GP Out of Hours (OOHs) Development Fellowship

Are you a recently qualified GP or a less recently qualified GP looking for a change? Are you looking for an opportunity to get some more experience in day time and OOHs

general practice and work as part of a regular team? Do you want some dedicated paid time to focus on professional development activities? Do you want to develop your Urgent general practice skills?

If so, we want you!

Come and be a GP OOHs Development Fellow working within both daytime practice and OOHs services, with additional funded time to concentrate on your professional development.

The role has three distinct components:

1. 14 hours per week for the local OOHs service – all types of out of hours shifts will be an option including evenings, nights and weekends;

2. 20 hours per week within a set GP practice – there will be the opportunity to choose a practice from those supporting this programme;

3. Professional development – there will be the opportunity to consider activities such as medical education, leadership or teaching alongside working on a Quality Improvement project.

This post is available up to 48 hours, at a pro-rata salary circa £85,000 but there is flexibility for reduced hours commitments.

INTRODUCTION TO NHS FORTH VALLEY

NHS Forth Valley is a single, integrated healthcare system comprising acute care facilities and community based services, which are delivered through Forth Valley Community Services Directorate.

NHS Forth Valley is the largest employer in the area, with approximately 8000 staff from a wide range of professional and support occupations. Currently, services are provided in one acute hospital, four community hospitals and fifty-six health centres. Acute Services are based in purpose built 860 bedded Forth Valley Royal Hospital, which opened in 2011 and provides inpatient and outpatient services for a population of approximately 310,000.

THE FORTH VALLEY AREA

The working environment in Forth Valley Royal Hospital is excellent, as is the quality of life that the Forth Valley area has to offer candidates and their families. The area has stunning scenery including the Trossachs and Loch Lomond and is steeped in history with Callander House in Falkirk, Stirling Castle, Bannockburn and the Wallace Monument to name but a few of the sites in the area. It also has many cultural and leisure facilities such as the Helix Ecopark, home to the 100ft Kelpies, the Falkirk Wheel, the MacRobert Arts Centre at Stirling and the National Sports Academy at Stirling University.

Forth Valley also lies equidistant between Scotland’s two largest cities; Edinburgh and Glasgow. With their respective airports, both are little over half an hour from most parts of Forth Valley, on the Falkirk, Larbert and Stirling area lines.

House hunters in the Forth Valley area are spoiled for choice. On offer is a large array of established and new properties in a variety of attractive settings.

Education in the area is first class, with excellent provision of pre-school, primary and secondary education. State schools in the area have strong reputations and there is also easy access to the area’s private schools in Dollar and Crieff. On the further education front, Stirling boasts an excellent University and access to the University cities of Glasgow and Edinburgh is easy. There are also further education Colleges in Falkirk, Stirling and Alloa.

In addition to its scenery, history, culture and shopping facilities, residents in Forth Valley have on their doorsteps a varied range of leisure and sporting facilities. There are marvellous golf courses, hills and mountains and everything from sailing and windsurfing.

1. Background

NHS Forth Valley (NHSFV) serves a population of approximately 310,000 over 2,643 square kilometres. The region ranges from remote rural areas in the North West to heavily industrialised areas in the South East. A central acute hospital is based at Larbert with community hospitals in Stirling, Falkirk and Alloa.

Out of Hours services are provided by Forth Valley Health Board, through clinician led unscheduled primary care services from 18:00 to 08:00 weekdays, all weekend, public holidays and protected learning time sessions for general practices. Care is delivered from three centres:

Forth Valley Royal Hospital: working closely with the Emergency Department Stirling Care Village : co-located with a Minor Injuries Unit, Xray and 3 GP practices Clackmannanshire Community Healthcare Centre.

2. Job Purpose

This post is aimed at those who have completed MRCGP and are looking for varied experience of primary care in order to provide a professional development adjunct to future clinical activity.

