SCOPE OF SERVICES

The specific services needed by CDC Public Safety Department (PSD) from the winning PrivateSecurity Agency shall include but not limited to the following:

1) Protect the general interest of the Client (Clark Development Corporation or CDC).

2) To guard and secure all property of CDC or its officers/employees within its area ofresponsibility.

3) Strictly implement standing regulations, rules, guidelines and policies laid down bythe Client.

4) Prohibit the commission of any unlawful or illegal acts within the assigned CDCpremises or its area of responsibility.

5) Protect CDC or government property from theft, pilferage or damage.

6) Inspect and verify all documents of cargoes, goods, materials, products or similaritems, particularly the Bring-In or Bring-Out Permits, or Tally-In/Out Sheets comingin or out of Clark Freeport Zone.

7) Report and provide assistance in cases of any sign of disorder, riots, strike or anyserious violation of the law.

8) Report and provide assistance in cases of any emergency situations, such asvehicular accident, fire, typhoon, or natural calamities.

9) Perform their tasks in accordance with the Eleven General Orders, Code of Ethicsand Code of Conduct of Security Guards as stated in RA 5487 (Act Governing theOrganization and Management of Private Security Agencies).

A. MANPOWER QUALIFICATION AND OTHER CONDITIONS

In addition to the basic requirements of a Security Guard as stipulated in the Republic Act 5487,the Guards posted at CDC area of responsibility shall have the following specific qualificationand requirements:

a) Guards must be physically and mentally fit;b) Guards must be at least 5’4” in height;c) Guards must be at least 21 but not more than 40 years of age; d) Guards must have at least six (6) months experience as Security Guard;e) Guards must be College level or had 2 years in College;f) Guards must have undergone Basic Security Training from reputable institution authorized

or accredited by PNP Security Agency and Guard Supervision Division (SAGSD);g) Guards must have a valid Security Guards License issued by PNP SAGSDh) Guards with any history of drugs, violence of any type including deadly weapons violation,

theft of any types, fraud and/or forgery shall not be acceptable to CDC. Violation of thisrequirement will be a cause for contract termination.

i) Guards must understand oral and written instructions, policies, rules and regulations andimplement them in a tactful and non-confrontational manner.

j) Guards must have the necessary public relations & effective communication skills to dealwith employees and customers in a professional & courteous manner. They must maintainpoise, self-control, discipline, tact, diplomacy and mature judgment under stressfulsituations.

k) Guards must have a valid license to operate hand-held or base radios;

l) Guards must have valid Driver’s License (for Vehicle/Motorcycle Patrol units);m) All Guard Supervisors to be fielded by the Agency must be a college graduate, must have at

least three (3) years security related experience, possess a valid driver’s license, and musthave a Security Officer’s license issued by PNP SAGSD.

B. OBLIGATIONS AND RESPONSIBILITY OF THE AGENCYThe winning Supplier or Agency must have a Branch Office located inside Clark Freeport Zoneor within Region-3 prior the start of the contract. The Agency must secure a Certificate ofAccreditation with CDC PSD upon receiving the contract of security services. The Agency mustprovide all the equipment indicated in the CDC Minimum Required Equipment (Section VI & VIIof the Bidding Documents) prior the start of the contract. In addition, the Agency must complywith the following obligations and responsibilities:

1) The Agency shall assume all loses in their areas of responsibility due to theft/pilferages and negligence of their security guards. Serious violation of CDC Security policies, rules and regulations shall be sufficient to terminate the contract immediately and initiate legal action as warranted.2) Being the employer, the Agency shall assume full responsibility for the acts of its Security Guards during the performance of their duties.3) The Agency must cooperate with CDC Public Safety Department’s official investigation of

any suspected criminal activity or in case of theft or similar incidents in their assigned area.4) The Agency shall be responsible for the safety of their Security Guards in the performance

of their duties in providing security services to the Client (CDC);6) The Agency shall provide the total number of guards required in its assigned area every shift;7) The Agency must provide at least twenty (20) relievers for his specific Sector or area;8) The Agency must provide each Guard with one set of handcuff, baton, flashlight & raincoat;9) The Agency must provide each Guard on posts with firearm and hand-held radios;10) The Agency must provide each Guard with at least two (2) complete sets of Security

Guard’s uniform;11) The Agency must provide all equipment indicated in the Duty Rosters in every shift andshall immediately replace any equipment found to be defective or inoperative.12) All Security Guards equipment, tools and materials should conform to the applicable standards and must be covered with appropriate and valid licenses & documentsparticularly for issued firearms, hand-held radios, vehicles, etc.13) The Agency must provide at least four (4) qualified & licensed Security Officers who will supervise and oversee the Guards in their assigned Sector.14) The Agency shall have at least three (3) reserved Lady Guards ready for posting at

anytime.15) The Agency must ensure that all Security Guards to be supplied to CDC are properly

screened, and passed the neuro-psychiatric and drug tests. Copies of the results of saidtests shall be submitted to CDC PSD.

