Genèse du projet PIONeeR 2
Immune effector Tumor cell

Anti-PD-(L)1
Despite the ongoing immunotherapy revolution, lung cancer remains a huge biomedical challenge
Epidemiology
Most common cause of cancer death: ~1,59 million
5 years OS (advanced stages): < 5%
Treatment options
Targeted agents: ~15-20% of cases
1st and 2nd line chemotherapy
Immune Checkpoint Inhibitors (ICI) anti-PD-(L)1: the future backbone of cancer treatment
Push immuno-oncology further
Brahmer J et al. N Engl J Med 2015
Long term survival: ~25%
RATIONAL : PD1(L1) BLOCKADE IN LUNG CANCER
Objectifs Scientifiques 3
CONCEPT N°1: PROTOCOL PIONeeR – BioMarkers Program
UNDERSTAND BIOLOGICAL BACKGROUND BEHIND RESISTANCES TO PD1(L1) INHIBITORS
Objectifs Scientifiques 4
CONCEPT N°1: PROTOCOL PIONeeR – BioMarkers Program
UNDERSTAND BIOLOGICAL BACKGROUND BEHIND RESISTANCES TO PD1(L1) INHIBITORS
• Comprehensive multiparametric approach : tissue & blood biomarkers discovery & validation programs
• Periodic sampling, in responding &
non-responding patients
Objectifs Scientifiques 5
• Next generation ICI addressing mechanisms of anti-PD-(L)1 resistance targeting both innate & adaptive immunity
CONCEPT N°2: PROTOCOL PIONeeR –OPEN-LABEL MULTICENTER RANDOMISED UMBRELLA TRIAL
OVERCOME RESISTANCES TO PD1(L1) INHIBITORS
Objectifs Scientifiques 6
CONCEPT N°3 : ANTICIPATE NEW TARGETS

The goal is to assess the efficacy of a panel of anti-NKp44/46-L and anti-tumor cells mAbs generated by Innate Pharma, in healthy and tumor conditions with a special focus on lung cancer
BTLA
BTLA is an escape mechanism in cancer,
including NSCLC. Consequently anti-BTLA
mAbs restore tumor immunity. The goal is to
validate their efficacy and toxicity profiles in
vitro and in animal models, before translating
those findings to perform first in man studies.
7
The PIONeeR challenge is to
Understand PD1 /L1 primary resistances
Systematic re-biopsy
Large biomarkers program to predict benefit
Bypass the primary resistances
A randomized trial
Based on PDL-1 based combinations with 4 innovative candidate drugs
To improve OS, while minimizing IRAE
Develop the next generation of drugs & diagnostics in IO
New targets
To restore or boost anti-tumor immunity
Thanks to a large public/private international consortium of excellence
Precision IO diagnostics &
therapeutics, from PIONeeR concepts to product-based solutions for lung
cancer patients
