1 Introduction

Welcome to the OSVDPA Assessment Guide, we greatly appreciate your participation in the most important component of our certification scheme—assessing the competency of DPOs.

The OSVDPA has attempted to create assessments that are relevant to the many different types of DP operations conducted by various vessel types, while at the same time, providing practical ways for instructors and mariners to conduct these assessments. But customization and multiple options can cause confusion. For this reason, we have included this guide providing step-by-step instructions for Certified Instructors and Qualified on Board Assessors (QOBA) to use as they administer and score Phase 3, Phase 4, Phase 5, and Revalidation Assessments. In this document, users will find the following information:

1. A summary of the assessment system,2. Step-by-step instructions to administering an assessment,3. A glossary of assessment-related terms,

4. Instructions for how to use each assessment document, and5. Information on the OSVDPA assessment policies and procedures.

Do not feel the need to read this guide straight through. It is structured to provide the level of information you need. For example, if you are familiar with the OSVDPA assessment system and just need a refresher or “go-by” please use Section 3, the Step-by-Step guide. Alternatively, if you need more detailed information about the assessment steps or an assessment document, utilize Section 4 or Section 6, respectively. 2 Summary

This section provides a summary of the OSVDPA assessments, answering the “who,” “what,” “where,” “when,” and “why” of OSVDPA assessments. More information on each of these is found in the rest of this assessment guide. In general, at the end of Phase 3 (Simulator), Phase 4 (Watchkeeping), and Phase 5 (Upgrading), the OSVDPA requires DPOs and Prospective DPOs to pass a practical assessment showing they can conduct a DP operation. Every five years DPOs must again pass an assessment. Each of these assessments has the following characteristics:

Who: Assessments are conducted by either Certified Instructors or Qualified on Board Assessors (QOBAs), these positions are summarized below:

• Certified Instructors are individuals that have been certified by the OSVDPA as capable of training and assessing DPOs. In addition to usually having servedas a DPO, these individuals have passed the OSVDPA assessment and had their ability to conduct the assessment graded by an OSVDPA Auditor.

• QOBAs are Certified DPOs that have recorded 150 days of Sea Time and 450 hours of Practical Experience, and passed a flag-stated approved onboardassessing course. They are allowed to conduct assessments on DP vessels. If a DPO or Prospective DPO works for an “Enrolled Vessel Operator,” theQOBA can be someone from the same employer as the DPO or Prospective DPO. If the DPO or Prospective DPO does not work for an “Enrolled VesselOperator,” the QOBA must be from a third party.

What: OSVDPA assessments attempt to measure a DPO’s mastery of the knowledge and skill the OSVDPA believes every DPO must have.

• Assessments are composed of “Assessment Items.” Like questions on a test, these are the points of measurement that determine the assessment score.• Each assessment is composed of 45 assessment items. 15 of these items are Tier 1 Items and the remaining 30 items are Tier 2 Items.

o Tier 1 Items are those that if done incorrectly, can cause the vessel to lose heading or position. The Assessee must correctly complete all 15 Tier 1Items in order to pass the assessment.

o Tier 2 Items are items relating to DP system set up, monitoring, control utilization, or other related items. The Assessee must correctly complete 80percent of these items to pass the assessment.

• All assessments do not utilize the same Assessment Items. Items are selected to customize the assessment to the DPO or Prospective DPO.• To pick which Assessment Items are included, the mariner or instructor conducting the assessment can choose to use a pre-printed assessment or make their

own using a current OSVDPA standing order.o Pre-printed assessments are call “scenarios.” Each scenario provides a narrative of a DP operation, what happens during the operation, the 45

Assessment Items to be included, and a recommended order the items should be recorded in.o Instructors and mariners can also create their own assessments from a list called a “Standing Order” published by the OSVDPA.

Where: OSVDPA assessments can be conducted on simulators or DP-equipped vessels. Each is summarized below:

• Simulators: The Phase 4, Phase 5, and Revalidation assessments can be conducted on OSVDPA-approved simulators at OSVDPA-accredited TrainingProviders. The Phase 3 Assessment must be conducted at the Training Center where the Phase 3 (Simulator) Course was taken.

• Vessels: Assessments can be conducted on any classed DP vessel or unclassed vessels that have been approved by the OSVDPA (called “Accepted Unclassed Vessels” or “AUVs”). AUVs have passed an audit performed by an individual approved by the OSVDPA for that purpose.

When: • The OSVDPA requires Prospective DPOs to pass practical assessments after Phase 3 (Simulator) Course and after Phase 4 (Watchkeeping).• DPOs that want to upgrade from a Class B DPO Certificate to a Class A DPO Certificate must pass an assessment after completing Phase 5 (Upgrading).• All DPOs must pass an assessment every five (5) years after completing the revalidation requirements (150 days of Sea Time and 450 hours of Practical

Experience).• If these assessments are not passed, the DPO or Prospective DPO is provided two more opportunities to pass this assessment. No more than two attempts

can be conducted in the same day, and all three attempts must be completed within 120 days.

Why: The OSVDPA believes that DPO certification is a product of taking courses, gaining experience, and then proving you have gained knowledge and ability by passing assessments. Additionally, the OSVDPA believes that training is most meaningful when it is relevant to what the Prospective DPO does on a daily basis. Thus, we have made our assessments customizable. Finally, the OSVDPA believes DPOs must prove they have kept their skills current, as such, we require revalidation assessments every five (5) years.

3 Step-by-Step Instructions

This section provides short step-by-step instructions for how to conduct OSVDPA assessments. More detailed information is found for each step in Section 4 and additional information about the documents mentioned below is found in other sections of this guide.

Step-by-step instructions are continued on the following page.

Step 1: Confirm the DPO/Prospective DPO is eligible to take the assessment. Ensure the DPO or Prospective DPO has completed the applicable requirements for that phase. Assessments conducted prior to the DPO/Prospective DPO completing the requirements shall be rejected by the OSVDPA. (See Section 7 for more information on the specific requirements of each phase).

Step 3 (Standing Order Users): Choose an acceptable standing order version.

Check your email or the OSVDPA website to ensure the standing order version number (the last three digits in the document code) is currently accepted by the OSVDPA. (See Section 4 for more information about standing orders and when standing orders are updated).

Step 3 (Scenario Users): Choose an appropriate scenario. Along with the DPO/Prospective DPO being assessed, review "Assessee Copy" of the "live" scenarios on the OSVDPA website and select one that is appropriate for the level of the assessment being conducted, the vessel the DPO/Prospective DPO serves on, and the vessel's industrial mission. (See Section 4 for more information about scenario selection).

