Q1- CHC staff have regularly attended appropriate AAC meetings. The steering committee (CHA/CHIP workgroup) met in February and will meet again in April. See meeting minutes in attachments. This group is tasked with determining outcome objectives for the priority areas and to develop strategies for 2017-2018.A Celebration of Health event is being planned for August as a way to wrap up the PICH grant and to celebrate the successes Activate Allen County has had thus far.Q2- The Executive Council of Activate Allen County met in May. A program evaluation is underway which will provide findings and recommendations for moving forward with the structure of AAC. A community celebration is planned for August 11 and 12 to celebrate the

Updates were also shared on the CHA/CHIP and Community Health Assessment. See meeting minutes included in attachments.The Community Health Assessment results were shared at a review held in June. The official release of results will take place in August at the community celebration.Q3- The steering committee met in July, but Shelly was unable to attend. See meeting notes included in attachments.The executive council did not meet this quarter.The results of the Community Needs Assessment were unveiled at the Activate Allen County Stakeholder meeting event on August 11. Successes of the PICH grant and AAC were shared. There was also a panel of speakers that shared their involvement with AAC throughout the year, including Mayors from Spencerville and Bluffton and CEO from St. Rita’s Medical Center. A community celebration was also held on the following day (Saturday, August 12).The formal structure of Activate Allen County is still a work in progress, but with

the PICH grant ending at the end of September, funding has been allocated from the local hospitals to keep AAC running. The co-directors for AAC will be moving into their new offices in the Chamber of Commerce building. They do not have a clear definition of their job description, but most of their work will be focused on the CHIP. CHC will be actively involved in the wellness workgroup with a possibility of co-chairing this group. This would be identified as the CHC Coalition.Q4-

Q1- Bike and Pedestrian Task Force – The Bike/Ped Steering Committee is chaired by Monica Harnish, CHC Staff, and met in January to set goals for the year, schedule quarterly meetings and to prepare for the February Task Force meeting. At the Task Force meeting in February, the overall goal and plans for the year were shared with the group.See attachments for meeting notes of each meeting.Tobacco Free Coalition – The Tobacco Coalition met in February. Shelly Miller, CHC Coordinator, continues to chair this group. Shelly shared the CHC Tobacco objectives with the group and asked for feedback for project focus. Several

suggestions were made. Shelly also shared the Ohio Strategic Tobacco Plan and mentioned an opportunity for a LIFT training to be offered in our area to help supplement our lack of cessation resources. See attachments for meeting notes. The next meeting is scheduled for April 12. The focus of this meeting will be to develop a plan to address tobacco aligning with CHIP strategies and how to increase local resources for cessation.Q2- Bike and Pedestrian Task Force – The Bike/Ped Steering Committee met in April to plan the Task Force meeting in May. The featured speaker was Larry Cress from the Wheelhouse Bike Ministry. A crash data report was shared as well as engineering updates. Sophie Finlayson Schueler from Regional Planning shared her experience of participating in the “How we Roll” ride and discussed how the program options can be offered here. The Allen County MoveSafe Coalition was announced to the task force and those present were invited to join the group. See attachments for meeting notes of each meeting.Shelly also attended a meeting in April about a bike share program for Lima. A preliminary discussion took place as to the feasibility of a bike share in Lima and related issues and concerns. A small

