4. Apply to no more than 20 colleges/universities which may include up to six (6) CUNY colleges.

5. Enter ALL colleges to which you are applying into the “colleges I’m applying to” section of your Naviance

account under the “colleges” tab. (See Page 3 screenshot)

6. Complete and submit the [GREEN] Regular/Rol Decision Transcript Request & Parent Consent Form for Regular Decision and/or [PINK] Early Action/Early Decision Transcript Request & Parent Consent Form.

7. If you did not attend a New York City public school during the 9th, 10th, or 11th grade, you MUST:

a. Check “I have attended a non-NYC public high school (i.e. private, parochial, out-of-state school, etc.)” on part I of the Transcript Request & Parent Consent Form.

b. If you are applying to CUNY, submit the CUNY Document Return Receipt (see page 8) We will not be able to submit your transcripts to CUNY without the CUNY Document Return Receipt.

8. If required by the individual college, complete any writing supplement or supplemental

application.

I. STUDENT RESPONSIBILITIES

9. Pay the application fee(s) or mail an application fee waiver to each college - colleges will not review your application until payment is received. The College Office is NOT responsible for mailing, and will NOT mail, your payment or fee waiver(s) to colleges/universities. LAST DAY TO REQUEST ANY FEE WAIVERS IS NOVEMBER 3rd. For additional information see Section V – Fee Waivers, Page 6.

10. If any of the colleges you wish to apply to request a teacher recommendation, please follow the guidelines below: a. Find out how many letters of recommendation, if any, are required by each college to which you are

applying b. Ask your teacher in person and at least four weeks before your earliest college application deadline if

he/she is willing to write a recommendation on your behalf. c. Once your teacher has agreed to write a letter of recommendation for you, provide your teacher(s) with

a [YELLOW] Teacher Recommendation Request Form and copies of your (1) Student Questionnaire, (2) Résumé, (3) Personal Statement.

d. In the “colleges I’m applying to” section in your Naviance account, under Teacher Recommendations, select the name(s) of the teacher(s) who have agreed to write a letter of recommendation.

e. Once your teacher(s) has/have agreed to write a letter of recommendation on your behalf, please do not request any additional teacher recommendations.

f. The deadline to request teacher(s) for a letter of recommendation is NOVEMBER 22. After this date you will not have access to this feature on Naviance.

11. Send your SAT, SAT Subject Tests, and/or ACT scores to individual colleges through collegeboard.com or

actstudent.org. Francis Lewis High School’s CEEB code is 331926. The College Office is NOT responsible for sending and will NOT send your SAT/ACT scores to colleges.

12. Check your Naviance account and your e-mail daily for any requests or updates from the College Office or

from any of the colleges to which you have applied.

1. Early Action (EA)/Early Decision (ED) Applications: Bring the following to the College Office on or before Monday, October 16th:

a. Printed copies of your Student Questionnaire, Personal Statement, and Résumé. b. Printed “colleges I’m applying to” page (see below) from Naviance signed by your parent/guardian. c. Completed [PINK] Early Action/Early Decision Transcript Request & Parent Consent Form signed by

you and your parent/guardian.

2. Regular Decision Applications: Bring the following to the Auditorium on or before your Designated Date (see schedule on page 9):

a. Printed copies of your Student Questionnaire, Personal Statement, and Résumé (if you have not already submitted them with your Early Action/Early Decision application materials).

b. Printed “colleges I’m applying to” page from Naviance signed by your parent/guardian. c. Completed [GREEN] Regular/Rolling Decision Transcript Request & Parent Consent Form signed by

you and your parent/guardian. d. Do NOT bring your undergraduate admissions application or any form of payment to the College

Office. The College Office is NOT responsible for sending and will NOT send your applications to the colleges.

II. SUBMITTING COLLEGE APPLICATION MATERIALS TO THE COLLEGE OFFICE

ALL APPLICATION MATERIALS MUST BE SUBMITTED AT THE SAME TIME.

Please note: Your application materials can be submitted ON or BEFORE your DESIGNATED DATE, but no later than Friday, November 17th.

1. CITY UNIVERSITY OF NEW YORK (CUNY) APPLICATION

a. Register for a CUNY Portal account as an “Applicant” and be sure to include your Social Security Number. If you do not have a Social Security Number, CUNY will assign you an ID Number.

b. Once you have created a CUNY portal account, click on “Apply Online”, select Freshman, and choose either General Freshman Admission or Macaulay Honors

c. If applying to Macaulay Honors College (https://macaulay.cuny.edu) please note that the deadline is December 1st at 6 p.m. EST.

d. On “Section 2: Student Information”, make sure to properly enter your 9-Digit OSIS Number – this is how CUNY retrieves your Francis Lewis H.S. Transcript.

