This fall NEDSRA will be upgrading to a BRAND NEW registration software that will allow us to enhance our services to our users! This new registration software will allow participants, families, friends and community members the capability of viewing their household accounts online. Through the online system, users will be able to register for programs, view account status, update household information, print monthly calendars for enrolled programs, and so much more. We are thrilled for the capabilities and ease of registration that this new software will provide to our users! In order to process your fall registration in this new software, we will need you to submit a new 2019 Annual Information Form (AIF) BEFORE you can register for any fall programs, regardless of how you register. To make sure you can get into all your favorite fall programs, submit your AIF today, don't delay! Please be aware that we will need 3 buisness days to process your new AIF. After your AIF is processed, you will receive your online registration credentials and link to the registration website, via email.

Registration Notes: • The participant must have a 2019 AIF on file.• The account must be cleared of any outstanding balances• Payment in full is required.• Regardless of how you register, when your registration is processed you will receive an email notification. Important Tidbits:This fall there will be a registration window. The registration window is when programs will be open for you to register. The fall registration window will open on August 19 and will close on September 6. Registration deadline for all programs, unless otherwise noted, is September 6. Registration forms will not be accepted until the AIF has been received and processed by NEDSRA. Registrations received prior to 8:30am on the registration open date will be processed manually in random order. Processing registrations manually takes time. Registering online at 8:30am on August 19 is the most effective and efficient way to get into your favorite programs.

At Synergy Adaptive Athletics, we believe everyone should have equal access to the wide range of benefits that sports opportunities can provide. We help athletes with a primary physical disability realize those benefits by providing the tools and resources necessary to help each athlete perform to their utmost potential and to achieve their personal sports goals. Synergy Adaptive Athletics is a collaborative program of Northeast DuPage Special Recreation Association (NEDSRA) and Western DuPage Special Recreation Association (WDSRA) that provides adaptive sports opportunities to individuals of all ages with a primary physical disability.

BOCCIAAre you a wheelchair user with cerebral palsy or a high spinal cord injury? Boccia may be for you! Boccia is a precision ball sport contested by athletes on the local, national, and even international level. The sport is open to individuals with spastic hypertonia, dystonia, athetosis or ataxia in all four limbs or with severe locomotor dysfunction. Saturdays 2:00 to 4:00pm at Ackerman Sports & Fitness Center in Glen Ellyn.CLIMBINGClimbing fosters an amazing sense of community, trust, courage and accomplishment. It can be easily adapted to almost any ability level. Through climbing, you can break through barriers and summit to new heights. First and third Mondays of each month from 6:30 to 9:00pm at Vertical Endeavors in Glendale Heights.VI BOWLINGSynergy is proud to offer this program that enables ambulatory youth and adults with visual impairments the chance to improve their bowling skills in a social setting. Staff will provide instruction, adaptive resources and support to help you achieve success. This program is run in a drop-in format so you can sign up for one date or multiple dates throughout each season. Guiderails are provided. Saturdays 12:30 to 2:00pm at Fox Bowl in Wheaton.

ARCHERYArchery is an exciting lifetime sport that grows with you. Whether starting at a beginner level to learn the fundamentals of archery safety and basic technique, or to further develop more advanced skills, Synergy Archery will provide you with the tools and support to reach your goals. This program is open to anyone with a primary physical disability, including individuals with a visual impairment. Facility and dates TBA.

