General Information:

Connie’s Kids CDC is licensed by the State of Wisconsin, Department of Children and Families. I am licensed to care for no more than 8 children at any one time. I am inspected regularly to ensure that I continue to meet licensing standards.

Connie’s Kids CDC is covered by liability insurance for both the operations and the premises.

Parents and/or authorized guardians are required to sign their child in and out every day with their full name. The sign in clipboard is located by the front entrance and is required by law to be completed each day your child attends child care. It is also important to sign out with the accurate time. If you happen to be late, we need you to sign out at the actual time of pick-up, not our closing time of 6:00PM. Signing in or out with the improper time puts provider in an awkward position.

Connie’s Kids CDC has an open door policy. Our doors are always open to families for any questions, concerns, or suggestions you may have. We encourage families to drop in as often as you like during regular business hours. Family involvement is always encouraged unless denied by court order.

Connie’s Kids CDC will serve children 6 weeks through 12 years of age. Connie’s Kids CDC will comply will all laws governing facilities and operations. Child care services are available without discrimination on the basis of race, sec, color, creed, political persuasion, national origin, disability, ancestry or sexual orientation.

Child care services will be provided between the hours of 6:30 A.M. and 6:00P.M. ~ Monday thru Friday ~ January thru December.

Non-Discrimination Policy and Anti-Bias Statement

Enrollment opportunity are available without discrimination on the basis of race, sex, color, creed, political persuasion, national origin, disability, ancestry or sexual orientation. Connie’s Kids CDC will comply with the requirements and objectives of state and national laws that are the basis for this policy.

Connie’s Kids CDC does not discriminate against children or families with disabilities. Children or families with mental or physical disabilities are encouraged to participate in all the activities and opportunities here at Connie’s Kids CDC. Provider will assess children on an individual basis to determine whether a child with a disability can be cared for at Connie’sKids CDC with reasonable accommodation. Some examples are:

The revision of policies and procedures to be inclusive. The removal of physical barriers.

The Addition of adaptive equipment. The provision of additional provider training.

Provider is committed to treating all families with dignity and respect for their individual needs and differences. Provider promote an anti-bias environment and curriculum, and maintain confidentiality.

Provider is dedicated to providing an anti-biased environment for children and adults. An anti-biased environment is one that encourages an active approach to challenging prejudice, stereotyping, and any other kind of bias. Holidays

Connie’s Kids CDC will be closed on the following holidays:

New Year’s Eve / Day Martin Luther King Jr Day Memorial Day Fourth of July Labor Day Good Friday Thanksgiving Day / Friday after Thanksgiving Christmas Eve / Christmas Day

If the holiday falls on the weekend, we will be closed on either the Friday before or the Monday after. All regular fees will be charged for these holidays.

The following items are posted on the parent board for your review:

issued by the department as soon as it tis received. Weekly calendar

These items will remain posted until the violations(s) has been verified as corrected and the action is closed.

Children will only be released to persons listed on the enrollment form. If anyone other than the child’s parents or someone who is listed on the enrollment form is to pick up a child (ren). Provider will need to be notified in writing. The person picking up the child (ren) must show a driver’s license or other picture ID.

If parents wish to allow school-age children to leave or arrive to the center unescorted, they must provide written authorization. School-age children who leave the

center unescorted must be traveling to home, school or another activity where adult supervision is present.

If the parent or other authorized person arrives to pick up a child and that person appears to be intoxicated or under the influence of drugs, all reasonable steps will be taken to prevent the person from leaving with the child, including offering to call a cab or another contact person. While I can’t legally withhold a child from a legal guardian, provider will not hesitate to call local authorities if provider feel the child (ren) are in danger.

Every center that provides care for children and is licensed by the State of Wisconsin is a mandatory reporting agency for suspected child abuse. Although this is rare, provider is required by law to report to Children’s Protective Services any abuse that is observed or suspected my occur. Provider is trained on how to recognize signs of abuse and /or neglect.

Parent communication

Effective communication is essential to a successful program. Connie’s Kids CDC publishes a monthly newsletter that addresses upcoming events, planned curriculum, pertinent parenting information, and articles on events affecting our center. The front entrance is our communication zone where poster and flyers of reminders will be posted near the daily sign in and out sheet. Provider education credentials and or degrees will be displayed in this area also. Provider will also occasionally send out a group email with notices of upcoming events so be sure to update or include the best email address for you to receive important messages. Parent teacher conferences are offered two times per year to discuss children progress. Provider offers two family outreach, and social opportunities Per year.

Communication is one of the hallmarks of family-friendly center. Families indicate that they highly value a wide range of communication methods. The methods receiving the highest ratings are those most likely to be in place at centers; communication methods provider use include notices posted in places like cubbies or on children bags home, conversations at drop-off and pick-up times, individualized written notes, and individual parent conferences. Parent teacher conferences are offered two times per year to discuss children progress. Social opportunities and family outreach is offered two times per year. Provider will post a weekly calendar on the parent board to keep parents informed of the planned themes, projects, activities, and skills practiced each week.

Parent involvement

Families are strongly encouraged to be involved with Connie’s Kids CDC. It is not a requirement for families of children enrolled at Connie’s Kids CDC, I have found that if each family volunteers just one hour each month, it makes a tremendous difference in your child’s day.

The most common means of involvement are: Participating on the Board of Directors. Sharing family stories or information about your job. Attending field trips. Assisting provider in daily lessons preparations. Contributing an article in the monthly newsletter.

Each family with a child enrolled at Connie’s Kids CDC is required to raise $250.00 per year through a family donation of money, time or materials; or a combination of both to support center yearly quality improvement goals.

Enrollment and discharge of enrolled children:

All children will be enrolled on a two week trial period. During this period either the provider or parent my terminate contract without advance notice.

Parents must meet with provider to discuss child (ren) specific needs, tour facility and to review program policies and procedures. Provider will make reasonable accommodation for children with disabilities as specified under the Americans with Disabilities Act.

Each child participating with Connie’s Kids CDC is required to have a file that includes:

Child enrollment form (on file prior to the first day of attendance) Health history and emergency care plan (updated every three (3) months. Child Care immunization record (must be on file before first day of attendance and

updated as changes occur). Alternate arrival / release agreement Transportation permission(if applicable written authorization outlining any plan for

school age programs to come to or leave the program from school, home or other activities unless the parent is signing the child in/out.

Intake for child under (2) years (if applicable on file before the first day of attendance, this form must be updated every (3) months).

Field trip permission slips Allergy or dietary form Parent / Provider contract Emergency card Schedule agreement

First week’s tuition payment Photo ID of parent or guardian responsible for child (ren). Parent orientation of policy and rules and regulations.

The following items must be completed and returned to provider within 30 days of enrollment:

Child health report (updated as changes occur)

Provider will inform parents of any updates that are needed and give parents one (1) week to submit updated materials.

A child may be discharged from the center for reasons such as, but not limited to:

Failure to pay fees on time (grounds for immediate termination, without advance notice).

Lack of parental cooperation. Inability of child care program to meet the needs of the child. Provider will consult

with parents concerning how any issues might be solved before ending the child care arrangements. Families could be referred to other community resources.

Repeated failure to pick up child (ren) at scheduled time. Failure to complete and return required forms.

Provider will give two (2) weeks written notice of intent to discharge a child (ren) and will try to inform parents of local resources that may be of help to them, except when the discharge is due to failure to keep current with fees owed. Should families remove the child (ren) during a notice period provider initiates, fees will not be charged for the remaining unused days.

Families must give two (2) weeks written notice of their intent to withdraw the child (ren), and are required to pay for those two (2) weeks, whether children continue to attend or not. All outstanding fees must be paid…

Payments and Refunds

All fees are to be paid in advance of services (fees are due on Friday of week prior to attendance) $20.00 late fee assessed each week fees not received when due).

Returned checks will result in $25.00 fee. (If this occurs families lose check writing privileges). Provider will then only accept cash, money order or cashier checks for child care payments.

Families with past due fees must pay all outstanding balances, including any late fees charged, before the child (ren) will be allowed to return to child care.

No credit will be given for holidays, sick days, vacations, or any other-special occasion fees or absences which shorten week. (Fees are to be paid in advance on Friday each week for the following week services). If there is a third party payment, such as an employer or W2, a special payment schedule will be arranged and detailed in the child care contract. Families will be responsible for any specified co-payments or unpaid amounts.

Connie’s Kids CDC does not charge a registration /enrollment fee.

Full-time rates are offered for children in care for 6 hours or more (5) days per week. Part-time rates are charged if children will attend fewer than (6) hours each day, or

fewer than (30) hours per week. Provider will accept children for drop-in care if prior enrollment arrangements have

been made and space is available. Provider will offer a sliding scale fee for financially challenged families based on

families last year’s income tax forms. Provider will establish regular rates based on child (ren) hours of attendance.

(Additional fees will be assessed for additional hours if hours exceeds contracted hours).

Rates are higher for children under two (2). Extra fees are assessed for late payments. Extra fees are assessed for late pick-ups. No reductions will be made for additional children from one family. No refunds for absences due to illness or any other reason. Families are required to notify Connie’s Kids CDC two full weeks in advance when child

will withdraw to accompany family on vacation. (Families are required to pay one-half the regular week rate, in advance, for vacation time in order to ensure readmission)

Provider will notify parents four weeks in advance of closure due to provider vacation or an educational conference.

