ENROLLMENT NOTE:   If you choose not to enroll yourself and/or your Dependents because you have other (health, dental, vision) group in‐surance, you may be able to enroll yourself or your Dependents in this plan if you or your Dependents lose eligibility for that other coverage (or if the employer stops contribu ng toward your or your Dependents’ coverage).    However, you must request enrollment within 31 days a er your or your Dependents’ coverage ends. In addi on, if you have a new dependent as a result of marriage, birth, adop on, or placement for adop on, you may be able to enroll yourself and your Dependents.  However, you must request enrollment within 31 days a er marriage, birth, adop on, or placement for adop on.       

TERMINATION NOTE:   Employees who terminate employment are eligible to con nue Health, Dental and Vision coverage temporarily, under Federal COBRA guidelines.  Upon termina on, if you were receiving coverage, you will receive a le er outlining your con nua on op ons.

Way Sta on “Healthy Incen ves”  Program 

$5 per pay discount on health plan premium (available to all health plan enrollees) In an effort to encourage a healthy lifestyle and manage costs for all, Way Sta on, Inc. (WSI) has developed a Healthy Incen ve Program for staff enrolled in any of the (2) Health Plans offered.  This is a voluntary program offered by WSI to help control the rising costs of health coverage and to encourage healthy habits for employees and their families.  We believe the Healthy Incen ve Program will enhance physical, emo onal, and intellectual health of our employees and their families through various means of awareness, educa on and healthy programs.  In addi on over the long term, this will assist us in decreasing health care costs.  The goals of the Healthy Incen ve Program are to:  

increase employee par cipa on in wellness ac vi es  change par cipant behavior to more healthy lifestyles;  ul mately improve the health status of par cipants; and  to manage rising costs of health benefits 

 The program is designed to provide every employee enrolled in the health plan an opportunity to earn an Incen‐ve of an addi onal $5 per pay discount on their health plan premium contribu on.   

 Employees who meet designated “Healthy Incen ves” are eligible to receive this discount.  Details are below:  

Employees must earn five (5) “Healthy Incen ves” in order to receive the $5 per pay incen ve.  See a ached chart for the list of “Healthy Incen ves” you can chose from.  Employees need to provide proof to WSI HR in order to receive the incen ves.  See chart below for 

details. Due to HIPPA privacy regula ons and confiden ality, proof submi ed to earn incen ves should just verify enrollment and/or comple on.  Refrain from submi ng any documenta on that shows a diagnosis. 

Incen ve amounts earned will then be processed by Payroll at the beginning of the month following confirma on of your “incen ves” earned. 

Upon enrolling in one of the health insurance plans listed above, an employee has up to (6) months to earn “Healthy Incen ves” in a given plan year.  Plan year begins January 1 through December 31.

What is an FBA?  HRA?  FSA?

FBA money is “use it or lose it” per plan year.  All money that goes “unused” at the end of the plan year can be used for all eligible IRS 213d expenses (i.e. qualified health, den‐tal, vision, over‐the‐counter expenses, etc).   You will have 75 days a er the plan year to use this money, or it’s forfeited.

Health Reimbursement Account (HRA) ...Did you know?  Our company offers employees high‐deduc ble health insurance plans that allow employees to u lize an HRA.  An HRA is a Health Reimbursement Account that allows you to pay for/ be reimbursed for qualified Medical and RX expenses that are subject to Deduc ble.  Choice‐Strategies (“CHOICE”) administers this HRA plan for us.  1)  How much am I allowed to be reimbursed? 

Refer to our Health Insurance Op on chart for HRA amounts  

2)  How will I be able to access HRA funds?   Via Healthcare payment card:   Members can pay for eligible expenses using a CHOICE payment card (i.e. Debit Card)     Via Reimbursement: 

Members can pay for eligible expenses with any form of payment and request reimbursement (using                                     appropriate reimbursement forms sent to CHOICE).  Reimbursements are made by check or direct deposit.  

