1.Graduateleaderswithameaningfulcareerpath;2.Relentlesslycompeteandwinchampionships;and3.ServeasapointofprideandunityforourglobalUNLVcommunity.

B.RESPONSIBILITIESASANATHLETEUNLVstudent-athletesarehighprofilecommunitymembers.Yourconductonandofftheplayingsurfacemattersandwillreflectuponyou,yourfuture,yourteam,yourcoach,theDepartmentofAthleticsprogramandUNLV.Youareexpectedtoconductyourselfresponsibly,honestlyandwithintegrity.Ataminimum,youareexpectedto:
● complywithallUNLVDepartmentofAthleticsrulesandregulations● participateinNCAACompliancesessionsandaskquestionsifthereisanyconfusionregardingthe
materialdiscussed● readallformsthoroughlybeforesigning● complywithallNCAArulesandpromptlyreportanyruleinfraction● complywithallteamrules,policiesandpractices● displaygoodsportsmanship● protectyourhealth,bothmentallyandphysicallyand● reportpromptlyanyhealthconcernsyoumayhave.
C.ATHLETICSPORTSMANSHIPUNLVANDTHEMOUNTAINWESTCONFERENCEETHICSANDSPORTSMANSHIPAllaspectsofintercollegiateathleticscontests,includingfanbehavior,shallbeconductedinasportsmanlikemannertoensurethehealthandsafetyforallfansandparticipatingstudent-athletes.EachmemberinstitutionintheMountainWestConferenceholdstheresponsibilitytoensurethatallindividualsassociatedwithitsathleticsprogramconductthemselvesinasportsmanlikemanner.Therefore,UNLVDepartmentofAthletics,inconjunctionwiththeMountainWestConferenceexpectsthateachstudent-athletegovernthemselvesinasportsmanlikemanner.UNLVwillnotcondoneinappropriateandunsportsmanlikebehavior.Failuretoabidebythisexpectationmayresultindisciplinaryaction.ROLEOFSPORTADMINISTRATORTheUniversityofNevada,LasVegasiscommittedtoexcellence,bothathleticallyandacademically,andtothepersonaldevelopmentoftheholisticstudent-athlete.OurgoalistopromotesuccessineverythingwedowhileremainingrootedinourRebelWays.Theroleofthesportadministratoristoprovideprogrammaticoversight.Thesportadministrator’sgoalistoensurethedepartment’smissionandcorevaluesarereflectedwithineachindividualsportsprogram.Thisoversightincludes,butisnotlimitedto:

universitypoliciesandprotocol)● Assessingandensuringsport’sprogrammaticneedsarebeingplannedforandaddressedincludingbut
notlimitedtoconductingstudent-athleteexitinterviews.

