A Note from the Director February 2020 Dear Friends, Crossriver Quality Health Partners is proud to present our 2019 Annual Report in which we

summarize some of our collective accomplishments and illustrate the benefits of Clinical Integration

across our region. As we face 21st century challenges in a rapidly evolving health care environment

where value is paramount, Crossriver is ready to serve as a resource and guide.

Highlights from this past year include first, improving the quality of care throughout the region. Our

Physician Champions, as well as members of the Performance Improvement and Quality

Committee, developed several new quality metrics and created up-to-date, evidence-based

protocols to ensure that the care we deliver across our network is first rate. Second, we created

individualized quality scorecards for each of our providers, which deliver a high level of detail and

insight into the care they are providing. Given the challenge of improving quality, our providers and

staff were certainly up to the task. Because of their exceptional work, we have performed favorably

on our Employee Health Plan contract now two years in a row, and have been very happy to

distribute significant shared savings to our participants. This year marked the first time many of our

providers have ever received payment for quality outcomes and is surely a trend that will continue.

We have entered a collaborative Accountable Care Organization and, with guidance from Caravan

Health, are focusing on the quality, cost, and coordination of care for our Medicare patients. We

have gained the attention of local employers who are starting to recognize us as a potential partner.

Finally, we are celebrating growth with the addition of Scotland County Hospital and Clinics as our

newest Crossriver Partner.

The work we have undertaken is both modern and transformative, and reflects the combined,

aligned efforts of all of our providers and staff. We have begun the first steps on this journey

together, and I hope you enjoy the report.

About Crossriver Quality Health Partners

Crossriver Quality Health Partners is a group of healthcare providers from our region who have made a collective commitment to performance improvement by focusing on the quality and efficiency of care for the patients they serve. To support those efforts, they are able to contract together to help achieve their common healthcare goals. Crossriver seeks to:

Improve the patient experience, not just patient satisfaction scores, but access to healthcare through patient portals and access to non-provider members of the healthcare team who can manage many day-to-day matters of chronic disease management and wellness,

Improve the health of the network’s patient population using evidence-based, chronic disease management programs, evidence-based referrals, and wellness programs,

Reduce the per capita cost of healthcare through wellness programs, evidence-based referral,

Crossriver Quality Health Partners (CQHP) is comprised of the following groups of leading hospitals, physicians, and clinicians in the region. Crossriver consists of over 300 providers providing healthcare in Illinois, Iowa, and Missouri.

Blessing Hospital

Quincy, IL

A 327-bed acute-care, not-for-profit, non-tax supported, independent hospital. Fully accredited by DNV-GL Healthcare.

Blessing Physician Services

Quincy, IL

A multi-specialty practice group consisting of over 100 providers, clinic and clerical staff who serve residents of western Illinois, northeastern Missouri and southeastern Iowa. Operates medical offices in Quincy as well as rural clinics.

Illini Community Hospital

Pittsfield, IL

A 25-bed critical access hospital that is part of the Blessing Health System.

Hannibal Clinic Hannibal, MO

A multi-specialty practice group consisting of over 50 providers in 25 various specialties who serve the communities of northeast Missouri. Operates medical offices in Hannibal as well as rural clinics.

Riverside Dermatology Hannibal, MO

A full service medical and aesthetic dermatology office and spa serving the communities of northeast Missouri.

Scotland County Hospital

Memphis, MO

A not-for-profit critical access hospital with hospital-based rural health clinics serving the communities in northeast Missouri.

SIU Center for Family

Medicine Quincy, IL

Federally Qualified Health Center within SIU School of Medicine designed to promote comprehensive, continuous family care for Quincy and surrounding communities.

Crossriver Staff

Christopher Solaro, MD, PhD, CPE

Executive Director

Dr. Solaro is the Executive Director for Crossriver. He completed an MD and PhD at Washington University School of Medicine followed by internship and residency at the University of Texas Southwestern Medical Center at Dallas. In addition to his work with Crossriver, Dr. Solaro is the Chief of Medicine for Blessing Hospital and a board-certified Emergency Medicine Physician.

Jordan Kiel Executive

Administrator

Jordan is the Executive Administrator for Crossriver. Jordan holds a Bachelor of Science degree in Healthcare Administration from Indiana University as well as Master of Business Administration degree from the University of Southern Indiana. In addition to his work with Crossriver, Jordan is also the Executive Director of Current Health Solutions.