e. On “Section 3: College Choices”, make sure to list up to 6 college choices in order of preference. f. On “Section 4: ASAP/SEEK/CD” check yes to applying to SEEK or College Discovery only if you meet the

financial guidelines outlined on page 6 of this bulletin and you meet the academic guidelines specific to each college (see attached CUNY Admissions Profile,).

g. The CUNY application fee is $65. Your application will not be considered for evaluation until you have submitted some form of payment.

h. Selective Programs in CUNY such as the B.A./M.D. program at Brooklyn College, the Sophie Davis School of Biomedical Engineering program, the Bernard and Anne Spitzer School of Architecture and the Grove School of Engineering at City College require a supplemental application and have different deadlines. Visit www.cuny.edu/deadlines for more information.

i. Some CUNY colleges and programs, such as Baruch College and Hunter College, strongly recommend or require a specific supplemental application and/or essay.

2. COMMON APPLICATION

a. Add the colleges of your choice to your “Dashboard” including the SUNY colleges that participate with the Common Application.

b. On the “Education” section, under “Current or Most Recent Secondary School,” please do not enter your guidance counselor’s contact information, instead enter your college advisor’s information as follows:

c. On the “Education” section, under “Grades,” leave Graduating class size blank as it is yet to be determined, class rank reporting is exact, GPA scale is 100, and GPA weighting is weighted.

d. If applying to EOP and/or HEOP, you must meet the financial guidelines outlined on page 6. In addition, you must meet the academic guidelines specific to each college.

e. If applying to a SUNY college via the Common Application, please make sure to properly enter your 9-Digit OSIS Number on your online Common Application – this is how SUNY retrieves your Francis Lewis H.S. Transcript.

f. The application fee may be different for each college that participates with the Common Application.

All SUNY colleges offering 4-year Bachelor degree programs (except for Empire State College and Fashion Institute of Technology [FIT], participate in the Common Application) a. For Empire State College, FIT, and all SUNY Community Colleges apply via www.suny.edu/apply b. Make sure to properly enter your 9-Digit OSIS Number on your online SUNY application – this is how

SUNY retrieves your Francis Lewis H.S. Transcript. c. If applying to EOP, you must meet the financial guidelines outlined on page 6. In addition, you must

meet the academic guidelines specific to each college (see attached SUNY EOP Information Summary) d. The application fee is $50 per SUNY college. e. Make sure to complete the SUNY Supplemental Application if applying via www.suny.edu/applysuny.

4. PRIVATE AND OUT-OF-STATE COLLEGE APPLICATIONS NOT ON THE COMMON APP

a. If the college of your choice does not participate with the Common Application, visit the individual website to apply.

b. The application fee is different for each college. c. If applying to HEOP, you must meet the financial guidelines outlined on page 6. In addition, you must

meet the academic guidelines specific to each college.

1. All students should complete the Free Application for Financial Aid (FAFSA) starting October 1, 2017,

regardless of your parents’ or guardians’ income. For more information on how to apply, visit: www.fafsa.ed.gov.

2. Some very selective colleges require students to complete the CSS Profile to receive non-federal financial aid. Visit https://cssprofile.collegeboard.org/ for more information.

3. All students applying to a college in New York State should also complete the application for the NYS Tuition Assistance Program (TAP). Visit www.hesc.ny.gov for more information.

IV. FINANCIAL AID & SCHOLARSHIPS

4. The Excelsior Scholarship is intended to supplement all current aid programs, including but not limited

to TAP, PELL, AND SUNY/CUNY Tuition Credit programs. For information on how to apply visit: www.HESC.ny.gov

5. All Scholarship application materials (except for Excelsior) must be submitted to the College Office at least 4 weeks before the deadline.

6. Submit the completed [BLUE] Scholarship Transcript Request & Parent Consent Form signed by your parent/guardian to the College Office.

7. Check the “scholarships & money” section under the colleges tab in your Naviance Account for scholarship updates.

1. If you are in need of a fee waiver for SAT, ACT, TOEFL, or college applications bring in your parent/guardian’s Federal Income Tax Return (Form 1040) for 2016 AND a print-out of the email that confirms that you have submitted the lunch form application (www.applyforlunch.com, to the College Office, Room 111. Additional documentation may be required.

2. Fee waivers are granted during Periods 4 through 8 in September, October, and through the first week in November only. LAST DAY TO REQUEST ANY FEE WAIVERS IS NOVEMBER 3rd. No fee waivers will be issued in November during the collection of application materials. NO EXCEPTIONS.