group is researching company options and follow-up meetings will continue to take place.Monica attended a Transportation Advisory Groups meeting in June. The groups discussed the bike share opportunities in Lima, pedestrian infrastructure, updates on proposed projects (USBR 25/40, Miami-Erie Canal project, Riverwalk expansion to OSU/Rhodes campus), update on the Active Transportation Plan, and new projects. See agenda included in attachments. No meeting notes were available for the quarterly report deadline.Tobacco Free Coalition – The Tobacco Coalition met in April, May, and June. The focus of the meetings has been on developing a plan to address tobacco, aligning with CHIP strategies, and how to increase local resources for cessation.See attachments for meeting notes.Q3- Bike and Pedestrian Task Force – The Bike/Ped Steering Committee met in July to plan the Task Force meeting in August. Q1- Food Policy Council – The Food Policy Council held several meetings in the first quarter. The full group met in January. Most of the discussion was on the Farmer’s Market grant and planning for applying and how to revitalize the current Farmer’s Market. Also discussed was what could be done with additional funding sources. Several ideas are being tossed around – supporting the aquaponics garden, developing a commercial kitchen, supplementing the Land Bank program. A follow-up meeting was set for February 17 to work on the Farmer’s Market grant. A meeting was held with the vendors on March 9. Shelly was unable to attend this meeting. A Farm to School meeting was held on February 22. Thom Mazur, MPO at the Regional Planning Commission called this group together to gauge if this is a project we need to move on or just let go. A brief history of work done thus far was provided as well as some possibilities for options of Farm to School practices. Main topic of discussion was how to grow interest in produce farming.Shelly met with Beth Seibert to plan a Gardening Forum for residents in Allen County that are interested in gardening. See more in notes for objective 8.

Healthy People 2020 – met in February and March. In February, a presentation was given for social media tips and tricks. March’s meeting including information on Food Safety for bake sales, farmer’s markets, etc. from a Sanitarian at Allen County Public Health. CHC Staff, Tami (chair) and Shelly, both attend regularly and share CHC updates as appropriate.Safe Communities – meets the second Monday of every month. Shelly attends the meetings regularly and provides updates on CHC initiatives as applicable. An increased focus on bike safety is being looked at with this group, but typically, initiatives revolve around motorcycle safety, seatbelt use, and drunk driving. Through membership of Safe Communities, Shelly was able to become involved with a new Pedestrian Safety Committee, solely tasked with addressing the issue of bike and pedestrian safety issues. This group was developed as a subcommittee of the Safety Review Team which has a fatal review board. This group is also being considered as a subcommittee of the Bike and Pedestrian Task Force, assisting with education and enforcement goals for 2017. See notes in objective 4.Q2- Food Policy Council – The Food Policy Council did not meet this quarter. A

Farmer’s Market Advisory committee has been established. Meetings were held in April and June. The Health Commissioner was invited to be a part of the committee and attended both meetings. Since then, she has decided it would be best for Shelly to attend the meetings in her place. A meeting will take place in July that Shelly will attend. See meeting notes included in attachments.The gardening forums continue to take place every month and are well attended. See more in notes for objective 8.Shelly attended the Ohio Food Council Workshop in June on strategic planning for food councils. Kayla Monfort, PICH, attended a workshop in May on stakeholder engagement and relational community organizing.Healthy People 2020 – met in April, May, and June. In April, a Campus Sexual Assault Survivor Advocate shared information about the program available to the county’s local colleges on educating students about sexual assault and way to prevent it from happening. In May, the City of Lima Neighborhood Specialist shared updates on neighborhood projects and what services are provided through her position. In June, information was shared on the local LOSS (Local Outreach to Suicide Survivors)

See meeting notes included in attachments.Safe Communities – meets the second Monday of every month. Shelly attended the meeting in April. Updates were shared for the MoveSafe Coalition with a focus on reaching out to Nelson Packaging to provide education on which way to walk. This is a mid-size factory located on the outskirts of Lima that has been identified as having a number of employees who walk/bike to work and have had problems with walking on the wrong side of the road, biking at night without proper lighting, and other issues that put their employee’s safety at risk.Shelly also attended a meeting at LCC with a subgroup of members from the Safe Communities Coalition meeting with the principal to discuss a project on driving safety for teenagers, relating it to the bike/pedestrian crash issue in the City/County. The school has a group of students that is ideal for this type of project. The principal was going to take the information to the group and then schedule a follow-up meeting. A meeting was scheduled in June, but Shelly was unable to attend.Q3- Food Policy Council – The Food Policy