Gerri Buss, MS, RN Manager of Quality

& Performance Improvement

Gerri is responsible for overseeing all aspects of Quality and Performance Improvement. She assists the participating partners with process improvement and project management. She also serves as the ACO Champion for our relationship with Caravan Health. Gerri is a Lean Six Sigma Black-belt and received a Bachelor of Science degree in Nursing from Southern Illinois University at Edwardsville, a Master of Science in Nursing from the University of Missouri at Columbia, and is actively pursuing a Doctorate of Nursing Practice degree at Northern Kentucky University.

Kelly Awerkamp Manager of Data

Analytics

Kelly is responsible for all data projects that come through Crossriver. She manages the intake and production of each project and works on various platforms to analyze the data. Kelly holds a Bachelor of Science degree in Finance from Eastern Illinois University.

Brittany Terwelp Business Analyst

Brittany serves as report writer, developer, and data analyst for Crossriver. She extracts data from various platforms to develop reports and analyze key measures across the CIN. Brittany is actively pursuing her Lean Six Sigma Green-belt and holds a Bachelor of Science Degree in Mathematics with an emphasis in Actuarial Science from Illinois State University.

Liz Hoffman Administrative

Assistant

Liz manages the day to day office operations for Crossriver. She maintains the provider list and manages all aspects of marketing and communications, including the website, newsletter, and podcasts. Liz is a Lean Six Sigma Yellow-belt and holds a Bachelor of Science Degree in Healthcare Administration from Purdue Global University.

Board of Directors & Committees Board of Directors The Crossriver Board currently has three Administrative and eight Provider Directors. These directors oversee

the three active sub-committees and guide the Crossriver Management Team and Physician Champion Team.

During 2019, the Board met on a monthly cadence. Moving forward into 2020, they will meet every other month

on the fourth Thursday.

IT & Analytics Committee (ITAC) This committee works to communicate framework and processes to the Board as they

pertain to updates in technology and data analytics. They serve as a working group

responsible for the development and vetting of strategies, requirements, and policies

regarding information technology and analytics capabilities needed to support Crossriver’s

goals. This includes, but is not limited to, improving quality, coordination of efficiency of

care, and assuring success under value-based reimbursement arrangements by Crossriver

and its network participants.

Performance Improvement & Quality Committee (PIQC)

This committee works in conjunction with ITAC and the Physician Champion Team to

provide quality data, provider payout calculations, and evidence based protocols. The goal

of the committee is to improve clinical performance and service quality across the

Crossriver provider network, including quality of care, patient experience, appropriateness

and efficiency of care, and total cost of care.

These committees meet together on the third Thursday of the month.

Operations & Finance Committee This committee is responsible for the development and vetting of strategies,

requirements, and policies regarding professional standards, credentialing, recruiting,

communications, and other operational activities. In addition, they are responsible for the

development and vetting of strategies regarding budget, payer strategy, contracting, the

Performance Improvement Plan, and other activities needed to support Crossriver’s

financial and budget goals. This group meets on the second Thursday of the month and

works to advise and assist the Board in its oversight of the operations of Crossriver.

Physician Champion Team The Physician Champion Team is an advisory group of providers and nurses from each participating entity, as

well as Crossriver quality and data analytics staff. The purpose of this team is to improve quality through the

review of evidence based practices and to develop new provider protocols. As an advisory group, this team is not

held to the same rules and regulations as a committee. There is no appointed chairperson for this group. They

meet on the first Wednesday of the month.

Provider Engagement Activities Crossriver Connection Newsletter The Crossriver Connection is a bi-monthly

publication distributed throughout the

Crossriver Network. The Crossriver

Connection is an avenue in which the CIN

stays up to date on new initiatives and

goals for the Caravan Health Accountable

Care Organization, current evidenced

based protocols, provider spotlights, best practices and much, much more. In the September/October issue, a

new ‘How’d they do it?’ feature was added to spotlight the great work our top performing providers are doing

within their practices.

Crossriver Connection Podcast Crossriver launched our first podcast on June 3rd, 2019 with an episode providing a snapshot of who Crossriver is,

what our mission is, and where we are heading in the future. To date, three episodes have been recorded that

spotlight who we are, provider quality dashboards, and an overview of the ACO. Plans are in place for a 2020

schedule.

