Saint Mary’s Newsletter October 2011 Issue 5 We are now half way through this financial year and you will see again in this newsletter a tremendous level of achievement across the division. Despite the challenging times, everyone at Saint Mary’s Hospital has continued to embrace the Trust objectives to keep our patients safe and provide them with high quality care whilst also delivering the levels of activity we have not experienced previously. The next couple of months will see the final stages of the Making it Better reconfiguration with the main impact being on the services in Obstetrics and Newborn Intensive Care. A great deal of preparation has taken place to ensure we are ready to welcome both our new women and their babies, but also staff. By November 2011 we will have appointed an additional 55 midwives, 18 nurses, 8 junior medical staff, 3 consultants and 4 administration and clerical staff. In addition we will have 5 more intensive care/high dependency cots, 9 more birth rooms including 2 additional birth pools and 4 fully equipped obstetric HDU rooms supported 24/7 by critical care nurses. The final few weeks will be challenging as we have seen a further increase in our birth numbers but we know the continued professionalism of our staff will ensure we successfully and safely transfer the services from Salford Royal NHS Foundation Trust. The performance within the division has remained strong and compliance with our key performance indicators has meant the division will continue to secure funding for CQUIN measures, eg VTE compliance (£420,000 last year) and for all the additional activity undertaken. This funding underpins the divisional financial position, which in the long term helps us to secure employment for all our staff and to also expand the workforce and facilities to deliver this activity. During the next six months you will see Professor Donnai and I more frequently appearing within your departments. This is an opportunity for us to see first hand the excellent work being undertaken, but more importantly an opportunity for you to raise with us any issues or provide us with any feedback which you feel will improve either staff or patient experience. Neonatal Wall ProjectOn 30th August the Newborn Intensive Care Unit unveiled its new wall project – a series of photographs of some ofthe children who have previously been cared for on the NICU. The artwork aims to give hope and encouragementto other families whose children are still on the unit. Some of the pictures also assist with way finding on the unit.The initial idea came from one of the mums, Sue Emms and the overall design was pulled together by staff inMedical Illustration who also took the photographs.

Geraldine Thompson, Head of the Trust’s Clinical Photography & Medical Illustration Services, said “Thedepartment is proud to have been involved in a project that has been driven by one of the Trust’s ‘Quality’objectives – Patient and staff experience.” The project was especially close to the heart of Sam Goddard fromMedical illustration, whose son Harry is pictured.

The artwork was unveiled to staff, families and everyone involved in the project. Families, including Phil and JulieNeville, who’s daughter Isabella appears in the art work, enjoyed a slice of celebratory cake provided by Mr andMrs Aziz, parents of Alizay, another ex-patient. Alizay’s dad, Raf Aziz, of Old Trafford, said: “We are so privilegedto be able to take part and if my daughter was old enough to speak, she’d say thank you and God bless all thestaff on the unit.”

There has been very positive feedback from both families and staff and the unit has a much more family friendly feel.

Recognition for quality care in Saint Mary’s

July saw the launch of our annual quality week in Saint Mary’s, which not only launched the division’s first AnnualReport, but also culminated with a presentation of the first Divisional Staff Recognition Awards to acknowledgethe hard work that has been carried out over the last 12 months.

It was standing room only when staff came from across the division, not only to receive awards but also toshow support to colleagues who had won. There was also a healthy and friendly rivalry with teams alreadyplanning how they could win next year and wondering what colour or shape award they might be able toadd to their collection. The overriding atmosphere, however, was one of pride and achievement.

Karen Connolly, Divisional Director, said: “We felt strongly that we wanted to do something to recognise theachievements of our staff. We are very aware that staff can sometimes feel theirhard work has gone unnoticed, especially with the financial challenges, servicechanges and reconfigurations that are happening, not just in Saint Mary’s butacross the Trust. We wanted to make sure that that didn’t happen and so weestablished these awards to celebrate an incredibly successful year.”

