INTRODUCTION
The goal of this study is to evaluate
the structural characteristics of filtering
blebs post glaucoma surgery from
an anatomical perspective using an
Ex-Press drainage implant with Optical
Coherence Tomography and Confocal
Microscopy. The Ex-Press drainage
implant is an aqueous shunt designed
to reduce intraocular pressure by
draining the aqueous humour from the
anterior chamber into a sub-Tenon's-
subconjunctival space. Once the
aqueous humour has been drained
to this virtual space it is continuously
discharged in three ways: transvenous,
transconjunctival or lymphatic. The
transconjunctival passage, which
was first cited by Kronfeld1 in 1952,
has recently been studied by Labbè
et al2 who have suggested that
aqueous transconjunctival transport is
carried by the conjunctival epithelium
goblet cells. In fact, it appears that
conjunctival microcysts are formed
due to the confluence of “modified”
goblet cells, meaning those cells
with reduced mucin due to aqueous
humour dilution (both being hydrophilic
molecules).
MATERIALS AND METHODSEighty-six eyes were examined. The
study involved 81 Caucasian patients
who had undergone glaucoma surgery
INTRODUZIONE
Lo scopo di questo studio è valutare da
un punto di vista anatomico l’aspetto
strutturale delle bozze post intervento
di glaucoma con impianto Ex-Press
mediante l’utilizzo della Tomografia a
Coerenza Ottica e della Microscopia
Confocale. L’impianto Ex-Press fa parte
degli impianti drenanti, ovvero shunts
dell’acqueo progettati per ridurre la
pressione intraoculare attraverso il
drenaggio dell’umore acqueo dalla
camera anteriore ad uno spazio
sottotenoniano-sottocongiuntivale. Una
volta deviato in questa area virtuale
l’umore acqueo viene continuamente
evacuato da questo spazio mediante tre
vie: via transvenosa, via transcongiuntivale
e via linfatica. La via transcongiuntivale,
evidenziata per la prima volta da Kronfeld1
nel 1952, è stata recentemente studiata
da Labbè e coll2 i quali hanno ipotizzato
che il passaggio transcongiuntivale
dell’acqueo avvenga tramite le cellule
mucipare dell’epitelio congiuntivale.
Le microcisti congiuntivali infatti
sembrerebbero costituite dalla confluenza
di cellule mucipare “modificate”, cioè a
scarso contenuto di mucina poiché diluita
dall’umore acqueo che viene richiamato
dalla stessa in quanto molecola idrofilica.

fra il 2011 e il 2013.
I soggetti inclusi nello studio erano
pazienti affetti da glaucoma primario
ad angolo aperto che nonostante
la terapia farmacologica massimale
presentavano una progressione del
danno glaucomatoso con peggioramento
del campo visivo.
I criteri di esclusione utilizzati sono stati
i seguenti: glaucoma ad angolo stretto,
glaucoma congenito, uveite, glaucoma
neovascolare, pregressi interventi
chirurgici per glaucoma (trabeculectomia,
impianto valvolare, ecc.), pregressi
interventi parachirurgici (Argon Laser
Trabeculoplastica, Laser trabeculoplastica
Selettiva).
Il tono endoculare è stato misurato prima
e dopo l'intervento con il tonometro di
Goldmann. Come criterio di successo
dell’intervento è stata presa una riduzione
di almeno un terzo della tensione
endoculare preoperatoria (senza utilizzo di
farmaci).
Nel corso della visita post-operatoria i
pazienti sono stati osservati alla lampada
a fessura e sono stati sottoposti ad
analisi in vivo con microscopia confocale
e tomografia a coerenza ottica del
segmento anteriore. Dopo aver eseguito
questi esami sono state formate delle
ulteriori sottocategorie in base all’aspetto
morfologico delle bozze.
Tomografia a Coerenza OtticaIl tomografo utilizzato in questo lavoro
è l’OCT Topcon 3D 1000, un OCT
Spectral Domain, che grazie al software
per il segmento anteriore è in grado
di delineare le strutture interne delle
bozze in base alle differenti proprietà
ottiche dei tessuti analizzati. Si tratta di
un esame non invasivo, non a contatto
(elemento importante in quanto non si
alterano meccanicamente le strutture più
delicate e si riduce il rischio di infezioni

