Initial case number assignment printout from FHA Connection
Evidence all parties have been cleared through LDP and GSA lists
Initial disclosures (dated within three days of 1003): GFE (Intent to Proceed/Changed Circumstances)

Salaried borrowers: Most recent pay stub(s) covering a minimum of thirty days earnings and W-2s as required by findings
Self Employed borrowers: Signed federal tax returns for the most recent two years. Other Income: If shown on 1003,
provide documentation as applicable in accordance with TOTAL Scorecard findings and Total Scorecard Guide
Verification of funds as required by TOTAL Scorecard (bank statements, VOD) Gift letter, if applicable, with evidence of
donor’s ability to provide and complete documentation showing the paper trail of the transfer
Document the source of funds for any large non-payroll deposits
Canceled earnest money check if purchase transaction
Verification of social security numbers
Divorce Decree, Bankruptcy Documents or other legal documents as applicable
Fully executed sales contract/purchase agreement with all addenda if transaction is a purchase
FHA Amendatory Clause and Real Estate Certification signed by all parties
Property: Complete appraisal ordered through Fairway AMC (Frisco Lending Services)
If Subject property is less than twelve months old: completed FHA Builders Certification (#92541)
Refinance Specific (Non Streamline):
Payoff statement current for the month of closing
Tangible Net Benefit Form – completed and signed by borrowers
Existing loan documents: Note, mortgage, HUD 1 with original case number (FHA to FHA refinance)
Copy of recorded Warranty Deed in borrowers names
Evidence of borrower’s social security numbers