Step 2: Choose how the assessment will be conducted. Determine if the assessment shall use a pre-printed scenario or a standing order as the basis of the assessment. (See Section 4 for more information about what factors should be considered when making this decision and the differences between scenarios and standing orders).

Step 5 (Scenario Users): Preparation. Complete Section 5 of the assessment report (or otherwise make this information available to the Assessee) and provide the Assessee the “Assessee Copy” from the scenario. For simulator-based assessments, it is recommended that some of this information be provided before the Assessee arrives at the training center (see Section 4 for more information on this point). Also at this time, the Examiner/QOBA can conduct the simulator familiarization time and operation planning time (See Section 4 for more information about when simulator familiarization is required and operation planning time).

Step 4: Ensure there is enough time to complete the assessment. Regardless of the method used to administer the assessment, there are restrictions on how long an assessment can take. Thus, the Examiner/QOBA shall review the Assessment Items listed in the scenario or standing order to ensure the Assessee has sufficient time to complete the assessment.

Step 5 (Standing Order Users): Item selection. Go through each section of the standing order and select the required number of Assessment Items. In completing this step, the Examiner/QOBA shall take care to select the Assessment Item(s) from each section that are most appropriate to the assessment being conducted, the types of vessel(s) the Assessee serves on, and the industrial mission(s) of the vessel(s) the Assessee serves on. As part of this process, the Examiner/QOBA shall ensure they have selected 15 Tier 1 Items and 30 Tier 2 Items. (See Section 4 for more information regarding this process).

Step 6 (Scenario Users): Signatures. Once all preparation and familiarization has been completed, the Examiner/QOBA and the Assessee shall sign Section 1 of the assessment report, thereby acknowledging they are beginning the assessment. After this point, no instruction or assistance shall be provided to the Assessee (except for when the Assessee requests a “time out” on a “cold simulator.” (See Section 4 below for more information about time outs and cold simulators).

Step 7 (Scenario Users): Conduct the assessment. Once Section 1 has been signed, the Examiner/QOBA shall begin the assessment. Providing the suggested instructions, directions, and prompts listed in the text of the scenario. As the Examiner/QOBA comes to each Assessment Item, the Assessor should follow the instructions listed therein and use the associated scoring criteria to record the appropriate score. The OSVDPA recommends the Examiner/QOBA utilize the scenario sheets to record the score, however, the Assessor can use other resources to record the score if they so choose. (For more information on how the scenario sheets are used see Section 6).

Step 9 (Standing Order Users): Conduct the assessment. Once Section 1 has been signed, the Examiner/QOBA shall begin the assessment, providing the directions and prompts listed in the Assessment Items and using the associated scoring criteria to record the appropriate score. The OSVDPA recommends the Examiner/QOBA utilize the standing order sheets to record the score, however, the Assessor can use other resources to record the score if they so choose. (For more information on how the standing order sheets are used see Section 6).

Step 8 (Scenario Users) / Step 10 (Standing Order Users): Record the Score. Once the assessment is complete, the Assessor shall record the assessment score by completing Section 3, Section 4, and Section 6 of the assessment report. Once this is completed, the assessment report shall be returned to the assessee.

Step 7 (Standing Order Users): Preparation. Complete Section 5 of the assessment report (or otherwise make this information available to the Assessee) and allow the Assessee to read the exercise description. At this time, the Examiner/QOBA can allow the Assessee to see the “Listed” Items (see Definitions Section) and can also conduct the simulator familiarization time and operation planning time (See Section 4 for more information about when simulator familiarization is required and operation planning time).

Step 8 (Standing Order Users): Signatures. Once all preparation and familiarization has been completed, the Examiner/QOBA and the Assessee shall sign Section 1 of the assessment report, thereby acknowledging they are beginning the assessment. After this point, no instruction or assistance shall be provided to the Assessee (except for when the Assessee requests a “time out” on a “cold simulator.” (See Section 4 below for more information about time outs and cold simulators).

Step 6 (Standing Order Users): Ordering of items and exercise selection. Once the appropriate 45 Items are selected, the Assessor should select an exercise similar to DP operations the Assessee conducts and reorder the selected items into an order that fits this operation. This reordering should be completed on paper or other medium (electronic tablets, etc.) that the Assessee does not have access to.

Step 9 (Scenario Users) / Step 11 (Standing Order Users): Debrief. Once the assessment report has been completed, the Examiner/QOBA shall allow the Assessee to see the complete scenario or standing order sheets for the purposes of learning what they did correctly and incorrectly during the assessment. However, the Examiner/QOBA shall not allow the assessee to keep or copy the scenario or standing order sheets and once the review is complete, the sheets shall be destroyed.

4 Detailed Instructions.

Step 1: Confirm the DPO/Prospective DPO is eligible to take the assessment. Ensure the DPO or Prospective DPO is eligible to take the assessment.

The Examiner/QOBA should ensure that the Assessee has met the applicable requirements. If an assessment is provided to a DPO/Prospective DPO ineligible to take that assessment, the OSVDPA may require the individual to retake the assessment. Additionally, the OSVDPA will investigate Examiners or QOBAs found to be repeatedly conducting assessments to those not qualified to take the assessments. Information on when DPOs or Prospective DPOs are eligible to take an assessment is included in Section 7 of this document, both OSVDPA logbooks, the OSVDPA MPP-1-CV, and the OSVDPA website.

Step 2: Choose how the assessment will be conducted. Choose if the assessment will be conducted utilizing a pre-printed scenario or via a standing order.

As previously stated, OSVDPA assessments are designed to be practical to implement and relevant to what the DPO or Prospective DPO being assessed does on a daily basis. As such, there are multiple ways that OSVDPA assessments can be conducted. The biggest difference in how the content of an assessment is selected is if the assessment is conducted using a scenario or a standing order. Both of these methods are summarized below.

• Scenario-based assessments. Scenarios are pre-printed assessments designed by the OSVDPA. Each scenario contains a narrative of what DP operation isbeing undertaken, what happens during the operation, and what Assessment Items are recorded by the Examiner/QOBA. Scenarios can be used on simulator-based assessments or vessel-based assessments that occur during a single watch. More information about use of scenarios is found in Section 6.

