SUMMARY

The Chittagong Fashion shares the 7 storied ( B+G+6), 26m high factory building with Overseas Apparels Ltd. under one the same management. The building construction completed in 2001 and the production started the same year. Presently, the factory has around 1000 employee working.

The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd.

(WEC). The purpose of the survey was to identify significant electrical safety issues and to provide

recommendations for remediation based on applicable standards specified by the Accord. The

scope of this initial electrical safety inspection was limited to the review and identification of major

electrical safety issues. The inspection did not include identification of minor deficiencies, which will

be further addressed as part of follow-up inspections.

Table below summarizes the major electrical safety issues identified during the inspection.

Recommendations have been provided to address each issue.

An implementation schedule shall be developed by the factory to remediate each of the

findings. The specific timing of improvements, including any requested extensions due to

design / installation constraints, shall be submitted to the Accord for approval.

Finding #: E- 1

There is no protection/barrier wall between transformer and surrounding area.

Category: TRANSFORMER ROOM

Finding: No protection between the transformer and surrounding area.

Recommendation: Construct a fire rated room for the transformers. Assign a qualified engineer to design a required transformer room according to BNBC, Section-2.6.3. The transformer must be installed with barrier walls between transformer and other panels. The walls must be fire resistant and should have height up to the ceiling. The wall should have the provision for necessary ventilation and fire rated door on required side.

No sufficient space to move around transformer inside its room.


Finding: Transformer room is congested.

Recommendation: Enlarge the transformer room as per standard (BNBC table 8.2.8) or maintain sufficient working space (preferably 1 meter) around the transformer.

Arcing horns for transformer HT Bushing not provided.


Finding:

Arcing horns on HT bushings of transformer, not installed.

Recommendation:

Arcing horns may be installed as per the transformer manufacturer's requirement. Consult with the transformer servicing company.

HT cable exposed near wall. HT and LT cable laid together in same tray.

Category: SERVICE LINE

Finding: HT cable laid over the LT cables.

Recommendation: HT and LV cables may be laid in different trays to avoid the crossover.

HT cable at certain places not protected, cable laid on floor.


Finding: Cable laid directly on concrete floor and not protected.

Recommendation: Remove all debris, dust and lint from HT cable. Install cable duct/tray to protect the HT cables and provide covers made of non-combustible material preferably metal to protect the cables’ insulation from any physical damage as well as prevent the ingress of debris, dust and lint.

Cable drawn in conduit entering building exposed at the entry point. Cable not fully protected at the entry

point.


Finding: Cable entering electrical room, through wall & fence, is not protected.

Recommendation: Cables passing through permanent walls must be protected in steel/PVC pipes and remaining holes around the pipe must be sealed with fire rated materials.

Protection of HT power cable above the ground is not adequate. Cable exposed from one side.


Finding: HT service cable dropping from pole is not protected near the base of the pole, above ground level.

Recommendation: HT cable dropping from HT pole must be protected in steel pipe of required size at least 2m from the ground level to protect the cable from any physical damage. The cable should be supported on covered tray or ladder throughout its length up to the HT panel base-plate (except the part of the cable laid underground at a depth of at least 1 meter).

Recommendation: Protective devices should be encased in metal casing made of 20 SWG thickness metal sheets.

Recommendation: Install cover plate on the top of the panel. Make circular hole at the top plate of panels and provide cable gland according to the respective cable size for cable entry and exit so that the cables are not stressed on the sharp edges of the hole of panels. Provide covers (of noncombustible material) if any additional gap remains after installing cable glands.

Recommendation:

Recommendation: Arrange periodic inspection & thermal scan to identify the overloading, loose connection, unbalanced load which may cause the burn and take action accordingly.

Recommendation: Disconnect the panel form power source and clean the interior of the panel regularly and seal the opening to protect ingress of lint and dusts. Provide covers if any additional gap remains after installing cable glands.

Multiple cable/wires terminated at MCCB


Finding: Multiple cables terminating to MCCB in panel.

Recommendation: Multiple cables connecting at a MCCB terminal must be removed. Individual circuit breaker must used for each load according to the respective cable-size.

Finding: Three phase MCCB connected to control two phases or less.

Recommendation: Check and redesign the requirements to control the circuits. If three phase protective device not required, the control devices suitable for the purpose may be selected (replaced).single phase circuit breaker may be used here.

Recommendation: Provide earth connection for body and doors of metallic distribution boards using green cables preferably braid so that the metallic door remains at zero potential all the time.

Recommendation: Cable terminating at Generator output terminal box must be supported on riser and protected. Install cable duct to protect the generator output cables and provide covers made of non-combustible material preferably metal to protect the cables’ insulation from any physical damage.

Recommendation: Cable ducts covered/closed with plastics (Polythene sheets) must be removed. Install metallic (non-combustible) cable duct over the floor and provide metallic cover on it to keep it dust and vermin proof. Keep 30% free inside cable tray/channels/ducts for proper heat dissipation.

Recommendation: Disconnect the power source of the cable laid into channel and clean dust and debris of all interior components. Establish a periodic cleaning program and maintain records of the activities. Provide cover made of noncombustible material on the channel for preventing ingress of dust and debris in future.

Recommendation: Connect generator earth directly to main earth strips with proper sized earth cable (green).

Recommendation:

Install additional lights in generator room to provide required illumination(about 150 lux) for ease of its maintenance work and operational work.

Recommendation: Remove the entire gas (CNG) cylinder from generator room. Any kind of combustible materials cannot be stored inside the generator room and near any electrical panel. Establish a routine cleaning program to keep the generator room neat, clean and dry.

Finding: Inadequate working space around the generators.

Recommendation: Enlarge the existing generator room to provide sufficient working clearance around generator as per BNBC table 8.2.9. or keep sufficient (1 meter preferably) around the generator for ease of its maintenance.

Recommendation: Generator room must be separated from the entrance with fire rated walls. . The wall should have the provision for necessary ventilation and fire rated door on required side.

Recommendation: Establish a routine cleaning program to keep neat and clean the transformer room. Shut the power of the transformer and clean the exterior of the transformer at scheduled period.

Re-wire able fuse used in wiring duct as protections for power socket out.


Finding: Rewire able fuse (cut out fuse) used for circuit protection.

Recommendation: Replace the semi enclosed rewire-able fuse with miniature circuit breaker (MCB) and place the protection devices inside an enclosed panel made of metallic sheet of minimum 20 SWG thicknesses.
