SPONSOR REGISTRATION

Gold Level Sponsor — $1,200 Your company will receive acknowledgement with the use of a logo on all collateral, a foursome of golfers, tee and table sponsorship, the ability to hang your company’s banner at the lunch buffet, and two additional buffet invitations.

Silver Level Sponsor — $750 Your company will receive acknowledgement with the use of a logo on

all collateral, a foursome of golfers, and a tee sponsorship.

Cart Sponsor — $500 (2 max) Your company will receive acknowledgement on all collateral, and

your company’s name or logo will be displayed in all 72 golf carts.

Closest to the Pin Sponsor — $500 (1 max) In conjunction with LeaderBoard Air Cannon, your company will receive acknowledgment on collaterals and on a tee sign at the designated hole.

Breakfast Sponsor — $300 (2 max) Your company will receive acknowledgement on all collateral, and your company’s name or logo will be displayed on a banner during registration.

Tee Sign Sponsor — $150 Your company name or logo will be on a tee sign at a designated hole and your company will receive acknowledgement on all collateral.

Table Sponsor — $100 Company name or logo will be displayed on a table at the reception.

Team Entry — $500 A team of four will be entered in to the tournament. Please fill out and mail in the Team Registration form on the reverse side to enter a team.

Individual or Teams with less than 4 players — $125/person One golfer will be entered in to the tournament and will be paired with other individual golfers for the day. Please fill out and mail in the Team Registration form on the other side to enter as an individual(s) golfer.

Reception — $35 Individual guests can attend the buffet reception after the tournament has concluded.

Easterseals RI provides exceptional services to ensure that children with developmental delays, or those who have been diagnosed with a condition known to impact development, and their families have equal opportuni-ties to live, learn, work and play in their communities.

In 2016, we touched the lives of over 400 children and their families.

Easterseals Early Intervention (EI) is a program designed to support families in enhancing the development of their infant and child (birth to age 3). We provide a comprehensive evaluation in all areas of development to detect early problems in learning, language, motor and social skills. Our services include occupational therapy, family training and counseling, nursing, physical therapy and speech/language thera-py. All services are provided in a child’s natural environment.