NCAACOMPLIANCEWHATISCOMPLIANCE?TheUniversityofNevada,LasVegas,iscommittedtotheprincipleofinstitutionalcontrolintheoperationofitsathleticdepartment.ThismeansfollowingtheletterandspiritoftherulesandregulationssetforthbytheNCAA,MountainWestConference(MWC),andUNLVadministration.TheOfficeofNCAAComplianceandStudentServicescoordinate,monitor,andverifycompliancewithallNCAArequirements.ThecomplianceofficealsoeducatesmembersoftheuniversitycommunityregardingNCAAandMWCrulesandregulations.Througheducationalprogramsandinterpretivesupport,theOfficeofNCAACompliancestrivestoensurethatallindividualsassociatedwithUNLVathleticsunderstandandabidebytherulesandregulationsoftheNCAAandMWC.ThegoaloftheOfficeofNCAAComplianceistotakeaproactiveapproachtoeducating,monitoring,andoverseeingtheintercollegiateathleticprogramsatUNLVandcultivatingapositiveenvironmentforNCAAcompliance.CODEOFCONDUCTAsvaluedmembersoftheUNLVDepartmentofAthletics,itisourgoaltoassiststudent-athletesinattainingaqualityeducationandearningadegree.Student-athletesatUNLVareexpectedtoconductthemselvesinamannerthatbestrepresentstheirteamandtheinstitution,whilebeingproductivemembersofboththecampusandsurroundingcommunities.Itistheirresponsibilitytoexhibitbehaviorsthatenablethemtomakethemostoftheacademicandathleticopportunitiesaffordedtothem.Participationinathleticsisaprivilege,notaright,subjecttoNCAAandMWCrulesandregulations.Toearnandmaintainthatprivilege,allstudent-athletesmustadheretotheUNLVStudent-AthleteCodeofConduct,aswellastheUNLVStudentCodeofConduct.TheintentofthispolicyisnottosupersedeUNLVorNCAApolicy,norisittojudgewhoisguiltyornotguiltyunderthesamestandardsaswouldapplyinacourtoflaw.Itismeanttosethighstandardsforthedepartment’sexpectationsofhowstudent-athletesrepresentUNLV,thedepartmentandtheirprograms,andtostatethepotentialconsequencesforthosestudentathleteswhomakepoorchoices,reflectingnegativelyonthemselvesandUNLV.AsUNLV’smostvisibleambassadors,student-athletesareexpectedtouphold,atalltimes,highstandardsofintegrityandbehavior,reflectingwelluponthemselves,theirfamilies,coaches,teammates,theDepartmentofAthleticsandthisuniversity.Student-athletesareexpectedtorespecttherightsofothers,andtoabidebyallrulesandregulationsofUNLV,theMWCandtheNCAA.Failuretodosomayresultinsuspensionfromtheteam,forfeitureofathleticsfinancialaidpursuanttoNCAAbylaw15.3.4.2.4Misconduct;further,UNLVmaycancelorreducethefinancialaidofastudent-athletewhoisfoundtohaveengagedinmisconductbytheuniversity’sregularstudentdisciplinaryauthority,eveniftheloss-of-aidrequirementdoesnotapplytothestudentbodyingeneral.Astudent-athletemayappealdisciplinaryactionifhe/shebelieveshis/hersituationpresentsspecialcircumstancesbywhichtheassignedsanctionstobeimposedaremanifestlyunfairorthereissignificantevidencethatwouldjustifyadifferentoutcome.

(c)Writtencorrespondence.Itispermissibleforanenrolledstudent-athletetoengageinwrittencorrespondence,provideditisnotdoneatthedirectionand/orexpenseofthememberinstitution.TheUNLVComplianceOfficeshouldbenotifiedifthisoccurs.
(d)Unavoidableincidentalcontact.Ifunavoidableincidentalcontactoccursbetweenastudentathleteandaprospect(evenattheprospect’shighschool),suchcontactispermissible,providedtheinstitutionhadnopriorknowledgeoftheoccurrenceofthecontact.TheUNLVComplianceOfficeshouldbenotifiedifthisoccurs.
(e)Officialvisits.Anenrolledstudent-athletemayparticipateasastudenthostduringaprospect’sofficialvisittotheinstitution’scampus.Asastudenthost,astudent-athletemayreceivethefollowing:(1) Amaximumof$40foreachdayofthevisittocoverallactualcostsofentertainingtheprospect
(andtheprospect’sparents,legalguardiansorspouse),excludingthecostofmealsandadmissiontocampusathleticsevents.ThesefundsmaynotbeusedforthepurchaseofsouvenirssuchasT-shirtsorotherinstitutionalmementos.Itispermissibletoprovidethestudenthostwithanadditional$20perdayforeachadditionalprospectthehostentertains.Intheeventthatseveralstudentshostaprospect,the$40perdayentertainmentmoneymaybeutilizedtocovertheactualandnecessaryexpensesincurredbytheprospectandallhosts.Onlyonestudenthostperprospectmaybeprovidedafreemealifrestaurantfacilitiesareutilized;

