Latar Belakang Kasus SLE di US lebih dari 300.000
Kriteria diagnostik yang sudah banyak digunakan adalah yang di buat oleh ACR (American College of Rheumatology) Kriteria ini di publikasikan tahun 1982 dan direvisi tahun 1997 Kriteria tahun 1982 sudah divalidasi, kriteria 1997 belum
Tujuan penelitian : The Systemic Lupus International Collaborating Clinics (SLICC)merevisi dan memvalidasi kriteria klasifikasi SLE dari American College of Rheumatology (ACR) agar meningkatkan relevansi klinis, mendapatkan persyaratan metode yang lebih ketat, dan pengetahuan baru mengenai imunologi dari SLE.
Metode Tahap Derivasi (menentukan kriteria baru dari kriteria yang sudah ada)
Memilih variabel yang relevan Mendapatkan pasien sampel dalam bentuk skenario Menentukan diagnosis dari sampel (mengklasifikasikan SLE atau tidak) oleh ahli
Metode Tahap Validasi
menilai kriteria yang baru, layak digunakan atau tidak dengan menggunakan sampel baru (690 sampel)
Hasil Tahap derivasi
Didapatkan 716 skenario pasien sebagai sampel Dari total sampel didapatkan diagnosis menurut ahli : SLE (n 293), RA (n 119), myositis (n 55),
chronic cutaneous lupus (n 50), undifferentiated connective tissue disease (n 44), vasculitis (n 37), primary APS (n 33), scleroderma (n 28), fibromyalgia (n 25), Sjo gren’s syndrome (n 15), ̈rosacea (n 8), psoriasis (n 7), sarcoidosis (n 1), dan Juvenile Idiopathic Arthritis (n 1)
Didapatkan kriteria diagnostik (table 3 terlampir) dibagi dalam kriteria “klinis” dan kriteria “imunologi”
Dikatakan SLE jika pasien tsb memenuhi minimal 4 dari kriteria klinis dan imunologi, yang didalamnya minimal 1 kriteria klinis dan 1 kriteria imunologi, ATAU jika pasien tsb telah di biopsi ginjal dan terbukti lupus nefritik dengan dibuktikan adanya ANA atau Anti dsDNA Antibodi
Hasil Tahap Validasi
Didapatkan 690 sampel baru untuk proses validasi kriteria yang baru SLE (n 337), RA (n 118), undifferentiated connective tissue disease (n 89),
primary APS (n 30), vasculitis (n 29), chronic cutaneous lupus (n 24), scleroderma (n 20), Sjo - gren’s syndrome (n 15), myositis (n 14), psoriasis (n 8), fibromyalgia ̈(n 4), alopecia areata (n 1), dan sarcoidosis (n 1).
Diskusi Malar rash & fotosensitif bukan kriteria yang terpisah, karena keduanya sering overlap
Kriteria artritis didefinisikan ulang Tidak perlu di periksa secara radiologi Morning stiffness selama 30 menit dapat di masukkan sebagai artritis
Kriteria renal, sekarang penghitungan proteinuri bisa menggunakan urin protein-creatinin rasio. (tidak memerlukan waktu lama(24 jam) untuk menampung urin, bisa menggunakan urin sewaktu/random)
Kriteria hematologi dibagi menjadi 3 : hemolytic anemia, leukopenia/lymphopenia, and thrombocytopenia
Diskusi Aspek terpenting pada klasifikasi SLICC yang baru adalah hasil biopsi
didapatkan nefritis yang berhubungan dengan SLE menurut International society of nephrology/renal pathology society 2003 (klasifikasi dari lupus nefritis) Didapatkannya ANA atau anti-dsDNA antibodi sekarang sudah bisa diklasifikasikan SLE
Kriteria klasifikasi SLICC lebih baik daripada kriteria ACR yang sudah di revisi untuk sensitivitas tetapi tidak spesifisitas. Kriteria SLICC lebih relevan dari segi klinis, karena dapat mendefinisikan lupus secara klinis apda pasien lebih banyak dari pada menggunakan kriteria ACR yang sudah ada.
Sensitivity. Identifies the proportion of people who test positive in a group of people known to have the disease or condition, or the proportion of people who are true positives compared with the total number of people who actually have the disease. When the observation or test is negative in people with the disease, the result is termed false negative. Good observations or tests have a sensitivity of more than 90%, and help rule out disease because there are few false negatives. Such observations or tests are especially useful for screening.Example: The sensitivity of Homan's sign in the diagnosis of deep venous thrombosis (DVT) of the calf is 50%. In other words, compared with a group of patients with deep vein thrombosis confirmed by phlebogram, a much better test, only 50% will have a positive Homan's sign, so this sign, if absent, is not helpful because 50% of patients may have a DVT.
Specificity. Identifies the proportion of people who test negative in a group of people known to be without a given disease or condition, or the proportion of people who are “true negatives” compared with the total number of people without the disease. When the observation or test is positive in people without the disease, the result is termed false positive. Good observations or tests have a specificity of more than 90% and help “rule in” disease because the test is rarely positive when disease is absent, and there are few false positives.Example: The specificity of serum amylase in patients with possible acute pancreatitis is 70%. In other words, of 100 patients without pancreatitis, 70% will have a normal serum amylase; in 30%, the serum amylase will be falsely elevated.