• Standing orders. The OSVDPA understands that despite our best efforts we cannot design assessment scenarios to replicate every vessel’s DP setup orevery DP operation. As such, we also allow Examiners/QOBAs to design their own assessment via the use of standing orders. Standing orders provide amenu of Assessment Items for Examiners/QOBAs to pick from, thereby allowing a customized assessment to be created while at the same time ensuringthat all assessments are of comparable difficulty and content. Standing orders can be used for simulator-based assessments and vessel-based assessmentsconducted during a single watch or those conducted over longer periods. More information on standing orders is found in Section 6.

Step 3 (Scenario Users): Choose an appropriate scenario.

Once the Examiner/QOBA has decided that they will utilize a scenario for the assessment, they must select which scenario to utilize. To make this decision, the Examiner/QOBA and the DPO or Prospective DPO being assessed shall review the “Assessee Copy” or the front page of the assessment sheet for each scenario that is currently “live” on the OSVDPA website and pick the scenario that is the best fit for the DPO or Prospective DPO being assessed. Factors that should be considered during this decision are the current level of the DPO or Prospective DPO (i.e. some scenarios are tailored to DP-1 vessels and thus not a good fit for Phase 5 Assessments or Class A DPO Revalidation) and the DP operations they do on a daily basis. It is also important that the Examiner/QOBA select a “live” or currently accepted scenario. The OSVDPA routinely changes which scenarios are live to ensure that the scenarios are fresh and true measures that the assessee is proficient at DP operations (not memorizing what items are part of the test).

Step 3 (Standing Order Users): Choose an acceptable standing order version.

Once the Examiner/QOBA has decided to use a standing order, they must ensure that they use an acceptable standing order version. Every standing order published by the OSVDPA has a version number, this number is the last three digits of the document code (the version number is highlighted in the following example OSVDPA AS-2-002, the Assessment Standing Order (Version 2)). Specifically, the OSVDPA accepts assessments which utilize the current version of OSVDPA AS-2-CV or one of the two previous versions. Every time the standing order is updated, it is sent via email to every Examiner, QOBA and the Point of Contact for every Enrolled Vessel Operator and Accredited Training Provider.

Step 4: Ensure there is enough time to complete the assessment. The OSVDPA provides considerable leeway in how assessments are taken, however once an assessment is started, it must be completed or the Assessee shall be considered as having failed the assessment. If you are conducting an assessment, you should ensure you have the following amounts of time available:

• If conducting on a simulator: For simulator-based assessments, the OSVDPA allows up to one (1) hour of instruction and familiarization on the use of the simulator. After this time, the OSVDPA recommends no more than three (3) hours be allowed for any simulator-based assessment (for a total recommended time of four (4) hours per assessment).

• If conducting on a vessel: The OSVDPA prefers assessments be completed straight through, and thereby recommends the QOBA allot no less than four (4) hours. However, the OSVDPA understands this is not always possible. Thus, the OSVDPA allows 14 consecutive days for the assessment to be completed, provided the entire assessment is conducted on a single vessel.

Step 5 (Scenario Users): Preparation. Once the Examiner/QOBA has decided what scenario they are going to utilize, the Examiner/QOBA should provide to the DPO or Prospective DPO the information and time they need to prepare themselves for the assessment.

For simulator-based assessments, the OSVDPA requires that DPO or Prospective DPOs be offered instruction on how the simulators are used and up to one (1) hour to familiarize themselves with the simulator (instruction time can be part of this hour). Additionally, the OSVDPA allows for up to 30 minutes of operation planning time for all assessments (this can be conducted concurrently with the simulator-familiarization time).

Also for simulator-based assessments, the OSVDPA understands it can be difficult for DPOs or Prospective DPOs to utilize “cold” or unfamiliar DP simulators. Thus, to expedite assessments, the OSVDPA requires the following information be provided to DPOs or Prospective DPOs no less than 48 hours prior to a simulator-based Phase 4, Phase 5, or Revalidation Assessment.

• A description of the layout of the DP console, screens, and menus found on the DP simulators; • A description of the vessel being simulated, its equipment, and capabilities; • The ASOG, CAM/TAM, and/or decision support tool that will be used; and • The checklist that will be used for the DP operation.

The OSVDPA does allow for the Assessee to waive their right to receive this information early by signing the appropriate box on the assessment report. Also during this time, the Examiner/QOBA shall complete Section 5 of the assessment report (or otherwise make this information available to the Assessee, possibly including some of the above-described information to the Assessee completing simulator-based assessments) thereby providing the Assessee with information about what they will be utilizing during the assessment. Finally, the Examiner/QOBA should provide the Assessee the “Assessee Copy” from the scenario (if this hasn’t been done already). Step 5 (Standing Order Users): Item Selection. Each standing order provides a range of options for which Assessment Items should be included in the assessment. These items are grouped together by subject matter, and each group provides instructions for how many items from each group should be selected. As such, the Examiner/QOBA should go through the standing order and select the requisite number of items from each group, ensuring that these selections are best fit for the DPO or Prospective DPO being assessed. Factors that should be considered during this decision are the current level of the DPO or Prospective DPO (i.e. some scenarios are tailored to DP-1 vessels and thus not a good fit for Phase 5 Assessments or Class A DPO Revalidation), the equipment contained on the vessel(s) the DPO or Prospective DPO serves on, and the DP

operations they do on a daily basis. In some cases, if an item (or a sufficient number of items from a group) is not practical or relevant to a DPO or Prospective DPO the standing order provides alternative items which can be substituted. After the Examiner/QOBA has completed they item selection process they should go back through the standing order and ensure they have selected 15 Tier 1 Items and 30 Tier 2 Items. (See Section 4 for more information regarding this process). Step 6 (Scenario Users) / Step 8 (Standing Order Users): Signatures. Once all preparation and familiarization has been completed, the Examiner/QOBA, the Assessee, and the Master of the Vessel (if applicable) shall sign Section 1 of the assessment report, thereby acknowledging they are beginning the assessment. These signatures ensure that everyone involved in the assessment process is cognizant of the restrictions surrounding an OSVDPA assessment. Specifically, the signatures make it clear that all parties know:

• They must abide by OSVDPA policies and procedures surrounding assessments, • The Assessee has completed the requirements of the previous phase (or the revalidation requirements), • Assistance shall not be provided to the Assessee, and • The Assessment will be halted if any of the parties feel continuation endangers the crew, the vessel (or other property), or other parties (this portion is

obviously only relevant to vessel-based assessments). As a further point to the restriction on assistance being provided to the Assessee, if the Assessee is taking the assessment on a cold simulator, or a simulator that has a DP system different than DP system found on the vessel they serve on, they can call a “time out” during the assessment to ask where a certain control, function, feature, or display is located. In calling for a time out, the Assessee must state what control, function, feature, or display they are seeking. Step 6 (Standing Order Users): Ordering of items and exercise selection. Once the appropriate 45 Assessment Items are selected from the options provided on the standing order, the Assessor should select an exercise from those listed in the standing order. In OSVDPA assessment terms, an exercise is the DP operation that is being conducted. Each standing order provides a few possible exercises, the purpose of these is to make a frame within which the assessment items are fit, thereby making the assessment seem more like a true-to-life DP operation, rather than an unconnected list of items that must be done correctly. Similar to the selection of Assessment Items, the Examiner/QOBA should select the exercise which best resembles the operations the DPO or Prospective DPO engages in. Once this exercise is selected, the Examiner/QOBA should ensure the Assessment Items are ordered in a way that logically fits this operation. This reordering of items can be conducted on the standing order sheet, or it can be done on paper or other mediums (electronic tablets, etc.), provided that the Assessee does not have access to this information. As an additional note about exercises, vessel-based assessments completed utilizing a standing order can be completed over the course of no more than 14 consecutive days. This method is referred to as “layering” the assessment on top of real operations. Under this format, a QOBA records the assessment items as he or she observes these items being completed during the normal course of operations. In the case of a layered assessment, an assessment exercise must still be recorded. Specifically, the QOBA shall choose the assessment exercise which most closely resembles the vessel’s operation. Step 7 (Scenario Users): Conduct the assessment. Once Section 1 of the assessment report has been signed, the Examiner/QOBA shall begin the assessment, provide the suggested instructions, directions, and prompts listed in the text of the scenario. As the Examiner/QOBA comes to each Assessment Item, the Assessor should follow the instructions listed therein and use the associated scoring criteria to record the appropriate score. The OSVDPA recommends the Examiner/QOBA utilize the scenario sheets to record the score, however, the Assessor can use other resources to record the score if they so choose. (For more information on how the scenario sheets are used see Section 6). As previously

stated, the OSVDPA recommends that the Assessee be provided three (3) hours to take the assessment, however, that is only a recommendation, the Examiner/QOBA can allow more time at his or her discretion.

Step 7 (Standing Order Users): Preparation.

Once the Examiner/QOBA has decided what Assessment Items and exercise will be used, they should provide to the DPO or Prospective DPO the information and time this individual needs to prepare themselves for the assessment.

For simulator-based assessments, the OSVDPA requires that DPO/Prospective DPOs be offered instruction on how the simulators are used and up to one (1) hour to familiarize themselves with the simulator (instruction time can be part of this hour). Additionally, the OSVDPA allows for up to 30 minutes of operation planning time for all assessments (this can be conducted concurrently with the simulator-familiarization time).

Also for simulator-based assessments, the OSVDPA understands it can be difficult for DPOs or Prospective DPOs to utilize “cold” or unfamiliar DP simulators. Thus, to expedite assessments, the OSVDPA requires the following information be provided to DPOs or Prospective DPOs no less than 48 hours prior to a simulator-based Phase 4, Phase 5, or Revalidation Assessment.

• A description of the layout of the DP console, screens, and menus found on the DP simulators;• A description of the vessel being simulated, its equipment, and capabilities;• The ASOG, CAM/TAM, and/or decision support tool that will be used; and• The checklist that will be used for the DP operation.

The OSVDPA allows the Assessee to waive their right to receive this information early by signing the appropriate box on the assessment report.

Also during this time, the Examiner/QOBA shall complete Section 5 of the assessment report (or otherwise make this information available to the Assessee, possibly including some of the above-described information to the Assessee completing simulator-based assessments) thereby providing the Assessee with information about what they will be utilizing during the assessment.

Step 8 (Scenario Users) / Step 10 (Standing Order Users): Record the Score.

Once the assessment is complete, the Assessor shall record the assessment score by completing Section 3, Section 4, and Section 6 of the assessment report. All of these sections must be completed. It should be noted, the assessment reports are designed to capture all of the information about the assessment while preserving the DPO or Prospective DPO's data. Specifically, when these reports are received by the OSVDPA during a certificate application process, the OSVDPA shall record the data and then remove the last two pages of the assessment report, thereby ensuring that this information is not detrimental to a DPO or Prospective DPO’s employment. More information on the assessment reports is found in Section 6 below. Once this is completed, the assessment report shall be returned to the Assessee.

However, the Examiner/QOBA shall not allow the Assessee to keep or copy the scenario or standing order sheets and once the review is complete, the sheets shall be destroyed.

Step 9 (Standing Order Users): Conduct the assessment.

Once Section 1 has been signed, the Examiner/QOBA shall begin the assessment, providing the directions and prompts listed in the Assessment Items and using the associated scoring criteria to record the appropriate score. The OSVDPA recommends the Examiner/QOBA utilize the standing order sheets to record the score, however, the Assessor can use other resources if they so choose. (For more information on how the standing order sheets are used see Section 6).

5 Definitions

This section lists and defines the terms that are to be used in connection with OSVDPA assessments.

Ask. An instruction provided to Examiners/QOBAs. For assessment items listed as “ask” items, the Examiner/QOBA shall ask the Assessee for the required information. For comparison, other assessment items use instructions including: “do not prompt,” “observe,” or “prompt.” “Prompt” items are different than “ask” items because “ask” items require verbal information from an Assessee, whereby “prompt” items require the Assessee to perform a task.

Assessee. The DPO or Prospective DPO that is taking the assessment.

Assessment Items. The points of evaluation which are combined to compose an assessment. As listed in this document, each Assessment Item contains a unique identifier, a description of the skill or knowledge measured by the item, set up and scoring criteria to assist the Examiner/QOBA when they utilize the item in an assessment, the competencies covered by the item, if the item is a Tier 1 or Tier 2 Item (see below), which DP equipment class should utilize the item, which Phases should utilize the item, and how it should be conveyed to the DPO or Prospective DPO.

Assessment Report. The form where the assessments are recorded. Specifically, the assessment reports are as follows:

One copy of each of these forms is included in the OSVDPA LB-1-CV, the OSVDPA In-Scheme Logbook (Current Version) and one copy of Form P4A-1-CV is included in the OSVDPA LB-2-CV, the OSVDPA Post-Scheme Logbook (Current Version) for Revalidation Assessments. Additional copies these forms—to use for retakes—are available at Accredited Training Providers or the OSVDPA.

Cold Simulator. A simulator replicating a DP system that the Assessee has not used before or not used recently.