16) The Agency must ensure that Security Guards to be provided to the client shall undergoand passed the Physical & Medical Examination at CDC Health Office. Guards shall submitto the CDC Physician their chest X-ray P.A film result size 11x14 prior physicalexamination.

17) The Agency must provide trustworthy, efficient & reliable Security Guards.18) The Agency must ensure that the Guards to be posted at CDC have valid clearances from the National Bureau of Investigation (NBI), Phil. National Police, Regional or Municipal Trial Courts where they reside, Barangay and PSD Police Clearance. Copies of the said clearances shall be included in their individual Personnel Folder to be submitted at PSD.17) A Summary Inspection & Inventory Report of all Vehicles, Firearms & Equipment of the Agency shall be submitted every shift daily to PSD Manager for monitoring.

Upon issuance of the Contract, the winning Supplier shall request for the installation of aseparate electric meter for their base radio & battery chargers. Furthermore, excluded from thetotal number of Security Guards to be provided, the Agency must have one (1) Officer or Over-All Supervisor who shall oversee that the performance of the security services contract is beingcomplied. The designated OIC may be a Branch Manager or Operations Officer who shall bethe Supplier’s contact person at PSD until the duration of the contract. The said Supervisor shallestablish a routine communication with PSD and provide prompt and timely response to anyconcerns or problems that may arise during the contract period.

Time and frequency of direct meetings may vary as determined by CDC PSD. The OIC shallcontact PSD to review overall performance, receive special instructions regarding securityservices, or discuss other pertinent items regarding the contract and the Supplier’sperformance. The Agency is also required to provide free of charge two (2) Canine units for atleast five (05) working days in one year at eight hours per day, to be used by CDC PSD on “asthe need arise” basis.

C. UNIFORM AND APPEARANCE STANDARDS Guards shall be neat and clean in appearance and shall wear prescribed uniforms with the

appropriate Agency issued identification cards, and required paraphernalia prior to posting. All Guards must wear the PNP SAGSD prescribed uniform while working in CDC premises.

No personnel will be allowed to work within CDC without the appropriate uniform,identification card, and required firearm/equipment.

D. ASSIGNMENT AND REPLACEMENT OF PERSONNEL The specific Post Location or Assignment of the Guards to be posted within the area of

responsibility of CDC shall be determined by the CDC PSD.

The Post Location or assignment maybe changed whenever the exigency so requires,upon approval of the PSD Manager or his/her authorized representative.

The PSD Manager, Asst. Managers, Operations Officer, Field & Sector Commanders orauthorized representative of PSD shall have the right to inspect at all times the Guardsassigned on CDC PSD posts. The PSD Manager or his authorized representative mayalso order a certain post “Open” if the Guard on-duty was found with incompleteequipment such as Firearm, Radio, Motorcycle or similar required equipment.

Cross-posting of Guards on another Sector/Area shall be allowed only if they are fromthe same Security Agency. A Security Agency who won in the bidding of any two (2)identified Sectors/Area of CDC PSD may interchange their Guards on the saidSectors/Area only. Except or when in the exigency of the service which shall bedetermined by the PSD Manager when situation warrants that cross-posting of Guards inone Sector shall be allowed to be assigned in another Sector occupied by anotherwinning Agency.

No changes, replacement or substitution of Guards shall be made without prior approvalof the PSD Manager. The Agency is required to submit first a written request if it wish toreplace or pull-out any Guard from CDC posts.

The PSD Manager on the other hand, may request for the immediate replacement of anyGuard posted at CDC area at anytime and this does not need any further justification.

E. SCHEDULE OF WORKThe winning bidder shall commence with the work on the date specified in the contract for aperiod of three (3) years. The contract may be extended or renewed when the exigency of theservice so requires provided that the Agency has had an exemplary performance and providedfurther, that the extension is approved in writing by the approving authority of the contract.

The winning Supplier (Security Agency) shall provide a total of One Hundred Twenty-Nine (129)Security Guards for Forty Three (43) Posts. The schedule of work of Security Guards shall beeight (8) hours per shift, 24 hours a day, seven (7) days/week including weekends and Holidays

for all CDC Buildings and Facilities. Duty Hours shall be from 0700H – 1500H (Dayshift), 1500H– 2300H (Swingshift) and 2300H - 0700H (Midnight Shift). However, Security Guards must be atthe PSD Building thirty (30) minutes before the scheduled duty hours for regular Guardmountformation, inspection and briefing.

