B. ACTIVITIES UNDERTAKEN (Enumerate and describe below the different activities during the One Health Week)

1. SBFP * Provided the Master list of feeding participants to the Feeding teacher in-charge * Supervised and gave assistance during feeding time. * Gave deworming tablets to all pupil participants including the teacher and helpers * Gave multivitamins after each feeding. * Pupils were provided health kits.

2. NDEP * The Philippine National Police Officers conducted a 1 week Classroom to classroom Drug. Awareness and Rehabilitation Education Lecture to all Grade VI pupils. Classroom teachers were There to assisted them.

3. ARH * Adolescent Reproductive Health were conducted during Science and H.E classes in Grade V and Grade VI.

4. WINS * Hand washing and tooth brushing where lectures demonstration were conducted to Kinder Pupils and Feeding and Participants. Returned demonstration were also Done. Grade V teachers and Kinder teachers assisted me during this time.

5. Others * Assisted our School Physician during the P.E athletes as a requirement before they can play the sports event they joining.

* Distributed permits to the classroom adviser distribution and collections for the deworming of all Grade levels.

* Serve as a resources speaker during Parent Orientation for Feeding Participants, Dengue Awareness and OK sa Dep ed program

* Conducted/Attended consultations and treatments among teachers and pupils. * LGU/NGO/GPTA Parents joined hand in the cleaned up drive in our school for dengue Free

School.

C. ISSUES AND CONCERNS FACILITATING FACTORS (Discuss major outstanding factors that contributed to the successful conduct implementation.)

The successful implementation of the One week Health activities were not be possible without the Cooperation and helped of everyone. From our Principal Mr. Diomedes T. Rosete, Master Teachers, Teachers, GPTA, Parents, Non teaching personnel and pupils. Along side with this success were the helped of our stakeholders.

HINDERING FACTORS (Discuss major factors that caused delay or impeded implementation.)

The only hindering factors in the delay of the implementation were the inclement weather that resulted to classes suspensions.