Decision Support Tool. A decision support tool is a guideline or standard operating procedure which directs, informs, or guides a DPO’s operational decisions. The most prevalent example of a decision support tool is an Activity Specific Operations Guidance (ASOG). However, other examples of decision support tools are a vessel’s operations manual, Standing Orders, or charterer’s instructions.

Do Not Prompt. An instruction provided to Examiners/QOBAs. For assessment items listed as “do not prompt” items, the Examiner/QOBA should not prompt, hint, or otherwise tell the Assessee to complete task listed in the item. Instead, the skill being assessed by the item is determining if the Assessee knows to perform the task. For comparison, other assessment items are listed as “observe,” “prompt,” or “ask.” As a contrast with “observe” items, “do not prompt items” cannot be hinted at or prompted under any circumstances.

Examiner. The individual that scores a simulator-based assessment. The Examiner must have completed formal assessment training, demonstrated competency in assessment administration and scoring, and be a DP Instructor Certified by the OSVDPA.

Exercise. An exercise is the DP operation that is being conducted during the assessment. As such, the exercises provide a frame within which the Assessment Items are fit, thereby making the assessment seem more like a true-to-life DP operation, rather than an unconnected list of items that must be done correctly.

Layered. An assessment that is conducted during the course of a real DP operation onboard a vessel. During a layered assessment, as items required by the real operation match the pre-selected assessment items the DPO or Prospective DPO’s performance of the items is recorded (or simulated or mimicked items are recorded during the operation. Layered assessments can be conducted over periods up to 14 days. Layered assessments must be conducted using a standing order. Specifically, QOBA’s conducting layered assessment shall pick the eligible exercise which most closely resembles the operation being conducted.

List. An instruction provided to Examiners/QOBAs. This item states that if the Examiner/QOBA allows the DPO or Prospective DPO to see a portion of the items that will be included in the assessment for the purposes of understanding the kind of DP operation being conducted, this item may be one of the ones shown. List Items are also usually those items which the Examiner/QOBA prompts the Assessee to complete or asks the Assessee about during the operation. As a contrast, those items denoted as “do not list” should not be shown prior to the start of the assessment and are usually items that are not mentioned to the Assessee prior to the time they occur during the assessment.

Mimic. As used in connection with Assessment Items, “mimic” means to imitate a particular action, event, fault, failure, or condition that cannot otherwise be conducted for the purpose of assessing the DPO or Prospective DPO’s competency. Mimicking is most often conducted by the QOBA telling the Assesse what happened and asking the Assessee to demonstrate and/or explain what they would do if such an event actually occurred. For example, a QOBA might mimic a vessel losing a breaker to assess a DPO’s reaction to such a failure. At no time should mimicking be used during simulator-based assessments. Mimicking should only be used during offshore assessments when conducting or simulating the Assessment Items is not possible, is prohibited by the vessel operator or charterer or endangers the safety of the vessel, crew, or the environment.

Observe. An instruction provided to Examiners/QOBAs. For “observe” Assessment Items, the Examiner/QOBA should observe the Assessee performing the task during the course of normal operations. For comparison, other Assessment Items are denoted as “Do not prompt,” “prompt,” or “ask.” Items. As a contrast, with “do not prompt” items, observe items can be prompted by the Assessee if the subject of the assessment item does not come up during the course of normal operations.

Pass/Fail. All items contained within an assessment are scored on a pass or fail scale. Decisions made by the Examiner /QOBA as to if an Assessee passed or failed the assessment item are determined by using the scoring criteria provided by the OSVDPA within the Assessment Item.

Prompt. An instruction provided to Examiners/QOBAs. For Assessment items listed as “prompt” items, the Examiner/QOBA should prompt the Assessee to perform the task covered by the assessment item (if the Assessee does not perform this task on their own). As an alternative, assessment items may be listed as “Do not prompt,” “observe,” or “ask.”

Qualified on Board Assessor (QOBA). A DPO certified by the OSVDPA to conduct the Phase 4 and Phase 5 Assessments. In conducting these assessments, the QOBA utilizes criteria established by the OSVDPA to evaluate whether the DPO or Prospective DPO has passed the assessment.

Scenario. Scenarios are pre-printed assessments designed by the OSVDPA. Each scenario contains a narrative of what DP operation is being undertaken, what happens during the operation, and what Assessment Items are recorded by the Examiner/QOBA. Scenarios can be used on simulator-based assessments or vessel-based assessments that occur during a single watch.

Simulate. As used in connection with Assessment Items, “simulate” means to imitate a particular action, event, fault, failure, or condition that cannot otherwise be conducted for the purpose of assessing the DPO or Prospective DPO’s competency, usually through the use of the vessel’s onboard simulator. At no time should simulating be used during simulator-based assessments.

Standing order. The standing order is a way of conducting OSVDPA Assessments when the Examiner or QOBA wants to create their own assessment instead of conducting one of preprinted scenario assessments.

Tier 1 Item. Assessment items are divided into two types: Tier 1 and Tier 2. Tier 1 Items are items that if, done incorrectly or not completed, cause the vessel to lose position or heading. Each assessment must cover no less than 15 Tier 1 Items all of which must be completed correctly for the assessment to be passed.

Tier 2 Item. Assessment items that are not Tier 1 Items. Tier 2 Items cover set up, monitoring and system operations. Assessments must contain no less than 30 Tier 2 Items. An Assessee must correctly complete no less than 80 percent (24 items) of these items correctly to pass the assessment.

Time Out. A stoppage of a simulator-based assessment caused by the Assessee not knowing the controls or displays of a Cold Simulator. When requesting a time out, the Assessee must clearly state what control, feature, display, option, or function they are seeking.

Unprompted Knowledge. Some Assessment Items seek to measure not if a DPO or Prospective DPO can correctly complete a task but rather that the task must be completed in order to provide for safe DP operations. For example, a DPO should know (without prompting) to assess the environmental conditions against the vessel’s capabilities prior to the start of an operation. Within assessments, competencies addressing unprompted knowledge are usually denoted as “Do not list” items.

6 Assessment Documents.

This section provides a summary of all assessment-related documents, instructions for how each document should be used, and a description of how OSVDPA documents are titled (and what information can be gained from document’s titles). Specifically, this section provides instructions for the following:

• OSVDPA CT-1-CV, the OSVDPA Competency Standard (Current Version): This document lists every ability or skill which the OSVDPA expectsDPOs to know or be able to perform.

