PREPARING FOR PARTICIPATION IN THE BBASI CT PROGRAM

Successful participation in the BBASI program is a collaborative process that begins in advance at home.

The Participant and Parents/Guardians should carefully review, know and follow the guidelines, rules and

regulations stated in the Registration Contract, Program Manual and any/all other Program Forms and

Materials (Forms & Materials). Reading all in entirety is a requirement of the BBASI Program.

All Participants attending the Program are required to be mature, focused, and able to behave responsibly at all times. Please also discuss with your Participant any individual suggestions and guidelines you have as a family for his/her stay away from home.1

I. BEING A RESPECTFUL MEMBER OF THE BBASI COMMUNITY

BBASI is a co-ed community and is comprised of hard-working dancers from diverse backgrounds, cultures and countries, and teachers/staff who work tirelessly to make this experience the most valuable it can be. Sensitivity to the rights, property and needs of others is expected from everyone. Some of the general guidelines to ensure a positive environment for all are:

Appropriate and respectful language and behavior is expected at all times. All are here to train hard, have fun and support one another. Bad attitudes and/or behavior will not be tolerated.

Please be sensitive to the fact that some Participants may not speak English and come from different cultures/countries.

Every Participant is expected to attend all classes/activities and to arrive on-time. Every Participant is expected to dress tastefully and appropriately at all times. If there is an issue/problem/emergency at any time, advise nearest BBASI Staff immediately. Do not

attempt to resolve any material situations by yourself. Cell phone/electronics use will only be allowed during free time and is prohibited during class,

Program organized activities, and after Lights Out. All are expected to respect the needs, wishes, privacy and property of others at all times.

II. BBASI CT CAMPUS

BBASI Program takes place on the open campus of The Westover School (Westover), in a rural town of Middlebury, CT. Classes for younger Participants are all on campus, while older Participants may also have class at an off-site studio a fifteen minute walk from campus. The Program’s supplemental activities take place on campus, with the exception of weekend trips. Westover’s campus is a 100-acre green private school campus which includes; studios, dormitory, a performing arts complex, dining hall, pool and other facilities, and encompasses natural fields, woods and lake areas. All participants and staff are housed at the Westover dorms. The facilities are managed and operated by its regular year-round staff, some of whom remain living and/or working there during the summer. Westover remains open in an administrative mode and runs a limited youth Program during BBASI. Please note that BBASI cannot ensure an allergen-free environment at Westover’s facilities. BBASI may utilize an off-site studio as part of the Program. This studio is located approximately 0.4 miles from Westover’s main campus and requires the crossing of one road in front of Westover School and a short walk along the sidewalk pathway. If used, only Participants ages 12-14 would be utilizing this off-site

studio and would be accompanied by BBASI Staff at all times. Upon check in at Westover, you may wish to walk the distance yourself. To grant such permission visit your online BBASI Dashboard > “Forms” tab > “Campus Authorizations” Web Form.

Supervision Program’s on-site staff, including; the Coordinating and Artistic Directors, senior staff, and counselors are paid staff residing on campus for the duration of the Program. Lifeguards and nursing staff are present part-time. Volunteer parent chaperons are additionally used for off-campus trips. The Program’s medical care is supervised by an off-site licensed Medical Director. Participants ages 12-14 are allowed to navigate the campus, in groups of three of their own choosing, with the express written permission of their parent/guardian. To grant such permission visit your online BBASI Dashboard > “Forms” tab > “Campus Authorizations” Web Form. All younger Participants are required to remain with their assigned group and counselors at all times. All Participants are supervised on any off-campus trips.

Residency at Westover The dorms are situated on the second and third floors of the main building, with a limited number of air-conditioned rooms available. The Program staff resides on the same hallways as students. Sleeping area corridors are locked after 8pm, and can be accessed only using BBASI Staff security key cards. Propping open ANY of the doors is a major safety violation and is PROHIBITED. Male and female Participants are housed in separate corridors. Participants are expected to respect this division and the privacy of others at all times. There are numerous common areas for group activities. All Resident Participants will be required to sign a Residency Agreement which details relevant policies and expectations in full.