WHEELCHAIR BASKETBALLWheelchair basketball is designed for individuals with a primary physical disability affecting his/her lower body. Athletes with a spinal cord injury, cerebral palsy, spina bifida, amputation, limb length difference, ankle fusion, or other diagnosis that affects one’s ability to play competitive stand-up basketball, may qualify. Individuals do NOT have to be an everyday wheelchair user to play this sport. The sports wheelchair is simply a tool used to allow everyone to compete on an equal playing field. Winter Break Escape CampYouth with an Intellectual Disability or AutismLooking for something fun and exciting to do over winter break? This is the program for you! Winter Break Escape is broken up into six fun-filled days. Activities will include field trips, arts and crafts, cooking, Sensory Station time, and more! You can register for one day or all six. If your child requires medication during the program, a Medication Release Form must be filled out on the first day your child attends camp. Individual Music LessonsYouth and Adults with an Intellectual Disability or AutismHalf-hour, individual music lessons will help participants develop appreciation for music, promote fine motor skills and learn a new hobby. Participants can choose from keyboard, drums, guitar or brass. It is recommended that participants bring their own instrument so they can become comfortable with their instrument and continue to practice at home. Huskies Transition Basketball TeamYouth and Adults with an Intellectual Disability Your basketball legacy starts today! Be a part of NEDSRA’s newest basketball team, the Transition Huskies. This will be a NEDSRA team made up of students at Transition Programs within our member partner districts who are looking to stay active and competitive through Special Olympics. You will work as a team to compete against other high schools, as well as other Special Recreation Associations. We will also be competing in the Special Olympics tournament in January and the ITRS Tournament in February. A detailed schedule of games, practices and pickup times will be created and distributed during the first few weeks of the program. Not all games will be on a Tuesday evening to accommodate other high schools. The program will be once per week, with the exception of December 23 and 30. Please note that students of TLC and District 108 will have priority until after the registration deadline. Special Olympics Medical Form InformationIn order for an athlete to compete in Special Olympics sports, a current Special Olympics medical form must be on file at the NEDSRA office. It is valid for two years from the date the doctor signs the form. Medical forms ARE NOW ABLE TO BE FAXED OR SCANNED AND EMAILED. Please feel free to contact Sean Tovey at 630-576-4035 or [email protected] to see when your athlete’s application expires. Please use the application found on page 43 of the brochure.

The Booster Club Supporting NEDSRA Special Olympics Sports is a group of parents who realize and fill a need for NEDSRA’s Special Olympics athletes, and have been recognized by the Illinois Park & Recreation Association for fundraising efforts on behalf of these special athletes. The Booster Club supports more than 130 athletes, many who live on their own in group homes with no financial backing. Costs to participate in Special Olympics rise each year. TREC is a recreation-based program developed to meet the unique needs of transition-aged individuals with an intellectual disability who desire to reach their full potential through recreational and leisure activities and community involvement.

TREC Eligibility Requirements • Individuals must be able to eat independently.• Individuals must be between the ages of 18 and 38.• Individuals must be able to successfully function in a 1:6 staff ratio.• Individuals must be able to use the restroom and change themselves independently.• Individuals must actively participate in all aspects of program schedule and demonstrate flexibility.• Individuals are expected to demonstrate appropriate behavior with peers, staff and community members.

Individual Music LessonsYouth and Adults with an Intellectual Disability or AutismHalf-hour, individual music lessons will help participants develop appreciation for music, promote fine motor skills and learn a new hobby. Resident/Non-resident PolicyNEDSRA programs are supported by user fees, grants and donations, and subsidies from 11 partner communities through tax dollars. As a result, individuals residing within NEDSRA’s 11 partner communities are eligible for resident rates and are ensured registration priority. Non-resident registrations will be processed after the registration deadline. Additionally, residents qualify for door-to-door transportation when applicable and may qualify for fee assistance. Program FeesEach program description in the brochure lists a registration fee.

•All participants residing in one of NEDSRA’s partner communities are eligible for the resident fee – “Res. Fee.”•Participants residing outside of one of NEDSRA’s partner communities are considered non-residents. Non-resident fees are 50% more than the resident fee.

Suggested Ages & AbilitiesNEDSRA offers most programs and special events to targeted age and ability groups. Anyone wishing to participate in a program that does not appear to match his or her specific age or ability is encouraged to contact the NEDSRA office for additional information.

Inclusion ServicesNot every person with a special need will require NEDSRA programming. In some cases it will be appropriate for NEDSRA staff to work in cooperation with our member partners to advise and/or assist residents wishing to participate in recreational programs offered by their local park district or recreation department. We encourage that inclusion be considered as a recreational option. • One-to-one consultation • In-service training with park district and village recreation staff • Assistance with the aid of leisure buddies • Provisions of community resources These services are provided to residents at no cost. We will do everything possible to make your inclusion experience a success.