(e)Astudent-athletemaynotbeinvolvedinthearrangingfororprovidingto,directlyorindirectly,anytypeofofferorinducementthatinvolvessexualliaisonsoractivitiesfortheprospectortheprospect’srelativesorfriends.Alsoprecludedisarrangingforthepurchaseof,orprovidingto,directlyorindirectly,anytypeofalcoholicbeverage,illegalorcontrolledsubstance,daterapedrugorotherdrugofanytype.
(f)Duringanofficialorunofficialvisit,student-athletemaynotbeinvolvedinthePatronagetoadultestablishments,orarrangingfororprovidingto,directlyorindirectly,anytypeofadultentertainmentoractivityfortheprospectortheprospect’srelativesorfriends.Thisincludesbutisnotlimitedto:theviewingofpornographicmaterial,patronageatTopless/Gentlemen'sClubs,solicitingPrivateDancers,prostitutionorCall-GirlServices.
(g)Astudent-athletemaynotbeinvolvedintheonoroff-campusarrangingfororprovidingto,directlyorindirectly,anytypeoflegalizedorillegalgamblingactivity.Thisincludesbutisnotlimitedto:CasinoTablegames,slots,dice,crapsorsportwageringactivities.NOTE:ThelegalgamblingageinNevadais21.TheUNLVStudent-Athleteshallsignanagreementacknowledgingcompliancewiththispolicyonannualbasis.
EXTRABENEFITSAnextrabenefitisanyspecialarrangementbyacollegeoruniversityemployeeorrepresentativeoftheschool’sathleticintereststoprovideastudent-athleteorthestudentathlete’srelativeorfriendabenefitnotclearlystatedintheNCAAMANUAL.Ifabenefitisnotgenerallyavailabletoallstudents,itisnotpermissiblebyNCAArules.Forexample,studentathletesmaynothavefreephotocopying,telephonecallsortypingofcoursepapersattheexpenseoftheAthleticDepartment.ViolationoftheNCAA“ExtraBenefits”rulebyastudentathletemayresultinlossofeligibility.COMPETINGINNON-UNLVRELATEDCOMPETITIONTheDepartmentofAthleticscanprovideexpensesforstudent-athleteswhohavebeenselectedorqualifyfornational,internationalornon-sanctionedUNLVathleticevents,teamsorcompetitions.Youmustspeakwithyourcoachtodetermineyoureligibilityasitwillbedeterminedonacase-by-casebasis.10.17.02.18VoluntaryAthleticallyRelatedActivitiesInorderforanyathleticallyrelatedactivitytobeconsidered“voluntary,”allofthefollowingconditionsmustbemet:(Adopted:4/18/01)

CURRENTSCHOLARSHIPSTUDENT-ATHLETESNOTENROLLEDINSUMMERSCHOOLCurrentstudent-athletes,includinggraduatetransfers,whoarenotenrolledinsummerschoolarenotallowedtoreceiveactualandnecessaryexpensesbutmayparticipateinrequiredsummerathleticsactivitieswithoutbeingenrolledinsummerschool.However,UNLVmayusetheStudentAssistanceFundtoprovidemealsandlodgingtostudent-athleteswhoareparticipatinginrequiredsummerathleticactivities,butarenotenrolledinsummerschool.CURRENTNON-SCHOLARSHIPSTUDENT-ATHLETESNOTENROLLEDINSUMMERSCHOOLCurrentnon-scholarshipstudent-athletesareabletoparticipateinsummerathleticactivitieswithoutbeingenrolledinclasses,providedthestudent-athleteisacademicallyeligibleforcompetitionthesubsequentfallterm.UNLVisnotabletoprovideexpensestonon-scholarshipstudent-athleteswhoareparticipatinginsummerathleticactivitieswhilenotenrolled.However,UNLVmayusetheStudentAssistanceFundtoprovidemealsandlodgingtostudent-athleteswhoareparticipatinginrequiredsummerathleticactivities,butarenotenrolledinsummerschool.PROSPECTIVESTUDENT-ATHLETESENROLLEDINSUMMERSCHOOLProspectivestudent-athletesthatareenrolledinsummerclasses,havenotyettriggeredstudent-athletestatus,butareeligibletoparticipateinsummerathleticactivitiesareeligibletoreceiveactualandnecessaryexpenses.STUDENT-ATHLETESHOUSINGPROSPECTSItisnotpermissibleforcurrentenrolledstudent-athletesorcontinuingstudent-athletestohouseorlodgeprospects(e.g.,thoserecruitswhohavesignedanNLI,transfersandwalk-ons)duringthesummer.Althoughpermissibleinsomecases(e.g.,prior-pre-existingrelationships),prospectsresidingwithcurrent/continuingstudent-athletesfreeofchargeoratreducedcost–areinviolationofNCAA13.2.1&13.2.2.AllarrangementsmustbeclearedbytheNCAAcompliancestaff.Youareremindedthataprospectremainsaprospectiveuntiloneofthefollowingoccurs(whicheverisearlier):

PROSPECTS&FACILITYUSEWithregardstoincomingProspectiveStudent-AthletesparticipatinginvoluntarysummerconditioningitispermissibleprovidedtheyhavesignedaNLI,institution'swrittenofferofadmission,and/orfinancialaidortheinstitutionhasreceivedhisfinancialdepositinresponsetoadmission.
● Aprospectivestudent-athletewhohassignedaNationalLetterofIntent(NLI)(orafour-yearcollegetransferprospectwhohassignedawrittenofferoffinancialaidand/oradmission)mayparticipateinvoluntaryweightliftingorconditioningactivities(e.g.,conditioningonthetrack)ontheinstitution’scampusinthepresenceoftheinstitution’strengthandconditioningcoach,providedsuchactivitiesarenotprearranged,thestrengthandconditioningcoachisperformingnormaldutiesandresponsibilities

