As mentioned in our Fall 2013 edition of The Register, the terms of some of our Board of Directors came to an end on December 31, 2013. We appreciate the support and dedication of Thomas C. Barber, MD; Kevin J. Bozic, MD, MBA; Catherine H. MacLean, MD, PhD; J. Wesley Mesko, MD; and Patience H. White, MD, MA. Starting January 1, 2014, we welcomed five new Board members: Robert L. Krebbs, BA; Gregory B. Krivchenia, II, MD; Colin Nelson, BA; Scott M. Sporer, MD; and Bryan D. Springer, MD.

Nominated by America’s Health Insurance Plans as our second payer representative, Robert Krebbs is Director, Payment Innovation at WellPoint. In his position, he has accountability for the design, development, and rollout of value-based payment initiatives. Mr. Krebbs’ responsib-ilities include the management and oversight of the award winning Quality-In-Sights®: Hospital Incentive Program (Q-HIP®) and the facilitation of WellPoint’s Hospital National Advisory Panel. His work has led to collaboration with multiple clinical registries and the integration of clinical registry data into value-based payment solutions for both physicians and facilities. Most recently, he has been focused on leading the development of medical neighborhood programs at WellPoint aimed at aligning with the American College of Physician’s Patient-Centered Med-ical Home Neighbor (PCMH-N) concept.

As one of our two representatives of the American Academy of Orthopaedic Surgeons (AAOS), Dr. Greg Krivchenia is with First Settlement Orthopaedics, a private practice in the mid-Ohio Valley. He is a graduate of the University of Cincinnati College of Medicine, where he also completed his residency in orthopaedic surgery. He is an active member of both local and state medical societies in Ohio and West Virginia, and is past President of the West Virginia Academy of Medicine. He currently serves as a representative to the AAOS Board of Councilors for the West Virginia Orthopaedic Society.

Colin Nelson, the new Chairman of AJRR’s Public Advisory Board (PAB), is Senior Research Associate at the Informed Medical Decisions Foundation, where he oversees a portfolio of shared decision-making programs in orthopaedics and spine care. His responsibilities include synthesizing medical research and translating it into clear, evidence-based materials for patients so they can make informed decisions about their health. He is a member of the Medicare Evidence Development and Coverage Advisory Committee and associate editor of The BackLetter. Mr. Nelson’s award-winning writing for health care providers and the general public has appeared on sites such as CNN.com, WebMD, MedPage Today, and AOL Health. For ten years, he was editor of the newsletters Bone & Joint and Sports Medicine Digest. He holds a BA from Williams College.

Our other new representative from AAOS is Dr. Scott Sporer. He holds an academic appoint-ment at the rank of Associate Professor of Orthopaedic Surgery at Rush University Medical Center (RUMC) in Chicago and is the Co-Director of the Joint Replacement Institute at Cadence Health. He attended medical school at The University of Iowa before finishing his orthopaedic residency training at Dartmouth-Hitchcock Medical Center. He completed a fellowship in adult reconstruction at RUMC and currently has a clinical practice focusing on primary and revision hip and knee arthroplasty. His strong interest in clinical outcomes research led him to complete a master’s program at Dartmouth College in the evaluative clinical sciences.

Our Champions this quarter are George F. Chimento, MD, Sec-tion Head of Adult Reconstruction and Joint Replacement at Ochsner Health System in New Orleans and Kevin Meek, RN, BA, MHI, Chief Nursing Officer at OASIS Hospital in Phoenix.

Dr. Chimento is the Section Head of Adult Reconstruction and Joint Replacement at Och-sner. Ochsner’s main hospital, Ochnser Medical Center, joined the AJRR in mid-2012 and three additional Ochsner hospitals joined AJRR a year later, all under his guidance. Ochsner also plans to participate in our Level 2/Level 3 pilot program. Their participation will be extremely valuable to us. Dr. Chimento graduated from Georgetown University School of Medicine in 1990, going on to complete his internship and residency at Tulane University Medical Cen-

ter. Following residency, he served four years in the U.S. Air Force at Barksdale Air Force Base. Upon completion of his Air Force com-mitment, Dr. Chimento became a fellow at the Hospital for Special Surgery in New York City. He spent 5 years in private practice in the New Orleans area, and joined Ochsner in 2006.