Putting People FirstNo eligible participant shall be denied equal access to programs, activities, services or benefits on the basis of race, gender, creed, national origin or disability, or be limited in the exercise of any right, advantage or opportunity.

Transportation ServicesDepending on registrations received, pickup and drop-off locations may be subject to change or cancelled if a two-person minimum is not met. Be sure to mark on your registration form if you will be using a pickup point.

A $10.00 fee will be charged for every 15 minutes, or portion thereof, that a participant is picked up late. A $10.00 fee will be charged when a participant does not report an absence in a timely manner and a NEDSRA vehicle is sent to the participant’s location for door-to-door transportation services. A notice will be sent to you indicating a fee being charged to your account. Continued tardiness at pickup times, or lack of cancellation for door-to-door routes, may lead to suspension from, or removal of, NEDSRA transportation services.

Brochure Changes/Error DisclaimerWe apologize for any errors and will make every effort to advise participants of any program changes and corrections as quickly and efficiently as possible.

Inclement WeatherPrograms may be cancelled due to severe or prohibitive weather. All participants will be notified if a program is cancelled. A maximum of one session per program will be rescheduled if possible. If a make-up session is not possible, a prorated refund will be issued.

Financial AssistanceWe believe all residents, regardless of financial limitations, should have access to NEDSRA’s programs. We offer a financial assistance program so that financial need is not an obstacle to having fun. Partial assistance is available for a maximum of two programs per season. A participant’s local park district or recreation department may offer additional financial assistance. Please contact their customer service directly for more information.

Illness PolicyNEDSRA has the right to refuse any participant who is ill or has a fever. If a participant becomes ill during a program, a parent/guardian or emergency contact will be notified and asked to pick up the participant.

NEDSRA Behavior CodeThe following guidelines have been developed to make NEDSRA’s programs safe and enjoyable for everyone. Additional rules may be developed for particular programs as deemed necessary by staff. Staff, participants and parents shall: • Show respect to all participants, staff and parents • Refrain from using foul language • Refrain from causing bodily harm to other participants, staff or parents • Demonstrate respect for equipment, supplies and facilities • Represent NEDSRA in a manner that is consistent with the goals, objectives and ethics of the Association. In addition, bullying behavior of any nature will not be tolerated.

NEDSRA reserves the right to dismiss a participant, staff, volunteer or parent if it is in the best interest of the Association. Each situation will be evaluated on its own merit.

Active Duty Military Fee Assistance Policy Fee assistance is now available to active duty military personnel and their dependents residing in NEDSRA’s member partner communities. For details, contact Nora Sandoval at 630-576-4010.

Program Fee RefundIf a participant withdraws from a program, a full refund is issued if requested two (2) days prior to the program’s start. If the refund is requested due to unforeseen circumstances such as hospitalization, relocation, or death in the family, a refund may be approved beyond this time frame. In these situations, written documentation may be required.

Refunds will not be granted when missing a program due to a planned vacation or as a result of the common cold, flu, etc. Refunds may be issued as a credit toward future programs or in the form of a check. If a program involves the purchase of tickets, contracted services, or non-returnable items, the cost of these may be deducted from the refund amount.

Promptness Is AppreciatedWe ask that participants arrive no earlier than 15 minutes prior to the start of a program or scheduled pickup time, and that they are picked up promptly at the end of a program or scheduled drop-off time.

Your Safety Is ImportantTo ensure the safety of our participants, we appreciate your adherence to the following guidelines: 1. NEDSRA staff is responsible for the safety of program participants during designated program times. Please adhere to scheduled drop-off and pickup times. 2. Participants enrolled in NEDSRA programs will not be released to anyone other than a parent, guardian or family member without prior written consent from the parent/guardian. 3. When arriving at or departing from program sites, parents should check in with a NEDSRA staff member before leaving or taking a participant home. 4. A child or minor will be allowed to walk home from a program location only if written permission from the parent/guardian has been received. 5. For programs involving door-to-door transportation, NEDSRA staff will pick up and drop off at the participant’s driveway. Arrangements should be made to accommodate participants from this point. 6. If you will not attend a program for which you registered, please notify the NEDSRA office.