Karen, along with Professor Dian Donnai, Clinical Head of Division, presentedthe awards to the winning staff members.

After presenting the awards Professor Dian Donnai said:

“This division is very highly regarded and respected both for the care we provide to patients and the way our directorate teams work togetherand are managed. In addition we have a very high research profile which I like to think is the best in the Trust. At our divisional reviews, we in themanagement team bask in reflected glory when we present the outcomesof all your work. In spite of our significantly increased workload, we’vemanaged to maintain very high quality. We really do appreciate all of your workand are really proud of you.”

Quality WeekIn addition to the awards, a poster display in the atrium showed some of thequality achievements of our Clinical Teams in the last 12 months and was wellattended by staff and patients.

Of particular interest were a set of posters showcasing how the Division has usedinformation from PPI activity, Complaints, NHS choices and Patient Informationleaflets to improve patient experience.

Comments were invited from patients and were posted on the PPItree. Staff were encouraged to participate in the “Change one Thing”campaign. Below are some of the patient comments we received:

Enhanced Recovery Programme in GynaecologyThe gynaecology directorate presented an update of their progress on the implementation of theEnhanced Recovery Program (ERP) based on an audit undertaken in July 2011 at the Division of SurgeryAudit day on the 13th October 2011.

The main focus of ERP is to optimise the patient’s health prior to admission and to rehabilitate themto self care and therefore discharge earlier than previously expected.

The presentation highlighted that above 80% of patients are now being admitted on the day ofsurgery (previously approximately 30%) and that this had had no adverse impact on thedirectorate's avoidable theatre cancellation rates.

Significant improvements had been found in rehabilitating patients sooner, in particular regardingsitting patients out of bed on the day of surgery and of mobilising patients on day one postsurgery. In terms of monitoring patient safety, this had had no affect on gynaecology’s positionon falls rates.

A positive reduction in the number of patients being given opiate analgesia post operatively,and a significant improvement in the commencement of oral analgesia on the day of surgerywas also noted. However, the directorate plans to undertake detailed work to review patientsatisfaction with pain management.

Above 80% of patients are now being discharged on the predicted expected date ofdischarge (EDD). This has reduced the total length of stay on the ward by just over one dayand again, in terms of patient safety, there has been no deterioration in the directorate’sre-admission within 30 days rates.

As many of you will know, CMFT are thepreferred bidders to acquire TraffordHealthcare NHS Trust, with theconsultation due to be complete in May 2012.

Moving forward, the gynaecology team atSMH has commenced work with thegynaecology team in Trafford to develop acombined service. The proposal is that

women in Trafford will still be ableto access out-patient, emergencygynaecology and daycase surgery inTrafford, but those requiring morecomplex in-patient treatment willcome to Saint Mary’s. Moreinformation will be available over the coming months.

The implementation of the enhancedrecovery programme has resulted in anoticeable decrease in the demand forbeds, which has supported theharmonisation of wards 62 and 63.

The gynaecology directorate are alsosupporting the plans for Making it Better

in agreeing to share theatre40 with the obstetricdirectorate for emergenciesduring the day andenabling the shared use ofsome of the women’s out-patient facilities.

Our specialist maternity-cardiac team iscelebrating after winning a prestigiousParliamentary award for its work caring for women with complex heart diseasethroughout pregnancy, the leading cause of maternal mortality.

The team won Highly Commended in

the 'Best example of a service forwomen with complex medical needsin pregnancy' category at the All-Party

Parliamentary Group (APPG) Maternity

Services Awards.

The service was set up in 2004 by Dr SarahVause, Consultant Obstetrician, and DrBernard Clarke, Consultant Cardiologist,with the support of a specialist midwife.The number of new referrals to the servicehas doubled since its launch, increasingfrom 43 in 2004 to 86 in 2010 and therehave been no unexpected admissions of ourpatients to Intensive Care or maternal deaths.