• OSVDPA AS-1-CV, the Assessment Guide and Item Bank (Current Version): This document lists every assessment exercise and Assessment Item thatmay be found in a Phase 3, Phase 4, Phase 5, or Revalidation Assessment. Additionally, this document provides instructions for how an OSVDPA assessmentshall be conducted.

• OSVDPA AS-2-CV, the Assessment Standing Order (Current Version). This is a memo that is sent via email to each Examiner/QOBA listing whichassessment exercises and assessment items are to be utilized for assessments. Examiners/QOBAs can also chose to conduct some assessment through theuse of a scenario (see below).

• Assessment Reports. These are the forms utilized to record a DPO or Prospective DPO’s performance on an assessment. As the rules for using Form P3A-1-CV, Form P4A-1-CV, and Form P5A-1-CV are the same, this document only lists and provides an example of one of these documents. Specifically, theassessment reports have the following names and purposes.

o Form P3A-1-CV, the Phase 3 Assessment Report (Current Version). This form is utilized to record the Phase 3 Assessment. A copy of this formcan be found in OSVDPA LB-1-CV, the OSVDPA In-Scheme Logbook (Current Version), additional copies of this form are available from theOSVDPA or Accredited Training Providers.

o Form P4A-1-CV, the Phase 4 Assessment Report (Current Version). Similar to Form P3A-1-CV but for the Phase 4 and RevalidationAssessments. A copy of this form is found in OSVDPA LB-1-CV and OSVDPA LB-2-CV, the OSVDPA Post-Scheme Logbook (Current Version).

o Form P5A-1-CV, the Phase 5 Assessment Report (Current Version). Same as Form P4A-1-CV but for the Phase 5 Assessment.• Assessment Scenarios. These are pre-printed assessments provided by The OSVDPA. Each scenario provides a narrative of a DP operation, what happens

during the operation, the 45 Assessment Items to be included, and a recommended order the items should be recorded in. Scenarios offer less flexibility inassessment content, as opposed to standing orders, but with more ease in conducting the assessment because all of the information is provided.

Naming Convention: Most OSVDPA documents (except for scenarios) follow the below listed naming convention. Examiners/QOBAs can understand a great deal about whether and how a document should be used by its name. As such, the following bullet points attempt to explain the OSVDPA document naming convention:

• The OSVDPA attempts to put every rule it abides by, or requires others to abide by, in public documents called “standards”. All standards start with“OSVDPA”. For example, the assessment content standard is “OSVDPA AS-1-CV, the Assessment Guide and Item Bank (Current Version)”.

• The OSVDPA attempts to make securing a DPO certificate easier by publishing forms that dictate exactly what information should be provided to theOSVDPA. These forms are denoted by the first word in the document title. For example, to record a Phase 4 assessment, one should use the “Form P4A-1-CV, the Phase 4 Assessment Report (Current Version)”.

• The OSVDPA understands that modifications to the scheme will be necessary. In fact, we welcome your comments and suggestions as ways of identifyingand correcting issues within the scheme. As issues are addressed and documents updated, the OSVDPA has a version control and tracking system in itsdocument titles to ensure everyone is using the appropriate versions. Specifically, the last three digits of a document’s code reflect the version number. Forexample, when the OSVDPA began operation, it used the first version of its competency standard, “OSVDPA CT-1-001, the OSVDPA Competency Standard

In order to reduce the number of changes, the OSVDPA utilizes a system whereby when one document refers to another document, the version number is replaced by “CV,” meaning the referenced information is found in the most current version of that document. For example, the OSVDPA’s competency requirements for DPOs are found in “OSVDPA CT-1-CV, the OSVDPA Competency Standard (Current Version).” The “Form Library” section of the OSVDPA website lists the most current (and all accepted) versions of each document.

OSVDPA CT-1-CV, the OSVDPA Competency Standard:

Defined competencies are the basis of the OSVDPA scheme. All aspects of the OSVDPA scheme are evaluated by whether or not they cover the competencies listed in this standard. OSVDPA CT-1-CV was developed after a review of all existing international standards and regulations, industry-created guidance documents (for example, IMCA M 117 and MTS Operations Guidance), and an understanding of the cause of previous DP incidents.

Each competency is accompanied by a unique identifying number. These numbers are found throughout the OSVDPA scheme and are referenced across all OSVDPA products. The competency table is organized by subject and by the phase where Prospective DPOs are first assessed on their understanding of the competency. It is important to note that the phase number listed next to each competency is only the phase where the competency will first be assessed, many (if not most) competencies appear on the assessments of subsequent phases. See example below.

OSVDPA AS-1-CV, the Assessment Guide and Item Bank (Current Version):

This document is the catalog of all Assessment Items and assessment exercises utilized by the OSVDPA for practical assessments. Additionally, this document contains an extensive guide for how assessment should be conducted.

Following the assessment instructions, this document contains the assessment exercises. As stated above, an exercise is the DP operation that is being conducted during the assessment. As such, the exercises provide a frame within which the Assessment Items are fit, thereby making the assessment seem more like a true-to-life DP operation, rather than an unconnected list of items that must be done correctly.

An example of an Exercise is included on the following page.

Phase Number: The number of the first phase where the competency will be assessed. Competencies might also be included on the assessments of subsequent phases. For example, 19.8 may also be part of the Phase 3, 4, or 5 assessments.

OSVDPA AS-1-CV also lists the Assessment Items. Like questions on a test, Assessment Items are the individual points of measurement that form the basis of the assessment score. To ensure assessments can be made relevant to the vessel(s) and industrial mission(s) the DPO or Prospective DPO uses or engages in, there are more than 200 Assessment Items in the item bank, however, each assessment is composed of only 45 items. Below, is a summarized example of an Assessment Item.

Instructions and scoring criteria: This column can list up to three pieces of information. The top paragraph details how the assessment item should be set up or expressed to the Assessee (for example: tell the Examiner/QOBA to create a PRS failure or tell the Assessor to prompt the Assessee to set the position alarms. The second paragraph in the column explains what criteria the Examiner/QOBA should use to determine if the item was passed or failed. Often, this criteria is based on factors relevant to the vessel, such as the vessel’s ASOG. The third paragraph (not present on every Assessment Item), list special instructions, such as an international standard that should be used by the Examiner/QOBA as a scoring standard, or when the Assessment Item should not be conducted.

Competency Covered: This column lists the competencies from the OSVDPA CT-1-CV that are covered by the AssessmentItem.

Assessment Item: This column tells the Examiner/QOBA what task or operation must be completed. It is important to note, this column should not be used to tell or prompt the Assessee what to do during the Assessment Item.