The number of Guards maybe increased or decreased whenever the exigency of the service sorequires upon written notice to the Agency, duly signed by the head of CDC or his authorizedrepresentative.

F. EVALUATION OF PERFORMANCE AND INSPECTION

1. The Performance of the Supplier will be evaluated quarterly based on the quality of work and its compliance with the terms and conditions of the contract.

2. Quarterly Security Contract Performance Review will be conducted to assess the performance of the Supplier in terms of Contract Compliance, Manpower Requirements

Supplies Inspection, Personnel/Work Performance and other related requirement which will form part of the Performance Review of the Supplier.

3. Supplies, vehicles, tools and equipment shall be inspected daily to determine its compliance with the contract based on submitted bids. The Daily Inventory Report is a

pre-requisite to billings.

4. The Supplier shall maintain a satisfactory level of performance throughout the term of thecontract. Before end of each year, the procuring entity shall conduct an assessment ofthe performance of the supplier based on the performance criteria. The performancecriteria, to be applied shall include, among others, the following:

Quality of Service Delivered Time Management Management & Suitability of Personnel Contract Administration and Management Provision of Regular Progress Report

5. Based on its assessment, the procuring entity may pre-terminate the contract for failureby the supplier to perform its obligations following the procedure prescribed under GPPBResolution 018-2004 dated 22 December 2004.

6. CDC PSD shall also provide a monitoring and inspection checklist for security services that

shall be handled by the PSD Technical Review & Administrative Committee (TRAC) whichis composed of the PSD Manager, the Asst. Manager for Operations, Asst. Manager forAdministration, and Head of Operations Section. PSD TRAC will take note of deficienciesand deviations from the required results, and shall provide rating for the conditions of allitems of work in the checklist.

The checklist form shall be used as a tool for auditing the performance of the securityservices in providing quality services at Clark Freeport Zone.

There are four (4) general items of work, namely;

a. Operational Vehicles, Tools and Equipment b. Attendance of Personnel c. Discrepancy or losses on Assigned Posts

d. Discrepancy of Individual Guards

G. ACTION ON OFFENSES

Effective Supplier- Client relation should consider the prevention of situation that will resultin disciplinary action or termination of contract. Given such condition, it is the responsibilityof CDC to impose penalty on offenses committed by the Supplier or winning SecurityAgency.

The action of penalty shall be that which is expected to improve the effectiveness of theSupplier’s personnel in rendering security services.

H. METHOD OF PAYMENT

Payment shall be made on a Semi-Monthly Basis and shall be based on the actualnumber of personnel deployed as verified and certified by CDC PSD.

Payroll of personnel shall be based on the following cut-off: 11-25th of the month and 26-10th of the month

The Supplier shall open an ATM Payroll account with CDC Accredited Banks upon thestart of the contract for all of its hired personnel posted at CDC PSD

The semi-monthly supplier’s billing will be based on the semi-monthly contract less thesemi-monthly payroll of the personnel for the project

To ensure continuous compliance of the Supplier on its legal obligations, authenticatedcopies of Official Receipts of monthly payments made to SSS, PAG-IBIG, PhilHealth andBIR shall be required from the Supplier during submission of billing statement to CDC

I. SITE INSPECTION

Before submitting their bid, the bidder is expected to have thoroughly familiarized himself withthe site or Sector. The bidder shall submit a letter requesting for a site inspection indicating thedate and time for the same. In the conduct of a site inspection, the bidder shall be accompaniedby an authorized officer to be designated by the PSD Manager or his authorized representative.

The bidder acknowledges that he has inspected the site and tenders his proposal cognizant toand accepts all the conditions and restrictions imposed by CDC. (Certificate of Site InspectionForm attached).

J. POST DUTY ASSIGNMENT & LOCATION

The winning Supplier (Security Agency) shall provide a total of One Hundred Twenty-Nine (129)Security Guards for Forty Three (43) Posts. All Security Guards posted at CDC PSD muststrictly follow the Eleven General Orders, Code of Conduct and Code of Ethics as stated in theRA 5487 and must comply with all the specific instructions required in their assigned post.

SCOPE OF WORKS

The specific services needed by CDC Public Safety Department (PSD) from the winning PrivateSecurity Agency shall include but not limited to the following:

1) Protect the general interest of the Client (Clark Development Corporation or CDC).

