ABSTRAK
Tuberkulosis (TB) merupakan penyakit pcringkat kedua yang menyebabkan
kematian karena infeksi setelah HIV/AIDS. Indonesia merupakan negaxa ketiga di
dunia dalam urutan jumlah penderita TB setelah India dan Cina. Faktor kuman
saja tidak dapat menjadi faktor tunggal dalam kejadian TB, letapi harus disertai
dengan faktor lain. Defisiensi vitamin D dan polimortisme Fokl, Bsml, Apal, dan
Taql gen reseptor vitamin D (RVD) berperan penting dalam hubungannya dengan
kerentanan terhadap TB. Tujuan penelitian ini adalah untuk mengetahui peran
defisiensi vitamin D dan polimoriisme Fold, Bsml, Apal, serta Taql gen RVD
terhadap TB anak_
Penelitian ini merupakan analisis observasional dengan rancangan kasus
kontrol. Penelitian dilakukan di RSUP Dr, Hasan Sadikin Bandung dan RSU
Cibabat Cimahi sejak Mei 2008 hingga Maret 2009. Kelompok kasus sebanyak 42
anak yang menderita TB dan memenuhi kriteria penelitian, diambil secara
consecutive sampling, sedangkan kelompok kontrol sebanyak 42 anak yang tidak
menderita TB. Dilakukan pengambilan darah untuk pemeriksaan kadar vitamin D
[25-(OH)D dan 1,25-(Ol-I);D] serum, serta polimorfisme Fo/cl, Bsml, Apal, dan
Taql gen RVD. Analisis statistik yang digunakan adalah uji Chi~kuadrat, uji
Mann-Whitney, uji t, uji kecocokan Model Hardy-Weinberg, menghitung odds
rasio (OR) dan 95% CI, Serta analisis regresi logistik ganda.
Pada penelitian ini didapatkan angka kejadian deisiensi kadar l,25(OH)1D
serum pada kelompok kasus TB sebesar 28,6%, sedangkan konlrol hanya 9,5%,
hasil uji Chi»kuadra! diperoleh nilai p = 0,026 dengan besamya OR (95% Cl):
3,80 (1,ll-12,98). Kejadian polimorfisme Fakl gen RVD pada kelompok kasus
TB sebesar 66,7% dan kontrol sebesar 40,5% yang menunjukkan adanya
perbedaan bennakna Q: = 0,016) dengan besarnya OR (95% CI): 2,94 (l ,2l-7,\6),
sedangkan Apal, Bsml dan Taql tidak bennakna (p>0,05)_ Hasil analisis
muhivariabel dengan menggunakan regzesi logistik ganda, variabel yang
berpengaruh terhadap kejadian TB adalah jenis kelamin OR (95% CI): 2,276
(0,841-6,16l); polimoriisme Fokl OR (95% CI)i 2,346 (1,053-5,225);
pnlimorfisme Apal OR (95% CI): 0,81 (0,912-3,593) dan delisiensi vitamin D
dengan OR (95% CI): 5,645 (1,441-22,113). Peluang terjadinya TB pada anak
perempuan dengan defisiensi vitamin D serta polimnrfisme Fakl (genotipe FF)
dan Apal homozigo! (genotipe aa) adalah 0,98, sedangkan pada laki-laki hanya
0,955_
Disimpulkan bahwa defisiensi vitamin D (l ,25(OH);D) dan polimorlisme Fokl
gen RVD merupakan faktor risiko texjadinya TB pada anak. Perempuan dengan
deiisiensi vitamin D serta polimoriisme Fakl dan Apal homozigot memiliki
peluang terbesa: untuk terjadinya TB pada anak.

ABSTRACT
Tuberculosis (TB) is the worlds second commonest cause q/ death #om
injections disease, aher HIV/AIDS. Indonesia is the third most populous' countries
have the largest numbers of TB cases aher India and China. .Mycobacterium
tuberculosis itscy' is not the only single factor of TB occurence, but also it should
be accompanied by other factors. Wtamin D dchciencies and polymorphism of
Fold Bsrnl, Apal and Taq] vitamin D receptor (VDR) gene has been considered
important related to the susceptibility of TB occurance_ The aim of this research
was to #nd out the role of vitamin D deficiencies and polvmorphism of F0lKL
BsmL Apal and Taq] VDR gene on TB in children.
The study was an observational anab/tic research with case control design,
Ihe study was done in RSUP Dr. Hasan Sadikin Bandung and RSU Cibabat
Cimahi from May 2008 to March 2009. Case group of 42 children with TB and
meet research 's criteria, were taken by consecutive sampling, while control group
consisted of 42 children without TB, The blood sample were taken to measure
serum vitamin D level [25-(OIHD and 1,25-(0H)2D], andpolymorphism ofF0l(L
Bsml ApaL and T aql VDR gene. Statistic analysis in this study was Chi-square
test, Mann-Whitney test, t test, Hardy- Weinberg principle, to calculate odds ratio
(OR) and 95% confidence interval (Cl), and double logistic regression anab/sis,
The incidence ofserum l,25(0I~D1D level dqiciencies in this TB case group
was 28.6%, ana' onLv 9.5% in control group. We got signyicant result using Chi-
square testp value = 0.026 with OR (95% CI): 3.80 (1.1Ifl2_98). Pobimorphism
incidence ofFok1 VDR gene in TB case group was 66.7% and 40.5% in control
group, which showed signyicant dwrence (p = 0.016) with OR (95% CD; 2,94
(I.2IfZl6), but ApaL Bsml and Taq] no sigriyicant d#rence @1>0_05).
Multivariable analysis result using double logistic regression, the variable that
influenced TB occurance was gender, OR (95% CI): 2.276 (0841-6.16l),'
polymorphism ofFokL OR (95% CD: 2.346 (l.053-5.225); polymorphism ofApa.
OR (95% CD: 0.81 (0.9l2-3.593) and vitamin D dqiciencies with OR (95% CI);
5.645 (1,441-22,1 13). Chance of TB disease on girls with vitamin D deficiencies
and pob/morphism of Fakl (FF genotype) and Apal homozygote (aa genotype)
was 0.98, while on boys onb/ 0. 955.
It is concluded that vitamin D deficiencies (l,Z5(0H);D) andpolymorphism of
Fokl VDR gene are risk factors of TB in children. Girls with vitamin D
defciencies and polymorphism of Fokl and Apal homozygote have greatest
chance of having TB in children.