Respect for Property of others Individual and all common areas (including studios and changing areas) must be kept in a clean, safe and undamaged condition at all times. All are expected to treat Program’s facilities and the property of others with respect and care, and to be responsible in caring for her/his own property. Breakage, damage, or other repair needs should be reported to a counselor immediately. A Participant will be held responsible for any damage to property which occurs as a result of his/her action.

Visiting and Temporary Departure Policy To ensure the safety of all participants, NO GUESTS/VISITORS are permitted on campus at any time, other than during scheduled visiting days. If you would like to join your Participant on the off-site trips, we invite you to volunteer as a chaperone. A Volunteer Chaperon form can be found on the online BBASI Dashboard > “Forms” tab. Closer to the date of the trip, you will be contacted by the Coordinating Director if you are needed to assist. Please note that only chaperons confirmed by the Coordinating Director are allowed on trips. To ensure the safety of all, Participants are NOT allowed to leave the Program during a session for any reason (inclusive of weekends, excepting emergencies2 and supervised weekend off-site trips organized by the Program). For Participants enrolled in the six week session, parents/guardians will have the opportunity to visit with their Participant on campus, or temporarily withdraw their Participant off-campus

during the weekend between sessions. For more information about Visitor’s Weekend, please review Section VIII. VISITING DAYS on page 12.

Enjoying the Outdoors in State of CT The State of Connecticut is rich in its nature and wildlife3. While that is ideal for outdoor summer activities to supplement the time spent in ballet studios, we ask that you prepare your Participant in advance for living in a rural environment. Suggested guidelines which should be reinforced are:

Stay away from woods, lakes and any other parts of campus not utilized by the Program. Wildlife and all unknown animals are safe when humans stay away from them. No matter how cute

an animal may seem, always:

Calmly and immediately walk away from the animal, facing it at all times. Immediately notify staff of the sighting.

When participating in Program’s outdoor activities (outdoor fun, bon-fire, swimming, etc.) always: Apply your insect repellant4/sunblock lotion all over your body and clothing (for insect

repellant). Inspect your immediate area to ensure no visible bugs, insects, mosquitos, etc. Stay away from areas which have visible bugs, insects, mosquitos, etc. If such are sighted,

notify staff. If you believe you have been bitten, have a rash, and/or any other condition from contact with nature

and wildlife, notify staff immediately and ask to be taken to a nurse.

State of CT offers valuable information to all visitors. We encourage you to review it with your Participant for best preparation.5

III. HEALTHY LIFESTYLE

BBASI CT is dedicated to a healthy training environment and we insist on all Participants: Eating, at a minimum, 3 healthy meals daily Staying hydrated Getting sufficient sleep

Meals Westover dining hall is operated by a professional staff committed to offering a diverse and a healthy variety of foods so that each Participant should be able to find something they enjoy. Food is served in a buffet style format, with visible labeling of each offering. Resident Participants Other than on weekend off-site trips, the participants eat in the Westover dining hall three meals per day.

In addition, a morning and an afternoon healthy snack will be served during class breaks (Mon-Fri). During off-site trips, participants will be given an allowance to use for purchasing food. Non-Resident Participants Non-resident Participants will be served lunch, as well as a morning and an afternoon healthy snack during class breaks. Please ensure that the Participant eats a healthy breakfast, at least one hour before arriving to class in the morning. Unfortunately, Day Participants cannot remain for breakfast and/or dinner.

Allergies

While all food offered at meal times is visibly labeled, BBASI cannot ensure a nut, gluten, latex or any other allergen free environment. Participants are required to be aware of his/her own eating requirements and/or other restrictions, and be responsible enough to follow the protocol developed prior to arrival at the Program. Any allergy related concerns can be discussed in advance with the Coordinating Director and must be clearly noted in the Program’s medical forms accessible on the “Forms” tab of your BBASI Dashboard.