DP Class: This column states on which DP classed vessels the assessment item can

Exercise Number. The exercise number is referenced by the standing orders, which dictate which exercises are eligible during different time periods. Additionally, the assessment reports ask which exercise was used to conduct the assessment.

Exercise. This is the operation that is being conducted during the assessment. The exercises provides a story or frame within which the assessment items are fit, thereby making the assessment feel more like a real operation instead of a list of tasks. When conducted as a layered assessment, the QOBA shall still selects the eligible assessment that best mirrors the actual operation.

Conditions. When conducted on a simulator, the Examiner shall set the conditions to be equal to those listed here.

As stated elsewhere in this guide, the standing order is a way of conducting OSVDPA Assessments when the Examiner or QOBA wants to create their own assessment instead of conducting one of preprinted scenario assessments. For example, the vessel served on by the DPO or Prospective DPO might have a very specific set up, thus, the QOBA might use a standing order to design his own assessment to mirror that setup. Additionally, standing orders must be used when the assessment is conducted over a period exceeding a single watch.

This memo is called the standing order because the version stays valid until it is replaced by the OSVDPA. When it is replaced, the new standing order is sent to each Examiner, QOBA, Enrolled Vessel Operator, and Accredited Training Providers. The current standing order is also listed under the “Assessment” tab on the OSVDPA website. Finally, the OSVDPA understands that Examiners/QOBAs shall not always have access to email or the OSVDPA website, as such, the OSVDPA accepts assessments that utilize one of the previous two standing orders (for example, if the OSVDPA most recently issued OSVDPA AS-2-003, the Assessment Standing Order (Version 3), the OSVDPA shall accept assessments utilizing version 1 or version 2).

The standing order listing which assessment exercises and assessment items are to be utilized for each assessment, providing options for each subsection of Assessment Items. In selecting which item to utilize, the OSVDPA encourages Examiners/QOBAs to select the Assessment Item most relevant to the day-to-day operation of the Assessee. The OSVDPA has attempted to structure the subsections whereby every DPO or Prospective DPO, or DP vessel will be able to be able to complete the requisite number of items listed in that subsection. However, there are times when that is simply not possible (for example, some items are simply not appropriate for a Phase 3 Assessment or capable on a vessel with a limited number of reference systems), in these cases, the standing order directs the Examiner/QOBA what other subsection should include double the number of items to make up for these shortfalls.

Below is an example of how the standing order is laid out. With this example, the OSVDPA provides explanatory text.

Assessment Scenarios.

Scenarios are pre-printed assessments designed by the OSVDPA. Each scenario contains a narrative of what DP operation is being undertaken, what happens during the operation, and what Assessment Items are recorded by the Examiner/QOBA. Scenarios can be used on simulator-based assessments or vessel-based assessments that occur during a single watch. Because scenarios list exactly what Assessment Items are included in the assessment, scenario-based assessments can be less cumbersome for the Examiner or QOBA to conduct. To ensure this loss of customization does not cause scenario-based assessments to be irrelevant to the daily operations conducted by the Assessee, the OSVDPA provides multiple scenarios for Examiners and QOBAs to choose from.

The decision as to what assessment scenario should be chosen for a DPO or Prospective DPO’s assessment, is one that should be made in collaboration between the Examiner/QOBA and the Assessee. To do so, both parties can review the list of “live” (or currently accepted) scenarios found under the “Assessment” tab on the OSVDPA website. This page provides a description of the scenario, without providing the Assessee any information about the content of the assessment, thereby ensuring the assessment is a true measure of competency, not ability to memorize answers.

The Examiner/QOBA and Assessee should select the scenario that is most appropriate for the vessel(s) the DPO or Prospective DPO serves on and mirrors the industry mission the DPO or Prospective DPO participates in. This selection should be made on the OSVDPA website or in other ways which prevent the Assessee from seeing the entire content of the scenario. Once the scenario to be utilized is selected the Examiner/QOBA should print the entire scenario, providing the first page to the Assessee while ensuring the Assessee does not see any of the subsequent pages, see below for more information about the layout of a scenario.

Each scenario is composed of two pieces, the “Assessee Copy” and the “Assessor Copy.” The Assessee Copy is a single page and provides a broad overview of the intended operation that the scenario will be conducting. The Assessor Copy is multiple pages in length and describes each assessment to be utilized, when it should occur, how to set up the item, and provides an area to record the DPO or Prospective DPO’s score on the Assessment Item. A sample of an assessment scenario is included below.

Assessee Copy: This is the Assessee Copy of a typical scenario. As seen, the copy provides a very short description of what is going on in the Assessment. As listed in the narrative, the words in bold type and within brackets can changed to something within the brackets or something similar as the value, term, or item listed in the brackets. The title code of the scenario also provides information about the assessment, the first letter in the code specifies the type of vessel = logistics, C = construction, O = other, etc. The middle two letters specify the asset type, for example, “FL” for floating, or “SS” for subsea.

The last sentence of the “Assessee Copy” always states when the assessment should or should not be used. For instance, this sentence on the above scenario says, “As an assessment for a DP-1 vessel, this assessment is inappropriate for a DPO seeking a Class A DPO Certificate.

The conditions listed on the scenario have been formulated to provide guidance to Examiners when conducting simulator-based assessments. When conducted on a vessel, the condition information listed in the scenario is simply a guide, the Assessor does not have to wait for conditions to be within the parameters listed within the scenario.

Assessment Reports.

Assessment Reports are the forms utilized to record a DPO or Prospective DPO’s performance on an assessment. Specifically, these forms are titled: • Form P3A-1-CV, the Phase 3 Assessment Report (Current Version). This form is utilized to record the Phase 3 Assessment. A copy of this form can be

found in OSVDPA LB-1-CV, the OSVDPA In-Scheme Logbook (Current Version), additional copies of this form are available from the OSVDPA orAccredited Training Providers.

• Form P4A-1-CV, the Phase 4 Assessment Report (Current Version). Similar to Form P3A-1-CV but for the Phase 4 and Revalidation Assessments. Acopy of this form is found in OSVDPA LB-1-CV and OSVDPA LB-2-CV, the OSVDPA Post-Scheme Logbook (Current Version).

• Form P5A-1-CV, the Phase 5 Assessment Report (Current Version). Same as Form P4A-1-CV but for the Phase 5 Assessment.

The three forms are virtually the same, thus, Form P4A-1-CV shall be summarized and used for an example below, however, most of the same rules apply to the other assessment reports.

