MESSAGE FROM THE DIRECTOR

FY 2012-2013 was a significant year for Laguna Honda to which we can look with pride. The federal Centers for Medicare and Medicaid Services (CMS) awarded the hospital the highest rating possible—5 Stars. We served more San Franciscans than last year and successfully discharged more residents back to the community. We improved patient flow between San Francisco General Hospital and the community, while participating in the department-wide integrated delivery system planning and implementation. Staff attended several educational training sessions to expand our ability to care for more complex residents. We improved our emergency planning response proficiency through practice drills. We witnessed the success of these efforts in our coordinated system-wide response to disasters.

Through a strategic planning process for setting priorities and goals, staff established a solid framework for meeting our mission and beginning the transition to a managed care system that will come with healthcare reform.

My thanks to the Health Commission, Director Garcia, Laguna Honda staff, residents and their families, volunteers, friends, and neighbors, for supporting the mission of Laguna Honda.

WHO WE ARE Laguna Honda Hospital and Rehabilitation Center is a five-star rated1 general acute care facility providing acute care, skilled nursing and rehabilitation services to San Franciscans. Laguna Honda is one of the largest skilled nursing facilities in the country and one of the few publicly owned.

A longtime civic icon, Laguna Honda has served San Francisco’s population since the Gold Rush. Recently rebuilt in 2010, Laguna Honda is the first LEED-certified hospital in California and the most modern skilled nursing facility in the United States.

As part of the Department of Public Health’s network, residents at Laguna Honda have access to a comprehensive system of care that spans the continuum. Laguna Honda specializes in:

RESIDENT DEMOGRAPHICS In FY 2012-2013, Laguna Honda served a total of 1,191 unique individuals for a total of 275,985 patient days. The following charts describe the population by race/ethnicity, age, gender and payor source.

The number of Laguna Honda residents above the age of 85 has decreased slightly since last year from 15% in FY 2011-2012 to 13% in FY 2012-2013; the number of people served between the ages of 45-64 increased by 3%.

FY 2012-2013 HIGHLIGHTS Staff contributed with pride to achieve Laguna Honda’s mission by participating in a rigorous process to establish goals and priorities. Each Division/Department focused their goals on meeting the Hospitals’ six priorities for FY 2012-2013.

APPROPRIATE LEVEL OF CARE In FY 2012-2013, Laguna Honda

expanded access to care for more San Franciscans in need of skilled nursing care. This was accomplished by reducing length of stay and increasing discharges.

Average length of stay (inclusive of both acute and SNF) steadily decreased in the past three years.

Discharges Community discharges increased by 23% from 175 in FY 2010-2011 to 215 in FY 2012-2013.

Behavioral Health Summit On April 2, 2013, Laguna Honda convened a Behavioral Health Summit that brought together experts from the Department of Public Health and the community, to discuss best practices for caring for behaviorally complex patients. Panelists included practitioners from the Northern California Alzheimer’s Association, Crestwood Behavioral Health Inc., Department of Public Health’s Homeless Outreach Team, the University of California, San Francisco’s Alliance Health Project, San Francisco General Hospital’s Department of Psychiatry and Garfield Neurobehavioral Center. Discussion focused on identifying best practices, skill-building, sustainability, challenges and evaluation. Staff brainstormed how to apply best practices learned at the summit to the day-to-day hospital operations.

ACHIEVING 5 STARS

CMS Certification In December 2012, the

Centers for Medicaid and Medicare Services (CMS) awarded Laguna Honda the highest possible rating of 5-Stars. The annual CMS survey rates hospitals on three criteria: staffing, health inspections and quality measures. In three years, Laguna Honda went from a 2-star rated hospital to a 5-star. Hospitals with 5-star ratings are considered to have above average quality care and are

reflective of staff commitment to service excellence.

Quality Improvement Staff education and training is a vital component to maintaining a world-class skilled nursing facility. In FY 2012-2013, the Quality Management and Nursing Education Departments coordinated staff participation in a 12-month education course sponsored by the Institute for Healthcare Improvement (IHI) Open School. The on-line course focuses on quality improvement, patient safety, teamwork, leadership and patient centered care.

