Directions

Each student will have the chance to go to two Innovation

Stations daily (A and B). The attached sheets are

designed to help us match the student with stations that

best fit their individual needs.

Students: Make sure you put your name and your

teacher’s name at the top on BOTH Sheets A and B in the

boxes provided.

Choose five (5) stations that interest you for A and five (5)

stations that interest you for B. Rate them on a scale of

‘1’ to ‘5.’ The one that you rate a ‘1’ is the station that you

want the most, all the way to ‘5.’ Remember, you will do

this two times– once for Innovation Station A and again

for Innovation Station B. While we make every effort to

give students their 1st or 2nd choice, please know that

we cannot guarantee station placement.

Parents, please do this together with your child and sign

that you are aware of the choices made on both sheets.

Please do not choose any one of the Innovation Stations

that you have already participated in.

Send back BOTH sheets A and B sheets by FRIDAY

MORNING (October 11th) at 7:45! Placement will

begin on this day.