We recognise the unique position newly qualified GPs are in, in terms of career choices, geographical location, and being medically competent but wishing to gain further experience. During GP training, their GP trainer and local peer group at vocational training schemes support GP registrars. However, on completion of training this support ceases. At the end of GP training, registrars are competent but may lack experience. The first years after training can be a challenging time for new GPs who find themselves in the ever-changing world of modern general practice and the new independent practitioner may feel vulnerable and isolated. The purpose of this job is to increase experience to independently manage in and out of hours primary care situations.

This post will offer successful applicants the opportunity to develop their clinical and professionalcompetence in a purposeful and supervised manner.

Recent reports have highlighted the importance of leadership and development throughout the medical career. This post offers early exposure and training in this area.

Clinical development will be combined with supervised and directed activity linked to an area of mutual professional interest, including clinical teaching, healthcare quality improvement, and development of management and leadership skills.

At the end of the duration of this post, the post holder may achieve formal training and experience in Quality Improvement (QI) projects, the opportunity to learn context of healthcare through medical leadership academy, and make contacts and develop networks with other NHS Development and Leadership fellows. This will be dependent upon discussion with the successful applicant.

Providing High Quality Care to patients

The post holder will: Maintain GMC specialist registration and hold a license to practice Develop and maintain the competencies required to carry out the duties of the post Ensure patients are involved in decisions about their care and respond to their views

Personal and Professional Development (PPD)

PPD will consist of the following: Clinical teaching skills development through work with GP Training Programme Director

and Director of Medical Education in Forth Valley and Stirling University Management and Leadership skills development through the Medical Leadership

Academy and through Forth Valley Leadership Committees A quality improvement project that is primary care/interface based. It is anticipated that

the project will be written up so that it can be shared with other services in NHS Forth Valley

Research, Teaching and Training

7. Indicative Job Plan

Specialty: Primary Care

Contract: Full time / part time

Grade: Out of Hours GP Development Fellow

Places of Work: OOH PCEC at CCHC, FVRH and SCV, GP practice

The exact format of each post is flexible and to be mutually agreed with the successful applicant, the GP practice/ primary care setting and the Specialty Lead of Forth Valley GP OOH services . However, the following general principles will apply.

Within a 48hr contract, the fellow will contribute approximately: 20 hours Supervised Clinical activity within a GP practice 14 hours of OOHs [out of hours] clinical activity with Forth Valley GP Out of Hours

Service 14 hours Personal and Professional Development (PPD)

Within GP OOH services the successful candidate will have the following responsibilities

Responsible for: Provision with clinical and administrative colleagues of an unscheduled primary care service during the out of hours period for NHS Forth Valley. With responsibility for the prevention diagnosis and treatment of illness and for the proper functioning of the OOH service.

Type of activity Description of activity including when and where activity is conducted.

Telephone consultation

Clinical telephone consultation with patients and other care providers regarding patients. To assess and where considered clinically appropriate form a diagnosis, advise on and provide treatment for the patient. When considered necessary for safe care, the patient to be redirected to a face to face consultation, a home visit arranged or to another care provider.

Access to and use of the Adastra case management system. Documenting the consultation in the case management system.

Providing consultations to all patients in Forth Valley according to the priority displayed in Adastra, as allocated by NHS24 or Forth Valley OOH hub operator, and the time of call request.

Use of telephones in any of the OOH centres.

Consultations to be provided in between scheduled home visit and face to face consultations, or as a priority when requested by OOH hub staff.

Face to face consultation

Clinical face to face consultation with patients attending the OOH centres. To assess and where clinically appropriate form a diagnosis, advise on and provide treatment for the patient. When considered necessary for safe care, the patient to be referred on to another care provider.

Access to and use of the Adastra case management system. Documenting the consultation in the case management system.

Providing consultations to all patients in Forth Valley according to the priority displayed in Adastra, as allocated by NHS24 or Forth Valley OOH hub operator, and the time of appointment made.