Special Situations BBASI is dedicated to the belief that in order to be a healthy and successful dancer, one must be a healthy eater, stay hydrated, have substantial body weight and strength, and must get sufficient rest. As such, we do monitor the general well-being of all Participants. If, at any time, BBASI has any concerns about a Participant’s weight and/or ability to participate in class, BBASI reserves the right to require that the Participant is confidentially evaluated by the medical staff. For clarity of all, BBASI adheres to medically accepted BMI for determining safe, healthy and age appropriate weight levels. In the rare cases that the Participant may be determined to have a BMI level outside of healthy levels, BBASI, as in all medical situations, will be guided exclusively by instructions of its Medical Director and Program Director.

IV. TRAINING AS AN EDUCATED AND HEALTHY DANCER BBASI CT Participants come from various training backgrounds. Class training becomes progressively more difficult as the Teacher assesses the capability of participants and works to achieve appropriate progress. A strong emphasis will be placed on proper placement, technique and overall strength development. This may result in dancers feeling soreness and aches. It is of utmost importance that a Participant adheres to the following guidelines at all times:

A dancer knows her/his body best and must, at all times, self-moderate the training. Immediately advise the Teacher and/or Program Staff of any discomfort/pain.6 Delay in notification

may aggravate an injury. In most cases, the soreness/ache may just require a dancer to slow down a little and/or take a short

break. Academy’s Teachers/Program Staff will always be mindful of such situations and will work with the Participant so that they do not feel left out.

Class Placement At the start of each session, all participants are placed in groups based on the audition results. However, during the first 3 days of class, all participants are evaluated by the Academy’s Teachers and adjustments in placements may be made. BBASI Staff make their best effort to make the adjustments, if needed, in the most non-disruptive manner for the Participant. It is important that both the Participant and

parents/guardians understand that such changes are made strictly to ensure the most level appropriate and injury free training.

Class Dresscode Every participant is expected to dress according to the dress code in class. Female Dancers

Pain Identification / Communication / Action Procedures Just as with sports training, discomfort, aches, pains, and injuries are a known occurrence in ballet training. As such, in collaboration with the NYU Hospital for Joint Diseases Harkness Center for Dance Injuries, BBASI Programs have designed an approach to better educate and equip BBASI participants for managing their training. It is of utmost importance that parent(s)/guardian(s) and Participants jointly review, know and follow the information presented below. Participants should report discomfort/pain when it occurs. This will enable BBASI staff to offer support

and to engage in the proper course of action. Delay in reporting an injury/pain may aggravate it. BBASI’s course of action will be a direct response to Participant’s identification of the pain as per the

UNIVERSAL PAIN ASSESSMENT TOOL7. Parent/Guardian must ensure that the Participant is familiar with the UNIVERSAL PAIN ASSESSMENT TOOL below, as well as BBASI’s stated response actions.8

7 UNIVERSAL PAIN ASSESSMENT TOOL (UPAT) will be available as a reference throughout the Program. Parents/Guardians must

ensure that Participant knows how to use it, but does not need to memorize it. 8 BBASI encourages Parents/Guardians to discuss examples of past incidents with their dancers and where they would fall on this

UPAT. This would give Participant working benchmarks to rely on during BBASI, if such need arises.

Slips, Falls, Contact with another Participant The ballet training process challenges each dancer to learn new ballet elements and to develop one’s technique. It is a process during which slips, falls and contact with another participant may occur. Please discuss with your Participant BBASI’s standard procedures and responses to such incidents as outlined below:

A) BBASI Teachers feel strongly that highlighting such incidents during class brings emotional discomfort to those involved. As such, these type of incidents are ignored and the class continues without any attention by staff.

Dancer has difficulty getting up/continuing

A) Counselor will be called for assistance B) Protocol stated in the Universal Pain Assessment Tool will be followed.