Dr Sarah Vause said: "The thing that we are mostproud of is knowing that there are several womenwith severe heart disease who now have children,who would not have embarked on, or survived the pregnancy had it not been for the service we provide.”

"There are many more women where our inputresulted in better outcomes for the woman or thebaby, either by prolonging the gestation or byensuring that the woman was delivered at SaintMary's where all the facilities and professionals arein one place and have expertise in managing womenwith very high risk pregnancies.”

"We were absolutely thrilled when our team wonthis award. In Manchester we are fortunate enoughto have the people and the facilities to provide carefor this high risk group of women and their babies.The award recognises the work of all the membersof the team."

Their particular strength, and something which isunique to the North West, if not nationally, is thelocation of a wide range of disciplines on one site. In most other tertiary centres, the maternity unit is situated in a different building and often on adifferent site from the cardiac centre. We arefortunate to have, on one site and in one building,

a maternity unit with high dependency facilities,Newborn Intensive Care Unit, and the ManchesterHeart Centre with cardiologists, cardiac surgeons,anaesthetists and technicians as well ashaematologists and radiologists. This enables them to care for the sickest women and their babies in life threatening situations at any time of the day or night.

This was further strengthened when the team moved the clinic into an antenatal setting while still ensuring that women were able to see severaldifferent specialities in one place during oneappointment. Women felt this had changed thefocus from their cardiac problem to a more positivefocus on their pregnancy, birth and preparation forparenthood.

The high dependency facilities on the maternity unit have also been improved and expanded whichenables some women who require critical carearound the time of delivery to be looked after onthe unit instead of being transferred to the IntensiveCare Unit. This prevents mums being separated fromtheir newborn and allows for immediate bondingand breastfeeding.

Dr Vause, Dr Clarke and Lesley Bolger, the ECGtechnician for the service, went down to Parliamentin July to collect the award on behalf of the whole team.

Patient Safety First Week(12th – 16th September 2011)The Trust-wide focus for Patient Safety First weekthis year was Record Keeping/Documentation.

There was a 3 week run up to the start of PatientSafety Week focusing on:

• Medical Documentation (w/c 22nd August)

• Quality of Nursing documentation andfiling (w/c 29th August)

• Condition of records, filing andorganisation (w/c 5th September).

The week culminated with a Trust-wide day on 16th

September which comprised presentations from allDivisions recognising problems with documentation.

For Saint Mary’s, Shirley Rowbotham presented aHigh Level Investigation completed in Gynaecologywhich recognised the need to improvedocumentation of the Surgical Safety Checklist asone of the immediate actions. Considerable progresshas been undertaken within Gynaecology, led by DrFiona Reid and Louise Samworth.

There were also a number of other activities across theDivision aimed at raisingawareness and making someheadway into raising ourstandards:

Patient Safety First packs were distributed to all areas comprising Divisional and local Incidentinformation, journal articles/posters and a timetable for the presentation day.

Workshops were held for staff in which theywere asked to find information in patientrecords to respond to a complaint, comparing a good set of notes to a poor one and therebyraising awareness of the importance of goodrecord keeping.

A collective record of GMC numbers waslaminated and made available for reference in wardand out-patient areas for when medical staff arecompleting entries in patient notes.

An audit of the tracking system wasundertaken in Genetics to see if records have theirlocation tracked and if they are where they aretracked to (ie filing, awaiting clinic appointment,clinics, individual members of Medical, Counsellingand Clerical staff).

Patient Safety Walk Rounds wereundertaken by the Senior Management teamand Governance Leads and asked generalquestions of staff about patient safety.

Spot check audit of documentation forcurrent in-patient episodes within Obstetrics,Gynaecology & NICU. Results were fed backto the teams on the day for action.

NICU had drop in sessionswhereby various standards relating todocumentation were displayed. Therewere some interactive sessions wherepeople could look at good and poorstandards of documentation.

NICU erected a staff suggestion/mood board where staff couldexpress their thoughts/feelings aboutthings we do well and things whichwe don’t. There were also some oneto one session held regardinggeneral aspects of patient safety.

