Spring is indicative of renewal and re-growth, and the mood was no different at The Wellington, as we prepared for the opening of our new outpatient and day surgery facility: the Platinum Medical Centre (PMC)

I am proud to announce that after 18months of construction work, the PMC opened its doors to its first stream of patients on Tuesday 3rd May.

With such an extensive list of services available at the PMC, this edition will briefly introduce you to the facility, and the services available to your patients.

We have put the spotlight on just a few of our units, but you can view a full list of the specialities that will be housed at the PMC on page 5. In the forthcoming issues of Practice Matters we will be highlighting other specialities; updating you as the PMC grows and develops further.

The Platinum Medical Centre (PMC) has brought the experience and excellence of The Wellington Hospital together to create one of the most up-to-date private medical centres in the country.

The PMC is arranged over seven floors comprising 50 consulting rooms, a 10 bedded chemotherapy unit and four day surgery theatres, accompanied by a 12 bedded day surgery unit. The centre will increase the overall size of the hospital by one third, allowing more space for development in the main hospital, but also the availability of new outpatient and day case services.

The centre will undergo a phased opening, firstly treating outpatients and then cancer patients, before it is opened fully in July.

Cutting edge technologyThe Wellington Hospital invests in state-of- the-art technology as a priority and the Platinum Medical Centre (PMC), is a prime example of this philosophy. Equipped with some of the most advanced and sophisticated imaging and diagnostic technology, including a low dose PET/CT scanner and MRI capability.

There are plans for a lithotripsy machine as well as digital mammography in the very near future, a sure sign of The Wellington’s commitment to continual development.

First class facilitiesThe new consulting rooms and day surgery unit have been designed to the highest standard, so they are flexible and spacious; allowing a variety of specialist equipment to be housed within the rooms. All consulting rooms are designed for quick and efficient treatment; there are minor treatment, plaster and phlebotomy rooms on each floor.

With almost four decades of experience, we appreciate the need for ad-hoc availability. Standardised ad hoc rooms are also available for consultants who may need to add last minute clinics or see urgent cases, when GPs make urgent referrals.

Our new oncology service, run in partnership with Leaders in Oncology Care (LOC), means that cancer patients, who previously had to travel to other locations for their treatment, now have access to all the care they need in one place.

The Day Surgery Unit is spread over two floors, comprising of four day surgery theatres and a 12 bedded ward. The unit will be run by Susan Wilson and a team of highly experienced theatre staff. Laminar Flow, a device that allows for air to be purified during surgery, which is essential for orthopaedic surgery, is available in two of our four day surgery theatres.

A pre assessment unit will be situated at the PMC too which will improve our patients journey; allowing them to have their consultation, investigations, pre assessment and day surgery – all in one familiar place.

Patient focusedEvery area of the PMC has been designed with patients in mind; there are reception and waiting areas on each floor, where patients can relax in calming surroundings.

Our modern and spacious cafeteria - cafe 78, offers a selection of hot and cold quality food and beverages, which will be available all day.

Our opening hours reflect our patients’ need for flexibility:

Monday – Friday 8am – 8pm

Saturday 8am – 4pm

The centre is closed on Sundays

Though the PMC is a short walk from the main hospital, all patients are offered a shuttle bus service. A rotation system will be in operation throughout the centre’s opening hours, transporting patients from all three hospital buildings.

Our team closely liaise with radiologists to ensure that each examination is perfectly tailored to the patient’s and referrer’s specific requirements.

Our ultimate goal is to minimise the time from diagnosis to treatment and provide patients with the best possible outcomes.

Imaging technology offered at the PMC consists of the following:

MRI – Siemens Aera 1.5T

This state-of-the-art scanner performs all musculoskeletal, orthopaedic, neurology, vascular, oncology, abdominal (including dynamic liver, small bowel and MRCP) and pelvic (including rectal/ anal cancers, gynaecology, prostate and fistula) imaging.

Additionally, the Siemens Aera at the PMC offers:

• Neurology (including, Perfusion & Diffusion Weighted imaging, Spectroscopy, Tractography)

• Adult Cardiac MRI (including volumetric analysis, delayed enhancement, Adenosine stress

perfusion, assessment of cardiomyopathy, tumours, pericardial and valvular heart disease)

• Vascular (including specialised Multiphasic Angiography)

The Siemens Aera incorporates unique technology, such as TIM®: an innovative RF and coil architecture to pack more coil elements into a smaller space, producing far higher element configurations with enhanced image quality; and DOT®: which adapts to each patient’s capabilities, performing personalised high quality examinations that are easily reproduced with each visit, even when the patient’s condition changes.

