1We would like to introduce to you the Graduate Health Insurance designed specifically for the Graduate Students employed by the Texas A&M University System. This program is underwritten by Companion Life Insurance Company, and administered by Associated Insurance Plans International, Inc. Our representative in College Station is Dunlap Financial Services. Preferred Provider Network100% payment for covered medical expenses incurred at the Student Health Center.

100% payment for preventive care and immunizations - deductible and co-payment do not apply to treatment received In-Network

80% for In-Network treatment for accident and sickness, after deductible and any co-payment PHCS and Beech Street National Providers

60% reimbursement for out-of-network treatment for accident and sickness, after deductible and any co-payment

2When you receive medical treatment at your campus Student Health Center, covered medical expenses relating to an accident or illness are reimbursed at 100%. Preventive care and immunizations are covered at 100% without applying deductibles or co-pays when you visit an In-Network provider. When you are unable to utilize your campus Student Health Center, in a non emergency situation, you should obtain medical treatment from a medical provider who is part of the Preferred Provider Network. This Network is accessible through our website, www.TAMUinsurance.com. Through the website, you would type in your zip code, and a list of providers in the mileage range you specify will be shown to you. You may also call the Preferred Provider Network directly for the list of providers.Benefits are paid at 80% when you use the Preferred Provider Network, and at 60% if you do not use the Network.In the case of an emergency, you should obtain treatment from the nearest medical facility.Whether or not you use the services of a Preferred Provider, the maximum you will need to pay out of your own pocket for each accident or sickness is $3,000.Deductible$100 each plan year per insured ($300 maximum for family)

NOTE: The deductible is waived for medical treatment received at your campus Student Health Center and for preventive care at any In-Network facility or In-Network physicians office.

3Your medical expenses are subject to a $100 deductible each policy year. If you insure your eligible dependents, the Deductible is $100 per insured dependent, without more than $300 charged each policy year. The deductible is waived for medical treatment at your campus Student Health Center and for preventive care and immunizations received from an In-Network provider. Co-paymentsOutpatient Physician Visit and Urgent Care Treatment Center: $25 plus 20%/40% of the remaining charges (co-payment does not apply for In-Network charges for wellness and immunizations)

4When you visit a physician for treatment of an accident or sickness, or use an Urgent Care Treatment Center, you will need to bring $25, which is the co-payment, you will be responsible for 20% of the charges if you use a Preferred Provider, and you will be responsible for 40% of the charges if you do not. When you go to an Emergency Room at a hospital, you are responsible for a $250 co-payment, and for 20% of the charges if you use a Preferred Provider, and for 40% of the charges if you do not. Outpatient office visits to an In-Network physician for preventative care and immunizations are not subject to the $25 co-payment and the visit is paid at 100%.

Pharmacy

5You may obtain your prescription medication from the Student Health Center or the pharmacy of your choice. Pharmacy benefits at the Student Health Center are paid at 100% to the policy maximum. A $15 co-payment is applied to each 30 day supply of your medication. Outside the Student Health Center, you may use the Medco Prescription Drug Card by going to a participating pharmacy, presenting your Insurance ID card, and paying the applicable co-payment to the pharmacy, $15 for each 30 day supply of generic medications, $25 for each 30 day supply of brand medication, and $35 for each 30 day supply of single source medication (where no generic is available). Contraceptive medication is covered through the Student Health Center, as well as through the Medco Drug Card. Provided as any other illness, but the pre-existing limitation does not apply to maternity

Newborn care for 3 days (normal delivery) and 4 days (caesarean delivery)

6Maternity Benefits are covered the same as any other medical condition, however the pre-existing condition limitation of the policy does not apply to maternity. There is a benefit for newborn care which provides 3 days of coverage for a normal delivery, and 4 days of coverage for a caesarean delivery. If you do not have insurance coverage for your children at the time of delivery under this insurance program, you will have 30 days to add your newborn child to the insurance program as your dependent. If you have other children, you may already have them insured. If this is the case, no additional premium may be necessary, you would simply need to notify us of the newborns name and date of birth. 7This program provides additional benefits in the event of your Accidental Death or Dismemberment. There is a benefit for Repatriation of mortal remains in the amount of $25,000 for International students, or $15,000 for US students. For those of you traveling overseas, there is a Medical Evacuation Benefit which provides $50,000 of coverage to evacuate you to your home country, or another medical facility. Any evacuation must be approved by the company prior to the evacuation. Pre-existing ConditionsPre-existing conditions are covered if you have been continuously insured for 18 consecutive months prior to enrolling in the plan, or you have been continuously insured for 12 months under any TAMU student plan, or you are 12 months treatment free for your pre-existing condition.

Exception to pre-existing condition limitation: If you have not been continuously insured for 12 months under any of the TAMU insurance plans, or 18 months under any other insurance plan, there is a $1,000 maximum benefit for your pre-existing condition until you are continuously insured for 12 months under the TAMU student insurance plan once you are continuously insured, benefits would be paid for your pre-existing condition on the same basis as for any new medical condition. The pre-existing condition limitation does not apply to Maternity.

Creditable Coverage: Applies towards the pre-existing condition limitation. This is health insurance coverage you have had in place for 18 continuous months prior to enrolling in this insurance plan (this would include a foreign country who provides health coverage). www.TAMUInsurance.com

8This insurance program limits charges for pre-existing conditions to $1,000 for each condition UNLESS you have been treatment free for 12 months, or continuously insured under the student insurance program or graduate student insurance program for 12 months, or you have been continuously insured under another creditable insurance program for 18 months. For international students, any insurance program through your home country or government will qualify as creditable coverage. 9The Monthly premiums which you would pay after the state contribution are $47.87 for a Student Employee Only; $604.80 for a Student Employee and spouse; $254.20 for a Student Employee and all Children; and $822.27 for a Student Employee with Spouse and all Children.Continuation PlanIf you graduate or leave school prior to the policy termination date, you may remain on the insurance program for the balance of the policy year as well as for one additional year. You must contact AIP within 31 days of your change in status, complete an application, and pay your premiums directly to AIP.

If your employment terminates, but you remain a registered student of the TAMU System, you must contact AIP within 31 days of your change in status, and you may continue this plan, or change to any of the other TAMU student insurance plans, by completing an application and paying your premium directly to AIP.

Request for continuation must be received within 31 days of your employment termination date.

10When you leave school or terminate as an employee of the Texas A&M University System, you may continue your insurance coverage.