The form’s primary purpose is capturing and recording the OSVDPA assessment. But the OSVDPA understands data capture is not always positive for all parties involved. The OSVDPA’s aim is to improve the safety of the offshore industry, not adversely harm its participants. As such, when these assessment reports are submitted to the OSVDPA (usually as part of a certificate application or application for revalidation) the OSVDPA records the data and then removes the pages carrying the data from the logbook.

Within its primary purpose, the form records three areas of information. First, the administrative information about the assessment (e.g. who conducted the assessment, what scenario or standing order was used, if all the appropriate waivers were signed etc.). Second, the form records information about how the assessment was conducted, such as what systems and information was available to the Assessee and what were the conditions during the assessment. Finally, the form records the content of the assessment, such as what Assessment Items were included in the assessment and if the Assessee passed or failed each of these Assessment Items.

See below for instructions on each section of this form.

Section 1: Acknowledgements. This Section requires all of those involved in the assessment to sign the form acknowledging that they understand they are entering an assessment, they understand the rules of the assessment, will not break said rules, and will simulate/mimic (or even halt the entire assessment) if anything believed to endanger the safety of the crew, vessel, or other individuals or assets.

Examiners or QOBAs shall take care to remember to put their Instructor number or QOBA Number in this space, not their DPO Certificate Number.

Section 2: Assessment Information. This section records the administrative information about the assessment. Examiners and QOBAs shall take care to provide assessment content that has not been viewed before by the Assessee (see question 2a and 2b). Additionally, Examiners of simulator-based assessments shall take care to ensure Section 2e is completed (if necessary).

Section 4: Score Tally. While this information may seem redundant of some of the information in Section 2 and Section 3, it is repeated here because this page stays with the OSVDPA when the specific assessment information is removed from the Assessee’s logbook. Therefore, Section 4 should be completed completely.

Section 5: Vessel/Simulator and Condition Information. During simulator based assessments, this section can be utilized by the Examiner/QOBA to provide the Assessee information about the simulated vessel, the conditions, and the resources available to the Assessee. During a vessel based assessment this information can be utilized for that purpose or as a simple record of what was available.

For each item the Examiner/QOBA should clearly record the Assessment Item Number, if it was a Tier 1 or Tier 2 Item, and if the Item was passed or failed. Additionally, if so desired, the Examiner/QOBA can list the Item Number for the “List” items, in order to expedite the assessment by providing the Assessee some information about the operation they will be conducting.

7 Assessment Policies and Procedures

This section provides a detailed summary of the policies and procedures governing OSVDPA Phase 3, Phase 4, and Phase 5 assessments. More information about these rules can be found in OSVDPA MPP-1-CV, the OSVDPA Manual of Policies and Procedures (Current Version). Phase 3 (Simulator) Course: At the end of the Phase 3 (Simulator) Course (the Prospective DPO must be present for no less than 95 percent of the instruction hours contained in the course), the Prospective DPO shall be required to pass a simulator-based assessment covering the competencies tested in Phase 1, Phase 2, and Phase 3 of the OSVDPA Competency Table. The Phase 3 Assessment is scored by the OSVDPA-certified instructor conducting the assessment. The assessment must be taken at the Training Provider where the Prospective DPO took the Phase 3 (Simulator) Course and must be taken on the day the course is finished. If the Prospective DPO fails this assessment, they are provided two (2) additional attempts at passing the assessment. No more than two (2) of these attempts may be taken during any one (1) day and all three (3) attempts must be conducted within 120 days of the first attempt. If all of these attempts are failed, the Prospective DPO must retake the Phase 3 (Simulator) Course. The Phase 3 Assessment is conducted on simulators rated as meeting Class B or Class A as described in OSVDPA SIM-1-CV, the OSVDPA Simulator Standard (Current Version). Phase 4 (Watchkeeping): Once the Prospective DPO has completed the Phase 4 requirements, he or she is allowed to take the Phase 4 Assessment. Specifically, the assessment is only administered after the Prospective DPO has completed the following:

• 60 days of Sea Time, • 180 hours Practical Experience, and • The Phase 4 taskbook.

When conducted onboard a vessel, the assessment must be administered by a Qualified on Board Assessor (see Section 5). Understanding that working vessels cannot always dedicate time to DPO assessments, the OSVDPA allows vessel-based Phase 4 Assessments to be conducted over a period of up to 15 days. However, once the Prospective DPO starts the assessment on a vessel, he or she must finish the assessment on that vessel. Alternatively, the OSVDPA allows Prospective DPOs to complete the Phase 4 Assessment on an OSVDPA-approved simulator. In this instance, the assessment is conducted under essentially the same policies and procedures as the Phase 3 Assessment (see above). If the Prospective DPO fails the Phase 4 Assessment, he or she cannot switch mediums until he or she completes any required remedial learning, i.e., if the first attempt at passing the Phase 4 Assessment is on a vessel, the second and third retake attempts must be on a vessel. The Prospective DPO is provided three (3) opportunities to pass the Phase 4 Assessment. No more than two (2) of these attempts may be taken during any one (1) day and all three (3) attempts must be conducted within 120 days of the first attempt. If a Prospective DPO does not pass the assessment during these three (3) attempts, the Prospective DPO shall be required to complete an additional 30 days of Sea Time and 90 hours of Practical Experience before attempting to pass the Phase 4 Assessment for a fourth time.

Phase 5 (Upgrading): DPOs that have earned a Class B DPO Certificate can upgrade this certificate to a Class A DPO Certificate by completing Phase 5 (Upgrading). Please note that this is an optional Phase and is not required. Phase 5 (Upgrading) is conducted under essentially the same policies and procedures as Phase 4 (Watchkeeping), except the DPO is required to record the following aboard a vessel with a DP class notation of DP-2 or DP-3 vessel:

• 30 days of Sea Time, • 60 hours of Practical Experience, and • The Phase 5 Taskbook.

Once the Phase 5 (Upgrading) requirements are completed, the DPO is allowed to take the Phase 5 Assessment. The Phase 5 Assessment is conducted under the same policies and procedures as the Phase 4 Assessment. Revalidation: OSVDPA DPO Certificates are valid for five (5) years from the date printed on the certificate. To revalidate a certificate, a DPO must record the following Training and Certification Activities within the five (5) years preceding their submission of a Revalidation Application:

• 150 days of Sea Time, • 450 hours of Practical Experience, and • Passage of the Phase 4 Assessment (after completion of the above).