Service Excellence Staff participated in a department-wide Service Excellence training, where Laguna Honda, San Francisco General and Community Programs’ leadership helped spread the Service Excellence messages throughout the organization.

Service Excellence cards were created to acknowledge staff and volunteers who demonstrated the Seven Standards of Excellence. The cards are displayed on the Esplanade.

FISCAL RESPONSIBILITY

In the beginning of each fiscal year, Finance,

Human Resources and Executive staff conduct an extensive planning process with each division and department to review cost center management. Targets are set to assist departments in staying fiscally prudent. Creative efficiencies are put into place that help save substantial dollars throughout the year.

Linen Conservation And Awareness Program Environmental Services has partnered with the department's linen and laundry vendors to launch a campaign to educate staff about proper use of bed linens. The initiative—quality care without waste-- improved patient care outcomes, reduced linen usage by more than 20% and has resulted in potential savings of $400K per year.

AB 900 Support Laguna Honda partnered with the California Hospital Association to produce a state-wide video campaign that would stop a 25% Medi-Cal reimbursement reduction to skilled nursing facilities. Mayor Lee joined Executive Administrator Hirose along with Laguna Honda staff and residents to describe the devastating impact these cuts would have on individuals and their families seeking care.

INTEGRATED DELIVERY SYSTEM Since 2011, the Department of Public

Health has been planning the transition to an Integrated Delivery System (IDS). The goal is to ensure that we can effectively compete in a managed care environment as a provider of first choice.

Laguna Honda has been actively participating in this endeavor through the following initiatives implemented in FY 2012-2013:

Department-wide integration of Health at Home

Department-wide integration of Rehabilitation Services

Joint clinical nutrition coverage at Laguna Honda and San Francisco General Hospital

Joint Executive meetings with Laguna Honda, San Francisco General and Community Programs

Integration has also resulted in the relocation of department-wide programs from other City facilities to the Laguna Honda campus. In FY 2012 -2013, Community Oriented Primary Care Case Management, Nurse Advice Line, Community Placement, and the Targeted Case Management programs moved to the Laguna Honda campus.

Integrated Response to the July Airline Incident In an example of the effectiveness of an integrated delivery system, Laguna Honda and San Francisco General Hospital staff worked in unison to

respond to the Asiana Airline incident at SFO. Twelve patients were admitted to Laguna to vacate beds at San Francisco General for patients affected by the crash. Forty-nine staff members including physicians, nurses, pharmacy and porters worked long hours to ensure that arriving patients and their families felt welcomed and were well cared for. Department-wide Participation at Black History Month Celebration Commissioner Taylor-McGhee provided a moving key note speech to a packed crowd of residents and staff in celebration of Black History Month in February 2013. Staff throughout the Department of Public Health joined the celebration to provide education on African American health and equity. A rousing Black National Anthem was led by staff from the Department’s Community Programs. Over 60 residents, staff, family members and volunteers attended this annual event. Success is attributed to the teamwork throughout the Department.

SERVICE DELIVERY IMPROVEMENTS

VMI Roll-Out Videoconference Medical Interpretation (VMI) is now live at Laguna Honda’s clinic and will be rolled out to the rest of the hospital in the coming year. VMI provides instant connection to live interpreters in Spanish, Cantonese, Mandarin, Taishanese, Vietnamese, Russian, Korean, Arabic, and Tagalog and access to a total of 26 languages. The system has been in use at San Francisco General Hospital and Community Programs and provides vast improvements in patient care compared to the audio-only system.

Emergency Preparedness The hospital conducted multiple drills in FY 2012-2013 to prepare for an emergency. In May and June, 759 staff participated in Go Bag drills to support personal readiness. In March, 507 staff participated in a cold start power outage drill to prepare assigned staff to assume emergency roles and practice using the Hospital Incident Command System (HICS).

Pain and Healing Center The Laguna Honda Pain and Healing Center uses an integrated team approach to provide supportive and compassionate care for residents living with chronic pain. Since its launch in June 2011, the clinic has served 130 residents, and averages 35 visits per week. Practitioners from multiple disciplines including medicine, pharmacy, acupuncture, psychiatry, music, movement and massage therapists work together to provide comprehensive and innovative treatment for residents with all types of pain.

