Scroll down to the Topic/Week block that you wish to insert. Click on ‘Add an Activity or resource’. A window will appear. Select ‘Feedback’ from the ACTIVITIES list. The click the [Add] button at the bottom of the dialog window. 4 5 6 Insert ‘Name:’ as with any other MOODLE activity. Insert ‘Description’ – e.g. Please complete this short survey to help me improve your learning experience. Select ‘Display’ button. Feedback Options: Select whether you wish the survey results to be Anonymous – i.e do you want to know what each student answered or not? For simple and quick set up leave remaining options on NO. After Submitting: Insert a message of thanks to your students for completing survey. Again for simple set up… leave remaining options. FURTHER NOTES > The feedback activity module enables a teacher to create a custom survey for collecting feedback from participants using a variety of question types including multiple choice, yes/no or text input. Feedback responses may be anonymous if desired, and results may be shown to all participants or restricted to teachers only. Any feedback activities on the site front page may also be completed by non-logged-in users. Feedback activities may be used 1. For course evaluations, helping improve the content for later participants 2. To enable participants to sign up for course modules, events etc. 3. For guest surveys of course choices, school policies etc. 4. For anti-bullying surveys in which students can report incidents anonymously