● Aprospectivestudent-athletewhodoesnotmeeteitherofthecriteriaabovemaynotparticipateinvoluntaryweightliftingorconditioningactivitiesontheinstitution’scampus,unlesssuchfacilitiesareopentothegeneralpublic.

DISCIPLINEANDENFORCEMENTAnystudent-athletechargedorarrestedasaresultofanalcoholordrugrelatedincidentwhilethatstudent-athleteisaffiliatedwiththeDepartmentofICAwillbedisciplinedpursuanttotheSanctionsforInappropriateBehaviorPolicy.Student-athleteschargedwithstudentconductcodeviolationswillalsobedisciplinedthroughtheStudentJudicialAffairsOffice.Foradministratorsandstaffmembers,theNevadaRevisedStatutes,NRS284.406throughNRS284.407,providesforthetestingofemployeesandapplicantsfortheuseofalcoholanddrugs.RegulationshavebeenapprovedbythePersonnelCommissiontocarryouttheprovisionsofthestatutes.InaccordancewiththeStatepolicy,anemployeewhoconsumesorisundertheinfluenceofalcoholorwhopossesses,consumesorisundertheinfluenceofacontrolledsubstanceissubjecttodisciplinaryaction.Anemployeewhorefusessuchatestissubjecttodismissalorotherdisciplinaryaction.(StateofNevadaAlcoholandDrugTestingProgram,NevadaRevisedStatutes,NRS284.407throughNRS284.407.InfractionsoftheUNLVDepartmentofICAAlcoholPolicywillberegardedasUniversitydisciplinaryviolations,whichmay(atthediscretionoftheUniversity)beadjudicatedattheUniversitylevelinadditiontobeingadjudicatedinthecourtsystem.Incircumstanceswherealcoholisindicatedtobeaningredientofaproblem,students,facultyandstaffmayberequiredtoparticipateinanalcoholassessmentprogram.ApersonwhoviolateslaworUniversitypolicywhileintoxicatedremainsfullyresponsibleforhisorheractionsandtheconsequencesthereof.Theconsumptionofalcoholand/ortheresultsofsuchconsumptionareneverconsideredtocompromisepersonalresponsibilityoraccountability.AtpartiesoreventsheldonpropertywithinthejurisdictionoftheUniversity,violationsofthepoliciessetforthinthisdocumentmayleadtotheterminationoftheeventbyeitheraUniversitystaffperson(s)and/oraPublicSafetyofficial(s)anddisciplinaryactionagainstitssponsor.Intoxicated/disorderlypersonswillbebarredfromenteringtheUniversity’sfacilitiesorwillbeevictedfromthefacilitiesaccordingtoPublicSafetyprotocol.