Head impact/injury occurred during the

Any symptoms, post such incident, involving: headache, dizziness, fogginess, nausea should be immediately brought to attention of staff and Participant will be taken to nurse or appropriate medical specialist for evaluation.

Special Situations If at any time there is an issue/incident which is not addressed by stated guidelines above, the Participant

should advise the counselors immediately. If, at any time, the Participant has a condition which prevents

him/her from active participation in class, the Participant is invited to observe the class. Learning from

observation is very valuable and will ensure that the Participant remains on-pace with the class/Program.

At all times, the Participant will be required to abide by respectful behavior while class is in session.

Supplemental Activities BBASI CT includes other non-ballet activities, such as outdoor fun and free time. Stated guidelines will be applied to any/all incidents.

V. HEALTHCARE POLICIES AND PROCEDURES All at BBASI take the health of each Participant very seriously. Please discuss with your Participant that s/he should always and immediately notify Program staff of any problems or concerns. A licensed nurse will be available on-campus for regular visits, medication administration, and any other needs during the day. At all other times, a nurse will be always on call. The Program’s Medical Director is Town and Country Pediatrics and Family Medicine, PC which is based in Watertown, CT. If the need arises for a physician’s /specialist’s visit, the parents/guardians will be notified and the Participant will be taken to the physician / specialist by BBASI (applicable fees for the visit will be charged).

Medication (Prescription and OTC) CT Dept. of Health prohibits self-administration of any prescription and over-the-counter medications in youth programs. As such, all medications, vitamins, food supplements, etc. must be turned over to the nurse at check-in. All must be in unopened packaging and clearly labeled with Participant’s details and dispensing instructions. A physician’s written order must be included with ALL. The nurses will be available at check-in day to discuss any questions you may have. Please be advised that CT Dept. of Health mandates that all medication must be away from the reach of minors. In cases of legitimate requests for accommodations to emergency medication (ie – EpiPen, etc.), BBASI CT will need to be assured that such medication will be kept by the Participant in a safe and secure way preventing access by others. As such, it will be the responsibility of the family to provide a safekeeping device (we suggest something like a small bag with a lock/Packsafe, etc.) To grant such instructions visit your online BBASI Dashboard > “Forms” tab > “Medical Treatment & Administration of Medication” Web Form.

Communication The BBASI CT nurse/Coordinating Director will contact parents/guardians if Participant:

Admitted overnight to the health center Has a 101o fever for more than 24 hours Has a medical emergency Needs to visit any off-site medical specialist, such as a physician, dentist and/or OPT. (Any such

visits will be charged to parent’s/guardian’s insurance/credit card on file) Parents/Guardians may communicate directly with the Program nurse on routine matters. In cases of non-routine matters, and to ensure proper Program oversight, please contact the Coordinating Director first.

Lice Prevention & Screening Lice have become a common occurrence in schools and other places with large quantities of minors. While they are not harmful, they do cause substantial disruptions to all involved. As such:

Each family should thoroughly check their Participant prior to arrival to the Program. Please perform this check at least 3 days before Program's start, as sometimes a treatment, if necessary, may need to be repeated after the initial application.

Upon check-in, each Participant will be screened by either a BBASI Nurse or a Lice Technician. If there will be a positive identification, the parent/guardian must either:

Take the Participant home and perform their own treatment, or Have the licensed technician perform the treatment on-site. (*Please note that a treatment

may run as much as $200 - $300 or more, depending on the complexity of the process).

Please note that NO PARTICIPANT will be accepted into the Program until official confirmation of a lice free status. This includes RESIDENT and NON-RESIDENT Participants.

