Failure of medical therapy is defined as
1) A patient rendered nonambulatory because of pain from a weakened or fractured vertebral body; pain persisting at a level that prevents ambulation despite 24 hours of analgesic therapy
2) A patient with sufficient pain from a weakened or fractured vertebral body that physical therapy is intolerable; pain persisting at that level despite 24 hours of analgesic therapy
3) Any patient with a weakened or fractured vertebral body and unacceptable side effects such as excessive sedation, confusion, or constipation due to the analgesic therapy necessary to reduce pain to a tolerable level

✓ RM – molto sensibile con sequenze T1, T2 e STIR (caratteristico segnale della spongiosa nelle fratture acute fino ad 1 mese ed oltre nelle fratture non consolidate).
✓ SCINTIGRAFIA – è legata alla attività osteoblastica e rispecchia la reazione riparativa. Negativa in fase acuta. Positiva da 7-15 gg fino a 6 mesi e oltre. Utile per le fratture croniche
✓ BIOPSIA: lndicata nella dd tra crollo porotico e patologico qualora impossibile da dirimere con le altre metodiche (RM-PET)

• Type A fractures correspond to simple compression fractures due to hyperflexion.
• Type B injuries involve both anterior and posterior columns and are associated with distraction of adjacent vertebrae due to ligamentous injury anteriorly (uncommon, hyperextension) or posteriorly (common, hyperflexion).
• Type C injuries include the characteristics of type B with the added element of rotation/shear and are by definition unstable. These injuries most closely correspond to the dislocations or fracture-dislocations of traditional classifications.

• Frattura traumatica in pz giovane per ristabilire l’altezza vertebrale.
• Fratture tipo Magerl 2-3 ove la presenza di frammenti aumenta il rischio di fughe di PMMA (la cifo crea una cavità nella quale il cemento viene semplicemente depositato, non iniettato a pressione come nella VPL)
• Può essere riservata ai casi di VCF non trattati precocemente in cui vi sia necessità di effettuare un tentativo di recupero in altezza del soma fratturato.

Conclusions
Percutaneous vertebroplasty did not result in statistically significantly greater pain relief
than a sham procedure during 12 months’ follow-up among patients with acute osteoporotic
vertebral compression fractures.

A meta-analysis, recently published in Osteoporosis International, has found that the application of percutaneous Vertebroplasty (PVP) is safe and effective only in patients with acute osteoporotic vertebral compression fractures (OVCFs) having persistent and severe pain. The investigators recorded no benefits among patients with older fractures or those bearing non-severe symptoms.
PVP significantly reduced pain at all the time points, and that the risk of the new vertebral fractures was similar between the PVP groups and control groups.
The investigators included a total of 12 RCTs, comprising 1,624 patients, in the analysis. For the blinded studies, they found statistical differences between PVP and the sham injection group for the three primary outcomes in the subgroup of the Vertebroplasty for acute painful osteoporotic fractures (VAPOUR) trial. Although pain scores were similar between the PVP group and the sham injection group for the VAPOUR trial at each period, the effect size of PVP increased over time.

The updated version continues to misrepresent the evidence supporting early
intervention with vertebroplasty for patients with uncontrolled, severe pain and
fracture duration <6 weeks. The VAPOUR trial is the only blinded trial of
vertebroplasty restricted to this patient group. It showed the benefit of
vertebroplasty over placebo, particularly when the intervention occurred within
3 weeks of fracture.
