What?• Electronic Patient Discharge Plan (ePDP) is required
for patients discharged or transferred from SFGH to another facility (including inpatient psychiatry, 4A)
• If you include the Discharge Summary no additional note will be required after this is complete.
• Inter-service transfer summaries (e.g. MICU to medicine, medicine to surgery) should continue to be dictated or entered directly in LCR Reports/Notes section.

Why?• The new version is meant to provide improved
continuity of care between inpatient and outpatient care, improve the safety and quality of discharges at SFGH, and more clearly communicate the hospital course with other physicians.
• It is required to improve compliance with ACA and Joint Commission guidelines.

Who?• All patients discharged from a med/surg unit.• This includes patients who are occasionally
discharged from 5E/5R/4E and all patients discharged from 4B step-down and 5D telemetry.
• Med/surg and pediatric patients discharged from 6A.
• Outpatient surgery/PACU, ER, L&D, 6C/6H will not participate in this pathway.

Where?• LCR will have a new tab on the left column of the
patient’s record titled Discharge Process.
• Under this tab, select the ePDP option to complete the PDP for the patient.

When?• ePDP can be started at any time during the
patient’s hospital course (you are encouraged to start early!) and can be modified by any provider.

How?• The primary provider will complete the required
sections and any other applicable sections.• You may work on the ePDP at any point during the
hospitalization, using the Save button to save work as you go.
• When the provider is finished with PDP, he/she will use the MD Finalize button to submit and electronically sign the document.
• The ePDP and discharge instructions will ultimately be scanned and uploaded to the LCR in Reports/Notes.

here no additional summary needs to be written!

Clearer communication with outpatient providers is one of the benefits of the ePDP.
If PCP is within DPH/UCSF system, an email will be sent to the PCP advising him/her to review discharge documents.

Click the SAVE button located at
the bottom of each tab at any point while editing the
ePDP to save your work. You can then continue working on the document
or return at a later time.

After clicking the Save button, you
will be required to enter your
password to save your work.
You or any other provider may then continue working
on the saved version at any point in time.

You can edit any section of the ePDP by clicking on the
field and then hitting modify below or go directly to the relevant tab above.

IMPORTANT: Your signature on the FINAL
ePDP indicates that you are accountable for the entire content (even if multiple
providers contributed to the document over time).

To make edits to an already finalized ePDP,
go to Enter/Update PDP. Click Continue to make
changes to the document.