Kevin Meek, RN-BSN, BA, MHI, is the Chief Nursing Officer at OASIS Hospital in Phoenix, AZ. Kevin led OASIS through a seamless and quick enrollment and data submission process. Given Kevin’s dedication to OASIS’s participation in the registry, he facilitated OASIS’s participa-tion in our Level 2/Level 3 pilot program. Prior to his work at OASIS, Kevin worked on ambulances as an EMT for many years, he continued his career in health care by becoming an RN. His experiences include Trauma ICU, Emergency Room, Trauma Room, and Post-Surgical units. He has been an advoc-ate for patients and their families as a bedside RN, as well as an advoc-ate for his staff as a Charge Nurse, Director, and now Chief Nursing Officer. Kevin completed his Master of Healthcare Innovation degree from Arizona State University, and is one of a small few in the nation to have this degree.

Breaking News!

On November 18-19, 2013, AJRR Executive Director, Jeff Knezovich, attended the United States Bone and Joint Initiative’s (USBJI’s) “Best Practices in Patient-Centered Musculoskeletal Care Summit” in Washington DC. USBJI is a multi-disciplinary ini-tiative targeting the care of people with musculoskeletal conditions – bone and joint disorders. The summit provided an opportunity for stakeholders to collaborate on recommendations on patient care and delivery systems for people with musculoskeletal conditions. A special breakout session brought together the stakeholders who are advancing the idea of a national registry system. The AJRR was able to discuss its beginnings with an attentive audience.

On November 20, 2013, AJRR Executive Director, Jeff Knezovich, and Director of Analytics, Caryn Etkin, attended a meeting of the National Quality Research Network (NQRN). The NQRN, sponsored by the American Medical Association, is a network of private and public registries and other stakeholders interested in advancing registries to measure and improve patient outcomes. The meeting was intended to discuss the NQRN’s registry maturational framework.

Currently in its third year, Operation Walk USA is an independent medical charitable organization that provides free joint replacement surgery to patients who don’t qualify for government assistance and have no insurance. More than 120 volunteer orthopaedic surgeons and 70 participating hospitals in 32 states – from Arkansas to Alaska – conduc-ted over 200 surgeries December 2-7, 2013, a timely way right before the holidays for orthopaedic surgeons to give back to patients and help improve their quality of life. AJRR is pleased to once again assist Operation Walk USA with data collection and analysis on their patients. On December 3, 2013, The Joint Commission again sponsored a webinar to provide their constituents an update on AJRR’s 2013 progress and 2014 plans. AJRR Board of Directors member E. Anthony Rankin, MD, and AJRR Direct-ors facilitated a discussion for over 500 participants. Last year’s webinar proved to be a huge success for our marketing and outreach efforts, and we expect the recent webinar will be just as beneficial. Special thanks to Jean Range, Execut-ive Director, Certification Strategic Business Unit, Disease-Specific Care Certification and Bridget Chambers, Associ-ate Marketing Manager, Certification, for their support.

In 2013, The American Taxpayer Relief Act (ATRA) (Public Law 112-240) created an additional pathway for physi-cians to participate in the Physician Quality Reporting System (PQRS) and other value-based purchasing programs. Section 601(b) of ATRA recognizes the opportunity to leverage clinical data registries to measure and improve health care through a process whereby physicians participating in a Qualified Clinical Data Registry (QCDR) are “deemed” to have satisfied quality reporting requirements under PQRS and other Medicare quality improvement programs. The QCDR final rule with comment period was published in the Federal Register on December 10, 2013. AJRR, along with other members of the Physician Clinical Registry Coalition, are reviewing the final rule and providing comment to the Centers for Medicare & Medicaid Services (CMS).

On January 17, 2014, AJRR will have a meeting in Washington, DC with CMS representatives. The purpose of the meeting is to gain insight on the type of registry data being sought by CMS. The new ruling is somewhat vague as to how we can protect individual PHI.