Council met in September. The group has not met for quite some time, so many updates were provided on different projects taking place over the summer. Discussion also took place on some old business and the future focus of the group. Meeting dates for 2018 were set. See attachments for meeting minutes.The Farmer’s Market Advisory committee held meetings in July and August. See meeting notes included in attachments.The last community garden forum meeting took place in September. A review of the year took place as well as planning for next year. Shelly was unable to attend this meeting.A new gardening group has been established to focus on the issue of a production garden and many other food system components. The group is currently referred to as the “Kismet Initiative.” LACNIP community gardens have partnered with Jeff Givens who owns a large garden with aquaponics on the outside of Lima (Kismet Gardens). He is helping with the aquaponics system at LACNIP. He would like to provide an opportunity for training and entrepreneurship to improve healthy food access. Healthy People 2020 – met in August and September. In August, information was shared from the Alzheimer’s Association. Shelly provided updates on the community gardens and the bike rack/active commute program of CHC and informed the group about the upcoming Bike and Pedestrian Task Force Lunch and Learn on active commute. A representative expressed interest in getting a bike rack at the United Way building. (See notes in objective 4 for more information.) In September, Activate Allen County gave an update on their restructuring and provided a discussion on the upcoming bike and pedestrian counts. Shelly provided an update on the urban agriculture possibilities in the City of Lima and invited anyone who is interested to become involved. See meeting notes included in attachments.Safe Communities – meets the second Monday of every month. Shelly attended meetings in July and August. Updates were shared for the MoveSafe Coalition referencing the high number of pedestrian crash rates and pedestrian crossing issues. Shelly also informed the group that the CHC program is one of the recipients of the active transportation grant funding which

will focus on law enforcement training.MoveSafe – met in July to discuss topics, next steps, and how to use media to help with messaging. The main focus of this group will be on a community education campaign linked to the active transportation funding.Shelly also attended a planning meeting for an event taking place at Hermon Woodlands in October. They are looking at a style that can be freestanding and not a permanent structure. planning needs to take place, but the goal is to hold an experiential ride in the mid-late summer following a Professional Development Ride being held in Lima on June 19. Ride Buddy programs can be promoted and offered as part of the bike rack program offered by CHC, as well as to other businesses who have employees interested in biking to work and needing assistance with route planning.

*Shelly has applied for Active Transportation funding from ODOT to host a law enforcement bike training program in late summer and through the next two years. Funding announcements are supposed to be made in May.Q2- CHC and the Bike/Ped Task Force promoted the Professional Development Ride to members of the community. The ride had a full roster which included Derrick Schierloh with ODOT, Kirk Niemeyer with the City of Lima, Ted Baker with Cranker’s Cycling, and Ginny and Larry Cress with the Wheelhouse Ministry.Shelly has also aligned both organizations that applied for bike racks with the Wheelhouse Program. This program provides bikes to those in need of alternative transportation. Clients have to be referred by social service agencies to be considered a recipient. Shelly arranged for

The case workers will be provided information on how to refer clients to the Wheelhouse when they have a client in need of a bike for transportation. New Life Church is also interested and a meeting is being planned for next quarter.Experiential rides will be conducted with the church next quarter. Met Housing expressed interest as well so arrangements will be made with them next quarter.

The Active Transportation funding application was approved, so planning associated with that funding will begin next quarter as well.

The Move Safe committee did not meet this quarter, but they have posted bike educational material on social media. Shelly is waiting to receive an application from the Kiwanis member.Shelly has also sent a letter to the owner of a building with multiple store fronts. No word has been received from that owner.Q3- A bike rack application has yet to be received from the Rustic Café, but the owner is still interested. Coalition members have been assisting with communicating with the owner. Shelly has sent the information and application via email and through the mail.To find a third location, Shelly sent a letter out to some of the larger businesses in Delphos. Q1- Shelly contacted the Mayor of Bluffton to see if she would be interested in a smoke-free project. Shelly met with the Mayor of Bluffton in February to discuss a possible smoke-free storefront initiative. See attachments for meeting notes. The Mayor expressed interest in a smoke-free Main Street project. She was concerned with a couple businesses on Main St. that have outdoor seating, so she suggested that Shelly talk with them before moving forward. Shelly spoke with the two businesses, both of which are in support of the project, and has a follow-up meeting scheduled with the Mayor next quarter to discuss next steps and how to move forward.