The participants included in the study
were patients with primary open-angle
glaucoma which, notwithstanding
maximal drug therapy, showed
progressive glaucomatous damage with
worsening visual field.
The exclusion criteria applied were
the following: narrow-angle glaucoma,
congenital glaucoma, uveitis,
neovascular glaucoma, prior glaucoma
surgery (trabeculectomy, valve implant,
etc.), prior parasurgical interventions
(Argon Laser Trabeculoplasty, Selective
Laser Trabeculoplasty).
Intraocular pressure was measured
before and after surgery with the
Goldmann tonometer. Accepted
criterion for considering the intervention
successful was at least a third
reduction in pre-operative intraocular
pressure (without the use of drugs).
During post-operative examinations
patients were examined using the slit
lamp and underwent in vivo confocal
microscopy and anterior segment
optical coherence tomography analysis.
Following these examinations patients
were divided into further subcategories
based on the morphological
appearance of the blebs.
Optical coherence tomographyThe tomography system used in
this study was the Topcon 3D OCT
1000, a Spectral Domain OCT system
which, thanks to its anterior segment
imagining software, can outline the
interior structure of the bleb based on
the different optical properties of the
analysed tissue. It is a non-invasive,
non-contact (an important factor as it
does not mechanically alter the highly
delicate structures and reduces post-
operative infection risk), allows rapid
execution, is non operator-dependent,

non operatore dipendente, in grado
di fornire immagini ad alta risoluzione
e che consente una ricostruzione
tridimensionale della zona di tessuto
analizzata.
Sono stati utilizzati due metodi di
scansione: il metodo radiale, con centro
di rotazione a livello del flap sclerale,
capable of providing high resolution
images offering a three-dimensional
reconstruction of the analysed tissue area.
Two scanning techniques were used:
radial line scan, with rotation centre at
the scleral flap, 6.0 mm radial scan line;
and scan resolution of 1024 and 3D
grid with radial line scan of 6.0x6.0mm
and scan resolution of 512x128. Using

(asse Y), orizzontali (asse X) e coronali
(asse Z) (Fig. 1).
Nelle scansioni radiali che intercettano
il limbus e nelle scansioni lungo l’asse
y possiamo osservare: la posizione
del piatto dell’Ex-Press, il flap (quando
visibile), l’accumulo di liquido sopra il flap
(se presente) e la struttura della parete
della bozza. Grazie a queste scansioni
possiamo vedere inoltre se a livello del
flap si è sviluppata una reazione fibrotica
oppure se esso è adeso o meno alla
parete della bozza. Mano a mano che le
scansioni si spostano dal limbus verso la
periferia il piatto del dispositivo Ex-Press
non viene più visualizzato e si possono
apprezzare le altre componenti della
bozza ottenendo informazioni importanti
in caso di bozza diffusa con filtrazione
posteriore.
Grazie alle scansioni coronali (lungo
l’asse z) ricavate dalla ricostruzione
tridimensionale siamo in grado di valutare
soprattutto la lunghezza e la larghezza
del flap ed eventuali reazioni fibrose che
si possono sviluppare a questo livello;
inoltre si può osservare come varia lo
spessore della parete della bozza a vari
livelli di altezza partendo dalla base e
proseguendo verso l’apice della stessa.
Con l’ausilio del software incorporato
nello strumento abbiamo effettuato varie
misurazioni; in accordo con quanto
suggerito dalla letteratura in termini di
significatività3-4, i parametri misurati sono
stati i seguenti: lo spessore della parete
della bozza, la massima altezza interna e
la riflettività della parete.
Lo spessore della parete è stato calcolato
nel punto più spesso posizionando il

(Y-axis), horizontal (X-axis) and coronal
(Z-axis) images (Fig. 1).
From the radial scans that cross-
section the limbus and scan along the
Y-axis, we can observe: the position
of the Ex-Press plate, the flap (when
visible), the accumulation of liquid
over the flap (if present) and the
structure of the bleb walls. This scan
also makes it possible to identify if
a fibrotic reaction has developed on
the flap or if it has coated the bleb
walls. As the scan moves from the
limbus towards the periphery, the
Ex-Press plate is no longer visible and
other components of the bleb can be
studied, thereby garnering important
information if there is posterior bleb
filtration.
The coronal scan (along the Z-axis)
obtained from the three-dimensional
reconstruction, makes it is possible to
evaluate the length and width of the
flaps and possible fibrous reactions
that can develop. Furthermore, the
bleb wall thickness can be observed
at different levels starting from the
base and continuing towards the
apex.
The software incorporated in the
instrument facilitates the gathering of
various measurements. In accordance
with significant published works3-4,
the features measured were: bleb wall
thickness, maximum internal height
and wall reflectivity.
The wall thickness was calculated
at the thickest point by placing the
calliper between the outer and inner
edge (the one towards the scleral
flap). The maximum height of the bleb
is the distance from the inner edge
of the wall and the scleral flap. Wall
reflectivity was examined by analysing

