INFLAMMATION
Complex reactions in response to injurious agents that affect the body, fundamentally a protective response Inflammation consists of 1. Vascular response 2. Cellular response (Migration and activation of leukocytes) 3. Systemic response
5 Cardinal signs of Inflammation
ปวด Pain (dolor)
บวม Swelling (tumor)
แดง Redness (rubor)
รอน Heat (calor)
สูญเสียหนาที่การทํางาน Loss of function
(functio laesa) Suffix –itis : Appendicitis, Myocarditis
Otitis, Neuritis, etc.
2
Vascular and cellular reactions are mediated by chemical factors, derived from plasma proteins or cells (Cytokines) and are produced in response to or activated by the stimuli Inflammation is divided into – Acute inflammation – Chronic inflammation

Plasma cells ACUTE INFLAMMATION
Rapid response to an injurious agent that serves to deliver mediators of host defense (leukocytes and plasma proteins) to the site of injury
5
Three majors components: 1. Alterations on vascular caliber that lead
to an increase in blood flow
2. Increase vascular permeability due to vascular structural change that permits fluid, plasma protein (fibrin, complement) and leukocytes to leave the circulation
3. Emigration of the leukocytes from the microcirculation to the stimulated site
Stimuli for Acute inflammation
Infections and microbial toxins Trauma Physical and chemical agents Tissue necrosis Foreign bodies Immune reactions
Vascular Changes Changes in vascular flow and caliber – Brief vasoconstriction and Vasodilation – Earliest manifestation of acute inflammation – Stasis increased blood viscosity – Quickly followed by increased vascular
permeability
Increased vascular permeability – Hallmark of acute inflammation – Protein and fluid leakage from the vascular lumen
to the interstitial space
6
Extravascular fluid
Exudate : high protein concentration, turbid Transudate : low protein concentration, clear
Pus or purulent exudate (หนอง) : leukocytes-rich exudate with cellular debris
Cellular Events Migration of neutrophils to the site of injury :
Margination Rolling Adhesion Transmigration (diapedesis) Migration chemotaxis
7
CHEMICAL MEDIATORS OF INFLAMMATION
Mediators originate either from plasma or from cells The production is triggers by microbial products or by host proteins, or by other chemical mediators Mediators perform activity by binding their specific receptors (specificity) One mediator can stimulate the release of other mediators (cascade) Mediators have different effects on different cell types Most mediators are short-lived
Chemical mediators
Vasoactive amines: Histamine Plasma proteins: complement system, kinin system, clotting and fibrinolytic system Arachidonic acid metabolites: prostaglandins, leukotrienes, thromboxanes Cytokines and chemokines: Interleukin-1(IL-1), Tumor necrotic factor (TNF) Nitric oxide (NO)
Effects of Chemical mediators
Pain: Prostaglandin, bradykinin Increased vascular permeability, edema Vasodilation: histamine, NO Fever, acute phase symptoms: IL-1, TNF Tissue damage Chemotaxis, leukocyte recruitment and activation
8
OUTCOME OF ACUTE INFLAMMATION
การหายเปนปกติ (Complete resolution)
การซอมแซม (Healing by connective tissue
replacement, Fibrosis หรือ scar)
การอักเสบแบบเรื้อรัง (Chronic inflammation)
MORPHOLOGIC PATTERN OF ACUTE INFLAMMATION
Serous inflammation – Burn – Inflammation in the body cavity
Fibrinous inflammation – Severe injury, results in greater vascular
permeability – Leakage of fibrinogen (plasma protein)
Suppurative or purulent inflammation – Inflammation with pus or purulent exudate
formation – Acute appendicitis – Acute meningitis – Abscess : localized collections of purulent
inflammatory tissue – Fibrinopurulent inflammation
9
Abscess (ฝ)
Focal collection of pus (consisting of neutrophils, necrotic cells, and edema fluid) Abscess has a central, largely necrotic region rimmed by a layer of preserved neutrophils, and completely walled off by connective tissue

Skin Abscess
Lung abscess
Liquefactive necrosis : Lung abscess
เนื้อเยื่อปอดที่ตายจะมีการยอยสลายโดยเอนไซม hydrolase จากเซลล
อักเสบ ทําใหเนื้อเยื่อเละ ไมเหลือเคาโครงเดิม
Ulcers (acute or chronic) – Local defect or excavation of the surface of
an organ or tissue – Epithelial layer (mucosa) is lost – Most common encounter in
GI mucosa : oral ulcer, peptic ulcer Subcutaneous tissue : skin ulcer

CHRONIC INFLAMMATION
Inflammation of prolonged duration (weeks or months) in which active inflammation, tissue destruction, and attempts at repair are proceeding simultaneously.
Cause of chronic inflammation
Persistent infection Prolonged exposure to potentially toxic agents, either exogenous or endogenous Autoimmunity
Morphologic features
Infiltration with mononuclear cells, including macrophages, lymphocytes and plasma cells (Acute inflammation = Neutrophils) Tissue destruction Healing by connective tissue replacement of damaged tissue (fibrosis, scar)