Home visit consultation

Home visit consultation to patients on behalf of NHS Forth Valley. To assess and where clinically appropriate form a diagnosis, advise on and provide treatment for the patient. When considered necessary for safe care, the patient to be referred on to another care provider.

Access to and use of the OOH laptops through remote connection to the Adastra case management system to receive and review cases from the vehicles. Use of the OOH laptops through remote connection to the Adastra case management system to document the consultation and when onward referral of the patient is required, to forward the consultation notes to the secondary care provider.

Providing consultations to all patients according to the priority allocated by NHS24 or Forth Valley OOH hub operator.

Community Hospital visit

Provide the care as described for a home visit. Additionally document the consultation in the written clinical notes used in the community hospital, and record changes to medication on the written medication chart or directly on the HEPMA system if appropriate.

Dispense medication

Where it is considered clinically appropriate due to pharmacy opening times or ability to access a pharmacy, to dispense medication from stock available on home visits or in the OOH centres. To record the stock issued on the Adastra case record and on the paper prescription.

Professional to professional telephone support

Provide telephone decision support to other clinical professionals. Scottish Ambulance Service calls to be replied to within 15 minutes.

Patient related administration linked to clinical work.

In addition to the administration related to the above activities, transcribe consultation records sent from other NHS areas where the patient has opted to attend an OOH PCEC outside of Forth Valley.

Local clinical governance activities

Attend Significant Event Analysis (SEA) meetings.Contribute completed SEA reports to the SEA meeting.Complete a SEA report on any complaints received

Where agreed: Attend and contribute to OOH clinical governance meetings.

Act as clinical lead for shifts, supporting the hub operator in the clinical prioritisation of workload.

Training Provide supervision and support to GPSTs working in the OOH service. Support advanced nurse practitioners, and paramedic practitioners.

Details of the facilities and resources necessary to support delivery of the doctor’s duties and objectives for all programmed activities

Primary care level service facilities for consulting with patients, including but not exclusively:

Consulting room, chairs, desk, PC, lighting, power, heat and hand washing facilities.

Primary care examination equipment including stethoscope, ophthalmoscope, otoscope, Blood pressure monitoring, oxygen saturations monitoring, thermometer, urine multi-stick testing and pregnancy testing.

Vehicles and drivers to facilitate home visits.OOH centre receptionist staff, OOH hub operators and OOH operational management.

Access to and use of Adastra call management system, NHS clinical email, use of Clinical Portal and other clinical reference systems as agreed relevant to OOH service delivery.

Facilities in line with SPA agreed responsibilities and service objectives.

CONTRACTUAL DETAILS

Contracted activities for this appointment will be defined and agreed with the post holder, the Specialty Leads and the Service Manager.

This post is available on a one year fixed term basis.

The grade of this post is a Clinical Development Fellow (GP) at a salary of £85,000 per year based on 48 hours per week (also available at reduced hours)

The terms and conditions of service applicable to this appointment are the NHS Forth Valley Out of Hours Doctor terms and conditions of service as amended from time to time.

NHS Forth Valley does not negotiate salary placements. On commencement of employment, salary will be determined in accordance with the NHS Forth Valley Out of Hours Doctor terms and conditions of service

Formal appointment to this post will be conditional upon satisfactory completion of all pre-employment checks, including satisfactory medical screening.

This appointment will be superannuable under the NHS Superannuation Scheme, which is contracted out of the State Earnings Related Pension Scheme (SERPS). New employees will automatically be enrolled in the scheme unless they opt out. The employment is subject to one calendar months notice on either side.

Criminal Convictions - Protecting Vulnerable Groups (PVG) Scheme

The Protecting Vulnerable Groups (PVG) scheme has replaced the previous disclosure arrangements for people who work with vulnerable groups. This post involves the performance of exposure prone procedures and therefore the successful applicant will be required to produce evidence of their Hepatitis B and C Immunity Status prior to appointment and to undergo satisfactory health screening.