calibro fra la faccia esterna e la faccia
interna (quella che guarda verso il liquido
soprasclerale). La massima altezza interna
della bozza è la distanza che intercorre fra
la faccia interna della parete e lo sportello
sclerale. Per quanto riguarda la reflettività
della parete sono state analizzate le
immagini a falsi colori ottenute dal
tomografo; come in altri studi4 abbiamo
stabilito un grading: da nero a blu assenza
di reflettività o bassa reflettività (Grado
1), da verde a giallo moderata reflettività
(Grado 2), rosso alta reflettività (Grado 3).
Le misurazioni sono state effettuate sulla
miglior immagine radiale intercettante
il limbus ottenuta su una serie di tre
scansioni.
Microscopia confocaleLo strumento utilizzato in questo studio
è un microscopio confocale a scansione
laser: Heidelberg Retina Tomograph
3 (HRT3) con il Rostock Cornea
Module (RCM) con ottica 300 x 300
microns. Gli occhi analizzati sono stati
precedentemente anestetizzati mediante
l’instillazione di Oxibuprocaina Cloridrato
allo 0,4% (Novesina®, Novartis Farma spa,
Roma, Italia). Lo strumento consente di
analizzare la cornea e la congiuntiva da
un punto di vista ultrastrutturale valutando
le strutture cellulari ed extracellulari sia
da un punto di vista qualitativo che da un
punto di vista quantitativo.
Sono state eseguite molteplici scansioni
al fine di esaminare la maggior parte della
superficie della bozza procedendo dal
limbus superiore verso l’alto, fin tanto
che è stato possibile ottenere immagini
qualitativamente soddisfacenti. In ogni
regione esaminata le scansioni sono
state effettuate partendo dall’epitelio
congiuntivale ed andando in profondità
verso la sclera (focalizzando manualmente
i vari strati) procedendo a steps di 1-2
microns fin quanto è stato possibile
acquisire immagini dettagliate.

studies4, we established a grading
system: from black to blue with lack
of reflectivity or low reflectivity (Grade
1); from green to yellow with moderate
reflectivity (Grade 2); red with high
reflectivity (Grade 3).
The measurements were performed
on the best radial images cross-
sectioning the limbus chosen from a
series of three scans.
Confocal microscopyThe instrument used in this study
was a confocal scanning microscope:
Heidelberg Retina Tomograph 3
(HRT3) with the Rostock Cornea
Module (RCM) with 300x300 micron
optical. The eyes were anesthetised
prior to analysis using Oxybuprocaine
Hydrochloride 0.4% (Novesina®,
Novartis Farma SpA, Rome, Italy). It
is possible to analyse the cornea and
conjunctiva from an ultrastructural
perspective using this instrument,
thereby evaluating the cellular
and extracellular structure from
both a qualitative and quantitative
perspective. Multiple scans were
carried out in order to capture as
much of the bleb surface as possible,
the scanning proceeded from the
superior limbus upwards until the
capturing of qualitatively-viable images
was no longer feasible. In each section
examined, the scans started from the
conjunctival epithelium and moved
deeply inwards towards the sclera
(manually focusing on the different
layers), proceeding at 1-2micron
intervals until the most detailed images
possible were captured.
The images which were captured at
approximately 2 mm from the limbus,
in the virtual centre of the bleb, were
the ones selected for our calculations.

i nostri calcoli sono quelle acquisite a 2
mm circa dal limbus, nel centro virtuale
della bozza.
Le strutture esaminate con attenzione
sono state l’epitelio congiuntivale ed il
tessuto connettivo subepiteliale.
L’aspetto dell’epitelio superficiale della
congiuntiva bulbare alla microscopia
confocale in soggetti sottoposti ad
interventi di chirurgia filtrante mostra
delle lacune iporiflettenti ovalari di
aspetto irregolare definite “microcisti
epiteliali”5 (Fig. 2). In questo studio
abbiamo valutato il numero di microcisti
per campo, ovvero per ogni scansione
eseguita rispettivamente vicino al limbus,
al centro della bozza ed in periferia,
abbiamo selezionato l’immagine con il
maggior numero di microcisti ed abbiamo
calcolato la media tra le tre.
Per quanto riguarda il tessuto connettivo
subepiteliale, in accordo con studi
precedenti4-5, il parametro che abbiamo
analizzato è la densità stessa del
connettivo valutata in termini di reflettività:
grado 0 scarsa densità, grado 1 media,
grado 2 moderata, grado 3 elevata (Fig. 3).
Analisi statisticaL’analisi statistica utilizzata in questo

