Dear Friends,

As we mark 35 years of service to our community I am drawn to reflect on what that really means. Hospice of Santa Cruz County has been honored to accompany thousands of individuals and families in their last months and days of life, offering comfort and support so that they may live, and die, with dignity. To me, this reflects our commitment to being truly present to serve when support is needed most.

During our 2012-2013 fiscal year, we expanded our capabilities to meet the growing need for hospice and grief support services in our community. Our new Center for Compassionate Care in Watsonville opened in 2013 and is now fully operational. The Center continues to be a great source of pride for us at HSCC, and the welcome we have received from the Pajaro Valley community has been nothing short of exceptional. In 2012-2013 the number of people choosing our hospice services increased 20% and we are grateful for the ability to deliver hospice care and provide grief support, volunteer training, and community education from two locations.

As we are all aware, healthcare has been prominent in the news, and end-of-life issues are being discussed more than ever. In this past year the Center for Medicare and Medicaid Services recognized Santa Cruz County and our continued success in providing choice, dignity, and comfort at the end of life with fewer unwanted medical interventions and at a lower cost than other regions in the nation. We are working closely with our local Health Improvement Partnership and community leaders to explore these findings so that they may be replicated in other communities.

This year has been rich with change, insight, and forward motion. Our work could not be possible without the hundreds of volunteers who dedicate their hearts and hands to our organization. The support of generous individuals and foundations that give so generously to our programs is also essential. Thank you for partnering with us to ensure that our community has a vital and enduring hospice program.

Hospice Care & Grief Support We believe that quality of life is linked to what matters most to you and your loved ones. We are committed to serving individuals and families facing the end of life with compassion, expertise and presence. Bilingual and bicultural care teams ensure that all members of our community have access to the highest quality hospice care when it is needed most.

Community ProgramsAn extension of our hospice program, our community programs are a direct response to the needs of our community.

•488daysofprobonohospicecaretotaling$110,920wereprovided to eight patients. Our charity care ensures that individuals who are too young to qualify for Medicare coverage, those who have lost access to private insurance or are underinsured, and those who are homeless can access high quality care so they can live, and die, with dignity.

•175militaryveteransreceivedcompassionatehospicecare.As part of the We Honor Veterans program, our care teams connect veterans and their families with VA benefits, provide emotional and practical support and assist with securing

military funerals and memorial planning. Most importantly, these veterans are honored and thanked for their service to our country.

•70communityeventsandpresentationsreached2,952communitymembers,students,andprofessionals,buildingknowledge of advance care planning, hospice basics, grief & loss, and caregiver skills, and fostering confidence in caring for ourselves and others at the end of life.

Introducing the Watsonville Center for Compassionate CareDuring the 2012-13 fiscal year, HSCC continued to see growth in the number of seriously ill individuals choosing hospice care. In March 2013 we opened a second location to meet this need. Located on the campus of Watsonville Community Hospital, our new Center for Compassionate Care serves as home base for a team of nurses, physicians, social workers, hospice aides, chaplains, clinical team leaders and volunteers who provide expert, culturally sensitive care to patients, and their families. The team provides in-home and facility-based care for those living in Aptos, La Selva Beach, Watsonville, Pajaro Valley, and the surrounding rural areas, which account for one-third of those we serve.

The Center also has space dedicated for individual and group grief support services for all community members. A special children’s room is furnished with the latest therapeutic tools to help children process their grief in a healthy manner. Educational offerings and volunteer trainings held at the Center allow us to meet the growing

community need for quality end-of-life care services.

The Watsonville Center for Compassionate Care allows us to have a visible presence where we serve and to be good neighbors in our community. With the support of a committed leadership of board members and community advisors, we continue to seek partners to make this project a success. Naming opportunities are available for various rooms as well as for the entire Center.