Does your hospital want Blue Distinction Recognition? Join the AJRR!The Blue Cross Blue Shield© Blue Distinction Centers® are part of a national designation program that recognizes

hospitals demonstrating expertise in delivering quality specialty care safely and effectively. There are six main specialty areas, including Blue Distinction Centers for Knee and Hip Replacement®. In order for a hospital to receive Blue Distinc-tion recognition, they have to meet a wide range of criteria concerning three major categories: cost, quality, and access. There are many standards and protocols which have to be implemented at the hospital in order to receive this recognition. These criteria can vary from being affiliated with accredited institutions, obtaining Health Information Technology (HIT) certification, Intensive Care Unit (ICU) Communication, to most importantly, submitting knee and hip replacement data to at least one national or multi-center registries or databases, such as the AJRR.

ArthroplastyWatchLaunched on February 1, 2013, ArthroplastyWatch is a unique, online

venue for information on joint implants. ArthroplastyWatch, located in Lund, Sweden, was developed throughout 2011-2012 as an information project with the purpose of collecting data on arthroplasty safety issues from a wide range of information sources on the Internet, and subsequently, disseminating information in one single, publicly accessible, channel. Via specially developed search engines, data is collected around the clock and then examined in several steps of medical and statistical expertise. Two levels of notification are used when there is the potential for a safety issue to occur and there is still uncertainty. The two levels are characterized as 1) caution is warranted, and 2) warning is declared when a safety issue is imminent or already occurring. The initiative includes a large number of inter-national arthroplasty experts, many of whom, such as AJRR Medical Director David Lewallen, MD, serve on the Arthro-plastyWatch scientific advisory board.

Finally, Dr. Springer is our new representative from the American Association of Hip and Knee Surgeons (AAHKS). He completed his residency in orthopaedic surgery at the Mayo Clinic and his Fellowship in Adult Reconstruction of the Hip and Knee at the Harvard School of Medicine/Brigham and Women’s Hospital. He is currently an attending Orthopaedic Sur-geon at the OrthoCarolina Hip and Knee Center in Charlotte, NC, where he serves as Fellow-ship Director and helped to establish its electronic web-based total joint registry/database, which now includes data on over 20,000 total joint arthroplasties. In 2008, he was a John Insall Travelling Fellow. In 2013, Dr. Springer was selected as a member of the Knee Society and also was named by DePuy Synthes Joint Reconstruction and Orthopaedics Today as a member of Leaders in Total Joint Replacement: Generation Next.

We are excited to begin working with these new Board members and look forward to their valuable input and collaboration.

In December, AJRR added our 9th staff member. Matthew Timmers is a

Program Advancement Associate, and the first staff member in our new Pro-

gram Advancement division. These staff will bear primary responsibility for recruitment and enroll-

ment of hospitals as our analytics team moves out of recruitment mode and in to the analysis and reporting of Registry data.

Matthew is a native of Harwood Heights, IL. He is a 2012 graduate of the University of Illinois at Urbana-Champaign, where he earned a BS in Health Planning and Ad-ministration. While at the University of Illinois, he worked with the Director of Wellness and Sports Medicine at Carle Hospital. From 2012-2013, he worked as a Scheduling and Clinical Specialist with Mobile Doctors.

In our previous newsletter, we discussed plans to initiate a pilot project of our Level 2 and Level 3 data systems. Throughout 2013, AJRR staff prioritized the expansion of reporting capabilit-ies and the development of a system to capture Patient-Reported Outcome Measures (PROMs). In November, the pilot study of 15 participating hospitals was launched to: a) assess a hospital’s ability to utilize their Electronic Health Record (EHR) to extract and submit Level 2 data elements; and b) test workflow models for

completing PROMs tools with patients. Some of the early lessons learned include the need to capture some Level 2 data, such as post-operative complications, in the surgeon practice setting, rather than in the hospital. We also are learning that in many instances both pre- and post-op PROMs will be collected by practices. As such, we have begun discussions to develop contracts to enroll surgeon practices in the AJRR so that we can capture Level 2 and Level 3 data from all of our participants in all settings. Fortunately, pilot project hospitals represent a broad spectrum of EHR capabilities; a combina-tion of hospital-employed and private practice-employed surgeons; and both large/small hospitals and health systems. We expect that the lessons learned during the pilot study will help AJRR staff to correct deficiencies in the electronic system and data submission process and provide valuable information to better assist hospitals in minimizing submission bur-den. This effort will be valuable to all participating sites as they move toward submission of these additional levels.