2) To guard and secure all property of CDC or its officers/employees within its area ofresponsibility.

3) Strictly implement standing regulations, rules, guidelines and policies laid down bythe Client.

4) Prohibit the commission of any unlawful or illegal acts within the assigned CDCpremises or its area of responsibility.

5) Protect CDC or government property from theft, pilferage or damage.

6) Inspect and verify all documents of cargoes, goods, materials, products or similaritems, particularly the Bring-In or Bring-Out Permits, or Tally-In/Out Sheets comingin or out of Clark Freeport Zone.

7) Report and provide assistance in cases of any sign of disorder, riots, strike or anyserious violation of the law.

8) Report and provide assistance in cases of any emergency situations, such asvehicular accident, fire, typhoon, or natural calamities.

9) Perform their tasks in accordance with the Eleven General Orders, Code of Ethicsand Code of Conduct of Security Guards as stated in RA 5487 (Act Governing theOrganization and Management of Private Security Agencies).

A. MANPOWER QUALIFICATION AND OTHER CONDITIONS

In addition to the basic requirements of a Security Guard as stipulated in the Republic Act 5487,the Guards posted at CDC area of responsibility shall have the following specific qualificationand requirements:

a) Guards must be physically and mentally fit;b) Guards must be at least 5’4” in height;c) Guards must be at least 21 but not more than 40 years of age; d) Guards must have at least six (6) months experience as Security Guard;e) Guards must be College level or had 2 years in College;f) Guards must have undergone Basic Security Training from reputable institution

authorized or accredited by PNP Security Agency and Guard Supervision Division(SAGSD);

g) Guards must have a valid Security Guards License issued by PNP SAGSDh) Guards with any history of drugs, violence of any type including deadly weapons violation,

theft of any types, fraud and/or forgery shall not be acceptable to CDC. Violation of thisrequirement will be a cause for contract termination.

i) Guards must understand oral and written instructions, policies, rules and regulations andimplement them in a tactful and non-confrontational manner.

j) Guards must have the necessary public relations & effective communication skills to dealwith employees and customers in a professional & courteous manner. They mustmaintain poise, self-control, discipline, tact, diplomacy and mature judgment understressful situations.

k) Guards must have a valid license to operate hand-held or base radios;l) Guards must have valid Driver’s License (for Vehicle/Motorcycle Patrol units);m) All Guard Supervisors to be fielded by the Agency must be a college graduate, must have

at least three (3) years security related experience, possess a valid driver’s license, andmust have a Security Officer’s license issued by PNP SAGSD.

B. OBLIGATIONS AND RESPONSIBILITY OF THE AGENCY

The winning Supplier or Agency must have a Branch Office located inside Clark Freeport Zoneor within Region-3 prior the start of the contract. The Agency must secure a Certificate ofAccreditation with CDC PSD upon receiving the contract of security services. The Agency mustprovide all the equipment indicated in the CDC Minimum Required Equipment (Section VI & VIIof the Bidding Documents) prior the start of the contract. In addition, the Agency must complywith the following obligations and responsibilities:

1) The Agency shall assume all loses in their areas of responsibility due to theft/pilferagesand

2) negligence of their security guards. Serious violation of CDC Security policies, rules and 3) regulations shall be sufficient to terminate the contract immediately and initiate legal

action 4) as warranted.5) Being the employer, the Agency shall assume full responsibility for the acts of its Security 6) Guards during the performance of their duties.7) The Agency must cooperate with CDC Public Safety Department’s official investigation of

any 8) suspected criminal activity or in case of theft or similar incidents in their assigned area.9) The Agency shall be responsible for the safety of their Security Guards in the

performance of 10) their duties in providing security services to the Client (CDC);11) The Agency shall provide the total number of guards required in its assigned area every

shift;12) The Agency must provide at least twenty (20) relievers for his specific Sector or area;13) The Agency must provide each Guard with one set of handcuff, baton, flashlight &

raincoat;14) The Agency must provide each Guard on posts with firearm and hand-held radios;15) The Agency must provide each Guard with at least two (2) complete sets of Security

Guard’s uniform;16) The Agency must provide all equipment indicated in the Duty Rosters in every shift and

shall 17) immediately replace any equipment found to be defective or inoperative.18) All Security Guards equipment, tools and materials should conform to the applicable 19) standards and must be covered with appropriate and valid licenses & documents

particularly 20) for issued firearms, hand-held radios, vehicles, etc.21) 13) The Agency must provide at least four (4) qualified & licensed Security Officers who

will 22) supervise and oversee the Guards in their assigned Sector.23) The Agency shall have at least three (3) reserved Lady Guards ready for posting at

anytime.24) The Agency must ensure that all Security Guards to be supplied to CDC are properly

screened, and passed the neuro-psychiatric and drug tests. Copies of the results of saidtests shall be submitted to CDC PSD.