Health at Home Treatment Adherence Program Health at Home’s Treatment Adherence Pilot Program focuses on improving antiretroviral (ARV) medication adherence for San Francisco's vulnerable populations. A total of 28 clients were enrolled and followed for one to sixteen months. Every day a Health at Home Aide, supervised by a nurse and supported by a social worker, would either visit or call clients to support medical adherence. Outcomes for the pilot were:

1. Clients demonstrated an ARV medication adherence rate of at least 90%

2. Viral load was decreased 3. Primary care appointment

adherence rate was 95%

In conclusion the pilot confirmed that interventions to improve adherence among antiretroviral drugs produce better clinical outcomes, lower community viral loads and risk for disease transmission, and ultimately reduce the cost burden of this disease on the public health system.

Performance Appraisals Due to a successful campaign by Laguna Honda Human Resource Division, 99% of employee performance appraisals were completed this fiscal year

Improving Quality of Meals Nutrition Services has continuously focused on improving the meal experience by obtaining input from residents, expanding menu selections, providing culturally sensitive choices, updating equipment, and converting to the new Dinex system. Results of their efforts can be illustrated by the

improved ratings from the last three Resident Satisfaction Surveys administered by My InnerView in the areas of quality of meals and dining experience.

WELLNESS Wellness Program In October 2012, Laguna Honda created its first Wellness program.

Modeled after national best practices, Laguna Honda convened a Wellness Council comprised of staff, residents, community members, other City Departments, and experts to provide guidance and recommendations.

Wellness Mission: to provide a relevant, accessible and enjoyable wellness program that supports healthy lifestyles and behavior for Laguna Honda staff, residents, their families and the community.

Each month, Leadership staff participates in a guided exercise to support individual well-being. Yoga, zumba, meditation, dance, softball, lunchtime hikes and nutritional guidance are some of the activities that have been developed at the campus.

RESIDENT EXPERIENCE Anthony Yee Anthony was born in San Francisco and raised in Sacramento. After attending college, he returned to his family home in the Richmond district. He worked for Charles Schwab until the rigors of high finance took their toll and he made a move into the world of fine pastry. He enrolled in culinary school and began working in restaurants until he was diagnosed with a complex set of medical conditions that required him to be admitted to Laguna Honda. Anthony was unable to walk or grab objects.

Anthony’s goal was to return home after completing a rigorous rehabilitation program at Laguna Honda. His peer mentor Shannon, a former resident, would visit often. They had many discussions around his progress, living on his own, the work he wished to pursue after discharge, and what he wished to do for the rest of his life. At one point his peer mentor asked, “Why don’t you come live in my building? It’s great.”

On May 16, 2013, his parents and social worker escorted Anthony to the hospital lobby with his belongings as they prepared to take him to his new community home. Anthony was in his electric wheelchair. He stopped just before the door and asked his social worker, “Can you take my wheelchair out to the car for me? I need to do something.”

Anthony wanted to walk out of the hospital without help. He then stood up. Though wobbly, he stood proudly and assertively and walked out the front door of the hospital, without assistance, and stepped into a new beginning.

FAMILY EXPERIENCE Richard and Genevieve Callejo Richard Callejo was a prominent immigration rights attorney, which allowed him and his wife Genevieve to live all over the world, including Paris, Mexico City, Zimbabwe, Israel and Uganda. At a recent Fiestas Patrias (Mexican Independence) celebration at Laguna Honda, Genevieve, said, while dancing, “we have been dancing together for over 50 years!” “We have shut down clubs all over the world!” You can tell how much Richard enjoys the music as tears stream down his face.

In addition to his work defending the rights of immigrants, Richard had hosted several talk and informational shows on local television, and is often recognized by members of the Bay Area Latino community. As an immigration specialist, Richard was instrumental in forcing the courts to certify translators in the courtroom, after often hearing testimony that was skewed by poor translations. He wrote a book called “Justice in Any Language” and his talk show entitled “El Amanecer” featured such prominent speakers as Rose Byrd, Phil Burton, and Dianne Feinstein.