Communicable DiseaseFor the protection and safety of both employees and participants, NEDSRA maintains specific policies and procedures on individuals with communicable diseases. These policies are available for review by the public at NEDSRA’s offices.

Instructions for Completing the Application for ParticipationThe Application for Participation (App) must be filled in completely. Apps with blank sections or attachments (exception: let-ter from State Office of Guardianship, 2a below) will not be accepted.

This App is valid for 2 years from the date of the examination date, regardless of the parent/guardian/Entrant signature date.

Parent/guardian and doctor signatures must be original and both original signatures must be on the same App form. Faxedsignatures, phone consents or verbal consents will not be accepted.

If Entrant was listed on the Sex Offender Public Registry or the Child Murder and Violent Offender Against Youth Registrybut has since been removed, contact the Special Olympics Illinois office for instructions before submitting this application.

ATHLETE INFORMATION AND HEALTH INSURANCE & EMERGENCY INFORMATION1. The first two sections must be filled in completely. The ethnicity information is requested to assist in the organizational outreachefforts.

a. The section must be signed and dated as printed. MEDICAL CLEARANCE3. The Medical Clearance section must be completed, signed and dated by a medical practitioner licensed to administer physicalexaminations by the state in which he/she practices. As of September 1, 1990, the Special Olympics Illinois Application forParticipation is the only Medical Clearance form which will be accepted as valid by Special Olympics Illinois.

This person, by signing the Medical Clearance, is stating that the athlete is in good health and can safely participate in SpecialOlympics sports training and competition. It is strongly suggested that the person administering the physical examination possess thefollowing:

The Chapter office will validate the Application for Participation and send a copy of the App with anapproved stamp back to the SOAD (Special Olympics Athletic Director). An Application for Participation will not be validated untilall information is correct and completed on the approved form.

5. Special Olympics Illinois requires that all Applications for Participation be presented prior to and no later than the establishedMedical App deadline of a Chapter championship level event (Winter Games, State Basketball Tournament, Summer Games,Outdoor Sports Festival, Fall Games, Floor Hockey or State Bowling Tournament). All Apps for the event in question must be validthroughout the completion of that Chapter competition.

Mandatory WaiverNortheast DuPage Special Recreation Association is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. NEDSRA continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for programs must recognize that there is inherent risk of injury when choosing to participate in recreational activities.

You are solely responsible for determining if you or your child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered any illness, injury or impairment, to consult a physician before undertaking any physical activity. NEDSRA carries no accident coverage on participants and the cost of medical attention and/or hospitalization will be the sole responsibility of the individual in question and/or their parent or guardian.

Warning of RiskDespite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activities. Not all hazards and dangers can be foreseen. Participants must understand that certain risks, dangers and injuries due to acts of God, inclement weather, slipping, falling, equipment failure, failure in supervision, premises defects and all other circumstances inherent to recreational activities exist. In this regard, it must be recognized that it is impossible for NEDSRA to guarantee absolute safety.

Authorization for Emergency Medical TreatmentI authorize NEDSRA to arrange emergency medical treatment, in the event of injury to my child or me and in the event that I or my designated emergency contact cannot be reached by NEDSRA.

Over 200 participants and 150 volunteers gathered for the 40th Anniversary of the NEDSRA Track & Field Meet. The meet is held each year at Glenbard East High School in Lombard. The event this year welcomed spectators, donors, superheros, princesses, new athletes and so much more!

The Annual BBQ Bash had quite a transition this year! Moving from indoors to outdoors, from April to June and even heading to Oakbrook Terrace for a scenic outdoor location! Enlarged outdoor yard games, a bounce house, crafts, and a playground welcomed families. NEDSRA looks
