The intervention is aimed to build trust and capacity among citizens by involving them in improving

health service delivery mechanism and also holding Health Service Providers accountable and

augmenting their responsiveness to the health needs of the community. Mechanisms need to be

developed and implemented to enhance the Social Accountability of Service Providers and the Health

System through engagement of Village Health, Sanitation & Nutrition Committee (VHSNC) and

Block level PRI representatives in the State.

Under SAI, it is proposed and planned to undertake multiple complementary and interactive activities,

i.e. informing citizens, creating simple feedback mechanisms, improving service response and

enhancing community, household and individual health behaviours, thereby strengthening interface

between health services and community. Project Design comprises of 12 Districts. The intervention

will be initiated in 3000 Gram Panchayats of 51 Development Blocks of the project district. The

proposed districts are :1. Banda 2. Ambedkar Nagar 3. Mau 4. Basti 5. Kushinagar 6. Chandauli 7. SR

Nagar (Bhadohi) 8. Moradabad 9. Jyotiba Phule Nagar 10.Hapur (Panchsheel Nagar) 11. Sultanpur

12. Fatehpur

Objective : The Project envisages to inform the community of their health entitlements, facilitate

monitoring based on agreed benchmark indicators and enable feedback on the indicators to the health

system to facilitate corrective action in a framework of accountability, and enable the community to

be partners in the process of improving the functioning of health system as envisaged by National

Health Mission (NHM). The specific objectives of the Project are:

1. To develop a design for the Social Accountability Intervention at Block and Village level to enable

the community to be informed of their health entitlements,

2. To facilitate monitoring based on agreed benchmarks / indicators & enable feedback on the

indicators to the health system to facilitate corrective action in a framework of accountability, and

3. To enable the community to be partners in the process of improving the functioning of health

system as envisaged by NHM.

Section-I

Social Accountability Intervention: An Overview

WhatsApp Messenger is a cross-platform mobile messaging app which allows to exchange

messages without having to pay for SMS on Real Time Basis.

The Social Accountability Intervention Projects activities started in the field from 22.02.2016

after Orientation Programme of District Coordinators and Sub District Coordinators. As all

Reporting & Monitoring formats have been finalized and printed, Orientation of field staff

completed and they have been deputed in the field with templates of the formats for reporting

purposes.

Under the guidance of Mr. N. S. Ravi, IAS, Chairman, CDA-DDUSIRD and Dr. O. P. Pandey,

Centre Director, CDA-DDUSIRD an WhatsApp Based Reporting Initiative (WBRI) was taken as

a initiative by State Programme Management Unit. This help the State Program Management

Unit in Monitoring the project activities on day to day basis and by end of the month it will be

generated into monthly report for the project as real time communication between grass-root

workers and state functionaries.

Under this system an WhatsApp group consists of all the field staff (DC-SDC), SPMU and

UPHSSP team locally were formed. Under this Group all District Coordinators and Sub-District

Coordinators are advised to upload the day to day activities with relevant photographs, letters/

circulars etc. issued concerning the program. Not only this, any instruction from SPMU or

UPHSSP relating to the program can be promptly conveyed to all field staff. It was also instructed

to all the field staff that only 2-3 relevant photographs to be posted with proper caption on each

image they are going to post.

Based on photographs and other report received, a monthly progress report is being prepared and

documented by the SPMU for perusal of various authorities.

District wise observation of Village Health & Nutrition Day (VHND)

Under Social Accountability Intervention Project in the month of September,2016, A total of

1530 VHND were observed in 1530 Villages by Gram Panchyat Coordinators (GPC), Sub

District Coordinators (SDC) & District Coordinators (DC). The no of VHND mentioned in the

table are only those which have been attended by representatives of SAI. From October 2016

onwards total number of VHND done in the block will be reported and out of which how many

VHND were attended by SAI representatives will be reported.