After his three strokes, Richard had all but stopped speaking, and was admitted to Laguna Honda one day shy of his 83rd birthday. Genevieve says that since he has moved to the Spanish focus neighborhood, his speech, both in English and in Spanish has made considerable improvements, “like night and day, it has improved 100%, even 200%.” She is sure this is because of the stimulation he gets in his native language through culture, music, food and activities.

Genevieve and Richard’s grandson visit 3 to 4 times per week, and can often be seen strolling Richard through the gardens to visit the farm.

VOLUNTEER EXPERIENCE In FY 2012-2013, 493 individuals volunteered at Laguna Honda for a total of 32,365 hours.

In honor of their dedication, the Friends of Laguna Honda hosted its annual appreciation dinner for the hospital’s volunteers in April 2013. Over 100 volunteers attended the dinner, which was held in the Pavilion Café. Appreciation awards were presented to three volunteers: Bea Gunn Johnny Adkinson, and Sister Nancy Bolton.

Bea Gunn worked as a nurse at Laguna Honda for over 30 years. Bea became a volunteer after she retired, four years ago. Bea spends her volunteer time filling

in wherever she is most needed: going on community outings, in the nursing department or at the Positive Care and Hospice Neighborhoods. She recently started helping with the hospital’s free clothing program and was an interviewer for the Family and Resident Satisfaction Survey.

Johnnie Adkinson is a former resident who has volunteered at Laguna Honda for 10 years. He has contributed over 9,500 volunteer hours. He

works one-on-one with people at the Positive Care neighborhood five days a week. Once a week, he runs the volunteer Bingo. He is compassionate and empathetic having first-hand knowledge of what it means to be a resident at Laguna Honda.

Sister Nancy Bolton is a loving and spiritually generous chaplain whose guidance is a comfort to many in times of need. She has been a

volunteer at Laguna Honda for over 10 years. She has been faithful to visit, hold hands, cry with, laugh with and just hang out on the Positive Care neighborhood.

STAFF MILESTONESRECOGNITION AND AWARDS

Compassionate Care Leadership Award The Hospice and Palliative Care Program was the recipient of the 2013 Compassionate Care Leadership Award given by the Coalition for Compassionate Care of California. The award recognizes contributions to palliative and end-of-life care in California. The Coalition said that Laguna Honda has “transformed end-of-life care in the Bay Area, serving as a training site for hundreds of healthcare professionals and introducing the concepts of hospice and palliative care to an urban nursing home that serves economically disadvantaged and culturally diverse populations.”

UCSF Evidence-Based Practice Fellows In June 2013, five members of the Laguna Honda nursing staff completed fellowships in evidence-based practices. Maria Antoc, Grace Chen, Zexu Fang, Myrene Fuertes and Jacky Spencer-Davies, wrapped up a six month quality improvement program at the UCSF School of Nursing, where they researched and implemented nationally recognized innovations in nursing care. The fellowship aims to improve patient satisfaction and nursing quality by encouraging adoption of practice improvements. Nursing Operations Director Edward Guina coordinated the hospital’s participation.

Daisy Award The Daisy Award is a national program conceived by the Barnes family in memory of the excellent care their son Patrick received from nurses before he died at the age of 33. Each year during Nurses Week, the hospital recognizes the exceptional work nurses provide through the Daisy Award recognition. This years’ Daisy Award winner is Zosima Pangilinan, North 3 Certified Nursing Assistant. Zosima is an extraordinary

caregiver who sets the bar for quality and compassion in her profession.

EMERGE Leadership Fellow Loretta Cecconi, Interim Nutrition Services Director and Chief Dietitian was accepted into the 2013 EMERGE Leadership Program sponsored by LeadingAge of California. The program is an opportunity to engage in learning experiences that include visits to best practice community organizations, meetings with leaders in the senior and disability service sectors, and trainings on leadership development. Loretta says, “EMERGE will assist me as a leader dedicated to innovation and quality care.”

EMPLOYEES OF THE MONTH July 2012 – Spiritual Care Bob Deel is responsible for spiritual care at Laguna Honda. He coordinates all chaplains, church groups, and volunteers. He was given this award for developing a program called “No One Dies Alone” that ensures the availability of chaplains or volunteers at a person’s bedside at end-of-life.