 3)  What happens if I use my account for a non‐eligible expense? 

If you file a manual request for reimbursement, your request will be denied.  If you use your   healthcare payment card and the expense is deemed ineligible, you will be required to reimburse   your account for that transac on.  4)  How will I know if I need to submit a receipt to CHOICE for substan a on? 

You can review if your claim requires receipt by logging into your account online.  You need to submit receipts if you see a no ce.  If a receipt is needed, you will also be no fied by email or le er within a week of your payment card swipe. 

 5)  What if I don’t submit my receipts? 

You must provide the receipts within the  me requested, or the transac on will be deemed ineligible and you will be required to refund the amount of the transac on.  If you fail to submit receipts within 45 days, your card will be deac vated.  If you fail to reimburse the account, the amount of eligible  expenses may be added to your W‐2 or withheld from your pay. 

 6)  Where can I use my healthcare payment card? 

Your healthcare payment card can be used na onwide at qualified merchants (such as Pharmacies, Doctor’s offices and hospitals).  Your card should only be used to pay for qualified medical and RX De‐duc ble expenses under your plan.  YOU SHOULD ALWAYS SAVE YOUR RECEIPTS!

Tui on Reimbursement  

Tui on reimbursement for college courses which are job related.  To qualify you must apply for course reim‐bursement by April 1st each year, obtain a grade of “B” or be er and turn in the necessary paperwork when course is completed.  See the Tui on Reimbursement Policy.

C. Notice of Exchange Health Insurance Marketplace Coverage Options and Your Health Coverage Beginning in 2014, there is a new way to buy health insurance: the Health Insurance Marketplace. To assist you as you evaluate options for you and your family, this notice provides some basic information about the new Marketplace.

What is the Health Insurance Marketplace?

The Marketplace is designed to help you find health insurance that meets your needs and fits your budget. The Marketplace offers "one-stop shopping" to find and compare private health insurance options. You may also be eligible for a new kind of tax credit that lowers your monthly premium right away.

The 2017 open enrollment period for health insurance coverage through the Marketplace runs from Nov. 1, 2016, through Jan. 31, 2017. Individuals must enroll or change plans prior to Dec. 15, 2016, for coverage starting as early as Jan. 1, 2017. After Jan. 31, 2017, you can get coverage through the Marketplace for 2017 only if you qualify for a special enrollment period or are applying for Medicaid or the Children’s Health Insurance Program (CHIP).

Can I Save Money on my Health Insurance Premiums in the Marketplace?

You may qualify to save money and lower your monthly premium, but only if your employer does not offer coverage, or offers cov-erage that doesn't meet certain standards. The savings on your premium that you're eligible for depends on your household income.

Does Employer Health Coverage Affect Eligibility for Premium Savings through the Marketplace?

Yes. If you have an offer of health coverage from your employer that meets certain standards, you will not be eligible for a tax credit through the Marketplace and may wish to enroll in your employer's health plan. However, you may be eligible for a tax credit that lowers your monthly premium or a reduction in certain cost-sharing if your employer does not offer coverage to you at all or does not offer coverage that meets certain standards.

If the cost of a plan from your employer that would cover you (and not any other members of your family) is more than 9.5 percent of your household income for the year (9.56 percent for 2015), or if the coverage your employer provides does not meet the "minimum value" standard set by the Affordable Care Act, you may be eligible for a tax credit. (An employer-sponsored health plan meets the “minimum value standard” if the plan’s share of the total allowed benefit costs covered by the plan is no less than 60 per-cent of such costs.)

Note: If you purchase a health plan through the Marketplace instead of accepting health coverage offered by your employer, then you may lose the employer contribution (if any) to the employer-offered coverage. Also, this employer contribution—as well as your employee contribution to employer-offered coverage—is often excluded from income for federal and state income tax purposes. Your payments for coverage through the Marketplace are made on an after-tax basis.

