Tumor-Infiltrating Lymphocytes in Breast Cancer

ในปัจจุบัน Molecular subtype ของ breast cancer แบ่งเป็น 4 ประเภท กลุ่ม HER2 over-expression และ
Tripple negative เป็นกลุ่มที่พยากรณ์โรคไม่ดี ดังนั้น Tumor-Infiltrating Lymphocytes จึงเป็นตัวบอกพยากรณ์
โรคของ subtype เหล่านี้
Immune system and cancer
โดยทั่วไปแล้วระบบภูมิคุ้มกันแบ่งเป็น Innate immunity และ Adaptive immunity Innate immunity ได้แก่
Dendritic cell, Natural cell, Macrophage, Neutrophil, Basophil, Eosinophil และ Mast cell เป็นต้น คอยทำ
หน้าที่ต่อต้าน Antigen ทันที และทําหน้าที่กระตุ้นการเกิด Adaptive immune response ได้แก่ B lymphocyte, CD4+
helper T lymphocyte, CD8+ cytotoxic T lymphocyte หรือ NK cell ซึ่งจะเป็นตัวจดจําและจําเพาะต่อ Antigen
นั้นๆ และหากเม็ดเลือดขาวของ Adaptive immune response เหล่านี้ไป accumulate ที่ Tumor จะเรียกว่า Tumor
infiltrating lymphocytes (TILs)
Cancer immunoediting เป็นกลไกการต่อต้านเซลล์มะเร็ง และมีบทบาทสําคัญในการดําเนินของโรคมะเร็ง ซึ่ง
Cancer immunoediting มี 3 ระยะ ได้แก่ Elimination Equilibrium และ Escape โดยในระยะ Elimination นี้
Proinflammatory type I ทั้ง innate และ adaptive immune system ทํางานร่วมกันเพื่อต่อต้าน pathogen รวมถึง
Breast cancer tumor antigen ก็สามารถกระตุ้นภูมิคุ้มกันของร่างกายได้เช่นกัน1,2
โดยทั่วไปแล้ว tumor จะมี Immune checkpoint protein เพื่อป้องกันการทําลายของ tumor จาก T-cell
lymphocyte เมื่อ T cell มาจับกับ tumor ผ่านการจับกันระหว่าง PD-1 (Programmed death 1) และ PD-L1
(Programmed death 1 ligand) แล้ว จะทําให้ T-cell หยุดการทํางานไป ดังนั้นยากลุ่ม Monoclonal antibody ได้แก่
Pembrolizumab, nivolumab, lambrolizumab จะเป็นตัวยับยั้งการจับกันของ PD-1 และ PD-L1 มีผลทําให้ T-cell
ทํางานได้ตามปกติ2 (ภาพที่ 1) โดยทั่วไปแล้ว PD-L1 expression พบได้ประมาณร้อยละ 30 ของผู้ป่วย triple negative
และมี TILs สูง3

Tumor infiltrating lymphocytes (TIL)
Tumor infiltrating lymphocytes (TIL) เป็น adaptive anti-tumor immunity โดยปกติแล้ว TILs จะอยู่ทั้ง
ส่วนของ stroma และ intratumoral ของมะเร็ง TILs สามารถส่งตรวจจากชิ้นเนื้อนําไปย้อม Hematoxylin และ Eosin
(H&E)5 ทาง International TILS Working Group ได้กําหนดค่ามาตรฐานในการบอกปริมาณ TIL โดยมะเร็งที่มี TILs
มากกว่าร้อยละ 50 จะถือว่าเป็น Lymphocyte predominant breast cancer ซึ่งเป็นกลุ่มที่มีพยากรณ์โรคดีที่สุด6
TILs score แบ่งได้เป็น Low คือปริมาณของ Stromal TILs น้อยกว่าร้อยละ 10 ของพื้นที่ทั้งหมด, Intermidiate
คือร้อยละ 10-50 และ High คือมากกว่าร้อยละ 506,7
การศึกษาก่อนหน้านี้พบว่าการตอบสนองของระบบภูมิคุ้มกันของร่างกายต่อเซลล์มะเร็ง รวมถึง TIL จะมีมากใน
กลุ่ม Tripple negative, HER2-positive breast cancer, High grade (Grade III), High Ki67 expression
ซึ่งเป็นกลุ่มที่อัตราการแพร่กระจายของมะเร็งสูงและอัตราการรอดชีวิตต่ํา เมื่อเทียบกับ Luminal A7,8,9,10,11
TILs as a prognostic factor
- Tripple negative breast cancer
การศึกษา Meta-analysis เกี่ยวกับ prognostic factor ของ TILs ในผู้ป่วยกลุ่มนี้มีหลายการศึกษา ซึ่งแต่ละ
การศึกษาผลเป็นไปในทางเดียวกันคือ ผู้ป่วยมะเร็งเต้านมระยะแรก (early breast cancer) ที่มี TILs สูง จะมี Overall
survival, Disease free survival และ Distant disease free survival สูงกว่า TILs ต่ํา11,12,13,14,15,16