SANCTIONSFORINAPPROPRIATEBEHAVIORBecauseoftheuniquenatureofbeingastudent-athlete,therearemanyissuesthatariseasaresultofanystudent-athletebeingchargedwithamisdemeanor,felonyand/orastudentconductcodeviolation.Intheeventofsuchchargesand/orviolations,thefollowingpolicywillbeenforceduntilthematterisresolved.Intheeventthechargesalsoincludeastudentconductcodepolicyviolation,sanctionsmayalsobeimposedbytheUNLVHearingOfficer,AdministrativeOfficer,HearingPanelorVicePresidentforStudentLifeinanyorderorcombination(6.9)NSHECode.PleaserefertotheUNLVStudentConductandPolicy.Inallcases,thestudent-athletewillberequiredtocompleteallsanctionsimposedbytheAthleticsDepartmentandtheUniversity.Failuretodosowillresultinfurthersanctionsandmayresultindismissalfromthestudent-athlete’srespectiveteam.FELONYARRESTORCHARGEAnystudent-athletewhoisarrestedandchargedwithafelonyinanylocationwillbeimmediatelysuspendedfromallcompetitionuntiltheDirectorofAthleticsand/orhisdesigneehastheopportunitytoreviewalloftheinformation.TheDirectorofAthleticswilldeterminewhatimmediatesanctionsareappropriatebeforethestudent-athleteisreinstatedforcompetition.Thestudent-athletemaybepermittedtocontinuepracticingorworkingoutwithhis/herrespectiveteamandshallretainallgrants-in-aidpendingresolutionofthechargesaslongasthestudent-athleteremainsacademicallyeligibleforcontinuedenrollmentattheUniversity.AtthediscretionoftheDirectorofAthletics,afterreviewingallrelatedinformationandcharges,mayimmediatelysuspendthestudent-athletefromallpracticesandcompetitiondependingonthegravityofthechargesimposed.FELONYCONVICTIONAnystudent-athleteconvictedoforpleadingguiltyornocontesttoafelonychargeunderNevadalaworanyotherjurisdictionalequivalentshallpermanentlybedismissedfromtheteam.Thestudent-athleteshallretainhis/hergrant-in-aidfortheremainderoftheacademicsemester.Attheconclusionoftheacademicsemester,theIntercollegiateAthleticsDepartmentshallrecommendtotheOfficeofStudentFinancialServicesthatthegrant-in-aidshallnotberenewedforanysucceedingacademicyears.FelonychargeinvolvinggamblingorgamefixingAnystudent-athletewhoisarrestedandchargedwithafelonyinvolvinggamblingorgamefixinginanylocationwillbeimmediatelysuspendedfromallpracticeandcompetitionuntiltheDirectorofAthleticsand/orhisdesigneehastheopportunitytoreviewalloftheinformationanddeterminewhatinitialsanctionsareappropriatebeforethestudent-athleteisreinstatedforcompetition.Thestudent-athleteshallretainallgrants-in-aidpendingresolutionofthechargesaslongasthestudent-athleteremainsacademicallyeligibleforcontinuedenrollmentattheUniversity.WRITTENNOTIFICATIONOFUNIVERSITYAND/ORATHLETICDEPARTMENTSANCTIONSTheDirectorofAthleticswillnotify,inwriting,thestudent-athleteofanydepartmentalorUniversitysanctionsimposedasaresultofafelonychargeand/orconvictionandforchargesinvolvinggamblingorgamefixing.

intercollegiateathleticsorprofessionalathletics,throughabookmaker,aparlaycardoranyothermethodemployedbyorganizedgambling,shallbeineligibleforallregular-seasonandpostseasoncompetitionforaminimumofaperiodofoneyearfromthedateoftheinstitution’sdeterminationthataviolationhasoccurredandshallbechargedwiththelossofaminimumofoneseasonofcompetition.Arequestforreinstatementmaybesubmittedonbehalfofastudent-athletewhohasparticipatedinsuchactivityonlyuponfulfillmentoftheminimumconditionindicatedabove.Ifthestudent-athleteisdeterminedtohavebeeninvolvedinasubsequentviolationofanyportionofBylaw10.3,thestudent-athleteshallpermanentlyloseallremainingregular-seasonandpostseasoneligibilityinallsports.

Determiningwhatconstitutessexualharassmentunderthispolicywillbeaccomplishedonacasebycasebasisanddependsuponthespecificfactsandthecontextinwhichtheconductoccurs.Someconductmaybeinappropriate,unprofessional,and/orsubjecttodisciplinaryaction,butwouldnotfallunderthedefinitionofsexualharassment.Thespecificactiontaken,ifany,inaparticularinstancedependsonthenatureandgravityoftheconductreported,andmayincludedisciplinaryprocessesasstatedabove.Examplesofunwelcomeconductofasexualnaturethatmayconstitutesexualharassmentmay,butdonotnecessarily,include,andarenotlimitedto:PHYSICALASSAULT:

•Monitoringtheworkandschoolenvironmentforsignsthatharassmentmaybeoccurring;•Refrainingfromparticipationin,orencouragementofactionsthatcouldbeperceivedasharassment(verbalorotherwise);
•Stoppinganyobservedactsthatmaybeconsideredharassment,andtakingappropriatestepstointervene,whetherornottheinvolvedindividualsarewithinhis/herlineofsupervision;and
•Takingimmediateactiontominimizeoreliminatetheworkand/orschoolcontactbetweenthetwoindividualswheretherehasbeenacomplaintofharassment,pendinginvestigation.

