Student Plans

HEALTH PLAN DOCUMENT DETAILS

Health Plan documents are created just like other documents as outlined in the Working with Documents part of this manual. Please refer to the following sections for details on completing specific parts of the Health Plans.

Health PlanThe Health Plan document contains five sections that must be completed before the document can be finalized:

Health Plan Assessment

1. After the plan is created, the Health Plan Assessment page is displayed with information to be completed.

2. Click in the text box and fill in the Assessment information according to your district’s guidelines.

The items labeled with red text and an asterisk (*) are required information that must be filled in if they’re not already.

If you don’t know all of the information at this time, you may leave some of it blank, but you’ll need to come back later and fill it in before you can finalize the document.

3. Click the Submit button A message is displayed indicating that the plan information was saved.

Medical Diagnosis

1. On the Health Plan Menu Bar, click Medical Diagnosis.

The Medical Diagnosis List is displayed.

2. Click the Add New Medical Diagnosis link. The Health Plan Medical Diagnosis Definitionpage is displayed.

3. Click on an item in the ICD-10/DSM-IV Area box to select it.

The list in the ICD-10/DSM-IV Code box is updated with the relevant items for the ICD-10/DSM-IV Area selected.

4. Click on an item in the ICD-10/DSM-IV Code box to select it.

5. Click the Show Descriptions button. A description of the selected Code item will be displayed.

You must click the Show Descriptions button to update the description if you click a different item in the boxes.

6. Click in the Sequence box and change the number, if you wish.

The system Health Plan Medical Diagnosis items in sequence as you add them. However, If you know what the sequence should be, you may set the sequence numbers manually as you go.

7. Click the Submit button. The Health Plan Medical Diagnosis Definition page is redisplayed with a success message.

Repeat to add any additional Medical Diagnosis information that is available.

8. On bottom portion of the Menu Bar, click Medical Diagnosis.

The Medical Diagnosis List is displayed showing all of the items you just added.

9. Click in the sequence number fields for each Medical Diagnosis and renumber them.

You may wish to have the items appear in a particular order (i.e., severity, code, alphabetical), depending on your district’s guidelines.

10. Click the Submit Sequence Changes button. The list is reordered according to the sequence numbers typed.

If you enter sequence numbers that are not numbered consecutively starting with 1, you will receive a warning message alerting you to this. Duplicate and out of sequence numbers ARE allowed by the system, but the Health Plan cannot be finalized unless they are in sequence.

Nursing Diagnosis

1. On the Health Plan Menu Bar, click Nursing Diagnosis.

The Nursing Diagnosis List is displayed.

2. Click the Add New Nursing Diagnosis link. The Search Library for Nursing Diagnosis page is displayed.

3. Click the Nursing Area drop-down menu and select a Nursing Area as appropriate.

This menu provides a customized list of Nursing Areas used in your district.

4. Click in the Search Patterns boxes and type additional search criteria, if you wish.

You may enter key words or parts of words to narrow your search within the selected Nursing Area.

If no Search Patterns are entered, all of the items in the selected Nursing Area will be found.

5. Click the Submit button. The Nursing Diagnosis Search Results page is displayed showing all of the items matching the search criteria.

6. Click in the checkboxes to select the Nursing Diagnoses you wish to use.

7. Click the Submit button. The Nursing Diagnosis List is displayed with the items you just added.

Sequence numbers may be changed in the same way that they are for Medical Diagnosis List.

8. Click in the sequence number fields for each Nursing Diagnosis and renumber them.

You may wish to have the items appear in a particular order (i.e., severity, code, alphabetical), depending on your district’s guidelines.

10. Click Edit for one of the Nursing Diagnoses on the list.

The Health Plan Goal Definition page is displayed.

You may edit the description as necessary, and any changes you make will apply only to this Health Plan and will not affect the description in the library.

11. Click the Submit and View Goals button. The changes are saved and the Health Plan Goal List is displayed.

12. Click the Add New Nursing goal from Library link.

The Choose Health Plan Goal list is displayed.

13. Click the checkboxes to select any goals you wish to use for this Health Plan.

14. Click the Submit button. The Health Plan Goal list is displayed with the goals you selected.

15. Click Add New Nursing Goal. The Health Plan Goal Definition page is displayed.

16. Click in the Goal box and type a description for this goal.

The description should relate to the Nursing Area selected.

17. Click the Submit button. The page is redisplayed with the Interventions and Expected Outcomes table.

18. Click the Add Interventions and Expected Outcomes from Library link.

The Choose Interventions and Expected Outcomes page is displayed.

19. Click in the checkboxes to select the Interventions and Expected Outcomes you’d like to use for this Goal.

Interventions are things the school will do, and Expected Outcomes are things the student will do.

20. Click the Submit button. The Health Plan Goal Definition page is displayed for the Goal and now shows the Interventions and Expected Outcomes that you just added.

21. Click the Add New Intervention link. The Intervention Definition page is displayed.

If the Interventions available in the library are not sufficient, you can create your own custom Intervention.

22. Click in the Intervention box and type the definition for the Intervention you’d like to add.

Remember, Interventions are things the School will do under this Health Plan.

23. Click the Submit button. The Intervention Definition page is redisplayed with a success message.

Add additional interventions, if you wish.

24. Click the Cancel button The Health Plan Goal Definition page is displayed for the Goal and now shows the Interventions that you just added.

25. Click the Add New Expected Outcome link. The Expected Outcome Definition page is displayed.

If the Expected Outcomes available in the library are not sufficient, you can create your own custom Expected Outcome.

26. Click in the Expected Outcome box and type the definition for the Expected Outcome you’d like to add.

Remember, Expected Outcomes are things the Student will do under this Health Plan.

27. Click the Submit button. The Expected Outcome Definition page is redisplayed with a success message.

Add additional Expected Outcomes, if you wish.

28. Click the Cancel button. The Health Plan Goal Definition page is displayed for the Goal and now shows the Expected Outcome that you just added.

29. Click the Cancel button. The Health Plan Goal List is displayed.

Notes Section

1. On bottom portion of the Menu Bar, click Notes.

The Health Plan Notes page is displayed.

2. Click in the box and type any notes you wish to include on the Health Plan.

This is the place to include information that doesn’t specifically belong elsewhere in the plan according to your district’s guidelines.

3. Click the Submit button. The Health Plan Notes page is redisplayed with a success message.

Health Plan GuardiansYou can work with Health Plan Guardians in exactly the same way that you work with Student Guardians. Please refer to the Student Information part of this training guide for details.

1. On bottom portion of the Menu Bar, click Health Plan Guardians.

The Health Guardian List page is displayed.