นอกจากนี้จากการศึกษาของ Ibrahim และคณะปี ค.ศ.2014 ในกลุ่มมะเร็งเต้านม Early stage พบว่า ผู้ป่วยที่มี TILs
สูงจะลด Risk of recurrence ได้ร้อยละ 30 (hazard ratio [HR] = 0.70; 95 % CI 0.56–0.87; P = 0.001), ลด Distant
recurrence ได้ร้อยละ 22 (HR = 0.78; 95 % CI 0.68–0.90; P = 0.0008) และลดอัตราการเสียชีวิตได้ร้อยละ 34 (HR
= 0.66; 95 % CI 0.53–0.83; P = 0.0003) นอกจากนี้ทุกๆร้อยละ 10 ของ TILs ที่เพิ่มขึ้นจะช่วยลด recurrence หรือ
mortality ได้ประมาณร้อยละ 15-2011,12
การศึกษา Breast International Group (BIG) 02-98 trial พบว่า เมื่อเปรียบเทียบกันระหว่างผู้ป่วยที่มี TILs
สูงกับต่ําแล้ว กลุ่มที่มี TILs สูง จะมี 5-year disease free survival ที่สูงกว่า หรือเท่ากับร้อยละ 92 และ 62 ตามลําดับ
(HR, 0.30; 95% CI, 0.11-0.81; adjusted P .018) และ 5-year OS สูงกว่าเช่นกัน หรือเท่ากับร้อยละ 92 และ 71
ตามลําดับ (HR, 0.29; 95% CI, 0.091-0.92; adjusted P .036)7 (ภาพที่ 2) การศึกษาต่อมาของ Dieci และคณะปี
ค.ศ.2015 พบว่า 10-year OS ในกลุ่มที่มี TILs สูงและต่ําไม่ได้ต่างกันอย่างมีนัยสําคัญ17
โดยสรุปคือในคนไข้กลุ่ม Triple negative breast cancer TILs เป็น prognostic factor คนไข้ที่มี TILs
สูงจะมีพยากรณ์โรคที่ดีกว่า

- HER2 positive breast cancer
ถึงแม้ว่าผู้ป่วยกลุ่ม HER2 positive ส่วนใหญ่จะมีปริมาณ TILs ที่สูงกว่ากลุ่มอื่นๆ เช่นเดียวกับ Tripple
negative breast cancer แต่กลุ่ม HER 2 positive นี้ไม่ว่าจะมี TILs สูงหรือต่ําไม่ได้ช่วยเพิ่ม Relapse free survival18
Disease free survival และ Overall survival อย่างมีนัยสําคัญ7,17 ในทางกลับกัน หากเป็นผู้ป่วยมะเร็งเต้านมที่มี HER2-
positive และได้รับยาเคมีบําบัดเป็น Anthracycline พบว่าผู้ป่วยที่มี TILs มากกว่าจะมี Disease free survival มากกว่า
กลุ่มที่ TILs ต่ํา หรือร้อยละ 78.6 และ 47 ตามลําดับ (P=0.022) แต่ไม่ช่วยเรื่อง Overall survival ส่วนสาเหตุที่ทําให้
สามารถเพิ่ม DFS ในกลุ่มนี้ยังไม่ทราบชัดเจน7

