This edition of Trust News is areminder of the Christmas period andI would like to thank all staff whocontributed to the care of patients overthe holiday season.

I would also like to congratulate allthe winners of our Christmas CheerAwards who are featured in thismonth's Trust News - this year'sawards were the most successful everwith a record 271 nominations of staffand volunteers. Well done to everyone.

HHHAs I write this column we are all experiencing a verychallenging time in order to achieve the national targetof treating 90 per cent of outpatients and day cases and85 per cent of inpatients within 18 weeks of referral bythe end of March.

I am asking you all to concentrate your efforts tohelp each other achieve the target and deliver thismajor service improvement for patients. It issomething we need to achieve to be a provider ofchoice and it is what we would want for ourselves. With all of your help we can deliver better care forpatients.

HHH

We also have an unprecedented number ofpatients with Clostridium difficile infectionbecause of a general outbreak of diarrhoeaand vomiting in the hospital. I took thedecision to concentrate patients withsymptoms onto the Chelsea Wing. I wouldlike to assure you all that we arecommitted to having a successful adultprivate patient unit and I will ensure thatit re-opens as soon as possible. By takingthe action I did we have been able tocontinue to admit patients to the hospital.

HHHYou will all have read the adverse media coverage thatwe have received over the last few weeks. I would askthat you all endeavour to be good ambassadors of ourhospital and to share good news. For example, we haveone of the best clinical outcomes for maternity servicesin London. However, we do need to respect and learnfrom patient feedback. Perception of the service makesthe difference between recommending a service or not.

So we have our challenges and I know how hard youare all working to ensure that we retain our reputationand achieve our targets, for which I would like tothank you. I know that we can continue to be asuccessful hospital. Many thanks for going the extramile for our patients.

The Trust’s team of ultrasonographers are often unsung heroes, scanning pregnantwomen and carrying out general scans for hernias, gallstones and the like.

Tina Hutchings, Clinical Nurse Specialist in the Early Pregnancy Assessment Unit (EPAU),would like to give the ultrasonographers some “well-deserved recognition for their hard workand patience”.

Tina says: “We have seen an increasing number of patients and unfortunately this has broughtan increase in miscarriage diagnosis. It is difficult to give bad news at the best of times but tohave to break this to onepatient after another on somedays is very sad.

“I cannot run the EPAUwithout the ultrasonographers’support and so I just want tosay thank you to them.”

The ultrasonographers areplaying a key role in helpingthe Trust progress towardsachieving a maximum 18-week patient pathway for allpatients in the hospital.

Superintendent CarienMorabito says: “Our routinewaits for ultrasounds are nowdown to around two weeksthanks to the hard work anddedication of theultrasonographers who havebeen working early morningand late evening scanningsessions.”

As a result of an assessment of the risk ofjumping or falling from a height, a decisionhas been taken by the Trust to secure thestairwells around the atria, effectivelymaking them for staff use only.

From Monday February 11, all patients andvisitors will need to use the escalators and lifts totravel through the hospital and staff will use thestairs via swipe card access.

Card readers have been installed on either sideof each of the four stairwells (excluding the A&Estaircase which will be available to patients andvisitors who are unable to travel by lift).

From February 11 2008, the stairs will besecured and all staff ID cards will be able toaccess these doors - it is therefore vital that allstaff have current ID badges in order to use thestairs.

Please visit the Security office near mainreception as soon as possible if you do not have acurrent ID badge.

Focus on... Children’s Services – Safe in our hands

We are proud that our children’s servicesare among the best in the country.

A national review by the Healthcare Commissionrated their overall quality as ‘good’ - only 25% ofNHS trusts achieved a score of ‘good’ or ‘excellent’.

We bring together a range of general and specialistservices to provide integrated healthcare for childrenand their families in hospital, the community and athome.

Specialist services include:• Neonatal and paediatric surgery – our surgeons

carry out virtually all such surgery in North WestLondon

• Neonatal Intensive Care Unit – a regional centre ofexcellence

• Dedicated operating theatres for children• Dedicated 24-hour Paediatric Emergency

Department• Specialist nurses and play specialists• Hospital School

You can find out more about our children’s servicesby logging onto the Trust website atwww.chelwest.nhs.uk/services/womenchildren/paediatrics/index.html

Children and their families have enjoyed access tothe benefits of music therapy in the heart of theircommunity – thanks to an innovative partnershipbetween the Trust’s paediatric music therapists, two community groups and a local GP practice.

n Children’s services staff are pictured with a giantcheque for a grand total of £1,189.15 to be presentedto Children in Need.

Staff raised the money thanks to the generosity of staff, visitors and patients who dug deep back in November todonate money to Pudsey Bear and friends.