Immunization As required by state law, all schools and youth programs are mandated to ensure that participants have age appropriate immunizations and records of such. State laws also require schools and youth programs to grant exemptions from vaccination for legitimate medical and religious reasons.9 BBASI complies with state laws and understands that there are legitimate circumstances which require special accommodation of such requests for exemption. If your family qualifies for such an exemption, please contact the Program Director to obtain full details and required steps to determine compliance. If no advance clearance for an exemption is obtained from the Program prior to registration, and a request for exemption is not granted at a later time, Program will adhere to its standard Refund Policy. However, and in the case of RESIDENT FAMILIES ONLY, BBASI also believes that each family should be provided the option of voluntarily agreeing or declining to agree to allow their Participant to reside with an unvaccinated participant, regardless of the reason for such decision. Please refer to Roommate Preference Form on your BBASI Dashboard to confidentially advise BBASI of your preference. To respect privacy of all Participants and their Families, BBASI will not disclose the names of Participants who have been granted an exemption from vaccination, nor will BBASI disclose the decision of any family regarding allowing their Participant to reside with an unvaccinated Participant.

VI. ZERO TOLERANCE POLICY

The following conduct is subject to a Zero Tolerance Policy and in case of a violation, the Program reserves the right to immediately dismiss from the Program without any refunds for tuition and/or housing. Any dismissed Participant must leave the Program immediately. No smoking or tobacco use of any kind on or near the premises of any Program facility and/or activity. No consumption or possession of alcoholic beverages/drugs/any other illegal substances at any time. Sexually oriented behavior is prohibited, including public displays of affection. No physical / sexual / emotional abuse, threats, intimidation, commission of any violent act, and/or

other conduct which threatens or endangers the health, safety or emotional well-being of another. No harassment of another person, whether physical, sexual, verbal, written or via internet. No vandalism, defacement of property or facilities. No possession, use and/or display of weapons or harmful materials. Participants are expected to eat 3 healthy and complete meals daily, stay hydrated, and get sufficient

rest and sleep. No disrespect to your fellow Participants, faculty, staff, and chaperones. No self-harmful behavior including refusal to eat, causing oneself to be sick, or any other bodily harm.

VII. DISCIPLINARY ACTION

BBASI knows that all of its Participants are dedicated to maximizing this unique opportunity afforded them by their families and BBASI staff. As such, we would like all to be clear on BBASI’s policy in rare cases that

there is a need for disciplinary action. Please note that Program’s counselors/teachers are not allowed to discipline participants in a material manner. Only the Program Directors / RAF Executives have the authority to confidentially discuss any actions/measures with either the Participant and/or the parent/guardian. Zero Tolerance Policy. If a Participant is found to be in violation of the Zero Tolerance conduct, BBASI

reserves the right to immediately dismiss the Participant from the Program. In such cases, the Parent/Guardian will be asked to make immediate arrangements for the Participant to depart.

Attendance & Lateness. After 3 unexcused absences BBASI reserves the right to dismiss the Participant from the Program. Repetitive lateness will not be tolerated, as it is disruptive to class and other participants.

Electronics. While the use of cell phones and other electronics is allowed during free time, using these devices is prohibited during class and after Lights Out. Any violations will result in the electronics being collected.

Other Program Policies. Intentional and/or repeated non-compliance with Program Policies may lead to disciplinary action and/or dismissal.

BBASI CT understands that with so many Participants from different cultures, places, and ages some less serious issues may arise. In such cases, the Coordinating Director will discuss the issue with the Participant/s and will notify the parent/guardian in writing. If the issue arises again, the Participant and parent/guardian will be given a formal warning that a repeat will lead to dismissal from the Program.

We are thrilled to say that we have found your dancers to be hard-working, respectful, delightful and truly appreciative of this great experience. We want to ensure this continues. If you have any concerns, whether about your own Participant or someone else, we ask you to notify the Coordinating Director. Please do not contact counselors/teachers/or any other summer staff.

VIII. VISITING DAYS

In order to ensure an enjoyable experience for all, visitors are kindly asked to observe the following rules: Please do not arrive early. All are asked to adhere to Program’s Zero Tolerance Policy. NO PETS ALLOWED – We know your pets are part of the family but please leave them at home. Program welcomes families taking photos and videos in a non-disruptive manner and from a fixed

location. Program will also be professionally taping the performances and videos will be available for purchase.