25) The Agency must ensure that Security Guards to be provided to the client shall undergoand passed the Physical & Medical Examination at CDC Health Office. Guards shallsubmit to the CDC Physician their chest X-ray P.A film result size 11x14 priorphysical examination.

26) The Agency must provide trustworthy, efficient & reliable Security Guards.27) The Agency must ensure that the Guards to be posted at CDC have valid clearances

from 28) the National Bureau of Investigation (NBI), Phil. National Police, Regional or Municipal

Trial 29) Courts where they reside, Barangay and PSD Police Clearance. Copies of the said 30) clearances shall be included in their individual Personnel Folder to be submitted at PSD.31) A Summary Inspection & Inventory Report of all Vehicles, Firearms & Equipment of the Agency shall be submitted every shift daily to PSD Manager for monitoring.

Upon issuance of the Contract, the winning Supplier shall request for the installation of aseparate electric meter for their base radio & battery chargers. Furthermore, excluded from thetotal number of Security Guards to be provided, the Agency must have one (1) Officer or Over-All Supervisor who shall oversee that the performance of the security services contract is beingcomplied. The designated OIC may be a Branch Manager or Operations Officer who shall bethe Supplier’s contact person at PSD until the duration of the contract. The said Supervisor shallestablish a routine communication with PSD and provide prompt and timely response to anyconcerns or problems that may arise during the contract period.

Time and frequency of direct meetings may vary as determined by CDC PSD. The OIC shallcontact PSD to review overall performance, receive special instructions regarding securityservices, or discuss other pertinent items regarding the contract and the Supplier’sperformance. The Agency is also required to provide free of charge two (2) Canine units for atleast five (05) working days in one year at eight hours per day, to be used by CDC PSD on “asthe need arise” basis.

C. UNIFORM AND APPEARANCE STANDARDS Guards shall be neat and clean in appearance and shall wear prescribed uniforms with the

appropriate Agency issued identification cards, and required paraphernalia prior to posting. All Guards must wear the PNP SAGSD prescribed uniform while working in CDC premises.

No personnel will be allowed to work within CDC without the appropriate uniform,identification card, and required firearm/equipment.

D. ASSIGNMENT AND REPLACEMENT OF PERSONNEL The specific Post Location or Assignment of the Guards to be posted within the area of

responsibility of CDC shall be determined by the CDC PSD.

The Post Location or assignment maybe changed whenever the exigency so requires,upon approval of the PSD Manager or his/her authorized representative.

The PSD Manager, Asst. Managers, Operations Officer, Field & Sector Commanders orauthorized representative of PSD shall have the right to inspect at all times the Guardsassigned on CDC PSD posts. The PSD Manager or his authorized representative mayalso order a certain post “Open” if the Guard on-duty was found with incompleteequipment such as Firearm, Radio, Motorcycle or similar required equipment.

Cross-posting of Guards on another Sector/Area shall be allowed only if they are fromthe same Security Agency. A Security Agency who won in the bidding of any two (2)identified Sectors/Area of CDC PSD may interchange their Guards on the saidSectors/Area only. Except or when in the exigency of the service which shall bedetermined by the PSD Manager when situation warrants that cross-posting of Guards inone Sector shall be allowed to be assigned in another Sector occupied by anotherwinning Agency.

No changes, replacement or substitution of Guards shall be made without prior approvalof the PSD Manager. The Agency is required to submit first a written request if it wish toreplace or pull-out any Guard from CDC posts.

The PSD Manager on the other hand, may request for the immediate replacement of anyGuard posted at CDC area at anytime and this does not need any further justification.

E. SCHEDULE OF WORK

The winning bidder shall commence with the work on the date specified in the contract for aperiod of three (3) years. The contract may be extended or renewed when the exigency of theservice so requires provided that the Agency has had an exemplary performance and providedfurther, that the extension is approved in writing by the approving authority of the contract.

The winning Supplier (Security Agency) for SECTOR - 2 shall provide a total of Ninety (90)Security Guards for Thirty (30) Posts. The schedule of work of Security Guards shall be eight (8)hours per shift, 24 hours a day, seven (7) days/week including weekends and Holidays for allCDC Buildings and Facilities. Duty Hours shall be from 0700H – 1500H (Dayshift), 1500H –2300H (Swingshift) and 2300H - 0700H (Midnight Shift). However, Security Guards must be atthe PSD Building thirty (30) minutes before the scheduled duty hours for regular Guardmountformation, inspection and briefing.

