TEACHING NARRATIVE

My main research areas inform the themes of my teaching and I have been called upon each year to lecture at grand rounds, continuing education conferences, and in graduate courses of epidemiology, ergonomics, disease management, and injury control because of being an expert in the following areas: Occupational epidemiology of musculoskeletal diseases, especially low back pain; injury epidemiology, especially physical and psychosocial occupational risk factors and measurement of job stress; disability research, especially prevention of chronic work-disability and facilitation of sustained and safe return-to-work; and cardiovascular disease epidemiology, especially the role of occupational physical activity, working posture, and psychosocial factors for incidence and progression of atherosclerosis, varicose veins, acute myocardial infarction, and other diseases of the circulatory system.

My focus in teaching, however, has been to mentor doctoral students and post-doctoral fellows, some of which have now become junior faculty at UCSF (Dept. of Social and Behavioral Sciences; School of Nursing; Dept. of Family Medicine and Osher Center for Integrative Medicine) and at universities abroad (Canada; Hongkong; The Netherlands). My commitment has been to assist doctoral students and post-doctoral fellows in launching their independent careers. Consequently, my mentoring activities have exceeded service on qualifying examination and dissertation committees and include an active role as research advisor, provision of relevant data sets, and active collaboration on publications and grant writing activities that have led to several co-authored published papers and grant awards, including two five-year NIH K23 career-development awards, one for Dr. W.E. Mehling at the Osher Center at UCSF and one for Dr. Carisa Adams-Harrison at UCB/UCSF.

Additional teaching/mentorship activities take place in the context of Community-based Participatory Research (CBPR) projects, one recently conducted among San Francisco Chinatown restaurant workers together with the Chinese Progressive Association, the Public Health

In this project, like in previous ones with hotel workers in San Francisco and Las Vegas, we have been providing leadership trainings for low-wage workers, including research skills such as survey development, administration and analyses. These activities resulted in 12 peer-reviewed articles, 3 with community members as co-authors, two book-chapters, and one PhD dissertation.

Summary of Public Service Activities

Because of my expertise in relevant areas I have been called by several California state government agencies and by CDC/NIOSH to review epidemiological evidence in the areas of workers’ compensation and occupational health service research and to identify research needs for a national research agenda (NORA). Further, I have been repeatedly sought for expert testimony by the Occupational Health and Safety Administration (OSHA) regarding proposed ergonomic rulemaking and also in legal cases concerned with occupational health and safety in specific industries.

In addition to providing expert advice to these research and regulatory agencies I have been working closely with employer organizations and labor unions in developing applied research projects including intervention research addressing specific problems in their industries and of underserved and under-researched worker populations at high risk for occupational injury and illness.

In some of these projects, namely in the hospitality and transportation industry, I conducted community-based participatory research (CBPR) projects that involved extensive consultations with worker advisory groups and labor and management representatives throughout study planning, design, execution, analyses, and interpretation phases. This approach requires considerably time but has the distinct advantage of a high chance that research results lead to the implementation of sustainable changes. In fact, I witnessed organizational changes, reductions in workload, and improvement of working conditions in the course of these projects. Most recently I have been involved in a CBPR project assessing working conditions and health of immigrant restaurant workers in San Francisco Chinatown involving independent community based organizations and the city’s Public Health Department.

Netherlands, served on the Research Advisory Council for the Institute for Work and Health (IWH) in Toronto, Canada, and on the research advisory committee of the Labor and Occupational Health Program (LOHP) at UC Berkeley.

RESEARCH PROGRAM AND CURRENT RESEARCH ACTIVITIES

The occupational epidemiology of musculoskeletal and cardiovascular diseases constitutes my main research focus. I am especially involved in investigating the etiology of work-related low back injuries, and how to predict and prevent prolonged work disability after these injuries. I have co-authored several methodological papers on how to measure job stress, low back pain, work disability, and return-to-work outcomes. In several review papers I synthesized evidence from different research disciplines suggesting that ergonomic and organizational work modifications are effective in facilitating return-to-work after occupational injury or illness. I have published a series of epidemiological studies showing that both biomechanical and psychosocial job factors play a independent and significant role in the etiology of low back pain, the incidence of formally reported spinal injury, and the duration of work disability. Similarly, regarding cardiovascular diseases, I published results from a prospective population-based study showing that both physical and psychosocial job factors play an independent and significant role in the progression of atherosclerosis, incidence of acute myocardial infarction, and in predicting disability retirement. My current research investigates interactions between worktime, shiftwork, occupational and leisure time physical activity in the etiology of CVD and mortality. The occupational determinants of behavioral and psychological risk factors and health disparities are another emerging focus in my research.

Ongoing activities as principal investigator include several research projects with different study populations and co-investigators, both locally and internationally. (1) The Las Vegas Hotel Room Cleaners (Culinary Workers Union, Local 226, and School of Public Health, UCB). This is a participatory research project with predominant female immigrant workers to study job quality and physical and mental health risks associated with ergonomic and organizational work conditions in this growing sector of the urban service economy. (2) MUNI Health and Safety Study (UCB and UCSF). This ongoing project comprises two public transit operator cohorts with 5-10 years of follow-up and comprehensive data on working conditions, health behaviors, work disability, and cardiovascular and musculoskeletal diseases. Both cohorts have been the basis for several publications on work and health among bus drivers and the work was funded through a NIOSH R01 grant to study physical and psychosocial predictors of low back pain and chronic disability. (3) The ongoing Kuopio Ischemic Heart Disease Risk Factor Study of occupational physical activity and circulatory diseases has been funded by NIOSH and the Finnish Academy of Science to determine risk and benefits of physical activities at work (University of Michigan, Ann Arbor, University of Kuopio, Finland, and UCLA). This is a prospective population-based study of middle-aged Finnish men with repeat measures at 4, 11, and 16 years of follow-up. In previous analyses of this cohort comparing sonographically measured intima media thickness of carotid arteries at baseline and 4-year follow-up I was able to identify prolonged standing at work as a new emerging risk factor for the progression of atherosclerosis with an effect comparable to that of heavy smoking. These findings have now been replicated with 11-year follow-up, and increased systolic blood pressure has been identified as one pathway. Long work hours and high physical job demands leading to high relative aerobic strain were also identified as a new risk factor for atherosclerosis. Ongoing analyses investigate associations between work posture, energy expenditure, work time and other measures of occupational activities with clinical manifest cardiovascular diseases and 20-year mortality.

As co-investigator, I initiated (4) a population-based study of occupational risk factors for hip pain and degenerative coxarthrosis led by the National Institute of Occupational Health in Denmark. Published results show that physical workload predicts the incidence of these conditions independent

(5-6) Two randomized ergonomic intervention studies of computer work conducted in collaboration with the Ergonomics Program at UCSF (Director Dr. David Rempel) determined the effectiveness of ergonomic interventions on upper extremity and neck disorders among Boeing engineers and Kaiser call center employees. (7) The ergonomic risks for upper extremity disorders investigated in several worksites in a NIOSH-funded Collaborative Study led by Dr. Rempel. (8) In cooperation with Dr. Franche at the Institute of Work and Health, Toronto, Canada, I am studying the psychological and occupational determinants of work-related disability and successful return-to-work after occupational injury. (9) In cooperation with Dr. Minkler (UCB), R. Baker (LOHP), Dr. Bathia (City of San Francisco), and community groups I have been conducting community-based participatory research with low-wage Chinese immigrant restaurant workers in San Francisco Chinatown to improve occupational health and safety for this vulnerable population.