The Siemens Aera also offers a new dimension in patient comfort, including:

• A shorter length and larger 70cm aperture for a more open appearance, as well as a 250kg weight limit to ensure that patients of all sizes and conditions are able to lie comfortably throughout their examination

• A MoodLight on the front panel of the scanner to produce soothing colours and a more relaxed scanning environment

• An integrated music system with a wide selection of music to choose from, or patients may bring an iPod/iPhone/MP3 player or a CD to listen to during their examination

We have over 20 specialist radiologists that attend The Wellington Hospital to report MRI scans.

There are plans to expand the MRI service at the Platinum Medical Centre with the addition of a Siemens Skyra 3T MRI scanner, in Autumn 2011. This will enable us to provide an even greater range of MR examinations

Diagnostic CT and PET/CT – Siemens mCT – the most advanced PET/CT in the world.

The Wellington Hospital is proud to offer PET/ CT as a new modality. This will provide both consultants and patients with access to state-of-the-art technology and facilities, and also provide the highest level of diagnostic excellence. Our Siemens mCT is the next generation of dual modality scanners. It gives a new level of integrated imaging; CT in conjunction with high definition PET, or alternatively it can be used as a stand-alone CT. The result is whole-body PET/CT scanning in as little as 10 minutes with lower radiation doses than conventional PET/CTs , as well as high performance 128 slice diagnostic CT scanning, with or without intravenous contrast.

PET/CT is indicated for use across a wide range of oncology settings, including staging and assessment of treatment response. There are also indications for the use of PET/CT in non-malignant pathologies, cardiac and neurological applications.

minutes, with the benefit of less radioactive

tracer administered to patients thus reducing

radiation dose.

• Siemens ultraHD·PET, which sharpens images

and provides a 2mm uniform resolution across

the entire scan for enhanced detection and the

highest level of detail

• Open appearance with large 78cm aperture and

227kg weight capability, offering a high level of

patient comfort and maximum imaging flexibility

CT features include:

• 128 slice CT acquisition for faster imaging

capabilities

• CT dose saving software and hardware which

significantly reduces radiation dose to patients

• z-Ultra High Resolution technology, giving

industry-leading resolution

in functional hybrid imaging and radiology, with a

broad area of practice across both teaching

hospitals and the private sector.

Ultrasound – Philips iU22 Matrix

Our two Philips iU22 Matrix Ultrasound machines are advanced premium performance imaging systems. These units increase clinical confidence, reduce exam time, and bring more diagnostic information to clinicians. Some of the key features of the Philips iU22 system are:

• The ability to reduce failed ultrasound exams on technically difficult patients by as much as 69% with the C5-1 PureWave transducer

• Reduction of ultrasound exam time by as much as 50% with SmartExam.

• Ergonomic, intelligent, human-centred design for comfort and convenience

• Next generation digital broadband acoustic beamforming, built for the latest pulse shaping and coding techniques

Our physician led ultrasound sessions are undertaken by some of the most experienced specialists in their fields.

Digital Radiology – Siemens Ysio wi-D and AXIOM Multix M

Our two X-ray systems at the PMC are also manufactured by Siemens. These detector based X-ray machines are designed to eliminate manual image processing, and allow staff to maximise their focus on patient care.

• The Ysio wi-D system features wireless detectors that can be used in upright imaging, on the table and stand, or can be placed underneath or next to the patient. Equipped with over seven million pixels, these detectors have the highest pixel count of any detector of their kind, providing excellent image quality to assist with diagnosis

• The AXIOM Multix M is a high performance universal digital radiography system for all standard examinations, from the head to the spine, abdomen, and extremities

Both X-ray systems incorporate multiple dose saving systems.

Imaging and reports from all diagnostic procedures are available for consultants to view on their computers via the HCA-wide Picture Archiving Communication System (PACS). Exams are also available for both patients and referrers on easy to use CD, with in-built viewing software.

The Platinum Medical Centre, the new diagnostic and outpatient centre for The Wellington Hospital, is already winning praise from patients, doctors and the press!

The £33 million Platinum Medical Centre, in Lodge Road, St John’s Wood, will serve around 70,000 outpatients every year. The building, at 50,000 square feet and spread over seven floors, increases the size of The Wellington Hospital – already the nation’s largest private hospital, by almost a third. The building also has 50 consulting rooms and other extensive patient facilities.