The number of Guards maybe increased or decreased whenever the exigency of the service sorequires upon written notice to the Agency, duly signed by the head of CDC or his authorizedrepresentative.

F. EVALUATION OF PERFORMANCE AND INSPECTION

1) The Performance of the Supplier will be evaluated quarterly based on the quality ofwork

2) and its compliance with the terms and conditions of the contract.

3) Quarterly Security Contract Performance Review will be conducted to assess the performance of the Supplier in terms of Contract Compliance, ManpowerRequirements

4) Supplies Inspection, Personnel/Work Performance and other related requirementwhich will form part of the Performance Review of the Supplier.

5) Supplies, vehicles, tools and equipment shall be inspected daily to determine its 6) compliance with the contract based on submitted bids. The Daily Inventory Report

is a pre- 7) requisite to billings.

8) The Supplier shall maintain a satisfactory level of performance throughout the termof the contract. Before end of each year, the procuring entity shall conduct anassessment of the performance of the supplier based on the performance criteria.The performance criteria, to be applied shall include, among others, the following:

a. Quality of Service Deliveredb. Time Managementc. Management & Suitability of Personneld. Contract Administration and Managemente. Provision of Regular Progress Report

9) Based on its assessment, the procuring entity may pre-terminate the contract forfailure by the supplier to perform its obligations following the procedure prescribedunder GPPB Resolution 018-2004 dated 22 December 2004.

10) CDC PSD shall also provide a monitoring and inspection checklist for securityservices that shall be handled by the PSD Technical Review & AdministrativeCommittee (TRAC) which is composed of the PSD Manager, the Asst. Manager forOperations, Asst. Manager for Administration, and Head of Operations Section. PSDTRAC will take note of deficiencies and deviations from the required results, and shallprovide rating for the conditions of all items of work in the checklist.

The checklist form shall be used as a tool for auditing the performance of the securityservices in providing quality services at Clark Freeport Zone.

There are four (4) general items of work, namely;

a. Operational Vehicles, Tools and Equipment b. Attendance of Personnel c. Discrepancy or losses on Assigned Posts

d. Discrepancy of Individual Guards

G. ACTION ON OFFENSES

Effective Supplier- Client relation should consider the prevention of situation that will resultin disciplinary action or termination of contract. Given such condition, it is the responsibilityof CDC to impose penalty on offenses committed by the Supplier or winning SecurityAgency.

The action of penalty shall be that which is expected to improve the effectiveness of theSupplier’s personnel in rendering security services.

I. METHOD OF PAYMENT

Payment shall be made on a Semi-Monthly Basis and shall be based on the actualnumber of personnel deployed as verified and certified by CDC PSD.

Payroll of personnel shall be based on the following cut-off: 11-25th of the month and 26-10th of the month

The Supplier shall open an ATM Payroll account with CDC Accredited Banks upon thestart of the contract for all of its hired personnel posted at CDC PSD

The semi-monthly supplier’s billing will be based on the semi-monthly contract less thesemi-monthly payroll of the personnel for the project

To ensure continuous compliance of the Supplier on its legal obligations, authenticatedcopies of Official Receipts of monthly payments made to SSS, PAG-IBIG, PhilHealth andBIR shall be required from the Supplier during submission of billing statement to CDC

I. SITE INSPECTION

Before submitting their bid, the bidder is expected to have thoroughly familiarized himself withthe site or Sector. The bidder shall submit a letter requesting for a site inspection indicating thedate and time for the same. In the conduct of a site inspection, the bidder shall be accompaniedby an authorized officer to be designated by the PSD Manager or his authorized representative.

The bidder acknowledges that he has inspected the site and tenders his proposal cognizant toand accepts all the conditions and restrictions imposed by CDC. (Certificate of Site InspectionForm attached).

of their duties in providing security services to the Client (CDC);6) The Agency shall provide the total number of guards required in its assigned area every shift;7) The Agency must provide at least twenty (20) relievers for his specific Sector or area;8) The Agency must provide each Guard with one set of handcuff, baton, flashlight & raincoat;9) The Agency must provide each Guard on posts with firearm and hand-held radios;10) The Agency must provide each Guard with at least two (2) complete sets of SecurityGuard’s uniform;11) The Agency must provide all equipment indicated in the Duty Rosters in every shift andshall immediately replace any equipment found to be defective or inoperative.12) All Security Guards equipment, tools and materials should conform to the applicable standards and must be covered with appropriate and valid licenses & documentsparticularly for issued firearms, hand-held radios, vehicles, etc.13) The Agency must provide at least four (4) qualified & licensed Security Officers who will supervise and oversee the Guards in their assigned Sector.19) The Agency shall have at least three (3) reserved Lady Guards ready for posting at

anytime.20) The Agency must ensure that all Security Guards to be supplied to CDC are properly

screened, and passed the neuro-psychiatric and drug tests. Copies of the results of saidtests shall be submitted to CDC PSD.