NON-SCHOLARSHIPSTUDENT-ATHLETESAllcostsassociatedwithanyadditionaltests,consultations,and/ormedicalproceduresneededtogainapproval/certificationforparticipationinintercollegiateathleticsattheUNLVwillbetheresponsibilityofthestudent-athleteand/orhis/herprimaryhealthinsurance.NomemberoftheUNLVAthleticsDepartmentwillpermitastudent-athletetoparticipate,norwillUNLVprovideinsurancecoveragetoanystudent-athlete,whohasnotcompletedthepreparticipationphysicalexaminationprocedure.EXITPHYSICALEXAMINATIONSEverystudent-athletemustscheduleanappointmentwithamemberoftheUNLVSportsMedicineDepartmentwithin14daysoftheconclusionoftheirathleticeligibilityforthepurposeofan“ExitPhysicalExamination.”Thiswillincludeareviewofallinjuriesand/orillnessesreceivedduringparticipationatUNLVandreferralforfollow-upcareifapplicable.SICKLECELLTRAITGENERALINFORMATIONSickleCellTraitisnotadisease,butablooddisorderinwhichthebodyproducesanabnormaltypeoftheoxygencarryinghemoglobinintheredbloodcells.Apersonwithsicklecelltraitinheritsabnormallycurved(sickle)redbloodcellsinsteadofnormalroundredbloodcells.These“sicklecells”donotflowfluidlythroughbloodvesselsandcanclustertogethertoobstructthem.Thisobstructionofbloodflowcanleadtopain,infection,organdamage,andinsomecases,death.Sicklecelltraitiscommoninpeoplewhocomefromplaceswheremalariaiswidespread.PeoplemostatriskforhavingsicklecelltraitarethosewithancestorsfromAfrica,SouthorCentralAmerica,theCaribbean,Mediterranean,India,andSaudiArabia.Thetraitoccursinabout8%oftheU.S.African-AmericanPopulationandrarely(between1in2,000to1in10,000)fortheCaucasianpopulation.

UNLVATHLETICSANDSICKLECELLTRAITUNLVAthleticTraininghasamandatorysicklecelltraitscreening,viabloodtesting,forallincomingstudent-athletesduringthepre-participationphysicalexamprocess.Student-athleteswhohavesicklecelltraitareneverdisqualifiedfromparticipationinsportingevents,butaremonitoredduringactivity.IMPORTANT:STUDENT-ATHLETESMUSTHAVEVERIFIEDRESULTSFROMASICKLECELLTESTPRIORTOBEINGCLEAREDFORPARTICIPATION.THEREARENOEXCEPTIONS.Therefore,student-athleteswhoarriveon-campuswithoutasicklecelltestshouldimmediatelyreporttotheathletictrainingstafftoscheduletesting.Student-athletescanalsohavethetestingdonepriortoarrivingatUNLVattheirownexpense.FurtherinformationregardingSickleCellTraitandtheUNLVSportsMedicinePolicycanbefoundonhttp://www.unlvrebels.com/ot/sports-med-policy-procedure.html.
MEDICALDISQUALIFICATIONTheUNLVteamphysicianhasthefinalresponsibilitytodeterminewhenastudent-athleteisremovedorwithheldfromparticipationduetoaninjury,anillness,orpregnancy.Inaddition,clearanceforthatindividualtoreturntoactivityissolelytheresponsibilityoftheUNLVteamphysicianorthatphysician’sdesignatedrepresentative.SECONDMEDICALOPINIONPOLICYProvidedtheinitialexaminationisperformedbyaUNLVteamphysician,anystudent-athleteatUNLVcanchosetobeseenbyaphysician,chiropractor,physicaltherapistorotherhealthcareprovideroftheirchoiceoutsideoftheUNLVSportsMedicinenetwork.Inthisregard,thefollowingapplies:
1. UNLVisnotfinanciallyresponsibleforanymedicalexpenses,includingsurgery,MRI’s,diagnostictests,officevisits,physicaltherapyand/ortreatmentprovidedbyindividualsoutsideoftheUNLVSportsMedicinenetwork.
2. UNLVprovidesastaffofphysiciansandthesephysiciansarenolongerresponsibleforanymedicalproblemsthatmayarisefromthisinjury/illness.
3. Thestudent-athletereleasingtheUNLVteamphysiciansfromanyliabilitymustsigntheWaiverofLiability/SecondOpinionPolicyForm.
4. Thestudent-athletemustsignaReleaseofMedicalRecordsForm.5. AllmedicalrecordsgeneratedasaresultofthesecondopinionmustbeforwardedtotheUNLV
DirectorofAthleticTraining.FailuretodosomayprecludeUNLVfromallowingthestudent-athletetopracticeandorcompete.
6. TheUNLVteamphysicianswillevaluatetheforwardedmedicalrecordsandwilldeterminetheplayingstatusofthestudent-athletepriortoclearingthestudent-athleteforanypracticeand/orcompetition.
7. Thestudent-athletemustbeclearedinwritingbytheUNLVteamphysiciansbeforehe/shecanactivelyparticipateinanyconditioning,practiceand/orcompetition.
8. Theathletictrainingstaff,coaches,andadministratorsatUNLVarerestrictedfromofferinganyassistancetothestudent-athleteunlessdirectedbythe“secondopinionprovider”.
9. Intheeventofre-injurytothesamebodypart,thestudent-athletemustbeseenbythe“secondopinionprovider”andthenmustbeclearedbytheUNLVteamphysicians.
10. Itwillbetheresponsibilityofthestudent-athletetomakeallschedulingarrangementsforthesecondopinion.