August 2012 – Gardeners Gardeners John Alexander and Noah Santiago are responsible for maintaining and beautifying the 66 acres and 26 landscaped gardens at the campus. They were recognized for their work securing an exquisite orchard of fruit trees by entering and winning a contest sponsored by Dreyer’s Fruit Bars.

September 2012 – Information Technology IT Help Desk employees Jorge Wong and Barney Chan are often on the run responding to calls to repair staff computers. They were awarded for their outstanding customer service skills in keeping desktops, laptops, phones and iPads up and running.

October 2012 – Discharge Neighborhood The N3 Discharge care team and staff were acknowledged for their innovative work in preparing residents for discharge back to the community and establishing the discharge program.

November 2012 – Nutrition Services Diet Clerks and kitchen staff were awarded for their support, courtesy, and reliability in accommodating special food requests, early tray deliveries and last minute changes to resident’s food orders.

December 2012 – Human Resources The Human Resources team was awarded for their excellent customer service when responding to employee and labor union matters. They exemplify the principles of Service Excellence.

January 2013 – Facility Services Jim Yakas is an engineer who works the night shift. He was nominated by nursing staff for being diligent, supportive and responsive. In addition to his paid job, Jim has worked many years as a volunteer at the hospital taking residents on outings during his off time.

February 2013 – Nursing Chickie San Juan was nominated by a resident she has taken care of for many years. She described Chickie’s work as a patient care assistant to be of the highest level in terms of professionalism, efficiency, and resident centeredness. “She treats me like a person.” –Resident Elizabeth C.

March 2013– Rehabilitation Department The Rehabilitation Department was recognized for its successful work in integrating rehabilitation services at Health at Home, Laguna Honda and San Francisco General Hospital. They have contributed significantly to patient flow, working to increase referrals from San Francisco General Hospital and to get patients to their highest levels of performance so they can live independently in the community.

April 2013 – Dietitians The Dietitians received more service excellence nominations than any other department. They were commended for their positive attitude, their teamwork, their commitment to excellence and their service recovery.

May 2013 – Nursing Raul Ramirez, CNA was recognized for creating a friendly atmosphere, for turning negative situations into positive ones, for projecting an optimistic attitude, and for being passionate about excellence.

June 2013 – Activity Therapy John Chan, Activity Therapy Supervisor brings events, music, and tons of love and volunteers to the hospital. He was given the award for his outstanding contribution to Chinese New Year activities.

Training Future Healthcare Professionals Statistical trends warn that the baby boomer population is now nearing retirement and that preparing in-coming staff to fill their vacated jobs will be crucial to ensuring continuity in care. Rehabilitation Department Intern Laguna Honda’s Rehabilitation Department collaborated with the Mayor’s Office of Economic and Workforce Development to provide on the job training for students in the health care field. For one year, Aleia Gordon who graduated from Oklahoma City University in Biomedical Sciences, worked side-by-side with our physical, occupational, audiology and speech therapists preparing

for a future career in physical therapy. Aleia summarizes her experience as follows:

National Youth Leadership Forum The National Youth Leadership Forum on Medicine introduces outstanding high school students with an interest in medicine to all aspects of the medical field. Students are selected from all over the country to participate in this annual program. In June and July 2013, 30 students spent two days at Laguna Honda in addition to visiting local medical schools, hospitals and clinics.

Nursing Students Laguna Honda provides clinical training sites for many nursing students including RNs, LVNs, and Nursing Assistants. A total of 320 nursing students trained with Laguna Honda nursing staff.

SF Youthworks Interns Vienna Chen, Kimberly Wong and Becky Wong were part of a city-wide program created by the Mayor’s Office to provide mentorship programs encouraging high school students to consider careers in the public sector. They spent the summer assisting the nursing, communications and administration divisions at Laguna Honda. After her first week at the hospital, Kimberly ecstatically said: “This experience really makes me want to pursue a career in the medical field…maybe pharmacy.”

UCSF Family and Community Medicine Residents This is the second year of a long term care rotation at Laguna Honda Hospital for 30 second and third year UCSF Family and Community Medicine residents. Residents receive didactic and hands-on teaching from Laguna Honda attending physicians on a monthly basis, learning to manage a range of medical and psychosocial problems common to patients in long term care.