21) The Agency must ensure that Security Guards to be provided to the client shall undergoand passed the Physical & Medical Examination at CDC Health Office. Guards shall submitto the CDC Physician their chest X-ray P.A film result size 11x14 prior physicalexamination.

22) The Agency must provide trustworthy, efficient & reliable Security Guards.23) The Agency must ensure that the Guards to be posted at CDC have valid clearances from the National Bureau of Investigation (NBI), Phil. National Police, Regional or Municipal Trial Courts where they reside, Barangay and PSD Police Clearance. Copies of the said clearances shall be included in their individual Personnel Folder to be submitted at PSD.17) A Summary Inspection & Inventory Report of all Vehicles, Firearms & Equipment of the

Agency shall be submitted every shift daily to PSD Manager for monitoring.

Upon issuance of the Contract, the winning Supplier shall request for the installation of aseparate electric meter for their base radio & battery chargers. Furthermore, excluded from thetotal number of Security Guards to be provided, the Agency must have one (1) Officer or Over-All Supervisor who shall oversee that the performance of the security services contract is beingcomplied. The designated OIC may be a Branch Manager or Operations Officer who shall bethe Supplier’s contact person at PSD until the duration of the contract. The said Supervisor shallestablish a routine communication with PSD and provide prompt and timely response to anyconcerns or problems that may arise during the contract period.

Time and frequency of direct meetings may vary as determined by CDC PSD. The OIC shallcontact PSD to review overall performance, receive special instructions regarding securityservices, or discuss other pertinent items regarding the contract and the Supplier’sperformance. The Agency is also required to provide free of charge two (2) Canine units for atleast five (05) working days in one year at eight hours per day, to be used by CDC PSD on “asthe need arise” basis.

C. UNIFORM AND APPEARANCE STANDARDS Guards shall be neat and clean in appearance and shall wear prescribed uniforms with the

appropriate Agency issued identification cards, and required paraphernalia prior to posting. All Guards must wear the PNP SAGSD prescribed uniform while working in CDC premises.

No personnel will be allowed to work within CDC without the appropriate uniform,identification card, and required firearm/equipment.

D. ASSIGNMENT AND REPLACEMENT OF PERSONNEL The specific Post Location or Assignment of the Guards to be posted within the area of

responsibility of CDC shall be determined by the CDC PSD.

The Post Location or assignment maybe changed whenever the exigency so requires,upon approval of the PSD Manager or his/her authorized representative.

The PSD Manager, Asst. Managers, Operations Officer, Field & Sector Commanders orauthorized representative of PSD shall have the right to inspect at all times the Guardsassigned on CDC PSD posts. The PSD Manager or his authorized representative mayalso order a certain post “Open” if the Guard on-duty was found with incompleteequipment such as Firearm, Radio, Motorcycle or similar required equipment.

Cross-posting of Guards on another Sector/Area shall be allowed only if they are fromthe same Security Agency. A Security Agency who won in the bidding of any two (2)identified Sectors/Area of CDC PSD may interchange their Guards on the saidSectors/Area only. Except or when in the exigency of the service which shall bedetermined by the PSD Manager when situation warrants that cross-posting of Guards inone Sector shall be allowed to be assigned in another Sector occupied by anotherwinning Agency.

No changes, replacement or substitution of Guards shall be made without prior approvalof the PSD Manager. The Agency is required to submit first a written request if it wish toreplace or pull-out any Guard from CDC posts.

The PSD Manager on the other hand, may request for the immediate replacement of anyGuard posted at CDC area at anytime and this does not need any further justification.

E. SCHEDULE OF WORK

The winning bidder shall commence with the work on the date specified in the contract for aperiod of three (3) years. The contract may be extended or renewed when the exigency of theservice so requires provided that the Agency has had an exemplary performance and providedfurther, that the extension is approved in writing by the approving authority of the contract.