The first patient to arrive at the new centre was Mr Simon Rosenfeld, from North West London, who recently had an operation to remove a small lesion on his foot. “I am pleasantly surprised. It’s very 21st century and state of the art. It could change the whole aspect of the Harley Street area from now on! All the consultants will be moving up here!” he said. “It is so convenient and very friendly. It’s a real asset to The Wellington and the local community.”

Mr Rosenfeld was seeing consultant orthopaedic surgeon, Mr Simon Moyes. “These premises are lovely. The Platinum Medical Centre has a nice warm feel about it. When you compare and contrast this with some other hospital facilities across London, those are looking dated and this place is in a class of it’s own,” said Mr Moyes.

The Chief Executive Officer of The Wellington Hospital, Mr Keith Hague, said that increasing

demand from patients and the need to bring new areas of medicine to the hospital were the drivers behind the project.

“The Wellington has for many years had an international reputation for the complexity of the medicine practiced here but our patients rightly demanded an expansion of the services we offer,” he said.

“For example, patients needing chemotherapy will be able to receive their treatment here for the first time in many years and the development of our second large diagnostic and imaging unit will mean faster diagnosis and therefore treatment will begin earlier too,” said Mr Hague.

Moving the outpatients department to the Platinum Medical Centre will also mean major new developments at The Wellington South building. “We will have a new urgent care admissions unit – 10 suites with critical care facilities,” said Mr Hague. “We are also developing a new lung cancer centre at Wellington South and a new breast cancer centre and that’s just the beginning!”

A partnership in quality cancer care

The Wellington Hospital has chosen to partner with Leaders in Oncology Care (LOC) to help deliver the very best levels of care to cancer patients.

All oncology patients treated at the Platinum Medical Centre will benefit from The Wellington Hospital’s state-of-the-art facilities and advanced technology, and the expertise of LOC and their successful cancer management methods.

Leaders in Oncology Care Leaders in Oncology Care was founded in 2005 at 95 and 97 Harley Street to provide the very highest standards of cancer care. LOC had the idea of starting ‘doctors in chambers’ – a group of clinicians with the same speciality who, by combining their skills, could provide a better service for patients.

This model has proved very successful and has since been copied by doctors working in several other specialties. LOC has established rigorous standards in chemotherapy and other cancer treatments and has brought these excellent practices to the Platinum Medical Centre.

LOC at The Wellington HospitalThe new LOC facility at the Platinum Medical Centre is based on the ground floor, for ease of access. A replication of 95-97 Harley Street, it will

mirror their successful building design and proven clinical protocols that have been so highly revered.

The very same standards of care and patient support will be provided, using LOC’s current systems, to ensure their high standards continue.

The Wellington has invested in the most most up-to-date cancer specific imaging, such as low dose PET CT scanning, which is vital for accurate and rapid results.

We treat all types of cancers in patients of all ages. However, the new centre specialises in gastrointestinal cancers, together with liver cancer, head and neck cancers and gynaecological cancers, there is also a new breast care service available.

On-site oncology pharmacyLOC also operates the specialist oncology pharmacy at the centre. This dedicated pharmacy delivers the highest possible standards of pharmacy care and is managed by a team of experienced specialist staff. The fully equipped pharmacy area includes aseptic clean-rooms to ensure the safe and efficient preparation of chemotherapy drugs.

The London Hand and Wrist Unit first opened at The Wellington Hospital in 1995, with the aim of providing specialist care for patients with upper limb conditions and injuries.

Over the past 17 years the London Hand and Wrist Unit has grown considerably, now offering services at five central London hospitals with its main base at the prestigious Platinum Medical Centre.

The London Hand and Wrist Unit is unique in London; combining a specialist multi-disciplinary team utilising the vast resources of a major private hospital group to provide complete management of upper limb conditions. This provides patients with continuous care - from consultation with one of our hand and wrist consultants, with access to a complete medical imaging department, to surgery (if required) by eminent consultants, through to comprehensive hand therapy services.

The following areas have seen particular advances and are fully catered for at the Platinum Medical Centre:

• The management of abnormal and excessive vaginal bleeding. High resolution imaging, both ultrasound and MRI, have increased our understanding of the causes of these disorders. Outpatient and day case based therapies have dramatically reduced the number of women requiring hysterectomy. The new methods of management of uterine fibroids have provided innovative techniques by which this common problem can be dealt with conservatively.

