NPT Training Exercise

Professor Frances Mair, University of GlasgowFor the RESTORE team NPT training April 2012.

Page 2: NPT training session 2 for RESTORE

Objectives of this session • To further consolidate understanding of NPT and

the core constructs.• To provide a second opportunity for team

members to further develop knowledge and understanding of the NPT constructs, particularly, COHERENCE and COGNITIVE PARTICIPATION.

Page 3: NPT training session 2 for RESTORE

WHAT DO WE PLAN TO DO?• To use a RESTORE relevant example to allow

participants to consider the constructs and ideas of NPT.

• To undertake a “Fish Bowl” exercise that will more closely resemble RESTORE fieldwork.

• NPT trainers will function as focus group members and the rest of the RESTORE team to act as observers.

Page 4: NPT training session 2 for RESTORE

Outline of Session

• NPT Trainers will participate in a focus group discussion about the implementation of professional, trained interpreters in a primary care setting.

• 2 NPT Trainers will facilitate the session.• 2 NPT and 2 PLA trainers will be in the “fish bowl”

discussing implementation processes.• The rest of the RESTORE team will observe, listen and

connect the trainers’ discussions with NPT constructs on a large scale NPT wall chart.

• Then Break and Feedback Thereafter

Page 5: NPT training session 2 for RESTORE

What is the Scenario?

• More closely reflect fieldwork.• Setting is a practice team along with an interpreter

and a patient.• The team are being brought together to consider

whether or not they think it is possible to introduce a paid interpreter into the practice’s daily routine. Scenario was described in detail in the material circulated about the Guideline for Communication in Cross Cultural GP consultations.

Page 6: NPT training session 2 for RESTORE

Details of the Scenario

• The setting for the implementation in this fictional role play is Ireland.

• The intervention is the professional, trained interpreter example.

• NB. We realise that the practice structures and issues would be different in the different RESTORE countries BUT we are asking you all to IMAGINE you are in the Irish context when considering this.

• THIS IS AN EXAMPLE FOR TRAINING PURPOSES ONLY.• HEALTH WARNING AS BEFORE ABOUT THE FICTIONAL

NATURE OF THIS ROLE PLAY AND ROLES ASSUMED!

Key Points

• The different trainers in this exercise have all been assigned different “roles” for the purpose of this scenario.

• We intend for all RESTORE members to use the NPT wall banners provided.

• So write down questions on “stickies” AND then place on wall banners.

• DO NOT spend a long time pondering which NPT construct is correct. Place “stickies” where you THINK IS CORRECT. If uncertain put a ? on the “stickie”.

Page 8: NPT training session 2 for RESTORE

Your Tasks

• Your task is to connect the focus group conversations with the NPT constructs.

• Write down individual questions on “stickies”.• Place “stickies” on the appropriate section of the large

NPT amoeba wall chart.• There will be space on the chart for “stickies” that are

“definitely” about a specific construct and for those that are “potentially” about a specific NPT construct.

• There will also be space on the chart for “stickies” that have material that does not appear to be related to NPT.

Page 9: NPT training session 2 for RESTORE

Key Points

• The FishBowl participants will aim to try to use language which will aid identification of the NPT constructs.

• We may repeat key ideas to ensure all the key NPT concepts are captured.

• Assign NPT “stickies” quickly …do not spend a lot of time discussing where they should go if there is disagreement or uncertainty.

Page 10: NPT training session 2 for RESTORE

The aim of this exercise is to help you think about how to operationalise NPT in practice.

• We have a NPT amoeba wall chart to help illustrate how questions relate to each of the constructs.

• Remember the focus is on COHERENCE and COGNITIVE PARTICIPATION…although naturally the other constructs may naturally be addressed at times.

REMINDER - COHERENCE

• DifferentiationIs there a clear understanding of how introducing paid interpreters

into primary care consultations will differ from usual practice?• Communal specificationDoes the group achieve a shared understanding of the aims,

objectives, and expected benefits of using interpreters?• Individual specificationDo individuals have a clear understanding of what will be required

of them as individuals?• InternalizationDo individuals understand the value, benefits and importance of

using paid interpreters?

Page 12: NPT training session 2 for RESTORE

REMINDER – COGNITIVE PARTICIPATION

• EnrolmentDo individuals “buy into” the idea of using paid interpreters?• ActivationCan individuals sustain using paid interpreters?• InitiationAre key individuals willing to drive the implementation?• LegitimationDo individuals believe it is right for them to do this?
