Many of the assays performed in diagnostic and research laboratories involve the
identification and quantification of chemical species in complex biological matrices.
Today’s chemists demand more accurate and cost-effective analytical tools that can
be easily integrated into current laboratory practices.
LC/MS/MS and other chromatographic detectors are ideally suited to many
applications as the selectivity of the technique enables individual analytes to be
quantified confidently. MassTrak Solutions integrate chromatography, consumables,
mass spectrometry, software and support services for diagnostic and research
laboratories worldwide. Single-vendor support is also available through the Waters
dedicated Clinical teams.

The Waters method for Tacrolimus has been in routine use worldwide
for several years and has proven to be very robust and reproducible.
An immunosuppressants reagent kit is available to further facilitate
laboratory implementation and ensure reliable results. The MassTrak
Immunosuppressants Reagent Kit is 510(k) cleared and CE marked to
the IVD Directive for the quantification of Tacrolimus in whole blood for
kidney and liver transplant patients as an aid in the management of
Tacrolimus therapy. The reagent kit has been extensively tested against
key criteria in three different laboratories, this degree of testing means
that you can rely on reproducible and accurate results in your laboratory.

LC/MS/MS is now widely used for the therapeutic drug monitoring of
immunosuppressive drugs in routine clinical laboratories around the
world. The increased specificity of LC/MS/MS enables the measurement
of immunosuppressive drugs with much improved precision and accuracy
when compared with the traditional immunoassay methods. This is of
particular benefit when the concentration of immunosuppressive
drugs is in the low ng/ml range, for example when they are used in
combination therapy. In high throughput laboratories LC/MS/MS
has also proved to be a cost effective alternative to immunoassay
based methods.
The challenges facing laboratories wishing to implement LC/MS/MS
for therapeutic drug monitoring can include a lack of laboratory staff
to develop, support or implement LC/MS methods, access to reliable
calibrators, QCs and Internal Standards, application and services
support. Waters has addressed these key challenges by supplying the
total system, methodology and consumables such as LC columns and
reagents for the quantification of Tacrolimus.

Over the last few years, there has been a significant increase in
interest using LC/MS/MS analysis of small molecules associated with
endocrine disorders. LC/MS/MS systems have been adopted in many
clinical laboratories world-wide for the routine analysis of endocrine
related compounds such as:
25OH Vitamin D
Testosterone
Cortisol
Plasma Free Metanephrines
Aldosterone
Steroid Panels
For many of these analyses LC/MS/MS has been able to demonstrate
improved precision and accuracy when compared to commercially
available immunoassay methods, leading to improved clinical
decisions and consistent diagnosis.

Tandem Mass Spectrometry is now established as the technique of
choice used for high throughput screening of inherited metabolic
disorders and as an aid to diagnosis of metabolic disorders in children
and adults. Over 5 million babies are currently being screened using
Waters LC/MS/MS systems.
Using tandem mass spectrometers, more than 30 metabolic disorders
can be detected from a single blood sample (taken within a week after
birth). Screening by LC/MS/MS can mean the difference between a
healthy life and one of severe disabilities or even death. In addition
to the obvious health benefits, system solutions can provide cost
benefits as early diagnosis can prevent costly health care in the
future. Prior to the use of LC/MS/MS, routine screening of newborns
was limited to three or five metabolic disorders – each requiring
a different test.
Waters has been involved in newborn screening since the mid
1990s. T he current ACQUITY UPLC TQD combination builds on
nearly 20 years of proven field experience and sets new standards
for performance, reliability and ease of use. T his solution is offered
in collaboration with Perkin Elmer.

To ensure accurate and reliable measurement, LC/MS/MS, assays for
endogenous compounds such as steroids, metanephrines and 25OH
Vitamin D, usually require sample pre-treatment steps prior to analysis.
To overcome laborious manual sample pre-treatment and potential er-
rors due to operator variablity, Waters has developed automated off-line
sample pre-treatment solutions to enable the successful implementation
in clinical laboratories optimizing work effectiveness.

Traditionally, confirmation analysis of drugs of abuse was carried out by gas chromatography/mass spectrometry (GC/MS). However this can be
a time consuming and labor intensive procedure, as many drugs require derivatization and complex sample pre-treatment before they can be
analyzed by GC/MS. In addition, long GC run times limit sample throughput.
To address these challenges many laboratories are now employing LC/MS/MS systems to conduct their confirmatory analysis. At Waters, the
MassTrak Solutions approach minimizes your sample pre-treatment requirements and optimizes your laboratory throughput. The technique is
particularly useful for polar, non-volatile, thermally labile compounds which are difficult to analyze by gas chromatography-based techniques.
The introduction of UltraPerformance LC® has resulted in significant improvements for routine laboratory analysis. Methods are now faster, more
efficient in respect to resolution and ultimately more sensitive.
Some recent examples of our customer’s success with the implementation and routine use of UPLC, as highlighted by peer-reviewed
publication, include:
Sixty percent reduction in the analytical process for 8 drugs of abuse in a high throughput laboratory.

Waters has developed two separate and complementary approaches to accurately identify unknown compounds, leveraging LC/MS/MS technologies:
For Systematic Toxicological Analysis: ChromaLynx™, for forensic toxicology is a unique data processing software tool ensures you routinely perform
faster analysis and data interpretation. ChromaLynx automates the process of extracting mass spectra from complex chromatograms and searching
against library spectra. When combined with UPLC, this results in rapid chromatography (e.g., run times reduced from 30 minutes to 15 minutes).
A chromatogram that would usually require hours of manual data analysis can now be processed in minutes using ChromaLynx.
The ChromaLynx™ browser shows; (i) the total ion chromatogram (TIC); (ii) the list of proposed candidates; (iii) the spectral match for function 5 (cone voltage 65 V), of one of the proposed compound candidates (EDDP). The spectrum view window allows a direct visual comparison of the acquired spectral data with the library data. In this example an excellent average Match Factor was observed i.e., 924 out of a possible max. 1000.
Alternatively, if your analysis requires a more targeted screening approach, you can use MRM [Multiple Reaction Monitoring] to increase the
selectivity and sensitivity for unknown compound identification. The ACQUITY TQD system uses the same 15 minute chromatography as with
the STA method, and in combination with a dedicated library containing the most frequently encountered toxicological compounds, ensures rapid
and reliable results from sample injection to injection. The TargetLynx software ensures that comprehensive reports are generated instantly after
samples are analyzed. The capability to then confirm and quantify any identified compounds using the same UPLC/MS/MS System ensures you
maintain the optimal laboratory efficiency.*
Both MassTrak screening solutions, including their respective compound libraries, are available without requiring additional method development.
*A separate validated method is necessary for any drug confirmation and quantification.

service plan
An annual Performance Maintenance
visit keeps your Waters MassTrak System
running at optimal performance. During the
visit, your Waters certified field service
engineer replaces normal wear-and-tear
maintenance parts based upon a proven
protocol developed specifically for your
system. The engineer also looks for signs
of wear, corrosion, and damage to key
parts so we can correct problems before
they impact your productivity.