II. Notices applicable to group health plans with certain benefit designs A. Wellness program disclosure If it is unreasonably difficult due to a medical condition for you to achieve the standards for the reward under this program, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under this program, call the plan administrator identi-fied at the end of these notices and we will work with you to develop another way to qualify for the reward. B. Newborns’ act disclosure Group health plans and health insurance issuers generally may not, under Federal law, restrict benefits for any hospital length of stay in connection with childbirth for the mother or newborn child to less than 48 hours following a vaginal delivery, or less than 96 hours following a cesarean section. However, Federal law generally does not prohibit the mother’s or newborn’s attending provider, after consulting with the mother, from discharging the mother or her newborn earlier than 48 hours (or 96 hours as applicable). In any case, plans and issuers may not, under Federal law, require that a provider obtain authorization from the plan or the insurance issuer for prescribing a length of stay not in excess of 48 hours (or 96 hours). C. WHCRA enrollment/annual notice If you have had or are going to have a mastectomy, you may be entitled to certain benefits under the Women’s Health and Cancer Rights Act of 1998 (WHCRA). For individuals receiving mastectomy-related benefits, coverage will be provided in a manner deter-mined in consultation with the attending physician and the patient, for:

All stages of reconstruction of the breast on which the mastectomy was performed;

Surgery and reconstruction of the other breast to produce a symmetrical appearance;

Prostheses; and Treatment of physical complications of the mastectomy, including lymphedema. These benefits will be provided subject to the same deductibles and coinsurance applicable to other medical and surgical benefits pro-vided under this plan. Notice for Patient Protections The following notice is provided for all plans that require or allow for the designation of primary care providers by participants or beneficiaries:

You have the right to designate any primary care provider who participates in our network and who is available to accept you or your family members. For information on how to select a primary care provider, and for a list of the participating primary care providers, contact the insurance carrier or the plan administrator identified below.

For children, you may designate a pediatrician as the primary care provider.

You do not need prior authorization from your insurance carrier or from any other person (including a primary care provider) in order to obtain access to obstetrical or gynecological care from a health care professional in our network who specializes in obstetrics or gynecology. The health care professional, however, may be required to comply with certain procedures, including obtaining prior authorization for certain services, following a pre-approved treatment plan, or procedures for making referrals. This notice has information about your current prescrip-tion drug coverage offered by the group health plan through Way Station, Inc. and about your options under Medicare’s prescription drug coverage. This information can help you decide whether or not you want to join a Medicare drug plan. If you are considering joining, you should compare your current coverage, including which drugs are covered at what cost, with the coverage and costs of the plans offering Medicare prescription drug coverage in your area. If you do decide to join a Medicare drug plan and drop your current group health coverage through Way Station, Inc. , be aware that you and your dependents will be able to get this coverage back, subject to the terms and requirements of such group medical plan. This notice has information about your current prescrip-tion drug coverage with Way Station, Inc. and about your options under Medicare’s prescription drug coverage. This infor-mation can help you decide whether or not you want to join a Medicare drug plan. Information about where you can get help to make decisions about your prescription drug coverage is at the end of this notice. There are three important things you need to know about your current coverage and Medicare’s prescription drug cover-age:

1. Medicare prescription drug coverage became available in 2006 to everyone with Medicare. You can get this cov-erage if you join a Medicare Prescription Drug Plan or join a Medicare Advantage Plan (like an HMO or PPO) that offers prescription drug coverage. All Medicare drug plans provide at least a standard level of coverage set by Medicare. You can keep your current coverage from CIGNA OAPin High Deductible plan. However, because your cover-age is non-creditable, you have decisions to make about Medicare prescription drug coverage that may affect how much you pay for that coverage, depending on if and when you join a drug plan. When you make your decision, you should compare your current coverage, including what drugs are covered, with the coverage and cost of the plans offering Medicare prescription drug coverage in your area.
