    Physical e:ams are re>uired of all students entering kindergarten. All students entering Medford Pulicchools from a different school district are also re>uired to have a recent physical e:am. A privatephysician or the school physician may give the e:am. tudents in grades four and nine are re>uired tohave a (ealth Kuestionnaire completed y their Parentuired. =t is especially important for the physician to indicate in ,riting ,hen astudent may safely resume full activity.