All the VHND are being jointly organized by ANM, ASHA and AWW. ASHA's role is to inform

the intended beneficiaries (pregnant/lactating mother & children below 6 years) who are coming

under the schedule period of VNHD week. ANM provides health related services & AWW

distribute Take Home Ration (THR). Following are the monthly progress details of 12 pilot

districts:-

District and Month wise Village Health Sanitation & Nutrition

Committee (VHSNC) meeting status

Under Social Accountability Intervention Project in the month of September, 2016, a total No. of

1528 VHSNC were attended by Gram Panchyat Coordinator (GPC), Sub District Coordinators

(SDC) & District Coordinators (DC). Following are the monthly progress details of 12 pilot

districts. In the month of September 2016 a total of 275 ATR were generated. Till September,

2016, a cumulative total of ATR comes to 1663 out of which 1416 ATR were submitted to line

departments out of which 715 ATR compliances have been taken by the line departments either

by Panchyat or Block. It is observed that VHSNC meetings are planned well in advance by ANM

and inform the Gram Pradhan and other members and fix the date and time for organizing the

meeting. The objectives of VHSNC meeting is to update the members of the committee and

community regarding the services received by community and generate demand for better

services.

In case issues are raised which cannot be solved at Gram Panchyat level, ATR is prepared after

taking concerned from all the members and the community and send to the concerns

departments. The meeting is recorded in the VHSNC meeting register. The figures of VHSNC

mentioned in the table are only those which have been attended by representatives of SAI, from

October, 2016 onwards total number of VHSNC done in the blocks will be reported and out of

which VHSNC attended by SAI representatives will be reported. Following are the detail of

VHSNC & ATRs in 12 districts mentioned in Table 2 :-

Training of Trainers (ToT for District & Block level Trainers)

It feels essential to organizing this Training of Trainers, keeping in view the objectives and

strategy of Social Accountability Intervention in Health and Nutrition in identified Districts,

which gives priority to community participation in promoting Social Accountability through

activating Village Health, Nutrition and Sanitation Committees (VHSNC) at Gram Panchyat

level. Besides their basic responsibility, the trainers have to understand the training needs of the

target groups proposed to be trained, conduct the training programme, make arrangements for

organizing the same and evaluate the impact of the training. In addition to these responsibilities as

trainers they have also to play such important roles as facilitators and their most important role,

however, shall always be a change agent.

Purpose of this ToT was to create a pool of trained and enabled Master Trainers for VHSNC &

AAA training. These trainers included district and block level official of Health, PRIs, ICDS and

Rural Development departments. These trainer's were briefed about Social Accountability

Intervention and other thematic areas such as different programs of Health, ICDS and PRIs,

VHSNC, VHND so that they would be enabled to take their sessions in trainings to be organized

for ANM, ASHA, AWW & VHSNC members. They were also briefed about programme

evaluation formats, Action Taken Reports and Score Card Activity. A pool of these Master

Trainers is created as additional trained human resources apart from District Coordinators (DC)

and Sub District Coordinators (SDC). They might be the regular resource persons in future for

RIRD & DIRD at the Regional/District level.

Now, his training will help the Master Trainers in training the AAA and VHSNC members during

the training programme. As all the Master Trainers belongs to stakeholders i.e. Health, ICDS &

Panchayat departments, they may use the skill and information of Social Accountability

Intervention goal in other areas where SAI is not been implemented. The details of such training

programme organized are given below :-

Challenges and Achievements under SAI Project

1. Keeping in view the training need of VHSNC & AAA, an additional pool of Master

Trainers is created through organizing ToT at District level, for under no budget is

sanction in the project.

2. Training of GPCs on Application of Tablets were organized at District level, for which no

budget is sanction in the project.

3. Contact drives are doing by GPCs to ensure the presence of Gram Pradhan and AAA in

VHSNC training programme.

4. ANMs are not showing more interest in preparing Micro Plan for VHSNC meeting &

VHND.

5. Several VHSNCs have money in their account but ANMs are not taking initiative to spent

it on prescribed activities.

6. Some where the funds are spent by Pradhan but many of them are not giving bills &

vouchers to ANM for fund liquidation.

7. In the periods of AAA and VHSNC training of Chandauli Sadar Block, The CMO of

Chandauli district almost is trying to attend meeting to mobilized to VHSNC members. It

is an achievement of our advocacy. Also the CMO released order for ANMs to utilized

untied fund otherwise salary would be unreleased.

8. In every Block MOICs, CDPOs, ADOPs and others officials participated as resource

persons AAA and VHSNC members training.