The confocal microscopy showed
that the superficial epithelial layer
of the bulbar conjunctiva in patients
who had undergone ocular filtering
surgery displayed oval-shaped hypo-
reflective cells defined as “epithelial
microcysts”5. (Fig. 2) In this study the
number of microcysts was evaluated
per area. By this, it is meant that for
each scan done near the limbus,
the centre and the periphery of the
bleb respectively, the image with
the greatest number of microcysts
was selected and the average was
calculated between the three.
In accordance with previous studies4-5,
with regards to the subepithelial
connective tissue, the parameters
chosen to analyse were the connective
tissues own density in terms of
reflectivity: Grade 0 - low density,
Grade 1-average, Grade 2 - moderate,
Grade 3 – high. (Fig. 3)
Statistical analysisThe statistical analysis employed in

lavoro è servita a stabilire quali dei
parametri misurati con l’OCT e con
la microscopia confocale fossero
dirimenti in termini di significatività nella
distinzione fra bozze funzionanti e non.
Il Mann-Whitney U Test si è rivelato
il test più idoneo in questo tipo di
analisi in quanto consente il confronto
delle medie di due gruppi di campioni
indipendenti in cui non si può supporre
una distribuzione di tipo gaussiana.
Un ulteriore vantaggio di questo test è
che i due campioni in considerazione
non devono necessariamente avere lo
stesso numero di osservazioni.
L’ipotesi nulla assunta in questo caso è
che i due campioni siano stati estratti
dalla stessa popolazione (cioè che
tra i due non ci sia una differenza
statisticamente significativa).
Il valore di P utilizzato è stato 0.05.
Per P ≤ 0.05 si respinge l’ipotesi nulla.
Se P è > di 0.05 possiamo accettare
l’ipotesi nulla.
RISULTATIDei pazienti esaminati 35 erano
donne e 51 erano uomini con una età
compresa fra i 51 e gli 80 anni.
I valori di pressione endoculare prima
dell’intervento variavano fra i 24 ed i 37
mmHg.
In base alle misurazioni tonometriche
ottenute gli 86 occhi esaminati sono
stati divisi in 2 gruppi: Gruppo 1
(bozze funzionanti) composto da 39
bozze dove l'obiettivo pressorio dopo
l'intervento è stato raggiunto e Gruppo
2 (bozze non funzionanti) composto da
47 bozze nelle quali il tono endoculare
non è sceso fino al target pressorio
stabilito.
L'esame OCT ha consentito di effettuare
un'ulteriore classificazione degli occhi
esaminati creando delle sotto-categorie
in base all'aspetto morfologico delle
strutture analizzate4-6.

and confocal microscopy were void in
terms of significance in the distinction
between functioning and non-
functioning blebs. The Mann-Whitney
U Test proved to be the most suitable
test for this type of analyse as it permits
comparison between outcomes of two
independent sample groups wherein
a Gaussian-type distribution cannot
be assumed. A further advantage
of this test is that the two samples
being considered do not necessarily
have to undergo the same number of
observations.
The null hypothesis adopted in this case
is that the two samples are from the
same population (i.e. that between the
two, there is not a statistically significant
difference).
The p-value used was 0.05.
A ≤ 0.05 p-value disproves the null
hypothesis, while a > 0.05 p-value
supports the null hypothesis.
RESULTSThere were 35 women and 51 men in
the patient group ranging in age from
51 to 80.
Pre-operative intraocular pressure was
measured between 24 and 37 mmHG.
Based on the tonometric
measurements gathered the 86 eyes
which were examined were divided
into 2 groups: Group 1 (functioning
bleb) included 39 blebs where post-
operative target intraocular pressure
was reached; Group 2 (non-functioning
blebs) included 47 blebs in which the
intraocular pressure didn’t drop to the
pre-established target pressure.
The OCT exam made it possible to
execute a further classification of
the examined eyes, consequently
creating sub-categories based on
the morphological appearance of
the analysed structure4-6. The 39

Le 39 bozze funzionanti (Gruppo 1)
sono state divise in 14 bozze diffuse e
25 bozze cistiche.
Le 47 bozze non funzionanti (Gruppo 2)
sono state divise in 10 bozze piatte e 37
bozze incapsulate (Tabella 1). Ognuno
di questi gruppi presentava delle
caratteristiche peculiari anche all'esame
con il microscopio confocale (Tab. 1).
BOZZE DIFFUSE All'esame con la lampada a fessura
queste bozze appaiono molto estese e
poco elevate rispetto al piano sclerale.
L'esame tomografico presenta le
seguenti caratteristiche: uno spazio
iporeflettente sopra il flap, ristretto ma
continuo, compreso fra 0.05 e 0.19
mm (media 0.17±0.2); lo spessore della
parete della bozza compreso fra 0.15
e 0.26 mm (media 0.20±0.4); la media
della reflettività della parete di 1.1±0.1.
Alla microscopia confocale si
apprezzano microcisti epiteliali diffuse, in
media in numero di 28±10.2 per campo
e si osservavano poche fibre connettivali
scarsamente arrangiate con una densità
media di 1.3±03. In tutte le scansioni il
functioning blebs (Group 1) were
split into 14 diffuse blebs and 25
cystic blebs. The 47 non-functioning
blebs (Group 2) were divided into 10
flattened blebs and 37 encapsulated
blebs. (Table 1) Each of these
groups also presented particular
characteristics in the confocal
microscopy (Table 1).
DIFFUSE BLEBSUnder examination with the slit lamp,
these blebs appeared very extensive
and not very elevated relative to the
scleral surface.
The tomographic exam presented the
following characteristics: hypo-reflective
cells above the flap, which were small
but continuous, ranging from 0.05 to
0.19 mm (average 0.17±0.2); bleb wall
thickness ranging from 0.15 to 0.26 mm
(average 0.20±0.4); the average reflectivity
of the walls was 1.1±0.1.
The confocal microscopy revealed diffuse
epithelial microcysts, with an average of
28±10.2 per area and few connective
fibres were observed. Those that were,
were sparsely arranged and had an

