LOOKING FORWARD TO ANOTHER GREAT YEAR

The start of a new year is a good time to reflect on the previous year. In 2015, we saw our health system grow and expand to provide access to healthcare for more people in our community. By adding almost 30 physicians who provide primary care as well as specialty services, the DeKalb Medical Physicians Group enhanced the level of service available close to home—eliminating the need for DeKalb residents to fight traffic to see a doctor or receive specialized care.

One significant celebration was the 10th anniversary of the opening of DeKalb Medical at Hillandale. Just as the community surrounding the hospital has grown, so has the hospital. In addition to opening Hillandale Radiation Oncology and establishing a Heartburn Center on the Hillandale campus, a number of primary care physicians as well as specialists moved to the campus to provide care close to home for our neighbors. See page 8 for more information about the

hospital’s services and to enjoy photos from our celebration.

DeKalb Medical at North Decatur and DeKalb Medical at Downtown Decatur also saw growth and were both recognized for excellence in programs at those facilities. Recognitions included achievement of Baby-Friendly USA designation and 5-STAR

Hospital recognition for Obstetrics at North Decatur; a Certificate of Distinction for Advanced Certification as a Primary Stroke Center by the Joint Commission; and three Gold Seal of Approval® disease-specific certifications— Respiratory Failure, Acquired Brain Injury and Wound Care—from the Joint Commission for our Downtown Decatur Long Term Acute Care campus.

I’m honored to work with the dedicated physicians and employees who make these achievements possible and we all look forward to another year of caring for our friends and neighbors.

AVOID THESE WINTER HEARTBURN TRIGGERS

There’s nothing better than a cup of hot chocolate when it’s cold outside. It’s also normal to crave “comfort” food during the winter—chili, grilled cheese sandwiches, tomato soup and chicken pot pie. Unfortunately, the high fat and acid content of some of these items can trigger heartburn in many people. These foods can still be enjoyed if you make them with low-fat ingredients and substitute other ingredients that are less likely to cause heartburn.

DON’T MAKE EXCUSES WHEN UNUSUAL SYMPTOMS OCCUR

The tingling and slight numbness in his feet did not alarm Jack Audas when he returned from a business trip, during which he had climbed lad-ders and traveled across catwalks in a factory to check out the equipment his company produces. Attributing the symptoms to fatigue and over-worked legs and feet, he decided to give himself a few days to recover before calling a doctor.

However, after his legs gave way while enjoy-ing a race at Talladega two days later, Audas’ wife had him see his primary care physician (PCP) on Monday. “I’ve had a few sports injuries in my life but never been sick, so my visits to the doctor are usually just for checkups,” he says. Because Audas had a long-term relationship with his PCP, the doctor knew his complaints were serious. After examining Audas, his doctor immediately sent him to Sara Shuler, M.D., a neurologist at DeKalb Medical.

Overcoming Obstacles“I was able to walk, but I shuffled,” says Audas. “I didn’t feel ill—in fact, when Dr. Shuler told me that she was admitting me to the hospital immediately, I asked if I could come back after a meeting I had scheduled at work.”

With a resounding “no” to his question, Audas was admitted to the hospital just a few days after he first noticed the tingling.

Diagnosed with Guillain-Barré Syndrome, Audas spent six days in the hospital for treat-ment for the condition in which the body’s immune system attacks the nervous system. He was then transferred to DeKalb Medical’s inpatient rehabilitation unit to focus on physical and occupational therapy and discharged to home about six weeks later to continue outpatient therapy.

QUICK ACTIONresults in full recovery

“ The therapy was necessary because I had to learn how to walk, stand and move again because my nervous system no longer communicated with my muscles.” —Jack Audas

WINTER 2016 5

“The therapy was necessary because I had to learn how to walk, stand and move again because my nervous system no longer communicated with my muscles,” explains Audas. Even with the intensive therapy required, he says he stayed positive. His lifetime of playing sports—college football, water skiing, track, golf, scuba diving and weight training—gave him the experience of setting physical goals and working toward them. “I never asked why this happened to me, and I never got depressed,” he says. “In fact, I recovered more quickly than anyone thought I would thanks to my attitude and the wonderful care I received from Dr. Shuler and DeKalb Medical staff.”

PCP Relationship Is Key to Quick TreatmentAlthough Guillain-Barré Syndrome is rare—affecting fewer than 20,000 people each year in the U.S.—Audas’ experience points to the importance of having a relationship with a PCP, says Shuler. “We were able to diagnose the condition early because he went to his doctor when he noticed increased weakness,” she says. “His doctor knew him well and knew that Audas did not normally have complaints and was able to see that this was something unusual.”

Too many times, patients will attribute weakness, or inability to do something they’ve always done, to aging, says Shuler. Progres-sive weakness or tingling, or sudden onset of a symptom that is unusual, are reasons to see a physician immediately, she suggests. “It is best to first see a PCP who knows you because the same symptoms might be caused by osteoarthritis, orthopedic problems or neurological issues,” she says. “The primary care physician can get you to the right specialist.”

Even though his condition meant he was wheelchair bound early in his recovery, Audas has slowly built his strength and regained his ability to walk. “I’m stubborn so I did not like it when I was told it would take a long time to recover.” Within four months, he was using a four-legged cane, or as he admits, “I was carrying the cane most of the time, and I was climbing steps and hills. My next goal is to be out on the golf course.”

GET THE FACTS: GUILLAIN-BARRÉ SYNDROME

WHAT IS GUILLAIN-BARRÉ SYNDROME?Guillain-Barré syndrome (pronunciation: “ghee-YAN bah-RAY”) is a nervous system condition. It causes muscle weakness, loss of reflexes, and numbness or tingling in your arms, legs, face and other parts of your body. The condition is rare, and if not treated quickly, it can cause paralysis and lead to death. Most people, however, recover between three and six months after onset with minor lingering symptoms.Possible symptoms: Numbness or tingling in hands and feet, and sometimes around

the mouth and lips Muscle weakness in legs, arms and the sides of the face Trouble speaking, chewing and swallowing Not being able to move the eyes Back painSymptoms usually start with numbness or tingling in the fingers and toes. Over several days, muscle weakness in the legs and arms develops. After about four weeks, most people begin to get better.

WHAT CAUSES GUILLAIN-BARRÉ SYNDROME?Experts don’t know what causes it. In Guillain-Barré syndrome, the immune system attacks the covering of certain nerves, which causes nerve damage. There is evidence that some viral or bacterial infections may trigger the syndrome.

HOW IS GUILLAIN-BARRÉ SYNDROME DIAGNOSED?Timing of the onset of symptoms and progression of weakness, as well as recent infections, will be evaluated by the doctor. Tests, such as lumbar puncture and a nerve conduction study, will also be used to confirm a diagnosis.

HOW IS GUILLAIN-BARRÉ SYNDROME TREATED?This syndrome usually is treated in the hospital. The hospital staff will monitor breathing, heart rate and blood pressure to make sure symptoms don’t worsen. Some people need a ventilator to help them breathe.

Plasma exchange, a process that takes blood from the body and removes harmful antibodies before returning the blood, as well as intravenous immune globulin, a process that adds helpful antibodies to the blood, are two treatments that help fight the disease and speed recovery.

Peanut butter sandwiches are

a staple in children’s lunch

bags, but for some people,

peanut butter and other

peanut products represent

a dangerous health risk.

The peanut desensitization program has helped Shoshana Yaschik, 5, feel like a carefree, normal kid when it comes to eat-ing and being near peanut butter and other peanut products.

WINTER 2016 7

When Karly Yaschik gave her one-year-old daughter, Shoshana, a peanut butter sandwich, she was shocked to see Shoshana break out in hives and develop gastrointestinal symptoms with one bite. “Our pediatrician tested her and discovered that she is highly allergic to peanuts,” explains Yaschik. This was the beginning of careful monitoring of what her daughter ate, making sure friends and daycare providers knew about the allergy and carrying EpiPens, devices that inject a pre-measured dose of epinephrine to combat a dangerous allergic reaction.

“It is nerve-wracking to worry that she might accidently eat something with peanuts and it was sad that she always had to ask if a birthday cake or snack at a friend’s house had peanuts in it,” says Yaschik. “I want her to be a normal kid who doesn’t have to worry about an allergic reaction.”

The good news is that a desensitization program developed about 10 years ago and now offered by George Gottlieb, M.D., a DeKalb Medical allergist, has given Shoshana—now five years old—a chance to be “a normal kid.”

“Although the program was first developed 10 years ago, it is only in the past one or two years that enough research existed to demonstrate the success and safety of the approach,” says Gottlieb.

In the U.S., approximately 3 million people report allergies to peanuts and tree nuts. Peanut allergies most often appear in young children, and while some outgrow this potentially dangerous allergy, most do not. “Many childhood allergies, such as milk and eggs, are outgrown, but only 20 percent of people with peanut allergies outgrow them,” says Gottlieb. “Peanut and tree nut aller-gies are responsible for most deaths attributed to allergic reactions for a number of reasons, including the fact that you can’t always know all of the ingredients in many prepared foods and you can’t always trust the labels.”

The approximately eight-month-long desensitization process requires weekly visits for monitored exposure to peanut in order to identify the highest tolerated dose, gradually increasing the dose as tolerance improves. Between visits, patients eat the new dose of peanut each day. The full-target daily dose is eight peanuts for children and 12 peanuts for adults. “Eating the correct dose of peanut each day is needed to maintain the tolerance to peanut,” explains Gottlieb.

“It was strange to be told to eat peanuts when I had spent my entire life avoiding them,” admits Nick Richwagen, a 23-year-old who saw a flyer describing the program in Gottlieb’s office. “I get monthly shots for another allergy and thought it would be a good thing if I did not always have to worry about my peanut allergy.”

Although Richwagen’s allergy kept him from eating peanut products throughout his life, he did not have problems if someone else near him was eating peanuts. “There are a lot of people who are more sensitive to peanuts than I was, but I always had to be aware of what was in the food I was eating,” he says. He credits Gottlieb and his staff for offering a valuable service that does change the lives of peanut allergy patients. “Now, I can try new restaurants and new foods and not worry. I wish this had been available when I was younger because it would have been a big relief for me and my parents.”

For Shoshana’s family, the treatment has changed their lives significantly. “I can now make a peanut butter sandwich for her older brother with no worries,” says Yaschik. Although Shoshana eats eight peanut M&Ms each day to maintain her desensitization,

Nick Richwagen, 23, has had peanut allergies from a young age. The peanut desensitization program allows him to try new foods and restaurants without worrying about his allergy.

everyone was surprised to learn she did not like the taste of peanut butter. “She won’t eat peanut butter, but she knows the M&Ms are important to eat.”

Richwagen, on the other hand, had a different reaction to peanut butter when he was able to eat it. “Peanut butter sandwiches and Reese’s peanut butter cups—I now understand why people like them.”

CELEBRATINGsuccessSouth DeKalb residents joined over 100 DeKalb Medical physi-cians and employees for the 10th anniversary celebration of the opening of DeKalb Medical at Hillandale. The theme of the event was “Take a Loved One to the Doctor Day.”

The festivities included one-on-one conversations between physicians and attendees, doctor talks, healthy cooking dem-onstrations, health screenings, registration for a primary care physician, fresh produce and face painting. “The event was a great success and allowed many of our community members to learn more about the services and resources offered on our Hillandale campus,” says Vice President and Administrator Kim Bentley. “We are thankful for the physicians and staff who volunteered their time to talk and interact with our community members, and for providing care to those same people throughout the years.”

DeKalb Medical at Hillandale opened for patients on July 8, 2005, and has provided service to South DeKalb and surround-ing communities—areas that previously had no nearby hospital. Annually, the hospital serves more than 5,000 people seeking inpatient care. It provides over 16,000 mammograms, treats 60,000 people in the emergency room and performs 1,600 surgeries.

“As a unit secretary on the bariatric surgical unit at DeKalb Medical, I’ve seen how many people choose surgical weight loss as a way to become healthier,” says Brenda Horne, a 14-year DeKalb Medical employee. “However, when my gastro-enterologist recommended bariatric surgery as a way to lose weight, which would help with my irritable bowel syndrome and my acid reflux, I was hesitant.”

When she noticed that she was gaining more weight, she began to think seriously about surgical weight loss. “I was huffing and puffing as I walked up stairs, had developed high blood pressure and heart palpitations, and was ready to do something,” says Horne.

“I talked with one of my former co-workers who underwent the procedure and asked her about her experience,” adds Horne. “I also went to one of the educational seminars Dr. Steinberg and Dr. Rashid of DeKalb Medical’s Surgical Weight Loss Center offer to learn more about it.”

In July 2014, Horne underwent a vertical sleeve gastrectomy procedure to reduce the size of her stomach, which makes her feel full sooner and means she doesn’t eat as much at one time. In less than 18 months, she lost 76 pounds—going from 263 pounds to 187.

Surgical weight loss, however, is only effec-tive if people are willing to change all of their behaviors related to food, says Horne. This means making good choices about foods you eat and incorporating exercise into your routine. “I’ve been in school, taking courses online, so

before my surgery, I’d work a 12-hour day at the hospital, and then come home to log on to my computer and snack while I did my school work,” she explains. Now, her snacks are healthy and she makes sure she eats a balanced diet throughout the day.

“I also love going to work out three to four times each week,” says Horne. “I see the positive results—more energy, no need to take medica-tion for high blood pressure or acid reflux, and more focused.”

Her husband and her two daughters—both in their 20s—were completely supportive, says Horne. “They saw how determined I was to succeed and they were excited for me.” One of the many benefits of her new healthy self is her ability to keep up with her family. “We like to go to the mall together. Before I lost my weight, I would have to sit and rest while they continued shopping,” she says. “Now, I keep up with them and go anywhere I want to go.”

Bariatric surgical unit secretary Brenda Horne saw dramatic weight loss after she underwent a vertical sleeve gastrectomy procedure and incorporated healthy food and exercise into her lifestyle.

A GLOBAL PERSPECTIVE ON PATIENT CARE

Before Jessica L. Malmad, M.D., M.P.H., entered medical school, she knew she wanted to be part of the solution to increase access to basic primary care services for the underserved. Not only did she enter medical school with a master’s of public health in global health from Emory, she also spent years gaining experiences domestically and in Latin America in order to become fluent in Spanish and to help her better serve a Latin-American population.

“I wanted to be a primary care provider long before I entered medical school because my experiences in public health demonstrated the importance of educating people about prevention,” says Malmad, who is board-certified in Family Medicine. “I want my patients to understand that they often have significant control over preventing the development of chronic diseases.”

For those already afflicted by one or more chronic diseases, Malmad often uses diagrams and pictures to explain the effect chronic diseases have on her patients’ bodies and how their medication acts within their bodies, in hopes that this knowledge will help them understand why taking their medications as prescribed can help them stay healthy.

Because she has a special interest in women’s health, Malmad says, “I look forward to working with women of all ages to educate them on things such as their contraceptive options and updated guidelines for cervical and breast cancer and other recommended screenings.”

Malmad also knew that she desired to work in a setting that enabled her to interact with people from all over the world. “Stone Mountain Primary Care is a great location for me because of our community’s diversity. Every culture seems to interpret and manage illness and disease differently, and I look forward to both learning from my patients in this regard and also teaching them what I can about health.”

WINTER FITNESS TIPSWinter season is a time to become creative with your workouts as there is less daylight in the evening and harsh temperatures, which prohibit you from working out outside on certain days.

1. If you must work out outside, dress in layers. Layer your clothing. It is easier to cool the body down than it is to warm the body up during this season.

2. Take advantage of the weather. Try working out indoors and changing up your routine to challenge your body in different ways.

3. Take advantage of the early sunsets and get enough rest. Your body needs rest to recover from the day’s workout and routine activities.

4. Water is the best thing to drink to stay hydrated. It’s healthier to drink water while your exercising than when you’re done.

5. Strive for the five P’s: Proper preparation prevents poor performance. Pack your gym bag the night before so you do not have to miss a workout.

6. Don’t wait until spring to clean your house. Cleaning your house is an easy way to burn calories.

7. Integrate exercise into your busy work schedule. Take stairs instead of elevators, take a walk during your lunch break or encourage your boss to have walking meetings.

OB Tour—Let’s Get Acquainted The class begins in our auditorium, where you learn about Labor & Delivery, the importance of skin-to-skin contact after birth and Rooming-In on the Mother-Baby unit. You’ll receive information on parking, infant safety, visitors, baby photos and pediatricians, and take a walk through our Labor & Delivery and Mother-Baby units. You can complete your pre-registration, so remember to bring your insurance card.

GOLFERSset their sights highMore than $198,000 was raised thanks to the 164 golfers who participated in the 21st Annual Charles B. Eberhart Memorial Golf Classic event at the beautiful Capital City Club Crabapple in October. Funds from this year’s tournament will support Building the Future of Emergency Medicine: When Every Second Counts, a $2.1 million capital campaign to renovate and reinvent the emergency depart-ment at DeKalb Medical at North Decatur so that evaluations and care are administered more efficiently to impact more lives.

The new architecture will feature a “Super Fast Track” check-in for urgent care patients and a separate immediate care area for patients experiencing stroke, heart attack or other life-threatening symptoms. LEGACIES LIVE ON

CELEBRATING THE GENIUS AND GENEROSITY OF DR. M. BOBBIE BAILEY

It’s no surprise that there is an intersection named after

Dr. M. Bobbie Bailey in Georgia. The extraordinary

innovator and philanthropist left her mark at many important

intersections in Atlanta. From donating 32 Steinway pianos

to Kennesaw State University to establishing women’s athletic

scholarships and funding technological advances at DeKalb

Medical, the entrepreneur’s generosity was integral to the

growth of arts, culture, education and the delivery of medicine

throughout the community.

The boldly progressive, outspoken, generous entrepreneur

and Ansley Park resident, who passed away in July, leaves

behind a legacy that will continue to touch all of us for

generations to come. As the third of eight children raised in

LaGrange, Georgia, Bailey learned a love of mechanics from

her father and was tuning race cars by the time she was 12.

With her extensive know-how and adventurous spirit, Bailey

started and ran four hugely successful companies that fueled

her love for, and ability to give back to, her community.

Preferring to remain behind the scenes, Bailey’s quiet gifts

helped DeKalb Medical reach far and wide in its ability to

take care of “its own.” Her philanthropic support of DeKalb

Medical helped purchase a PET/CT scanner to provide more

accurate and rapid diagnosis of cancers. She was so impressed

with the technology and the precision of the da Vinci Robotic

Surgical System that she purchased one for DeKalb Medical—

a technology that has been responsible for many lifesaving

surgeries. In addition, her donation of a Breast Magnetic

Resonance Imaging Unit has been a gift to women that has

enhanced early breast cancer detection capabilities in DeKalb

and surrounding counties. In recognition of her generosity,

the Dr. Bobbie Bailey Tower was named in her honor when

it opened in 2007.

Described as innovative, fearless, driven, inspiring, generous

and straightforward, Bailey’s contributions and character

have blessed our community for more than 50 years and will

continue to shape all we aspire to for many more to come.

PHYSICIAN CONTINUES TO TOUCH LIVES EVEN AFTER HE IS GONE

In August 2015, DeKalb Medical lost one of its finest friends and founding fathers with the passing of H. Duane Blair, M.D. Not unlike the approach he used to tend the champion roses he grew in his DeKalb County backyard, Blair applied love and tenderness to helping DeKalb Medical grow into a hospital system prized by the community.

An inseparable part of the roots of DeKalb Medical and its genesis into a burgeoning healthcare system, the renowned general surgeon was with DeKalb Medical from the very beginning. When DeKalb General Hospital first opened its doors in 1961, Blair served both as chief of surgery and chief of staff. The beloved physician and graduate of the Emory School of Medicine spent 42 years practicing medicine and mentoring many of Atlanta’s top physicians.

Born during the Great Depression, Blair witnessed firsthand what it means to struggle. In his roles as chairman of the DeKalb Regional Health System and as a lifelong trustee of the DeKalb Medical Foundation,

he made sure that DeKalb Medical never lacked the resources it needed to ensure area residents had access to the highest-quality care. Never one to be in the spotlight, Blair was blessed to touch the lives of more than 5,000 patients and, through a generous donation to the DeKalb Medical Foundation, his legacy of commitment and compassion lives on.

Blair championed many causes and made many critical decisions that have benefitted the citizens of DeKalb County. “As we look at where our health system has come, it’s clear that the leadership and foundational work of Dr. Blair and his colleagues are what make us able to capably serve our community today,” said DeKalb Medical president and CEO John Shelton. We will miss the keen intellect and generous spirit of this general surgeon and philanthropist who has contributed so much to the medical community and helped shape DeKalb Medical’s reputation as the hospital system with a “heart and soul.”