Following the open class performance on Friday, July 15th, six-week Participants may wish to leave campus with their families and/or friends. In order to ensure the safety and well-being of all Participants, BBASI requests that those families wishing to do so abide by one of the two following options: Off Campus Dinner: Visitors may temporarily withdraw their Participant during check-out for

three week Participants, and return him/her to campus no earlier or later than 9:00PM -9:30PM. Off-Campus Weekend: Visitors may temporarily withdraw their Participant during check-out for

three week Participants, and return him/her to campus no earlier or later than 7:00PM - 9:30PM on SUNDAY, JULY 17th.

If you are the family of a six week participant wishing to take advantage of either of these options, please indicate your preference on the CampSite web form “Visitors’ Weekend Permissions”

IMPORTANT: Designated visiting days are the only days/ times when the Program Participants are allowed to have visitors. Visiting at any other time during the Sessions is not an option.

IX. ARRIVALS & DEPARTURES

Resident Participant Check-In (Westover School): Resident Participants should plan to arrive on Sunday (June 26th or July 17th) between HOURS OF: 12 PM – 5 PM EDT (Arrivals outside of this time frame will not be processed). ADDRESS: 1237 Whittemore Road, Middlebury, CT 06762 To minimize registration lines, RAF will be implementing a staggered arrival schedule. Each family will receive email instructions about how to sign up for a registration time slot available on a first come, first serve basis.

Upon arrival:

BBASI Staff will register you and make sure all pre-registration materials are in order. BBASI CT nurses will review medical records to discuss any questions/ requests/ special instructions.

- You must give the nurse all your prescription and/or OTC prepackaged medication, vitamins, etc. at this time. PLEASE NOTE: for safety of all, no medications, vitamins, etc. are to be kept in the Participant rooms. The nurse will administer the medication to all who so require. If you have medication which you must always have in your possession, please refer to section V. Healthcare Policies and Procedures on page 10 of this manual.

Each Participant will be screened for lice as per Program’s lice policy. If you plan on leaving any spending money for your Participant, there will be a designated station

during check-in time. In order to ensure that all have a record of this transaction, please do not leave money with BBASI Staff in any other way/time.

You will receive a welcome packet, including your schedule and studio assignments (*Please note these may change after first few days of placement classes).

You will be introduced to the counselors and your roommates. It will be the responsibility of each family to survey the room for damage and report anything unusual. Participants will be held responsible for any damage caused to their assigned room or common areas.

PARTICIPANTS & PARENTS WELCOME ORIENTATION The Westover School Dining Hall

Session 1 & Session 6 weeks – Sunday June 26th: 2pm, 4.30pm Session 2 – Sunday, July 17th, 2pm, 4.30pm

Parents / Guardians may leave after the orientation session, but must inform the counselor that they are departing.

Resident Participant Check-Out (Westover School): Participants may check out either the Saturday following the last class day of class before 10AM, (July 16th or Aug 6th), or that Friday (July 15th or Aug 5th) after the performances. Please advise the Program staff of your decision in advance. Please make sure that:

Program staff surveys the room prior to your departure and records that no damage was done. You take back your petty cash, medications and all private belongings. RAF bares no responsibility

for unclaimed items. It is your responsibility to check your flight arrangements for any changes.

Non-Resident Participant Check-In (Westover School): BBASI requires that all non-resident Participants check in on Sunday, (June 26th or July 16th). All Participants will be screened by the Program’s medical staff prior to starting the Program. Please refer to page 14 for hours and the address of the Westover School. During the Program, a guardian will be required to sign in the non-resident Participant in the morning. If you will not be the person signing out your non-resident Participant at the end of the day, CT Program Staff must have a written note with details on who that person will be.