Provider Scorecards The first provider level quality scorecards were developed and distributed in 2019. These scorecards supply a

monthly snapshot of how each provider is currently performing compared to the Crossriver goal and their peers

on our 7 key quality metrics. Providers are able to quickly discern between metrics performing well to those that

have room for improvement. Crossriver also began awarding a monthly quality trophy for the top performing

provider in each metric. It is a traveling trophy, therefore the providers must work to stay at the top to keep it as

long as possible.

Lunch & Learns In July 2019, Crossriver hosted our first Coffee Klatch wherein providers and delegates from their departments

came together to discuss a number of Crossriver topics. A few questions that arose from this discussion are as

follows: Do specialists qualify for quality metrics? How can a specialist make an impact on the score? Is there

actual money tied to the scorecard? If a patient refuses a test or vaccination, does that count toward the metric?

All of these questions and more were answered and a lively discussion ensued. Future dates for Lunch & Learns,

Coffee Klatches, etc. will be distributed in 2020.

Shared Savings Distribution In the fall of 2019, Crossriver distributed shared savings to qualifying providers due to the CIN achieving success

in over 70% of the goal set by the 2018 Blessing Health Plan value-based contract. The CIN earned roughly

$415,000 from efforts centered on reducing the cost of care for the employee health plan, aligning members

with a primary care doctor, and improving the agreed upon 2018 key quality metrics.

Crossriver developed a process to distribute funds back to providers in which points are earned based on

personal performance related to the key quality metrics along with an annual volume multiplier for those

providers delivering high quality of care to a high number of patients. (A monthly snapshot of the quality points

earned can be found on the provider scorecard.) A calculation was established to determine the value amount of

each point: Total Points Earned by providers throughout the CIN is divided by Total Dollar Amount to be

distributed to determine a Dollar-per-Point Value. This methodology allows Crossriver to distribute funds back

to providers based on their individual performance of the key quality metrics at the end of the year.

In addition to distributing shared savings from our 2018 performance year, Crossriver will also have an

opportunity to soon distribute shared savings for our 2019 performance year. This will include our financial

achievements as it relates to both our Employee Plan agreements as well as our ACO program with Caravan

Health. We are excited to continue to be able to share our success via value-based incentives with our

participants and their providers.

Top Provider Notification In August 2019, the first Top Provider Notification email was distributed. This list utilized data from the provider

scorecards and illustrated the providers scoring in the top five percent of their peers. This publication is

distributed following the delivery of the provider scorecards.

Protocol Development & Distribution The Physician Champion Team works to develop evidence-based protocols. The team performs literature reviews, evaluates current nationally recognized standards, and invites subject matter experts to the discussions in order to create the protocols. Each draft is first presented to the ITAC/PIQC Committees for discussion, review, and approval. The protocol is then presented to the Board of Directors for final approval.

Approved protocols are distributed to providers in a number of ways; a) Posted on the Crossriver Website, b)

linked to the website via the Crossriver Connection publication, c) emailed to department supervisors or

designated administrative partners along with the monthly Provider Scorecards.

Mobile Application During the follow-up discussions with committee members regarding the provider survey, a

suggestion was made to develop a Crossriver mobile application in order to easily access the

provider protocols. After much research, it was determined that a mobile application would not

be cost effective for Crossriver when compared to the desired functionality of the app. Thus, a

solution was found, to create a link on the homepage of personal smart devices that directed

the user straight to the protocols page on the Crossriver website. Directions on how to link

directly to the protocols from your smart phone can be found in the Provider Digest of the

Crossriver website by typing the keywords: Protocols Mobile App Setup.

Website The Crossriver website (www.crossriverquality.org), is a multi-purpose point of access for the Crossriver

network. The public facing side of the site showcases the mission and vision of the organization, newsletters,

podcasts, and a comprehensive, searchable, and up-to-date provider directory. On the private, provider side of

the website, providers are able to log in to a password protected area and see a more robust provider directory

with contact information as well as articles and content not intended for the patient population. Providers have

access to update their own information, such as their education history, contributions to research, publications,

office hours, and personal biographical information.

Provider Survey In September 2019, Crossriver distributed its first annual provider survey. The survey was conducted via Survey

Monkey and was distributed to all entities within the Crossriver Network. It was comprised of nine (9) questions.

Some highlights include the following:

basic idea of a Clinically Integrated Network,

Majority of respondents had not yet listened to a Crossriver Connection Podcast,

Over 80% of respondents were aware Crossriver had entered into a value-based contract with the

Blessing and Hannibal Clinic Employee Health Plans,

Majority of respondents wish to obtain information regarding Crossriver via the newsletter, and

Over 90% of respondents are interested in attending a Crossriver sponsored Provider Continuing

Medical Education (CME) Event.