piatto del dispositivo Ex-Press risulta ben
visualizzabile ed a stretto contatto sia
con lo sportello sclerale (superiormente)
che con il letto sclerale (inferiormente);
a volte è presente una sottile area
allungata iporiflettente fra le due strutture
(Fig. 4).
BOZZE CISTICHEAlla lampada a fessura appaiono
mediamente rilevate rispetto al letto
sclerale, con caratteristiche molto
variabili per quanto riguarda l'estensione.
All’OCT si apprezza un accumulo di
liquido sopra il flap con altezza variabile
da 0.45 a 2.25 mm (media 1.81±0.6),
lo spessore della parete è compreso
fra 0.12 e 0.44 mm (media 0.29±0.3)
con una riflettività media di 1.2±0.3.
Si evidenziano inoltre multipli spazi
iporiflettenti stratificati a più livelli nella
parete delle bozza.
Alla microscopia confocale si osservano
numerose microcisti epiteliali (in media
32±0.98 per campo) ed il connettivo
risulta scarsamente arrangiato con una
densità media di 1.2±0.4.
Il piatto Ex-Press appare ben
average density of 1.3±03. In all of the
scans, the Ex-Press plate was clearly
visible and in close contact with both the
scleral flap (above) and the scleral bed
(below). At times, a thin area of elongated
hypo-reflective cells was present between
the two structures (Fig. 4).
CYSTIC BLEBSUnder the slit lamp, they were detected
relative to the scleral bed with widely
varying features with respect to the
extension.
The OCT revealed an accumulation
of fluid on the flap with height varying
from 0.45 to 2.25 mm (average 1.81±0.6),
wall thickness ranging from 0.12 to 0.44
mm (average 0.29±0.3) with an average
reflectivity of 1.2±0.3. In addition, many
stratified hypo-reflective cells were identified
at different levels of the bleb walls.
Confocal microscopy found numerous
epithelial microcysts were observable
(on average 32±0.98 per area) and
the connective tissue was scarcely
arranged with an average density of
1.2±0.4.

visualizzabile al di sotto del flap sclerale e
tra la faccia superiore del dispositivo e la
faccia inferiore del flap è sempre visibile
una sottile area iporiflettente (Fig. 5).
BOZZE INCAPSULATEAlla lampada appaiono poco estese,
rilevate e piuttosto localizzate. All’OCT
si apprezza un grande accumulo di
liquido sopra il flap, altezza compresa
fra 1.55 e 2.50 mm (media 1.9±0.4),
circondato da una parete con spessore
molto variabile compreso fra 0.29 e
0.52 mm (media 0.38±0.5) e reflettività
media di 2.2±0.4.
Alla microscopia confocale si
evidenziano poche microcisti isolate
(in media 19±6.3 per campo) ed
il connettivo risulta densamente
arrangiato (in media 2.0±0.5).
Il piatto Ex-Press nel 35% dei casi non
appare individuabile nelle scansioni
orizzontali a causa dell’effetto schermo
causato dall’alta reflettività della parete
della bozza; in alcuni casi a causa
dell’eccessiva altezza della bozza
l’immagine non risulta visualizzabile per
intero nello schermo (Fig. 6).

top of the device and the lower edge of
the flap (Fig. 5).
ENCAPSULATED BLEBSUnder the lamp, they didn’t appear
very extensive or apparent, and quite
localised. The OCT showed a large
accumulation of fluid on the flap,
between 1.55 and 2.50 mm high
(average 1.9±0.4), surrounded by a
wall with variable density, between
0.29 and 0.52 mm (average 0.38±0.5)
and average reflectivity of 2.2±0.4.
In the confocal microscopy a few
isolated microcysts were visible
(average 19±6.3 per area) and the
connective tissue was densely
arranged (average 2.0±0.5).
In the horizontal scan, the Ex-Press
plate was not detectable in 35% of
cases due to the effect of the high
bleb wall reflectivity on the screen.
In some cases, due to the excessive
height of the bleb, the image couldn’t
be seen in its entirety on the screen
(Fig. 6).

