Final Application (1003) and FHA addendum (92900a) completed in entirety and reconciled to supporting documents

Initial application and 92900a completed in entirety and signed by borrower and loan officer

Initial case number assignment printout from FHA Connection

Evidence all parties have been cleared through LDP and GSA lists

Initial disclosures (dated within three days of 1003): GFE (Intent to Proceed/Changed Circumstances)

Itemized List of Fees

TIL – Must be fully completed

Provider of Services Addendum

Servicing Disclosure

Anti-Steering Disclosure (FIMC Form required) lender paid transactions only

Notice to Homebuyer/Important Notice to Homebuyer (92900b)

FHA Assumption Disclosure

FHA ARM Disclosure (if applicable)

Borrower Authorization for Credit

For Your Protection (purchase only disclosure)

FHA Informed Consumer Choice Disclosure

Notice of Right to Copy of Appraisal Disclosure (ECOA) or Application Appraisal Disclosure with options for borrower to initial

Appraisal Processor Certification – (required if option to receive appraisal is checked on the Notice of Right to Copy of Appraisal disclosure)

FHA Identity of Interest Disclosure

Acknowledgement of Receipt of Homeownership Counseling

Homeownership Counseling Organizations List

Mortgage Loan Origination Agreement

Income/Assets : Tri-Merged Credit Report (Also for non-purchasing spouse if property is located in a community property state)

Supporting credit documents if applicable. (LOX’s,bankruptcy docs, etc.)

Employment/Income documents as required by TOTAL scorecard

Salaried borrowers: Most recent pay stub(s) covering a minimum of thirty days earnings and W-2s as required by findings

Self Employed borrowers: Signed federal tax returns for the most recent two years. Other Income: If shown on 1003,

provide documentation as applicable in accordance with TOTAL Scorecard findings and Total Scorecard Guide

Verification of funds as required by TOTAL Scorecard (bank statements, VOD) Gift letter, if applicable, with evidence of

donor’s ability to provide and complete documentation showing the paper trail of the transfer

Document the source of funds for any large non-payroll deposits

Canceled earnest money check if purchase transaction

Verification of social security numbers

Divorce Decree, Bankruptcy Documents or other legal documents as applicable

Fully executed sales contract/purchase agreement with all addenda if transaction is a purchase

FHA Amendatory Clause and Real Estate Certification signed by all parties

Property: Complete appraisal ordered through Fairway AMC (Frisco Lending Services)

If Subject property is less than twelve months old: completed FHA Builders Certification (#92541)

Refinance Specific (Non Streamline):

Payoff statement current for the month of closing

Tangible Net Benefit Form – completed and signed by borrowers

Existing loan documents: Note, mortgage, HUD 1 with original case number (FHA to FHA refinance)

Copy of recorded Warranty Deed in borrowers names

Evidence of borrower’s social security numbers