Obstetrics focused on legibility,correct mode of corrections,correct ‘headers’ on continuationsheets, completion of both pageson transfer sheets and whatshould be contained in CTGenvelopes.

European Human Genetics award forProfessor DonnaiProfessor Dian Donnai was awarded the 2011European Society of Human Genetics (ESHG)Education Award at its annual conference inAmsterdam in June. This is one of only two annual awards made by the Society.

The award is in recognition of Professor Donnai’s:

• Longstanding significant contribution to the clinical genetic sciences,

• Education of students and clinical geneticists worldwide,

• The creation of the Manchester DysmorphologyConferences, which have contributedsignificantly to strengthening the collaboration between dysmorphologists inEurope and beyond,

• Her wonderful dysmorphology workshops atthe ESHG meetings, and

• Her excellent clinical genetic books.

CPA surveillance visit to AndrologyThe Andrology Laboratory is part of the Department of Reproductive Medicine and provides a range ofdiagnostic and treatment services that includes semen analysis, vasectomy checks, anti-sperm antibody testing,freezing and storage of sperm, donor sperm banking and the quality assurance scheme for semen concentration,motility and morphology.

On 20th April 2011, Andrology had its Clinical Pathology Accreditation (CPA) surveillance visit. The laboratory wasassessed against eight standards (A-H):

The inspector reported that he was very impressed with the Andrology Department and was satisfied that thelaboratory was operating competently and providing a safe and secure environment for its staff. He commentedthat the department is staffed by an experienced and well trained group of people who are committed to themaintenance of a high quality service and continual improvement. He confirmed that the quality managementsystem in Andrology was well established and managed. The user survey also indicated that users were verysatisfied with the service provided by Andrology.

Mums from across Greater Manchester showed theirsupport for National Breastfeeding Awareness Week2011 at the end of June by gathering in a breastfeeding ‘flash mob’ at The Trafford Centre. Customersgathering in the Centre’s café bars witnessed thetakeover as the group of mums with their babies tookpart in a mass feeding event designed to make otherfamilies aware of the health benefits for both mumand baby.

Health experts say that breastfeeding helps to form abond between mums and their newborn babies andalso helps children to avoid obesity in later life. It isalso seen to limit the likelihood of common healthcomplaints in babies such as diarrhoea, ear and chestinfections, and eczema.

Lynsey Hicks, who attends the Baby Café run by SaintMary’s, took part in the event at The Trafford Centrewith her daughter Freya. She said: “I felt it wasimportant to take part to promote a positive attitudetoward breastfeeding both for new mums and thepublic in general.”

“I had no idea of the negative attitude some peoplehave to breastfeeding in public until I experienced itfirst hand. I wanted to show new mums that it’snothing to be embarrassed about and that they shouldbe able to feed their babies whenever and whereverthey like.”

“I always intended to breastfeed my daughter andnever considered anything else until she was diagnosedwith gastric reflux at eight weeks. She was strugglingto put on weight and I started to think about otherfeeding options to try and bulk her up. But midwivesat Saint Mary's reassured me that my breast milk wasmore than enough for Freya and really encouraged me

to stick at it. They also suggested that I attend theBaby Café to get support from other mums. I’m so gladI did – Freya is doing much better now and I’m glad Igot the support I needed to carry on.”

The Trafford Centre supported the event and hasrecently unveiled newly refurbished breastfeedingfacilities. Gordon McKinnon, Director of Operations atThe Trafford Centre, said: “We’re a family friendlycentre, which means we’re fully supportive of mumswho make the decision to breastfeed. If they require acomfortable, private space in which to feed their childwe have four sets of dedicated facilities including ournew Laura Ashley designed breastfeeding suite, but weconsider most public spaces appropriate and ourretailers are equally understanding.”