TILs as a predictive factor
ผู้ป่วยที่มี TILs สูงก่อนการรักษาจะทําให้การตอบสนองต่อยาเคมีบําบัด (Pathological response) เพิ่มขึ้น
ในผู้ป่วยกลุ่ม Tripple negative (OR=2.49, 95% CI: 1.61–3.83) และ HER2 positive breast cancer (OR=5.05,
95% CI: 2.86–8.92)13,14,16 การศึกษาต่อมา GeparSixto trial ปี 2015 พบว่า ในผู้ป่วยที่ได้รับ Neoadjuvant
chemotherapy เป็น Anthracycline และ Taxane-based แล้ว Pathological response rate ของผู้ป่วยที่ TILs สูง
คดิเป็นร้อยละ 59.9 เมื่อเทียบกับผู้ป่วยที่มี TILs ต่ําคิดเป็นร้อยละ 33.8 (P 0.001) และเมื่อเทียบกันระหว่างผู้ป่วยที่ได้
Nepadjuvant chemotherapy เป็น Anthracycline และ Taxane-based กับอีกกลุ่มที่ได้รับ Anthracycline Taxane-
based และ Carboplatin แล้วพบว่า Pathological response rate ของกลุ่มที่ได้ carboplatin ด้วยจะสูงกว่าคิดเป็น
ร้อยละ 75 เมื่อเทียบกับอีกกลุ่มร้อยละ 45 (P 0.002)19
เคมีบําบัดช่วยทําให้ antitumor immune response ดีขึ้น immunogenic tumor cell death มากขึ้น
เปลี่ยนแปลง Microenvironment ของ tumor แต่อย่างไรก็ตามปัจจุบัน TILs ยังไม่ได้เป็นตัวกําหนดว่าผู้ป่วยรายใด
ควรให้ยาเคมีบําบัด2,20
TIL in primary and metastatic breast cancer
ผู้ป่วย Metastatic breast cancer ทั้ง Triple negative และ HER2-positive เมื่อเปรียบเทียบกันระหว่าง
Primary tumor กับ Metastatic site พบว่าปริมาณ TILs ของ Primary tumor สูงกว่า คิดเป็นร้อยละ 34.6 และ 15.7
ตามลําดับ (p = 0.004) รวมถึงจำนวน TILs ที่สูงของ Primary tumor มากกว่าของ Metastatic site (ร้อยละ 52 และ 0)
ในขณะเดียวกัน TILs ต่ําพบใน Metastetic site ได้มากกว่า Primary site (ร้อยละ 56 และ 20)21
Targeting the immune system in triple-negative breast cancer
ดังที่กล่าวไว้ข้างต้น Immune checkpoint protein มีหน้าที่ป้องกันการทําลายของ tumor จาก T-cell
lymphocyte ดังนั้น immune checkpoint inhibitor ซึ่งเป็นหนึ่งใน targeted therapy มีหน้าที่ยับยั้งกลไกนี้ทําให้ T
cell ต่อต้าน Tumor ได้ปกติ การศึกษาของ Nanda และคณะปี ค.ศ.2016 พบว่า ถ้าให้ Pembrolizumab ซึ่งเป็นหนึ่งใน
anti PD-L1 ปริมาณ 10 mg/kg ทุกๆ 2 สัปดาห์ ในผู้ป่วย Advanced Triple-Negative Breast Cancer และมี PD-L1
positive แล้ว overall response rate เท่ากับร้อยละ 18.5 และระยะเวลาการตอบสนองประมาณ 18 สัปดาห์ แต่ผล
ข้างเคียงจากยาที่พบได้บ่อยได้แก่ ปวดข้อ เพลีย เมื่อยกล้ามเนื้อ และอาเจียน22
Conclusion
โดยสรุปแล้ว Tumor infiltrating lymphocytes เป็นตัวบอก prognosis ของโรค ถ้าหากผู้ป่วยมี TILs สูงกว่า
ร้อยละ 50 จะมีพยากรณ์โรคที่ดีกว่าและแนวโน้มตอบสนองต่อยาเคมีบําบัดมากกว่า แต่อย่างไรก็ตามปัจจุบัน TILs
ยังไม่ได้ใช้เป็นปัจจัยในการเลือกผู้ป่วยว่ารายใดควรให้ยาเคมีบําบัด
