Work progress during 2017-2018

In September we launched our second three year rolling business for the Public Health Directorate. This updated Plan includes priorities carried over at the end of 2018 and new projects for delivery over the next three years 2018-2021. This three year rolling Plan will continue to be reviewed and updated annually to capture the changes as projects are completed, and as new items are introduced. Although much progress has been made, we know that there is still much to be done to contribute to the Government’s goal that ‘we live longer, healthier lives’ and the Department of Health and Social Care’s (DHSC) strategic goal that ‘we will help everyone take greater responsibility for their own health, encouraging good lifestyle choices.’ This Quarterly Report covers the period from April 2017 to end September 2018 shows the progress of the work completed to date. The 2017-2020 Public Health Business Plan identified 76 projects for delivery over the next three years. As at end of Quarter 2

33/17 Launch a new Supervised Toothbrushing Programme: island- wide aimed at young children within Nursery Settings.

It has been confirmed that the supervised toothbrushing programme will be named ‘Smile of Mann’, and the Smile of Mann branding has all been finalised and approved. Every nursery / pre-school on the Island was invited to a forum which gave an opportunity to learn more about Smile of Mann, including personal feedback from staff that took part in the pilot programme. Following the event, all nurseries / pre-schools were provided with an enrolment form to confirm whether or not they would like to participate. 18 confirmed they would.

24 nurseries/pre-schools participating in Smile of Mann. Staff members from each participating establishments have received mandatory training on how to implement effective practice and correct methods of infection control.

Smile of Mann launched on 17th September, and remains open for other nurseries/pre-schools.

Undertake an Oral Health Needs Assessment: which will help to steer the development of a new Oral Health Strategy.

Planning for the needs assessment has commenced, and the first stakeholder meeting will take place in early October. Stakeholder engagement events are being planned and will take place in November/December 2018. The methods used to engage stakeholders will be focus groups and 1:1 interviews.

Q3 A number of stakeholder focus groups have taken place, with others lined up to take place over January / February. Secondary research on areas such as general anaesthetic, fluoride varnish etc. is being carried out alongside this work.

Sexual Health

36/17 Develop a needs assessment: Assess the role of substance misuse in sexual health and domestic abuse.

A Project Steering Group has met to scope out an overview for the needs assessment along with a draft Project Charter and Terms of Reference for both the Project Steering Group and the Stakeholder Group. The Stakeholder Group is being convened to finalise the scope of the needs assessment, the Project Charter and the Terms of Reference. Whilst these groups and the reporting and governance framework is being set-up, the Project Officer has undertaken a wide range of background reading to develop a clear understanding of the subject.

This needs assessment is well underway with the Project Steering Group and the Stakeholder Group set up and the reporting and governance framework in place. Subgroups from the Stakeholder Group have been convened and data and information is currently being gathered. Stakeholder Group meetings have been scheduled for the duration of the needs assessment with final amendments to the Project Charter and the Terms of Reference for both groups as well as plans created which include key tasks for the subgroups.

The Department of Home Affairs has drafted a high level strategy for domestic abuse which includes developing a Domestic Abuse Strategy. Therefore the scope of this needs assessment has been extended to include domestic abuse more broadly to provide a greater level of detail. Changes have been made to the governance framework, Project Charter and Terms of Reference to reflect the wider remit.

The majority of data and information has been gathered with the support of the subgroups and the report is being drafted in preparation for feedback from the stakeholders.

82/18 Commission an integrated sexual health service: Work jointly with hospitals’ and community.

Q1 The service specification for the integrated sexual health service is being finalised whilst who will be lead commissioner for the service is being reconfirmed by the new Executive Leadership Team.

An updated Public Health England ISHS specification has been published in August 2018 which varies considerably from the previous specification that we had based our initial service specification on. Therefore further comments are being received to update and localise this latest specification.

A draft service specification has been updated and circulated for comments. Benchmark costings for an ISHS on island are also being developed in preparation for the next meeting. Further updates will be discussed as well as the next steps: seeking expressions of interest from potential providers and presenting the options to the Executive Leadership Team for a decision on the future ISHS.

83/18 Commission a sexual assault referral service: For adults and children. Linked to a specialist provider in the UK (jointly with Isle of Man Constabulary)

Outline specification for a sexual assault referral centre for adults and children is being developed based on the service in Jersey and the NHS England service specification. A design and delivery project will commence in Q4. The Director of Public Health attended the Jersey SARC along with a key member of the DHA. Feedback from this visit along with findings from the SARS needs assessment were presented to key members of the Department of Health and Social Care, DHA and the Department of Education, Sport and Culture. It has been agreed that the DHA will create a project charter and project management will be provided by Public Health. Once the project charter is drafted, a steering group will be convened to confirm this and suggest members for an implementation group.

It was agreed that Local Area Coordination will be part of the Integrated Care Pilot Project longer term plans. A brief overview of Local Area Co-ordination was written by Public Health, and included in the Integrated Care Pilot Project business case. ABCD work is progressing by QUING (independent of Government).

As mentioned in the Sexual Health section, the Department of Home Affairs has drafted a high level strategy for domestic abuse, which includes developing a Domestic Abuse Strategy. Therefore the scope of the role of substance misuse in sexual health and domestic violence and abuse needs assessment has been extended to include domestic abuse more broadly to provide a greater level of detail. Due to this extended scope, there is no longer a need to undertake a Joint Strategic Needs Assessment as planned previously. Thus this project is now closed.