Rachel Varney, Infant Feeding Co-ordinator at SaintMary’s, said: “Breastfeeding Awareness Week is veryimportant to us as it’s a time when we can get out and about in the community to promote and celebratebreastfeeding. This year we marked the event bylaunching a new Breastfeeding Drop-in at LevenshulmeChildren’s Centre to provide breastfeeding informationand support to women in this part of the city”

The Baby Café in Moss Side, which is now in its 7thsuccessful year helping mothers with breastfeedingissues, also celebrated Breastfeeding Awareness Weekby holding a tea-party and inviting all our mums, pastand present, to attend.

Closer to home, there was an information stand in Saint Mary’s Atrium containing leaflets, posters, pens, DVDs, fridge magnets and shopping bags – allpromoting breastfeeding, with staff on hand to talk to mothers and members of the public.

Viewers will be aware of the recent Coronation Street storyline involving Carla’s character. What you may notknow is that our very own St Mary’s Centre SARC team were advisors on the storyline and were present when the scenes were filmed, with filming taking place in the hospital.

Coronation Street also did some publicity around this storyline and the centre was featured in the publicity,including Manchester Evening News, Real Radio and BBC Radio Manchester. Andrew Lancel, the actor who plays Frank Foster, appeared on Daybreak talking about the storyline and interviews with our team were also broadcast.

Coronation Street – St Mary’s SexualAssault Referral Centre (SARC)

SARC have now moved totheir new location, whichis on the second floor ofthe old Saint Mary’sbuilding (previouslyoccupied by the WarrellUnit in the Gynae block).This now providesadditional facilities forforensic examination withboth adult and children’sSARC being located inone unit. Whilst SARChave moved on a numberof occasions over theyears we now envisagethat this, in their 25th year,will be their home formany years to come.

K2 Guardian goes live at Saint Mary’sOver the past 12 months work has been in progresstowards the implementation of the K2 Guardian™system on the Birth Centre at Saint Mary’s Hospital.

Guardian™ is a fetal monitoring system that is able to provide the full electronic capture of patientinformation during childbirth via a touchscreen thatremains at the woman’s bedside. It relays informationrelating to CTGs, partograms, all labour events,outcome information, fetal blood sampling results and cord blood gas results direct from the blood gasanalyser. The data is shown in real-time and can beviewed by the bedside, externally at the central stationor remotely by senior clinicians via the internet, day ornight. This added accessibility enhances communicationbetween patients and clinicians and encouragesdiscussion and shared experience, essential for support,promoting best practice and patient safety.

Purposefully developed to withstand the rigours ofmedical litigation, and able to produce detailed reportsinstantaneously, Guardian™ is an invaluable tool forassisting in clinical governance and for meeting theneeds of its users at the point of care. Improved patientsafety benefits have been shown resulting from thecontinuous monitoring, real-time accessibility andcollation of extensive detailed information, as requiredby the Care Quality Commission.

To support the implementation and training, three members of the K2 team spent two weeks with us on Birth Centre delivering the training and implementation.

The system went live on 16th August and credit goes toall the staff who worked on that date as it was one ofthe busiest days and nights we have experienced todate in maternity. One of the K2 team members evenworked the night shift to ensure support and training.Staff blood sugar levels were maintained withcupcakes, kindly donated by K2.

The Band 7 co-ordinators are our K2 Champions – they have and continue to play a pivotal role in theimplementation of the system.

Fiona Bryant, Matron for the Birth Centre, said: “I amvery proud of the way all the staff and the Band 7 co-ordinators have embraced the new system and workedso hard with its implementation. We did “Go Live” onone of our busiest days. However, it was clear from dayone that the staff liked the new technology and werekeen to make the project work.”

The next step is to enable the K2 system to talk to PAS and CMiS, the other maternity information system used to capture antenatal, delivery details and postnatal information. Once this is achieved theteam can identify a date for ‘paper light GO LIVE’ ie,using only the electronic partogram, CTG and vaginalexaminations.