Provider & Entity Growth Scotland County Hospital

Crossriver was proud to welcome Scotland County Hospital (SCH) to the Clinically

Integrated Network in 2019. We look forward to collaborating with SCH to improve not

only the lives of their community members, but also the lives of the providers that

participate within our program. In working to fulfill our mission of uniting physicians and

other healthcare providers, we are confident that SCH will be a valuable addition to the

team, and are eager to learn alongside them as a likeminded and well-respected

organization. SCH is a non-profit district hospital in operation since 1970, designated as a Critical Access Hospital

in Memphis, Missouri. Since 1997, SCH has established three hospital-based rural health clinics in Scotland,

Clark, and Schuyler counties. SCH has continually demonstrated a deep commitment to the rural and remote

geographic area they serve and are the only hospital in a five county area of Northeast Missouri. Like the other

entities within Crossriver, SCH is dedicated to more than just providing emergent medical care. They are

dedicated to providing services to help prevent chronic medical conditions from occurring. SCH does this by

following their mission “to improve the healthcare of our communities with services close to home”. This

mission is inclusive of the uninsured and under insured.

Onboarding for SCH is complete and they are actively joining the Caravan Health ACO.

Riverside Dermatology

Crossriver was proud to announce the addition of Riverside Dermatology to the

Clinically Integrated Network in 2019. Riverside is a full service medical and

aesthetic dermatology office and spa that serves the communities of northeast

Missouri. Onboarding for Riverside is underway.

Performance, ACO Launch, & “Member” Growth Medicare Shared Savings Program – Caravan Health Collaborative ACO On January 1, 2019, Crossriver joined a Medicare Shared Savings Program (MSSP) - Accountable Care

Organization (ACO) with a convener organization, Caravan Health. Caravan has vast MSSP and ACO experience

with a proven track record of success and is presently one of the largest ACO conveners in the country. We

joined their largest Collaborative ACO program that is comprised of over 200,000 Medicare lives, this type of

Collaborative approach allows Crossriver to benefit from more sustainability through a “strength in numbers”

approach. With Caravan’s experience, they also streamline the application process and assign their team of

experts to assist ACO participants with focusing on key initiatives of the program, which are further illustrated

below.

Crossriver work related to these ACO initiatives began long before the official launch with educational efforts

focused on the key performance indicators (KPI’s) important to the overall success of the Collaborative ACO. The

ACO KPI’s include Annual Wellness Visits (AWV), Chronic Care Management (CCM), Hierarchical Category

Condition (HCC) Coding, and a variety of identified Quality/Promoting Interoperability Metrics.

In addition to these KPI’s, the Crossriver team also implemented a variety of administrative and compliance

related efforts to ensure a smooth and efficient entrance into the MSSP. This included, but is certainly not

limited to, some of the following items; distribution of ACO posters and desk notices, election of designated

ACO roles such as ACO Champion, Physician Lead, and Quality Manager, Data Analytics training, monthly

roadmap calls and quarterly steering meetings, and education to ACO participants and Crossriver Board

Members.

Preliminary Key Performance Indicator Efforts As illustrated above, the KPI’s specific to the Caravan Collaborative ACO are Annual Wellness Visits, Chronic

Care Management, and Hierarchical Category Conditioning Coding. The Crossriver team worked closely with

Caravan experts as well as leaders and staff members at our participating organizations to achieve optimal

outcomes related to these KPIs.

We were able to develop policies and protocols related to these items as well as work to provide AWV

Scorecards, HCC Facesheets, and Population Health data to support to CCM activity. All of these efforts in 2019

will allow Crossriver to continue to improve our outcomes and fine-tune our specific strategies related to these

KPIs. These efforts also allow our participants to achieve positive results for these KPIs even for non-ACO

membership, which can generate further quality and financial outcomes related to additional value-based care

and payor specific programs.

Quality Measures An additional KPI is related to our quality of care for a variety of metrics. These categories of metrics are as

follows:

Crossriver completed a review of these quality measures in the beginning of 2020 and will be submitting our

2019 data in the Lightbeam GPRO Module for patients randomly selected by the Centers for Medicare and

Medicaid Services (CMS). Crossriver will also be submitting calendar year data to Caravan via their portal for

seven of the ten quality measures, this will aid in determining the quality benchmark for shared savings

distribution per the ACO distribution procedure further outlined below.