“OUT-OF-TOWN”REHABILITATIONPROCEDUREAttimes,itmaybenecessaryforastudent-athletetoutilizean“out-of-town”rehabilitation/physicaltherapyfacility.Insuchsituations,permissionmustbegrantedfromtheDirectorofSportsMedicineand/orhis

DENTALCARETheUNLVAthleticDepartment’smedicalinsuranceprogramwillassumefinancialresponsibilityfordentalcareresultingfromthedirectparticipationintheintercollegiateathleticsprogramasapprovedbytheDirectorofAthletics,accordingtoNCAAregulations.Aswithotherinjuries,thisinsuranceisSECONDARYto,orinexcessof,personalfamilymedicalinsurancecoverage.TheUNLVDepartmentofAthleticswillnotbeheldresponsibleforanygeneraldentistry,includingteethcleaning,fillings,toothextractions,orotherpre-existingconditions,unlesstheconditionhasbeenaggravatedbyparticipationinintercollegiateathletics.AlldentalappointmentsmustbearrangedthroughthedirectorofAthleticTraining.Custommoldedmouthpieceswillbefittedforstudent-athletesparticipatinginhigh-risksports(e.g.football,basketball,etc.)andotherstudent-athleteswhoexpressadesireforthem.FootballplayersarerequiredbytheNCAAtowearamouthpieceatalltimesasapartoftheirrequiredequipment.TheUNLVAthleticsDepartmentanditsmedicalandcatastrophicinsuranceprogramWILLNOTassumefinancialresponsibilityfordentalinjuries,headandfacialinjuries,and/ormildtraumaticbraininjuries(concussions)ifthestudent-athleteisnotproperlywearinghis/herrequiredmouthpieceatthetimeofinjury.