The winning Supplier (Security Agency) for SECTOR - 3 shall provide a total of Eighty-Seven(87) Security Guards for Twenty-Nine (29) Posts. The schedule of work of Security Guards shallbe eight (8) hours per shift, 24 hours a day, seven (7) days/week including weekends andHolidays for all CDC Buildings and Facilities. Duty Hours shall be from 0700H – 1500H(Dayshift), 1500H – 2300H (Swingshift) and 2300H - 0700H (Midnight Shift). However, SecurityGuards must be at the PSD Building thirty (30) minutes before the scheduled duty hours forregular Guardmount formation, inspection and briefing.

The number of Guards maybe increased or decreased whenever the exigency of the service sorequires upon written notice to the Agency, duly signed by the head of CDC or his authorizedrepresentative.

F. EVALUATION OF PERFORMANCE AND INSPECTION

1) The Performance of the Supplier will be evaluated quarterly based on the quality ofwork

2) and its compliance with the terms and conditions of the contract.

3) Quarterly Security Contract Performance Review will be conducted to assess the performance of the Supplier in terms of Contract Compliance, ManpowerRequirements

4) Supplies Inspection, Personnel/Work Performance and other related requirementwhich will form part of the Performance Review of the Supplier.

5) Supplies, vehicles, tools and equipment shall be inspected daily to determine its 6) compliance with the contract based on submitted bids. The Daily Inventory Report

is a pre- 7) requisite to billings.

8) The Supplier shall maintain a satisfactory level of performance throughout the termof the contract. Before end of each year, the procuring entity shall conduct anassessment of the performance of the supplier based on the performance criteria.The performance criteria, to be applied shall include, among others, the following:

a. Quality of Service Deliveredb. Time Managementc. Management & Suitability of Personneld. Contract Administration and Managemente. Provision of Regular Progress Report

9) Based on its assessment, the procuring entity may pre-terminate the contract forfailure by the supplier to perform its obligations following the procedure prescribedunder GPPB Resolution 018-2004 dated 22 December 2004.

10) CDC PSD shall also provide a monitoring and inspection checklist for securityservices that shall be handled by the PSD Technical Review & AdministrativeCommittee (TRAC) which is composed of the PSD Manager, the Asst. Manager forOperations, Asst. Manager for Administration, and Head of Operations Section. PSDTRAC will take note of deficiencies and deviations from the required results, and shallprovide rating for the conditions of all items of work in the checklist.

The checklist form shall be used as a tool for auditing the performance of the securityservices in providing quality services at Clark Freeport Zone.

There are four (4) general items of work, namely;

a. Operational Vehicles, Tools and Equipment

b. Attendance of Personnel c. Discrepancy or losses on Assigned Posts

d. Discrepancy of Individual Guards

G. ACTION ON OFFENSES

Effective Supplier- Client relation should consider the prevention of situation that will resultin disciplinary action or termination of contract. Given such condition, it is the responsibilityof CDC to impose penalty on offenses committed by the Supplier or winning SecurityAgency.

The action of penalty shall be that which is expected to improve the effectiveness of theSupplier’s personnel in rendering security services.

J. METHOD OF PAYMENT Payment shall be made on a Semi-Monthly Basis and shall be based on the actual

number of personnel deployed as verified and certified by CDC PSD. Payroll of personnel shall be based on the following cut-off: 11-25th of the month and 26-

10th of the month The Supplier shall open an ATM Payroll account with CDC Accredited Banks upon the

start of the contract for all of its hired personnel posted at CDC PSD The semi-monthly supplier’s billing will be based on the semi-monthly contract less the

semi-monthly payroll of the personnel for the project To ensure continuous compliance of the Supplier on its legal obligations, authenticated

copies of Official Receipts of monthly payments made to SSS, PAG-IBIG, PhilHealth andBIR shall be required from the Supplier during submission of billing statement to CDC

I. SITE INSPECTIONBefore submitting their bid, the bidder is expected to have thoroughly familiarized himself withthe site or Sector. The bidder shall submit a letter requesting for a site inspection indicating thedate and time for the same. In the conduct of a site inspection, the bidder shall be accompaniedby an authorized officer to be designated by the PSD Manager or his authorized representative.

The bidder acknowledges that he has inspected the site and tenders his proposal cognizant toand accepts all the conditions and restrictions imposed by CDC. (Certificate of Site InspectionForm attached).

J. POST DUTY ASSIGNMENT & LOCATION

The winning Supplier (Security Agency) for SECTOR – 3 shall provide a total of Eighty-Seven(87) Security Guards for Twenty-Nine (29) Posts. All Security Guards posted at CDC PSD muststrictly follow the Eleven General Orders, Code of Conduct and Code of Ethics as stated in theRA 5487 and must comply with all the specific instructions required in their assigned post.
