Interventionsbeispiele
FIGURE 1 . Patient 17. Enhanced computed tomography
(CT) scan. A, Before closure, patent ductus venosus (DV)
with no right portal branch (RPB) and distal left portal
branch (LPB) visible. B, Seven months after surgical
closure of the DV, (arrow) RPB and LPB are patent, with a
normal appearance. Note that the whole liver is larger
after closure. PV = portal vein; SMV = superior
mesenteric vein; SV = splenic vein.
FIGURE 2 . Patient 10. End-to-side communication
between the portal vein (PV) and inferior vena cava (IVC).
A, Angiography with direct opacification of the superior
mesenteric vein (SMV) with a catheter coming from the
IVC and going to the SMV via the shunt shows the
absence of spontaneous opacification of the PV and its
intrahepatic branches. B, Occlusion test with a balloon
inflated in the IVC at the site of the shunt and injection
via a catheter placed in the SMV shows the absence of
vessels going to the liver. C, Three months after banding
of the shunt, coronal reconstruction of a computed
tomography (CT) scan shows a cavernoma (arrow) with
hepatopetal flow on Doppler ultrasonography (US), and
patency of the left portal branch (LPB), which was not
visible on preoperative imaging. Note the large liver cell
adenoma (star).

Zusammenfassung:
Kongenitale portocavale Shunts haben hohes Risiko konsekutiver
Komplikationen.
 Shunts können spontan verschließen.
Korrektur ab dem 2. LJ zur Prävention der Folgekomplikationen (?).
 Rekonstitution des intrahepatischen (physiologischen)
Portalvenenflusses nach Verschluß in allen Fällen.
Regression bzw. kein weiterer Progress (PAH) der
Sekundärkomplikationen nach Obliteration in allen Fällen.