The K2 system is also facilitating the INFANT study (Intelligence Fetal AssessmeNT

Monitoring) which is a multi-centrerandomised controlled trial. The aim of thisstudy is to determine if the addition of an‘intelligent’ (decision support) software,

giving real time analysis of cardiotocographsto midwives and obstetricians, reduces the

incidence of intrapartum neonatal mortalityand morbidity.

During March and April the senior midwivesfrom Saint Mary’s arranged to meet themidwifery and support staff at Salford Royal on a one to one basis. The aim of this was to introduce ourselves from Saint Mary’s, discuss with the SRFT team their personal and professional aspirations, all of which areaimed to help ensure for a smooth transfer of employment in November.

At the same time staff from SRFT were invitedto Saint Mary’s to see the unit, meet the staffand also follow up with a Question and Answersession.

The invitation was widely taken up by SRFTstaff who organised a bus for their staff sothere were no worries about car parking orhaving to find us.

Saint Mary’s Hospital have now completedthree unit visits that saw Salford midwives and

healthcare support workers visiting the hospital over the past month. The visits consisted of a tour of the unit,including the wards, Birth Centre and clinics and fittings for their new uniforms prior to their transfer later inthe year.

During the tour around the unit the midwives and health care support workers were very complimentaryabout our new unit here at Saint Mary’s. It was clear from these visits that having new people join our team atSaint Mary’s will bring opportunities for us to share ideas and practices in order to ensure we continue givehigh quality care for our mothers, their babies and families.

The new staff also took part in a Question and Answer session over tea and biscuits, which proved verysuccessful and gave them the opportunity to ask questions on a range of topics from shift patterns to thespecialist clinics we provide.

Eleanor Stanley, who arranged the visits said: "Itwas great to show the midwives and health caresupport workers around Saint Mary's and ourcurrent staff enjoyed meeting their newcolleagues. We look forward to working withthem and joining our team at Saint Mary's inNovember."

Senior midwives Donna Wilson, MarySymington, Louise Shaw and HeatherRamsbottom from SRFT who visited theunit with their colleagues had thefollowing to say: “All staff who visitedSaint Mary’s Hospital on the tours feltextremely welcome, in awe of thesize of the building but feltreassured about working as part of the multidisciplinary team atSaint Mary’s with the aim ofcontinuing to promote a highquality service”.

The charity team are helping to raise money to support excellence intreatment, care and research across the hospital and are on hand to helpstaff with any fundraising queries that they or patients may have. The teamcan offer support and advice on making the most of your fundraising insupport of the hospital generally or more specifically your ward ordepartment.

Recent fundraising includes that of Jo Hargreaves, who decided to celebratedaughter Frieda’s first birthday by organising a ‘Bring and Buy’ sale in support of the Newborn Intensive CareUnit. The sale at Chorlton Irish Club was a huge success with around 300 people attending throughout the dayhelping to raise over £4,500.

There are many ways that you can get involved and help to raise money for Saint Mary’s. For example, if youlike challenges then why not get involved in running events such as the Manchester Marathon and the GreatManchester Run, which will take place in 2012. You an even get involved in an overseas adventure such as theVietnam Hill Tribe Trek.

If sports aren’t your thing why not take a collection tin for your department or why not become a friend of the charity and help us to promote upcoming events and activities that will benefit your patients.

You can even support the charity by telling people about us and letting us know if someone you know isfundraising on our behalf. We are always looking for people to share their fundraising and patient stories with us to help us to talk about the wonderful work at the hospital.

Fundraising at Saint Mary’s

Genetic Medicine are developing a business case for new equipment and staff to enable them toprovide an array of diagnostic tests, particularly to stratify patients for new and effective cancertreatments. This will also position the directorate to potentially be one of five national HumanGenomic Centres.

As Saint Mary’s now has its own charitable fund status, if anyone would like to fundraise the monieswill be used for things specifically for Saint Mary’s, for example to purchase the equipment for geneticsor the finishing touches for midwifery led birth unit.