• Urinary incontinence is a common and distressing symptom. The latest investigation techniques allow accurate predication of the appropriate management protocol. Both medical and minimal access surgical treatment is provided at the hospital by an internationally recognised specialist team of urogynaecologists.

• Genital prolapse is a common problem. Repairs can be carried out using the latest laparoscopic, robotic and minimal access techniques. In many women such procedures can be performed under local anaesthesia.

• Gynaecological cancer management is provided by an integrated multidisciplinary team, which includes world class gynaecological oncology surgeons, medical and clinical oncologists in association with LOC. All aspects of radical surgical care are available, provided by registered sub-specialists. The team also specialise in the provision of fertility sparing surgery where appropriate. Individualised treatment plans are devised and carried out ensuring that women have the very latest and most successful regimes designed for their care.

• Recurrent pregnancy loss is a common distressing presentation for couples. The Wellington provides the latest diagnostic programs and access to specialists with international reputations in this important field.

• For the investigation of women with an abnormal smear, a full colposcopy investigation and treatment facility with accredited colposcopists is always available.

Services at the Platinum Medical Centre

Out-patient services will be focussed within the new Platinum Building where a gynaecology unit has been planned. The intention is to provide a complete service for women with emergency gynaecological requirements and a rapid referral service for general practitioners and other specialists.

Recent developmentsAt the Platinum Medical Centre the regular core of ENT Specialists have formed an ENT Unit to give our patients a more co-ordinated service. Whilst individual consultants continue to care for their own group of patients in The Wellington consulting rooms, we are now able to offer an emergency ENT service based in the consulting rooms during working hours, with an out-of-hours service on the 4th floor of Wellington South.

Nearly 80% of ENT practice is now outpatient based, and we have been fortunate with The Wellington’s investment in the latest diagnostic equipment and we are looking forward to our move to the spacious well equipped Unit in the Platinum Medical Centre. Another advantage will be the easier access to the Imaging Department and our ability to liaise with our colleagues in related specialties.

Members of the ENT UnitWhilst each consultant maintains their expertise over the whole range of the specialty it is inevitable that most of us have developed special interests. This enables The Wellington to offer the very best in all aspects of ENT practice.

New ENT Unit in Platinum Medical CentreWe will have fully equipped consulting rooms and a specialised Treatment Room and a comprehensive Audiology Unit. The Treatment Room provides facilities for the full range of ENT procedures:

• Microscope controlled otological procedures under local anaesthesia

• Nasal endoscopy stack facilitates many nasal procedures and enhances the post-operative care following endoscopic nasal surgery.

• Fibre-optic stack with online stroboscopy and video recording which allows detailed analysis of laryngeal and voice problems.

• Kay Swallowing stack provides highly sophisticated analysis of swallowing disorders.

• Planned laser facilities

The Audiology Unit has sound proofed rooms for the full range of audiological tests. The linked Vestibular Lab has electronystagmography to assess disorders of eye movements and record the results of caloric tests. There are facilities for the fitting and care of hearing aids.

Specialised ENT Units

Balance Unit: Consultant input is from Mr Abramovich, Prof Cheesman and Prof Saeed. The audiological assessments are made in the Audiological Unit and where appropriate additional Posturography studies are conducted in the Physiotherapy Department. This results in a comprehensive assessment of the body’s complete balance systems and we are often able to design individualised rehabilitation programmes which are then supervised by our specialised physiotherapists.

Swallowing assessment: Close cooperation with our Speech and Language Therapists supplemented by the pressure recordings from the Kay Swallow Station and videofluoroscopic images provided by the Imaging Unit, enables us to determine the exact problem in the swallowing, and allows for the monitoring of the swallowing therapy. Indeed the video images often help the patients in their therapy.

Paediatric cases: We are able to manage our paediatric patients in the outpatient clinics but currently all inpatient treatment is conducted in one of HCAs Paediatric Units at the Portland Hospital, Harley Street Clinic and Princess Grace hospitals.

Inpatient care at The WellingtonAll ENT patients are managed on the 4th floor of The Wellington South and we have a dedicated ENT Operating Theatre fully equipped with the full range of ENT procedures including laser surgery. The recent acquisition of the da Vinci Robot will further enhance our surgical expertise. Joint Skull Base cases are performed with our Neurosurgical Colleagues and are nursed in the Neurosurgical Unit on 3rd floor Wellington South.

The Wellington Hospital Pain Unit consists of five consultants, all of whom are specialists in the management of difficult pain problems. Patients are assessed after a full history is taken, and prescribed appropriate treatment specific for their problem.