Medicalbillsassociatedwithsuchinjurieswillbethesoleresponsibilityofthestudent-athleteand/orhis/herparent(s)orguardian(s).STUDENTHEALTHCENTEREachstudent-athletemayutilizethestudenthealthservicesand/orpurchasestudenthealthinsurancetocovernon-athleticinjuriesandillnesses.Alldeductiblesandincurredexpensesfromthestudenthealthcenterorstudenthealthinsurancewillbethefinancialresponsibilityofthestudent-athlete.MEDICATIONSAUNLVSportsMedicineDepartmentprescriptioncompletedbyaUNLVTeamPhysicianalongwithaMedicalReferralForm,willserveasauthorizationforpermissiblepaymentbytheUNLVStudentHealthCenterPharmacy.AllprescriptionsmustfirstbepresentedtotheUNLVSportsMedicineStaff.Anyprescriptionwrittenonastandardphysician’sprescriptionpadmustbepresentedtothepharmacistwithacompletedMedicalReferralFormtoguaranteepayment.Failuretofollowthisprocedurewillresultinthestudent-athletebeingheldfinanciallyresponsibleforpayment.PHYSICIANREFERRALS/CONSULTATIONSTheUNLVDepartmentofAthleticsandSportsMedicineDepartmenthasfosteredpositiverelationshipswithmanymedicalprovidersintheLasVegas,NevadaareawhohaveconsistentlyprovidedhighqualityservicetoUNLVstudent-athletes.MembersoftheUNLVSportsMedicineDepartmentwillreferstudent-athletestotheseproviders,unlessextenuatingcircumstancesnecessitateadifferentprovider.Allstudent-athletesarecoveredundertheUNLVAthleticDepartment’ssecondaryinsurancepolicywhentheyseeaphysicianintheUNLVSportsMedicineNetwork.Thisnetworkiscomprisedofawidevarietyofmedicalspecialistsfromthemedicalcommunity.ThegroupisdedicatedtoprovidingthebestpossiblehealthcaretotheUNLVstudent-athletes.ThisnetworkwasformedtoinsureaccurateandcontinuouscommunicationbetweenthephysiciansandtheUNLVSportsMedicineStaff.Allstudent-athletesmustbeseenandevaluatedbyamemberoftheUNLVSportsMedicineStaff.DecisionsregardingreferralsareatthediscretionoftheUNLVSportsMedicineStaff.AmemberoftheUNLVSportsMedicineDepartmentmustauthorizeandproperlyreferallstudent-athletestoseeaphysicianormedicalconsultant,and/orfordiagnostictests.Ifastudent-athletedecidestoseeaphysician/medicalconsultant,and/orundergoadiagnostictestWITHOUTpriorauthorization/referralfromamemberoftheUNLVSportsMedicineDepartment,thestudent-athleteand/orthestudent-athlete’sparent(s)orguardian(s)willbefinanciallyresponsibleforanyandallmedicalbillsincurred.Orthopedicand/orMedicalSecondOpinions:Ifastudent-athletedecidestoseeanorthopedicand/ormedicalphysicianwithouttheauthorizationofamemberoftheUNLVSportsMedicineDepartment,thestudent-athleteand/orthestudent-athlete’sparent(s)orguardian(s)willbefinanciallyresponsibleforanyandallmedicalbillsincurred.Ifastudent-athletedecidestoseekmedicalattentionoutsideoftheUNLVSportsMedicineNetwork,thestudent-athleteand/orthestudent-athlete’sparent(s)orguardian(s)willbefinanciallyresponsibleforanyandallmedicalbillsincurred.

Inordertoensureanatmosphereofcompliance,UNLVrequiresallstudent-athletesseekingexternalmedicalservices(Non-UNLVreferred)tocompletetheStudent-AthleteDeclarationofOutsideServicesform.Formscanbeobtainedthroughthecomplianceoffice.

GUIDELINESFORMEDICALCOVERAGEWithathleticpracticeandcompetition,thefirstrespondertoanemergencysituationistypicallyamemberofthesportsmedicinestaff,mostcommonlyacertifiedathletictrainer.Ateamphysicianmightnotalwaysbepresentateveryorganizedpracticeorcompetition.Thetypeanddegreeofsportsmedicinecoverageforanathleticeventmayvarywidely,basedonsuchfactorsasthesportoractivity,thesetting,andthetypeoftrainingorcompetition.Conditioningworkouts,weightliftingworkoutsandindividualskillinstructionscheduledintheearlymorninghoursoratnon-traditionaltimesmaybeconductedwithoutmedicalcoveragefromtheathletictrainingstaff.Thefirstresponderinsomeinstancemaybeacoachorotherinstitutionalpersonnel.Certificationincardiopulmonaryresuscitation(CPR),AutomaticExternalDefibrillators(AED),firstaid,preventionofdiseasetransmission,andemergencyplanreviewshouldberequiredforallathleticspersonnelassociatedwithpractices,competitions,skillsinstruction,andstrengthandconditioning.INJURYANDILLNESSPROCEDUREIntheeventastudent-athleteisinjuredorbecomesill,thestudent-athletemustfirstcontacttheAthleticTrainingDepartment.
1. Uponcompletionofanathletictrainingstaffexamination,anathletictrainingstaffmembermayreferthestudent-athletetotheteamphysician.
2. HeadCoaches,assistantcoaches,graduateassistantcoaches,managers,oranyotherUNLVpersonnelshallnotbepermittedtoscheduleappointmentsforanystudent-athletewithoutfirstconsultingtheDirectorofAthleticTraining.Anyreferralswithoutfollowingproperprocedureswillresultinthestudent-athletebeingheldresponsibleforpayments.Intheeventofanathleticrelatedemergency,ortheathletictrainingstaffisunavailable,anyUNLVpersonnelshouldprovidethestudent-athleteaccesstoamedicalfacilityofchoice.
3. Thestudent-athletewillberesponsibleforpaymentofanyfeesresultingfrommissedappointments,includingdental,eyeexams,oranyothermedicalproblem.