Non-Resident Participant Check-Out (Westover School): Non-resident Participants will be escorted from their last ballet commitment to the main Westover School entrance. The same individual who signed the Participant in, will be required to pick the Participant up. For any other arrangements, a written note from the parents must be received in advance by RAF. RAF is pleased to offer non-resident Participants the option to participate in the afternoon activities at no additional cost, but they must depart prior start of dinner.

Other Non-Resident FAQ: 1.) Can my non-resident Participant join in extracurricular activities?

Non-resident participants are welcome to join free of charge in all activities (lunch included) up to, but not inclusive of, breakfast and dinner on weekdays. All evening/weekend activities are for resident participants ONLY (excluding Bloch fittings). This includes weekend field trips (unless a parent/guardian is a confirmed chaperon).

2.) Where/what time should I drop off/pick up my non-resident student? Both drop off and pick up will be at the main Westover School entrance on Whittemore Rd. All non-

resident Participants must be dropped off/picked up ONLY from Westover School’s main entrance.

BBASI understands that many non-resident participants want to spend some time with their friends

and that parents/guardians work long days. As such, BBASI schedule is structured with these

considerations in mind.

In the morning. Participants are typically dropped off between 8:45am – 9:00am, depending on

placement. Consult your Participant’s schedule (distributed upon check-in at Westover School)

for more precise times. Please be on time, as Participants travel together from activity to

activity, the entire group will be held up due to one person’s tardiness.

In the afternoon/evening. Participants may choose to leave either a) immediately following

their final ballet commitment, or b) directly preceding dinner at around 5.30pm.

3.) Can I buy a ticket for my non-resident student to attend weekend field trips? The only way to guarantee that your non-resident student will receive a ticket for a weekend

field trip is if the parent/guardian of the student is a CONFIRMED CHAPERON for the trip. To volunteer as a parent chaperon, please see the Volunteer Chaperon Form on your BBASI Dashboard’s “Forms” Tab.

X. PRIVATE LESSONS

Some Participants may want to request supplemental lessons in addition to the regular weekly schedule. Private lessons are available after regular class times and on the weekends for an additional fee. Please note that these classes are in no way mandatory, nor will affect the Teacher-Participant relationship during group instruction. Additional information on how to register for private lessons will be sent separately.

XI. CAMPSITE – BBASI’S CENTRALIZED INFORMATION PORTAL

Using your BBASI Dashboard, (http://raf.campmanagement.com/campers) you will have access to all

program forms and information. Using CampSite, you will be able to manage several aspects of you

Participant’s relationship with BBASI using the various tabs under the BBASI banner:

MY ACCOUNT (upper right hand side)

This tab will allow you to add or edit your Participant’s basic information including a headshot, email

address, or household contacts. Information like your Participant’s DOB and academic grade level

can only be changed on the administrative end – your program contact can edit these fields for you.

ENROLLMENT (Second tab from the left)

This tab will allow you to add and view the enrollment options (i.e. registering for an audition or

program housing) for your Participant.

FORMS (Third tab from left)

This tab will allow you download, complete, and upload the required forms for your Participant’s

attendance in the BBASI CT Program.

FINANCIAL (Fourth tab from the left) From here, you have 24/7 access to your latest statement and invoice information. Using this tab, you may also safely encrypt a credit card number for BBASI Staff to use in the event of an emergency. You can read more about the privacy/safety policy in your registration contract.

Forms Under “Medical Forms,” you will note that there are 3 sections that must be filled out in their entirety. Please note, however, that these are not the extent of medical forms/authorizations required. In the Standard Forms sections, there are several supplemental medical forms/consents. Additionally, you will find a Physician Report to be filled out by your Participant’s primary physician. Please be advised that the BBASI Medical Director mandates that all medical examinations be done within 6 months of the start of BBASI (June 27nd, 2016). Should you have difficulty scheduling an appointment with your primary physician by the May 15th deadline, please inform your program contact. Please note that the physician’s report must be filled out by the Participant’s physician under whom s/he has been in the care of for at least 1 year. In addition to medical forms, you will note that there are two types of “Standard Forms” that may be completed and submitted in different ways:
