Immune Restoration Syndrome (IRS)
Définition : Réapparition ou aggravation de manifestations inflammatoires sous traitement spécifique lors de la restauration immunitaire sous HAART.
Pathologies sous-jacentes:« infectieuse »: virale (HHV8, JCVirus, CMV?, HSV1…), mycobactéries, parasites (cryptocoque, toxoplasmose, pneumocystose)…« dysimmunitaire»: sarcoïdose, Lupus, thyroïdite auto-immune, cryo-globulinémie..

Conclusion : Physiopathologie de l’IRS associée à la Tuberculose, chez des patients co-infectés
• IRS est la conséquence de l’exacerbation aigue d’une réponse immunitaire « normale » contre l’agent pathogène opportuniste présent associée à un syndrome inflammatoire diffus, intense non contrôlé
– Réponse CD4 Th1 mémoire effectrice , • Autorisée par la reconstitution immunitaire rapide sous traitement ARV
• permettant la reconstitution des lésions granulomateuses
• Responsable de l’augmentation de volume des lésions
• Représentant jusqu ’à 35% des CD4 circulants
– Réponse CD4 spécifique • d’antigènes mycobactériens de dormance contenus dans la tuberculine
• Mais non des antigènes associés à la virulence (ESAT-6, CFP10..)
• Associée au contrôle médicamenteux du BK
– Inductrice d’un syndrome inflammatoire aigu, responsable des manifestations locales et systémiques
Conclusion : Physiopathologie de l’IRS associée à la Tuberculose, chez des patients co-infectés
• IRS est la conséquence de l’exacerbation aigue d’une réponse
immunitaire « normale » contre l’agent pathogène opportuniste présent
associée à un syndrome inflammatoire diffus, intense non contrôlé
