Student Handbook 2010-2013

This edition of the Institutional Student Handbook familiarizes you, the students, with your second home, the De La Salle Health Sciences Institute. This will serve as your guide in establishing the foundation for your future careers.

Included in this new edition are the new campus development plan and

the DLSHSI creed which all students must know by heart. The policies on

student conduct stipulated in this handbook do not only apply once you

are in the campus. As Lasallians, it is expected that you uphold Lasallian

values even outside the school premises. The drug testing policy had

been further developed to aid in the drug prevention programs of the

institution. Likewise certain protocols for student activities have been

formulated to ensure a systematic way of monitoring these activities. In

addition, academic policies on adding and dropping of subjects, refund,

leave of absence were also drafted.

The Handbook is divided into four parts:

Part 1: YOUR FAMILY AND HOME, introduces you to the Lasallian Family

and the important people and sites in the campus.

Part 2: THE ACADEME, describes the policies and procedures from

admission until you complete your academic program.

Part 3: STUDENT SERVICES AND ACTIVITIES, orients you to the various

services that are available to you, and the co-curricular activities that we

encourage you to participate in.

Part 4: STUDENT CONDUCT AND DISCIPLINE contains policies that help keep

the peace and order in the campus. In this way, the campus becomes

conducive to learning and student development.

The last few pages of the handbook contain the Drug testing

acknowledgement slip (Appendix F), affidavit (Appendix G) and conforme

(Appendix H). Please accomplish these forms, have the affidavit

notarized and submit all accomplished forms to the Secretariat, Office of

Student Services, Basement, Wang Building.

Students under the MA in Nursing and PT Refresher Course are NOT

covered by the policies and procedures in this Student Handbook.

The Manual of Regulations for Private Higher Education (MORPHE) is

acknowledged as one of the basis for the formulation of provisions in this

handbook. The provisions in the MORPHE 2009 edition will supersede

any contradicting or conflicting statements in this handbook. Similarly, the

provisions in the same will also prevail in student issues and matters

which the student handbook does not address.

This Student Handbook shall be enforced effective from SY 2009-2010

until SY 2012-2013. The provisions of this Handbook shall take effect

upon its circulation to the DLSHSI students.
