1 Description As described by SAMHSA, ”integrated care is a team-based model of care, based on the blending of numerous provider disciplines’ expertise to treat a shared population through a collaborative treatment plan with clearly defined outcomes.” Teams vary across systems and organizations as they are influenced by setting, provider competencies, program goals, patient/consumer/population needs, funding perimeters, and other predetermined outcomes. This blended course will assist all members of the team to conceptualize their roles and responsibilities with a focus on key team-based concepts, including communication, practice flow, and shared decision-making strategies. Participants will work collaboratively during the session to develop a better team approach. Exercises provided will address specific team-based care topics such as: • Core components of team-based care • Impact of professional cultures on team dynamics • Practice flow • Patient identification • Shared treatment planning • Evidence-based interventions • Communications • Working with external referrals • Care management This program is an online blended course designed to provide a full knowledge base of team-based care methods and tools using a pre- and post-test, and lecture. The online version can be taken alone or with a live skills lab. The live skills lab is designed to be a fully interactive skills lab where participants will practice learned knowledge and facilitate practice change. This program will lay out how integrated care teams can effectively function and operate in various settings and address the current systemic challenges and cultural differences that can impede collaboration and integration. Participants will see and interact with experienced integrated care team members and practice various team roles to meet the whole person care needs of their patients. * Please note that your contact information will be shared with our joint provider, Vaya Health. FacultyLisa Tyndall, LMFT, PhD, is a licensed marriage and family therapist and an American Association for Marriage and Family Therapy approved supervisor. She received her doctorate in Medical Family Therapy from East Carolina University and currently serves as a Technical Assistant for the Center of Excellence for Integrated Care where she provides assistance to a variety of medical practice settings around integrated care program development. Her responsibilities include shadowing and one-on-one consultation, offerings of trainings, webinars, regular phone consultation and provision of resources and reference materials. She loves spending time with her two kids, fantastic husband, and endlessly optimistic German Shepherd.

Mel Ramage, RN, FNP, is a family nurse practitioner with focused training in high-risk pregnancy. She is a certified diabetes educator specializing in women who have type one and type two diabetes during pregnancy. She is also a certified advanced practice addictions nurse and is currently the clinical director of MAHEC Ob/Gyn Specialists’ Perinatal Substance Use Clinic/Project CARA. She is passionate about empowering patients in directing their own healthcare and is involved in local upstream movements to readdress healthcare delivery system. She also really enjoys a good run in the woods and watching Netflix.

Marie Gannon, LPC, LCAS, is a licensed clinical addiction specialist, professional counselor, and clinical supervisor with over 15 years experience working with patients with addiction disorders through the lifespan. She is the behavioral director of MAHEC Ob/Gyn Specialists’ Perinatal Substance Use Clinic/Project CARA. As co-founder of the MAHEC substance use clinic, her passion is providing opportunities for education to healthcare providers with the goal of reducing stigma against addiction. When she is not working, Marie loves snuggling her two kids and reading good books.

How to Register and ParticipateCreate an account at http://www.aheconnect.com/mahec. If you already have an account, click on “Login” to access your account. If you are new to the website, click “Signup” and setup your new account.

Register for a course. Click on the “Courses” tab, select a Whole Person Care Series course, and click “Register” to proceed.

Complete the course. After registering for the course, click on the “My CE” tab and locate your “Courses in Progress”. There you will find a link to the course. By clicking that course link, another window will open with all course information. Review the presentation, take the final examination, and complete the evaluation to activate your certificate link for that course.

Register for the live skills lab. After successfully completing the presentation and the exam, the live skills lab registration link will activate. Follow that link to the MAHEC registration page and complete your live skills lab registration.