Leigh Marsh, Acting Community Co-ordinator inPaediatrics, said: “During an Institute of Leadership &Management course run by Desiree Cox, the team were set amanagement challenge of raising £500 for Children in Need.

“The management team and their colleagues met theirchallenge by raising more than twice that amount by askingstaff and the general public to contribute their spare change. It was an enjoyable day and we hope to do the same againthis year!”

Children in Need

The project was madepossible by the funding of anextra post in the musictherapy team at Chelsea andWestminster thanks to agenerous charitable donation– Claire Flower and HelenLiebmann, who job share thepost, have a remit to developoutreach projects.

Claire explains: “NatashaNascimento, a home learningworker with the charity Kidswhose post is partly fundedby South Westminster SureStart Children’s Centre, identified a need for a drop-in group for pre-schoolchildren with disabilities andtheir parents or carers.”

Kids is a charity that workswith disabled children, youngpeople and their families. TheChildren’s Centre programmeis based on the concept thatproviding integratededucation with childcare, family support and health

services are key factors inachieving good outcomes forchildren and parents.

Claire continues: “Wecalled the group MusicIsUsand it was well received byfamilies when we ran it oversix weeks last summer at SureStart but we couldn’t find avenue when we wanted torun a similar group duringNovember and December.

“Luckily for us, we heardabout the new premises thatthe Victoria Medical Centrehad recently moved into inUpper Tachbrook Street andso we approached the PracticeManager, Andrew Kolenda, who offered us the use of alovely big room on a weeklybasis.”

Andrew says: “We movedinto the centre in May 2007– we are now the largest GPpractice in Victoria – and wehave a group room that wewant to promote as acommunity resource.

“We are here to help localpeople as part of thecommunity and so we weredelighted to offer the use ofour facilities when Clairecontacted us.”

Claire adds: “Being able touse the GP practice was agreat help because it issituated in the local area ofthese families which increasedattendance. I would like tothank the Victoria MedicalCentre and the other partnersin this project which is a

really good example ofcollaboration between anumber of organisations.

“MusicIsUs has had areally positive effect on thefamilies that attended thegroup and we could seechanges in the children evenin that short period oftime.”

Natasha Nascimento fromKids confirms: “I could seehow these changes weretransferred outside theMusicIsUs sessions. Asparents realised the relevanceof these changes, they haverequested even morecommunity sessions.”

MusicIsUs demonstrateshow the Trust’s small musictherapy service continues tobuild and develop itsreputation as one of theleading specialist musictherapy services for childrenin the NHS – the team canbe contacted on ext 6472.

The Maud Van Norden CharitableTrust, via the hospital Friends, hasdonated the majority of the fundsrequired for a much-neededDomiciliary Sleep Study Recorder, which means that the Trust nowhas two.

Dr Suveer Singh, Respiratory andITU Consultant and lead for the TrustSleep Medicine Service, said: “The

Sleep Clinic is an expanding serviceand we are delighted to receivecharitable funding from the Maud VanNorden Charitable Trust. On behalf ofthe hospital, I would like to thankthem.

“The Domiciliary Sleep StudyRecorder has enabled us to diagnosemore patients more quickly withobstructive sleep apnoea which is anepisodic obstruction of the airways atnight, which causes fragmented sleepand noticeable daytime sleepiness.

“The sleep service, which started in2004, is of growing importance tohabitual snorers, overweight patientsand those with cardiovascular andneurological diseases.”

The Sleep Study Recorder or ‘sleepmachine’ is taken home or worn inhospital, depending on the patient’scondition. Most NHS therapists may not betoo concerned with the financialimplications and costs involvedwith patient admissions to hospitalin their day-to-day clinical work.

However, Therapy Services at Chelseaand Westminster Hospital are takingPayment by Results – the new NHSfinance system - seriously and have takenactive steps to gain a greaterunderstanding of it.

Natalie McFarlane (Operational LeadTherapist) invited James Peskett, PolicyManager at the Audit Commission, andPeter Broughton, Senior InformationDesign Consultant from TheInformation Centre for Health andSocial Care, to present a ‘Beginner’sguide to Payment by Results’ for all

therapists.This session was interesting,

informative, pitched at therapists as astaff group and provided an interactiveforum to practise the PbR process firsthand.

James and Peter co-ordinated andfacilitated a group exercise in which staffsimulated the complicated PbR processof diagnosis, coding, adding bed daysand the subsequent allocation of fundingrelated to an inpatient admission.

Staff who attended now have a muchclearer idea of the PbR system, why thissystem was implemented, how it worksin practice, and how it may affecttherapists of the future.

Therapy Services are now in theprocess of answering more than 50questions generated by the session andwill use this as the basis of futuretraining.