3. Fee waivers are issued as follows: ¾ Up to 2 registrations for SAT Reasoning Tests and up to 2 registrations for SAT Subject Tests ¾ 2 registrations for the ACT ¾ 1 CUNY fee waiver that covers all 6 college choices. CUNY Fee Waivers expire February 1, 2018 ¾ The Common Application allows students to indicate that s/he qualifies for a fee waiver. The

College Office will then confirm this information online via Naviance. There will be no need to mail a paper waiver

¾ Colleges that do not participate with the Common App often accept the CollegeBoard fee waiver, available to students who used SAT fee waivers via their online account at www.collegeboard.org. Students are responsible to print these and mail them to the individual colleges.

¾ All college application fee waivers should be requested at one time

1. Register online:

SAT: www.collegeboard.com/SAT ACT: www.actstudent.org

2. Be sure to enter the Francis Lewis H.S. CEEB code: 331926 when you register for the SAT and/or ACT. It is the student’s responsibility to order (through the above-listed websites) their score reports to be sent to the colleges/universities.

3. When ordering SAT or ACT score reports to be sent to CUNY, use CUNY UAPC (Code 2950) to cover all the CUNY colleges. CUNY UAPC will then send your SAT or ACT scores to the individual CUNY colleges. Do not send your ACT or SAT scores to individuals CUNY colleges as it will be more costly.

4. You are entitled to have four 4 colleges receive your SAT scores at no cost. Select your four choices when you register for the SAT. You have only nine (9) days after taking the SAT’s to take advantage of these four (4) free score reports.

1. For colleges that request mid-year grades, you must submit a Mid-Year Grades Request and ParentConsent Form. Please note that mid-year grades are not available until February 1, 2018.

Adult Career and Continuing Education Services-Vocational Rehabilitation (ACCES-VR) starts with the presumption that all individuals with disabilities

can benefit from vocational rehabilitation services and should have opportunities to work in jobs integrated within their communities. The School of Cooperative Technical Education, or Co Op Tech, offers 17 different certification courses in a variety of careers from culinary

arts to computer networking. The program has a half-day model where students can attend their DOE high school or High School Equivalency (HSE) program for half a day and attend Co Op Tech for the other half of the day. Visit http://www.co-optech.org for more information.

The college application process can sometimes be a stressful and overwhelming time during your high school career. In order to alleviate the anxiety and learn more about this process, the College Office will be holding several topic-specific workshops to assist you during this very important “rite of passage.” All workshops will last for a full period and will be held at the College Office (Room 111). There are limited spots for these workshops, so make sure to register for them in person and at least a week in advance in the College Office. Early Action/Early Decision. This is a mandatory workshop for all students interested in applying to college through early admission programs. Advantages and disadvantages of applying Early Action or Early Decision will be discussed. Friday, Oct. 6 – Periods 3, 4 Tuesday, Oct. 10 – Periods 7, 8, 9, 10 Thursday, Oct. 12 – Periods 5, 6, 8, 9 College Search & Exploration. This interactive workshop will help you put together (or narrow down) your list of colleges/universities that best suits you, as well as learn more about admission possibilities and application strategies. Tuesday, Oct. 17 – Periods 3, 4, 6, 8, 9 Wednesday, Oct. 18 – Periods 5, 7, 8, 9, 10 The College Application Process. This informative workshop will familiarize you with the step-by-step application procedures to ensure that your college applications are completed in a timely and efficient manner. Topics covered include the application timeline, college application components (supplemental applications, teacher recommendations, submitting ACT/SAT scores, etc.) and how to utilize Naviance Family Connection. Thursday, Oct. 19 – Periods 5, 7, 8, 9, 10 Monday, Oct. 23 – Periods 3, 4, 6, 8, 9 Financial Aid & Scholarships. At this important workshop you will learn more about different sources and types of financial aid. Topics to be covered include FAFSA, PELL, TAP, CSS/PROFILE, scholarships, and other terms related to affording college. Tuesday, Oct. 24 – Periods 3, 4, 5, 7, 9 Wednesday, Oct. 25 – Periods 3, 4, 6, 8, 10

Opportunity Programs: SEEK, EOP, HEOP. These programs give access to college to students who meet financial and academic guidelines. At this workshop, you will learn more about educational opportunity programs offered at CUNY (SEEK/College Discovery), SUNY (EOP), and private New York State colleges (HEOP). Thursday, Oct. 26 – Periods 3, 4, 7, 8, 9, 10 The CUNY Macaulay Honors Application. At this informative workshop, you will learn valuable information about the admissions and application processes for this selective program at the City University of New York. Monday, Oct. 30 – Periods 3, 4, 5, 6 Tuesday, Oct. 31 – Periods 7, 8, 9, 10 The Common Application. At this hands-on workshop, you will learn how to use "common app" to apply to multiple schools through this single application. Wednesday, Nov. 1 – Periods 7, 8, 9, 10 Thursday, Nov. 2 – Periods 3, 4, 5, 6 The CUNY Application. This workshop will guide you through the process of applying to any of the eighteen 2-year and 4-year colleges that comprise the City University of New York.
