We are happy to celebrate all of the accomplishments during 2016 in spite of another year of

financial challenges and unprecedented increases in demand for services. Our staff, volunteers,

fundraising council, Advisory Council and Consortium Board continue to be creative in

stretching funding for services to meet the needs of older adults, and in their dedication to

volunteer to prevent longer wait lists.

We succeeded in becoming a Medicare certified provider to expand the Diabetes self-

management training and are working towards contracting with Medicare Advantage health plans

and other health providers to increase many of our evidence-based programs and classes. These

programs are extremely popular and have positive outcomes for the participants. This has been a

statewide partnership with the other 15 Area Agencies on Aging in Michigan (AAAs).

We continue to emphasize and practice our core values including:

The all-encompassing person-centered philosophy, supporting adults to function as

independently as possible

Increasing awareness of aging issues through education of the community, elected officials,

policy makers and others

Advocating effectively with older adults and persons with disabilities to bring about positive

change and outcomes and increased funding where it is needed the most

Supporting caregivers through the provision of effective information and supports

Continuing to improve the quality of services and include consumers in a meaningful manner

while maintaining sound policy and cost-effective practices and services

Continuing to provide comprehensive services without compromising quality, the safety of

consumers, the principles of person-centered care or being motivated by financial gain

Respecting the social, cultural and linguistic needs of diverse population groups in

communication, service delivery and program development

Strong community involvement of the TCOA Advisory Council and effective local support

from the TCOA Consortium Board

We continue to feel fortunate to live in the tri-county region with its rich and effective human

services, businesses and responsive local government collaborating so well. However, we need

adequate financial support including public and private fundraising as we are facing increased

need and dramatic changes.

We thank all of our dedicated and incredible staff for their commitment to clients and patience

during these changing times. We also appreciate the support of our Consortium Board,

Advisory Council and Friends for Independence Council.

Agency Updates & Accomplishments

Needs Assessment Process for Area Plan

In FY 2016, TCOA conducted an extensive needs assessment across Ingham, Eaton and Clinton

counties for input towards the Multi-Year Plan (MYP) for upcoming fiscal years 2017-2019. The

MYP is a proposal of services, budgets, goals and objectives the agency hopes to accomplish with

the funding received from the Aging and Adult Services Agency (AASA), and is completed

every 3 years. The needs assessment process included 16 Community Forums, 2 Public Hearings

and 150 Survey Responses. As a result, the proposed goals to be included in the area plan strive

to improve access to information, health, wellness and nutrition supports, protecting older adults

from abuse and exploitation and support individuals with dementia living in the community, as

well as their caregivers.

Community-based Care Transitions

Due to the success in reducing readmissions, Sparrow Hospital renewed the contract for

continuation the Community-based Care Transitions program in FY 2016. The program served

over 44% more clients in the 2016 fiscal year than in FY 2015. Trends confirmed positive

momentum of the program to reduce readmission rates within Sparrow Hospital. To further

reduce readmission rates in the tri-county area, discussions continued with other area hospitals

to expand the program.

Noteworthy Accomplishments At-A-Glance

Provided information and assistance, outreach and education (including elder abuse topics) to

over 19,000 older adults, caregivers and the general public

Over 1,600 frail persons received care management and essential in-home services supporting

them to remain independent in the community and out of the nursing home

Over 570 persons in the tri-county area participated in evidence-based classes and sessions

including fitness, disease prevention, Matter of Balance, Diabetes-PATH and SAVVY Caregiver

for persons caring for loved ones with dementia

Over 1,700 volunteers delivered Meal on Wheels, some delivering several times a month

Over 500,000 meals delivered to persons who are unable to prepare nutritious meals, many

connecting with the only person who regularly visits and checks on them

Over 1,800 received elder abuse education information or materials

Medicare and Medicaid counselors helped over 1,600 individuals to understand Medicare &

Medicaid, enroll in Medicare prescription drug coverage, review supplemental insurance needs,

apply for Medicaid Medicare Savings programs, and provide information to help identify and

report fraud, abuse or scams

The Friends For Independence Coordinating Council was formed to help raise local dollars to

supplement public funding by planning and organizing annual fundraisers, such as a dinner and auction, golf outing and direct mail appeals. The Council is a key part of increasing private sector

awareness and support for TCOA.

Tri-County Office on Aging in 2016. A special thank you goes to the members of the Friends For Independence Coordinating Council, Golf

Outing Planning Committee, Dinner and Auction Planning Committee and the Run for the Ages 5K Planning Committee. Assistance from all of these groups has helped raise funds to meet the con-tinuously growing demand for programs and ser-vices in the tri-county area. TCOA could not be successful without the hard work and support of many community volunteers. The 2016 vol-unteer numbers total about 22

full-time employees!

2016 Tri-County Aging Consortium Advisory Council

The Tri-County Office on Aging Advisory Council acts in an advisory capacity to the Consortium Board. At least one-half of the council consists of senior citizens, appointed by their

respective units of government. Remaining members represent community agencies. The Consortium Board approves their appointments. This composition offers the perspectives of

both seniors and service providers on aging issues.