BOZZE PIATTEAlla lampada a fessura appaiono
poco estese e senza una significativa
elevazione rispetto al piano sclerale.
All’esame OCT non si apprezza un
vero e proprio strato di liquido sopra
il flap oppure è assente, il piatto del
dispositivo Ex-Press risulta sempre
ben visualizzabile e la parete è molto
spessa (tra 0.32 e 0.48 mm con media
di 0.39±0.3) ed iperriflettente (in media
2.7±0.2).
Alla microscopia confocale non si
apprezzano microcisti, il connettivo si
mostra densamente organizzato e le
fibre connettivali appaiono iperriflettenti
(in media 2.5±0.4) (Fig. 7).
I dati relativi alle misurazioni effettuate
con l’OCT e la microscopia confocale
sono riportati nella tabella 2. I valori
riguardanti l’altezza e lo spessore della
parete sono espressi in millimetri (è
riportato sia l’intervallo massimo fra le
misurazioni che la media con relativa
deviazione standard), il numero delle
microcisti è riferito alla media per
campo e la reflettività della parete e del
connettivo fanno riferimento ai criteri di

significant elevation in relation to the
scleral surface. The OCT revealed a
barely visibly or possibly absent fluid
layer on the flap. The Ex-Press plate
was always highly visible and the wall
was very dense (between 0.32 and
0.48 with an average of 0.39±0.3) and
hyperreflective (average 2.7±0.2).
In the confocal microscopy no
microcysts could be seen. The
connectivity tissue was densely
organised and the connective fibres
appeared hyper-reflective (average
2.5±0.4) (Fig. 7).
The data related to the measurements
gathered using the OCT and confocal
microscopy are presented in Table 2.
The values regarding wall height and
density are expressed in millimetres
(both the maximum range between
measurements and the average
with relative standard deviation are
presented). The number of microcysts
refers to the average per area and
reflectivity of the wall and connective

tissue refer to formerly mentioned
evaluation criteria.
Statistical analysis of the data obtained
through the OCT was carried out
using the Mann-Whitney U Test. The
wall height, wall density and wall
reflectivity of Group 1 (functioning)
were compared with those of Group
2 (non-functioning). The results were
presented in Table 3. It can be noted
that the only statistically significant
parameter in terms of bleb functionality
was wall reflectivity.
Wall height and density were not
significant.
The parameters gathered using
the confocal microscopy were also
analysed statistically. Between the two
groups, both the number of microcysts
and connective tissue reflectivity
produced significant data for bleb
functionality (Table 4).
CONCLUSIONOver the years many published studies
have used ultrasound biomicroscopy
(UBM) to evaluate numerous post-
trabeculectomy filtering blebs; many
studies present reflectivity as the most
important criteria in the evaluation
of the bleb functionality7: heightened
valutazione enunciati in precedenza.
Nell’analisi statistica dei dati ottenuti con
l’OCT, effettuata con il Mann-Whitney
U Test, sono stati messi a confronto
l’altezza della bozza, lo spessore della
parete e la reflettività della parete fra
il Gruppo 1 (bozze funzionanti) ed il
Gruppo 2 (bozze non funzionanti).
I risultati ottenuti sono riportati nella
tabella 3. Possiamo osservare come
il solo parametro statisticamente
significativo in termini di funzionalità della
bozza sia la reflettività della parete.
L’altezza e lo spessore della bozza non
risultano significativi.
Anche i parametri ottenuti con la
microscopia confocale sono stati
sottoposti ad analisi statistica; fra i due
gruppi sia il numero delle microcisti
che la reflettività del tessuto connettivo
risultano essere dati significativi per la
funzionalità della bozza (Tab. 4).
CONCLUSIONICon l’UBM nel corso degli anni in
letteratura sono state valutate numerose
bozze filtranti post-trabeculectomia;
vari studi riportano come criterio più
importante nella valutazione della
funzionalità della bozza la sua reflettività7:

reflectivity leads to impaired filtration
ability. Today, thanks to anterior
segment OCT, it is possible to have
more detailed evaluation using a non-
invasive technique that eliminates
contact with the internal bleb structure.
Previous studies, which were also
carried out on post-trabeculectomy
blebs6-8, presented multicyst areas, thin
wall and low reflectivity as the principal
characteristics of bleb functionality.
On the other hand, flattened or
encapsulated non-functioning blebs
appear to be characterised by denser
walls and higher reflectivity.
Today’s published papers do not
include post Ex-Press blebs studies
which present data measured using
anterior segment OCT. In accordance
with what Ciancaglini et al4 observed
in post-trabeculectomy blebs, our
study did not uncover significant
differences in relation to density or
height when comparing functioning
and non-functioning blebs. The most
important differing factor between the
two groups was found to be the bleb
wall reflectivity. The functioning blebs
studied had lower wall reflectivity than
the non-functioning ones.
The confocal microscopy undertaken
una minor capacità di filtrazione. Oggi
grazie all’OCT del segmento anteriore,
siamo in grado di valutare più nel
dettaglio e con una metodica non a
contatto la struttura interna delle bozze.
Studi precedenti, eseguiti anch’essi
sulle bozze post trabeculectomia6-8,
evidenziano come caratteristiche
principali delle bozze funzionanti la
presenza di multipli spazi cistici, una
parete sottile ed una bassa relettività;
viceversa le bozze non funzionanti piatte
o incapsulate sembrano caratterizzate
da una parete più spessa e con una
maggiore reflettività.
Ad oggi in letteratura non ci sono studi
sulle bozze post Ex-Press che riportano
dati misurati con l’OCT del segmento
anteriore. In accordo con quanto
osservato da Ciancaglini e coll4 nelle
bozze post-trabeculectomia, nel nostro
studio non sono emerse differenze
significative per quanto riguarda
spessore ed altezza nel confronto fra
bozze funzionanti e non. L’elemento più
importante in termini di significatività fra
i due gruppi invece è risultata essere
la reflettività della parete della bozza.
Le bozze funzionanti analizzate avevano
una reflettività di parete più bassa
rispetto a quelle non funzionanti.

for this study found structural changes
effecting the connective tissue and
conjunctival epithelium which is borne
out in other published works9-10. Even
if the baseline studies were carried
out on post-trabeculectomy blebs4,
the fundamental parameters in the
evaluation of the functionality are
nonetheless the number of microcysts
and connective tissue density. Based
on the results obtained in relation to
the cases studied in this work, it can
be said that whether post Ex-Press
implant or post-trabeculectomy blebs
with a greater number of microcysts
tend to function better when compared
to blebs with a lower microcyst count.
This argues in favour of the importance
of transconjunctival filtration which
has been widely analysed in other
studies11-12.
It has emerged from this study that
scarcely arranged and low reflective
connective tissue is connected to
greater bleb functionality (conversely
in post-trabeculectomy blebs4 hyper-
reflective and densely-arranged
connective tissue is closely linked to
the procedure’s failure). More compact
connective tissue actually acts as
an obstacle to filtration through the
conjunctival epithelium
If the data obtained through the OCT
and in vivo microscopy were cross-
referenced, it can be extrapolated
from the OCT analysis that denser
connective tissue arranged more
complexly has higher reflectivity. On
the contrary, lower reflectivity in the
OCT correlates to looser and less
compact connective tissue. Therefore,
it is possible to deduce that the lower
the sub-epithelial fibrous tissue of
the conjunctiva is, the better the rate
of transconjunctival filtration of the
bleb will be. The higher the number
of epithelial microcysts is, the lower

questo lavoro ha evidenziato modifiche
strutturali a carico del connettivo
e dell’epitelio congiuntivale che
concordano con la letteratura9-10. Anche
se gli studi di riferimento sono stati
eseguiti su bozze post-trabeculectomia4,
i parametri fondamentali nella valutazione
della funzionalità risultano essere il
numero delle microcisti e la densità
del tessuto connettivo. In base ai
risultati ottenuti possiamo dire che,
per quanto riguarda la casistica di
questo lavoro, sia dopo Ex-Press che
dopo trabeculectomia le bozze con un
maggior numero di microcisti tendono
a funzionare meglio rispetto alle bozze
con un basso numero di microcisti.
Questo depone a favore dell’importanza
della via di filtrazione transcongiuntivale
ampiamente analizzata in altri studi11*-12.
Da quanto emerso in questo lavoro
il tessuto connettivo scarsamente
arrangiato e poco riflettente risulta essere
correlato ad una maggiore funzionalità
della bozza (come nelle bozze post-
trabeculectomia4) viceversa un
connettivo iperriflettente e densamente
arrangiato sembra maggiormente
legato al fallimento dell’intervento.
La presenza di un tessuto connettivale
più compatto infatti si comporta
come un ostacolo alla filtrazione
attraverso l’epitelio congiuntivale. Se
incrociamo i dati ottenuti con l’OCT e
con la microscopia in vivo, possiamo
ipotizzare che un tessuto connettivo
più denso, ed arrangiato in maniera più
complessa, all’analisi OCT risulti avere
una reflettività maggiore. Viceversa una
minore reflettività all’OCT correla con un
connettivo più lasso e meno compatto.
Quindi possiamo dedurre che minore è
la fibrosi subepiteliale della congiuntiva,
maggiore è la capacità di filtrazione
transcongiuntivale della bozza, più alto
è il numero delle microcisti epiteliali e

