Guide to Transition
The Supportive, Competitive, Integrative, Employment Training Team (SCIETT) is a collaborative endeavor of organizations and agencies which include: St. Johns CO School District, Project 10/Connect, Agency for Persons with Disabilities, Vocational Rehabilitation, Independent Living Resource Center, the Arc of St. Johns, WORKSource, United Cerebral Palsy of East Central Florida, and University of Florida - Center for Autism and Related Disabilities/Jacksonville as well as independent stakeholders. Technical assistance is provided through the Supported
Competitive Integrated Employment Training Team (SCIETT) Project, with the Florida Center for Inclusive Communities (UCEDD) at University of South Florida (USF). SCIETT team members assisted in the development of this “Quick Reference Guide” to assist St. Johns School Administrators, teachers, students with disabilities and their families in planning and preparing options for students with disabilities before they exit high school and start their adult life. The St. Johns CO Quick Reference Guide to Transition is a directory of resources and services offered to school administrators, teachers, students with disabilities and their families to steer student’s and their families to appropriate resources and to plan more effectively for student transition and adult options.

AGENCY FOR PERSONS WITH DISABILITIES (APD): APD assists individuals whose disability is:
o Mental retardation with an IQ of 69 or less o Autism o Cerebral Palsy o Spina Bifida o Prader-Willi Syndrome
Eligibility: Eligible to apply at age 3. If the person is not a client of APD, apply now.
Why: APD Home and Community Based Services (HCBS) Waiver (Medicaid Waiver) is a funding mechanism for:
o Respite o Support Coordination o Behavior Analysis Services o Behavior Assistant Services o Consumable Medical Supplies o Supported Employment o Supported Living Coaching o Occupational, Speech, and Physical Therapy o Transportation

Social Security Administration (SSA): SSA assists individuals whose medical condition is expected to last at least 12 month or results in death. Social Security Disability Income (SSDI)
o Not based on financial need o Medicare
Supplementary Security Income (SSI) specific requirements: o Income and assets must be below set limits
Include parents of minor children and spouse of married individual o Children can be eligible based on disability prior to age 18 if income/resource
limits are not exceeded
o Medicaid is provided by the state if approved (See Medicare/Medicaid)
o Individuals who are age 18 and are disabled
Parents income/assets stop counting (the month after their birth month)
Claims for SSI should be filed one month (not earlier) before 18th birthday
o SSA helps individuals with disabilities achieve independence by helping them to take advantage of employment opportunities: (Refer individuals to a “Community Work Incentive Counselor” at the Independent Living Resource Center for more information)

Division of Blind Services (DBS): The Florida Division of Blind Services has a rich history of providing services to person who are blind or have severe visual impairments and to persons who have disabling conditions that make it difficult to read regular print. Services include:

On Campus Transition (OCT) - University of North Florida (UNF)/Project Source: A transition program that helps students with developmental disabilities adapt to campus life. This is a partnership between the Arc of Jacksonville and the University of North Florida. St. Johns Housing Partnership, Inc: The St. Johns Housing Partnership is committed to promoting safe, decent and affordable housing in St. Johns County, Florida. We do this by creating
links between the public and private sectors on projects that create low- and moderate-income housing, rebuild neglected homes and neighborhoods, thus
benefiting disadvantaged areas and enhancing community economic and social development.
Staff in St. Johns CO includes carpenters, electricians, plumbers, masons, skilled builders and apprentices. Staff are trained to supervise volunteer
projects whenever necessary. Additional temporary construction staff are hired through a special partnership with Carpenters Local 627 when
additional funding opportunities are available.

Section 8 Housing
The Section 8 Rental Voucher Program increases affordable housing choices for very
low-income households (families, individuals with disabilities, elderly). Housing and
Urban Development (HUD) contracts with housing authorities to provide Section 8
voucher assistance.