Q2- Shelly met with Judy in April to update her on the two businesses that were of concern. They also discussed signage and how to get feedback from the businesses. Shelly has tried to attend Chamber meetings, but has not been able to make it fit with her schedule. Judy and Shelly did agree that they needed to meet with the Chamber Director. Judy also felt she should get feedback from the village solicitor and council members. See meeting notes included in attachments.Shelly scheduled a meeting with Judy and Phil Zimmerly, the Chamber director, in June. See meeting notes included in attachments. Phil had included the topic on the agenda for the Chamber Board meeting on the following day. After hearing back from Phil, the board is not in support at this time and wish to not move forward.Shelly has contacted the Village Administrator, Sean Champman, in Spencerville to see if they would be interested in partnering with CHC on this. They hold an annual summerfest event, but it is way too late to do anything with this event at this point in the year. Five stores are interested in developing a smoke-free policy and nine stores want signage for their storefronts. Q2- Shelly reached out to city officials on suggestions for events in the City that CHC could work with. It was suggested to work with the National Night Out event in Lima. In an effort to streamline all smoke-free event projects in each community, Shelly has reached out to the organizer of the Lima event but has been unable to connect.It was also recommended to work with the Lima Locos baseball team on some kind of smoke-free initiative. Shelly sent a letter to the owner of the team and requested a response form back, but has yet to receive any correspondence. She will follow-up with this next quarter. Q3- No response was received from earlier inquiries.Due to the work in Bluffton with a smoke-free storefront, Shelly contacted the director of Downtown Lima, Inc., Aubree Kaye. Shelly sent her the storefront survey. Aubree discussed the project with the Executive Board who was split on the topic. They were concerned about the restaurants and residences in the downtown area. Aubree was going to send out a blast email with the survey, but no responses have been received. Shelly has attempted to follow-up with Aubree but has been unsuccessful.Shelly also reached out the Allen Economic Development Group to see if they would be interested in making the Maker Fest event smoke-free. Shelly contacted the organizer of the event to see if this would be a possibility. He was very supportive and presented the opportunity to the Executive Committee. The committee decided to pass.With the invitation to attend a planning meeting for an event at Hermon Woodlands (refer to notes in Objective 2), Shelly asked if the event could be tobacco/smoke-free. The Parks Director supported the idea and felt it would be a good thing to do. Following the meeting, Shelly sent an email describing the initiative and the resources available. It was discovered there was an email issue

Q3- Johnny Appleseed Metropolitan Park District is hosting a “Meet the W.I.L.D (Wildlife in Legendary disguise)” event on October 21st at Hermon Woodlands. This event will be smoke-free. The event is not being over-promoted to the public as the location cannot accommodate a large crowd of people. It was been promoted in the JAMPD newsletter and on their website and will be on social media. At the time of the planning meeting, materials had already been developed, so the flyers, etc. will not have a smoke free promotion on them. However, the event will be promoted by word of mouth that is it smoke-free. Signage will be provided to the park district to use at this event and future events.Q4-

Q1- Shelly will follow up with the City of Delphos and the Parks Department next quarter.When Scheduling a meeting with Kelly at the Delphos Public Library for the Community Garden, Kelly expressed interest in a tobacco-free campus policy. When meeting with Kelly, they discussed the possibility of the tobacco-free campus policy serving as a tobacco-free public space policy with the gazebo out front and the garden space as well as other outdoor events that take place at the library. Shelly met with the Police Chief in June to discuss the possibility. He is completely on board and sees no issues with moving forward.