Promoting Interoperability (PI) Crossriver has been working with staff at partnering entities to assure the PI attestations and measures are

improved and submitted to the CMS-QPP Portal.

ACO Shared Savings Crossriver is pleased with our first-year efforts related to the Caravan Collaborative ACO. Through their strength

in numbers approach and elected KPIs, the projected Collaborative ACO savings are equal to roughly a 3.6%

overall savings rate. This is essential to Crossriver’s sustainability and allows us and our participants to share in

this success financially, as well as invest in additional performance improvement efforts in the future tied our

mission of the quadruple aim.

The Caravan Collaborative ACO model for receiving shared savings distribution is tied to a few different factors.

40% of all attribution is based on the number of attributed lives, an additional 40% is then allocated based on

performance related to the key quality measures, and the final 20% based on performance related to CCM

activity. With these factors considered, Crossriver is projected a shared savings amount of roughly $1,500,000

based on our ACO performance in 2019. This preliminary estimate will be finalized in the coming months and

distributed accordingly in the fall of 2020.

Employee Health Plan Contracts Blessing Health System and Hannibal Clinic Employee Health Plan Similar to 2018, Crossriver worked with key stakeholders to identify initiatives to better assist the Blessing

Health System Employee Benefit Plan in managing the health of their employee population and their overall

healthcare expenditures. These initiatives were also rolled out to the Hannibal Clinic Employee Benefit Plan

beginning on January 1, 2019.

Both Employee Benefit Plans executed a non-risk bearing services agreement with Crossriver for the following

services:

Managing the Total Cost of Care for health services received by their membership within 2019,

Solidifying a detailed process and mechanism for partnering with Blessing’s Care Coordination team to

provide Care Management services to Crossriver’s existing programs as well potential programs in the

future, and

Improving 7 Key Quality Measures by a composite percentage increase of 9.1%. This initiative would

encompass all populations served by Crossriver.

Through the efforts of our staff and many others involved, Crossriver is pleased with our preliminary results for

the 2019 Performance Year. We anticipate achieving roughly 65% of our annual stretch goal for the three

initiatives referenced above. These achievements not only aid us in sustaining Crossriver financially, but it also

provides us the privilege of sharing our successes with Crossriver participating organizations and their providers.

Transitions of Western Illinois In addition to the contractual initiatives above, Crossriver structured an agreement effective on July 1, 2019 with

Transitions of Western Illinois, located in Quincy, Illinois. This agreement enables Crossriver to work in

conjunction with Current Health Solutions to provide Care Coordination services to participants of the

Transitions of Western Illinois Employee Benefit Plan. While initiatives are just recently underway due to

anticipate claims data lag, Crossriver is eager to work with this population and to utilize the streamlined Care

Coordination processes referenced above to most effectively service this population.

Our Success Story:

How many lives have been impacted? In 2019, the staff at Blessing, Hannibal Clinic, Scotland County, and SIU ensured that:

644 additional patients were up to date on their Breast Cancer Screenings

899 more patients had completed the necessary Colorectal Cancer Screenings

1,159 additional patients now have their blood pressure under control

2,197 additional patients have their pneumococcal vaccine up to date

6,850 more patients either had a flu shot administered or were offered and declined

20,873 more patients were screened for depression than in 2018!

Looking Ahead

As we reflect on our past year’s progress, we are pleased with our initial efforts, but understand there is

more work to be done. We feel confident that through the guidance and leadership of our Board of

Managers, Committee Participants, Crossriver Staff, and many others that make our initiatives possible, that

we can continue to provide higher quality care to the communities in which we serve.

While many of the previously listed efforts will continue in 2020, we are eager to explore additional

opportunities to further engage our participating organizations and providers, as well as hold ourselves

accountable to the valuable work we complete. This commitment is evidenced by a variety of our actions,

including, but not limited to the following:

Increasing our number of Key Quality Measures in 2020 as well as setting the most aggressive

improvement goals in our history,

Electing to proceed with Caravan Health’s Pathways to Success, which allows us to further fast-

track our entry into Value Based Care through our first downside risk contract,

Further establishing initiatives and programs that help our partners lower their total costs of care

and the health of their population, which can be replicated and provided to additional customers

throughout region, and

Evaluating and researching Social Determinants of Health concerns within our community and

brainstorming opportunities to provide solutions to those in need.

to achieve the quadruple aim in the communities we serve.
