1. 1\t the direction of Major James Connolly, I provided Inspector Genera l Glenn Cu nha and personnel assig ned to his office access to the drug laboratory located at 305 South Street , ,Jamaica Pla in during the period from January 18, 201 3 through February 22, 2013. _

2. On ,January 22, 2013 at approximately 1500 hours, Taylor Torino, Office of the Inspector General, fo und a plastic bag marked "found on floor room 363 , 5/12/05 . DCS", conta ining a white rock substance (reference Caption "A") on a secondary desl\ in the office previously assigned to Charles B. Salemi. The sample was secured under the laboratory's packaging gu idelines and ente red into the Laborato ry Information Managernent System (LIMS) unde1· Case Number 13-o-1770.

3. On January 23 , 2013 at approximately 1530 hours, Tay lor Torino , found multiple test tubes and other conta iners with unknown substances in them (reference Capt ions "B" and "C"), in a desk drawer in the office previously ass igned to Chmles B. Salemi . Oi1e of the conta iners vvas labeled "7/20/1983". The samp les were secured under the laboratory's packag ing guidelines and entered into the LIMS under Case Number 13-021 07.

4. On January 24, 20'1.3 . at approximately 1000 hours, Maya French, assigned to the Office of the Inspector General , found a heat-sealed package marked "623044 AMPHET MOM/\ TAR + PP" containing one tab let (reference Caption "0 '') in a fo lder marked "Qua li ty Checks" in a fi ling cabinet marked "OA/QC Data Forms + Record s". The sample was secured under the laboratory's packag ing guide lines and entered into the LIMS under Case Number '13-0-1947.

5. On January 30, 2013, at approximately 1500 hours, Taylor To rino found miscellaneous pill s (reference Caption "E") in a workstation drawer form erly ass igned to Lisa Glazer . The sarnple was secured under the laboratory's packag ing guidelines and en tered into the LIMS under Case Number '13-02499.

6. On February 1. 2013 at approximately 1345 hours , Rose Bagalawis , Office of the Inspector General, found green vegetab le matter and miscellaneous contents (reference Captions ''F'' and ·'G") in a drying hood cabinet located in the laboratory. The sample were secured under the laboratory's packaging guidelines and later entered into the LIMS under Case Number 13-03681.

7. On February 14, 2013 at approximate ly 1030 hours , Taylor Torino found miscellaneous pills (reference Captions "H", "I" and "J") taped to a lab bench desk cabinet in the laboratory . The samples were secured under the laboratory's packag ing guidelines and entered into the LIMS under Case Number 13-03488.