Mary was suffering unbearably from abdominal pain. I had been asked to see her by a gastroenterologist who was desperately trying to find the cause of her pain, but all the investigations were normal.

I arrived to find a young lady lying in bed, clearly suffering, but giving me a watery smile. The story she told was typical of complex pain, with a seemingly ordinary beginning, but with profound consequences.

Three years previously, she had been taken to hospital with acute abdominal pain, which turned out to be an appendix abscess. The surgeon opened the abdomen and drained the pus out, but unusually left the wound open to allow all the pus to escape, a treatment that is now very rare. For two weeks, she suffered excruciating pain, while the wounds were washed, irrigated and dressed daily, eventually closing the wound with stitches. I discovered that Mary is extraordinarily brave, able to put up with unusually large amounts of pain without it affecting her daily life too much.

In the three years since, the pain had gradually increased; affecting all of the skin of the abdomen, such that it was impossible to touch or wear clothes without causing pain, especially over the area which had been operated on. Eating was now becoming a problem, since whenever food entered the stomach, the pain worsened.

When I examined her, the abdomen was scarred with previous surgery and the skin was glossy and shiny, indicative of chronic pain. I also noticed that the colour of the skin would change from white to red to blue, suggesting a highly active sympathetic nervous system, which was changing the calibre of the skin blood vessels continually.

Mary was showing classic signs of Complex regional pain Syndrome (CRPS). This happens when severe on-going pain becomes complicated by involvement of the sympathetic nervous system, the nerves that innervate the blood vessels and sweat glands for example, and it is harder for the pain to settle down unless this sympathetic drive is blocked. Mary’s pain was aggravated by eating, as the food in the stomach caused the sympathetic nerves in the abdomen to fire off, causing further sympathetic stimulation of the skin, increasing her pain.

At The Wellington Hospital, the facilities available to manage such difficult pain include one of the best neurological rehabilitation units in the country, involving physiotherapists, occupational therapists, psychologists and neurologists, who regularly treat patients from all over the world with injuries to the brain and nervous system. Consultants from the Pain Unit, with the help of advanced imaging

techniques using X-rays, CT scanning and ultra-sound, have advanced technological equipment, enabling them to freeze nerves, using needles and electricity, local anaesthetics, steroids and other drugs and techniques, all designed to manage and reduce pain.

Mary has improved remarkably since undergoing several treatments involving radiofreqency ablation of nerves in the abdomen. She also had her sympathetic nerves treated in the same way under CT guidance, where the splanchnic nerve (the sympathetic nerve that supplies the upper abdominal organs) was treated with Radiofrequency ablation. Radiofrequency ablation is used in ‘pulsed’ mode at a temperature of 42 deg. C, which doesn’t destroy the tissue or the nerve, but stops it working for several months. In some situations, it may be used at 80 deg. C for tissue destruction.

In addition to interventions, Mary was given special medications which reduce the hyperactivity of nerves, to reduce the Neuropathic pain caused by these hyperactive nerves.

Mary demonstrates a particularly complex pain problem which required the application of truly multidisciplinary treatments involving the use of medication, intervention and physiotherapy, in addition to occupational therapy and psychology. It also demonstrates the complexity of the pain system and the high level of sophistication of pain treatments that may be required, all of which are available in the Pain Unit at the Platinum Medical Centre.

NURSES CORNER

Many General Practitioners take an active interest in their patient’s treatment, progress and ongoing medical management whilst they are in hospital.

After all, it is often the case that an individual practitioner has cared

for the patient and their family, often several generations, for many years. Many GPs, as well as maintaining an ongoing liaison with their patient’s consultants, come and visit their patients in the hospital and wish to see the inpatient notes to keep abreast with treatment.

At The Wellington Hospital the nursing staff will be able to give you access to your patient’s inpatient records once the patient’s verbal permission has been gained by a senior member of the ward nursing staff. This consent then enables any medical practitioner, including the patient’s GP, who is not directly managing the patient’s care and treatment whilst they are an inpatient, to have access to their medical records.

However, although it is acceptable for the GP to view the records, once the patient’s consent is obtained, it is not permitted for the

GP to order tests for the patient without agreement from the treating consultant.

In addition, we inform the admitting consultant of any requests from external medical practitioners, as a courtesy.

All nursing staff are aware of the correct procedures for when requests are made. Please appreciate they are not trying to be obstructive and they may not be aware who you are by sight only, but are protecting patient confidentiality.