the intraocular pressure will be.
Future combined use of these two
diagnostic imaging techniques (along
with further technical advances) will
make it possible to understand new
mechanisms of post Ex-Press bleb
filtration; to play a predictive role with
respect to the functionality of the latter;
and to execute detailed analysis of the
complete bleb structure.
ABSTRACTIntroduction: The goal of this
study is to evaluate the structural
characteristics of filtering blebs
post glaucoma surgery from an
anatomical perspective using an
Ex-Press drainage implant with Optical
Coherence Tomography and Confocal
Microscopy.
Methods: Eighty-six eyes of 81
patients, who had undergone
glaucoma surgery via Ex-Press
implant between 2011 and 2013, were
examined. The filtering blips were
evaluated using Optical Coherence
Tomography (OCT) and Confocal
Microscopy. The principal features
assessed by the Tomography were:
bleb height, thickness of the bleb
wall and bleb wall reflectivity. The
principal features assessed by the
Confocal Microscopy were: number of
epithelial microcysts and subepithelial
connective tissue reflectivity.
Statistical analysis of the collected
data was carried out using the Mann-
Whitney U Test. This made it possible
to compare functioning blebs with
non-functioning ones.
Results: based on the intraocular
pressure measured post-operatively
two groups emerged: Group 1
(functioning blebs) eyes that showed
a reduction of at least a third of
preoperative tension and Group 2
(non-functioning blebs) where the

RIASSUNTOIntroduzione: lo scopo di questo studio
è valutare da un punto di vista anatomico
l’aspetto strutturale delle bozze filtranti
post intervento di glaucoma con
impianto Ex-Press mediante l’utilizzo
della Tomografia a Coerenza Ottica e
della Microscopia Confocale.
Metodi: sono stati esaminati 86 occhi di
81 pazienti operati di glaucoma mediante
impianto Ex-Press fra il 2011 ed il 2013.
Le bozze sono state valutate mediante
Tomografia a Coerenza Ottica (OCT) e
mediante Microscopia Confocale.
I parametri presi in considerazione alla
Tomografia sono stati: altezza della bozza,
spessore della parete della bozza e
reflettività della parete. I parametri valutati
con la Microscopia Confocale sono stati:
numero di microcisti epiteliali e reflettività
del tessuto connettivo subepiteliale.
L’analisi statistica dei dati raccolti è stata
effettuata con il Mann-Whitney U Test;
questo ha consentito di confrontare
le bozze funzionanti con quelle non
funzionanti.
Risultati: in base ai valori di pressione
misurati dopo l’intervento sono stati
formati 2 gruppi: Gruppo 1 (bozze
funzionanti) costituito da occhi che
presentavano una riduzione di almeno
un terzo della tensione preoperatoria e
Gruppo 2 (bozze non funzionanti) dove
la riduzione della pressione ottenuta
era meno di un terzo rispetto al valore

pressure reduction was less than a
third compared with the preoperative
level. Group 1 consisted of 39 blebs,
Group 2 from 47 blebs.
Based on tomographic appearance,
Group 1 was divided into diffuse blebs
(14) and cystic blebs (25). Group 2 was
divided into encapsulated blebs (37)
and flattened blebs (10). In comparing
the filtering and non-filtering blebs, the
parametres which produced significant
results were: bleb wall reflectivity
(tomographic analysis), microcyst and
conjunctival connective tissue density
(confocal microscopy).
Conclusions: this work has shown
that filtering blebs after an Ex-Press
implant have low reflectivity with
optical coherence tomography, show
an elevated number of microcysts per
area and low conjunctival connective
tissue density; while non-functioning
blebs demonstrated elevated bleb
wall reflectivity, a reduced number of
microcysts and denser subepithelial
connective tissue.
The OCT images made it possible to
deepen understanding of the implant
and the structural changes of the
filtering bleb over time and they also
provided significant surgical guidelines
in cases where re-operation was
necessary.

In base all'aspetto tomografico il Gruppo
1 è stato suddiviso in bozze diffuse
(14) e bozze cistiche (25). Il Gruppo 2 è
stato diviso in bozze incapsulate (37) e
bozze piatte (10). Nel confronto fra bozze
filtranti e non filtranti i parametri che sono
risultati significativi sono stati: la reflettività
della parete della bozza (all’analisi
tomografica), il numero di microcisti per
campo e la densità del tessuto connettivo
congiuntivale (alla microscopia confocale).
Conclusioni: da questo lavoro è emerso
che le bozze filtranti dopo impianto
Ex-Press hanno una bassa riflettività alla
tomografia a coerenza ottica, presentano
un elevato numero di microcisti per
campo e una bassa densità del tessuto
connettivo congiuntivale. Quelle non
funzionanti invece mostrano un'elevata
reflettività di parete, un ridotto numero di
microcisti ed un connettivo subepiteliale
più denso.
Le immagini ottenute mediante OCT
hanno consentito di approfondire la
conoscenza sul funzionamento del
dispositivo, sulle modifiche strutturali
della bozza filtrante nel corso del tempo
ed hanno fornito importanti indicazioni
chirurgiche nei casi in cui è stato
necessario eseguire un reintervento.