Child Absence

If child (ren) will not attend on a regular scheduled day families are required to let provider know by 6:00 AM or within (30) minutes of child’s scheduled arrival time. If child has not arrived (30) minutes after the scheduled arrival time provider will attempt to contact family to determine child (ren) whereabouts. Provider will leave a voice message if families are not home.

If child (ren) are expected to arrive at the center from someplace other than home (school etc.) and does not arrive as scheduled, provider will immediately attempt to contact the facility child arriving from, and families if necessary, to determine the child’s whereabouts.

Provider will not transport children who attend school to and from the center.

Each year children who have been enrolled six (6) months or more will be allowed two (2) weeks’ vacation, prorated for part-time enrollments). These two (2) week can be used for sick or vacation time. After the two (2) weeks are used, provider will require full payment for any absences, for the rest of the year.

Families are required to record child (ren) daily attendance records on sign in/our form. Fee information on late pick-up, absences, etc. is available under the fee and payment policy. It is important that families comply with contracted scheduled agreements.

Provider absence

Provider will take two (2) weeks of vacation each year. Provider will not charge a fee for these weeks. Provider will let families know four (4) weeks in advance of these vacation days. Families are responsible for making alternate child care arrangements.

In the event that Connie’s Kids CDC is closed because of illness, provider will notify families of all regularly scheduled children no later than 5:45 AM. Regular fees will be charged when program is closed due to provider illness.

In case of an emergency situation that requires my immediate attention, provider will call Cynthia Hutchins, program’s emergency backup person. Provider backup person will come to the center and stay with the children during provider absence. Families may or may not be called to pick up children depending on the severity of the situation. If parents are called, parents should pick up children, as soon as possible, or within one (1) hour of receiving the call.

Health – child illness / injuries

If your child (ren) is ill to the extent that it interrupts the care of or endangers the health of the other children, it will be necessary you to make other arrangements for his or her care. Should your child become ill during his or her day here, you will be notified and provider will determine the best course of action concerning appropriate care, which may include the child being taken home.

Provider will always respect families need to be at work when it comes to an ill child. Provider ask, however that when deciding if your child should be at home, families give consideration to the other children in care and ask yourself how you would feel if your child were here and well and another child were as ill as your child is now.

Provider “bottom line” for not providing care is fever in excess of 101 degrees, vomiting, excessive diarrhea, or any potentially contagious disorder. Further, there are times when a child is not that ill but is terribly uncomfortable and really needs some

“one on one” that I cannot provide and be fair to the other children. At those time provider will strongly urge families to consider keeping child (ren) at home.

Any medication to be given must come in a labeled prescription bottle, nonprescription medicine (also labeled with child’s name), if needed, will also be administered, if provided by families.

Provider will report all communicable diseases, when required, to the local health department and to families of all enrolled children. Families will only be notified when children have been exposed to a communicable disease.

Child’s Kids CDC has been authorized by state licensing agency to provide care for mildly ill children. This means provider will care for children who have a common, temporary illness that is non-progressive in nature and is not considered a communicable disease by Wisconsin Division of Public Health. (Example), children with a cold or upper respiratory illness may be cared for, however, children who are in the contagious stages of a communicable disease such as chicken pox, or pink eye, may not be permitted in care until the appropriate period of communicability has passed.

Superficial injuries will be washed with soap and water and covered with a bandage or treated with ice. Families will be told about the minor injuries when child (ren) are picked up, or delivered to families or other authorized person.

Provider has received training in first aid. Provider will follow standard emergency medical procedures for treating injuries. Provider have a current certification in infant and child cardiopulmonary resuscitation (CPR) including training in the use of an automated external defibrillator (AED).

If there is a need for emergency medical treatment, (911) will be called and the child (ren) will be taken to St. Joseph Hospital. Should an ambulance be needed, families will be responsible for the costs. Families will be contacted as soon. Provider will suggest to emergency responders that child be taken to hospital specified on child’s emergency medical form.

All medication administered, accidents or injuries that occur during the time child is in provider’s care, any marked changes in behavior or appearance or any observation of injuries to child’s body received outside of provider’s care will be entered into a medical logbook. As a licensed child care provider, provider is required to report suspected child abuse or neglect or appropriate authorities.

Provider will administer medications. Prescriptive and not-prescriptive medication will only be given to children if families have completed the authorization form provided.

All medicine must be in its original container bearing the label with child’s name, dosage and administration instructions. Provider will not exceed the age-related dosage without written doctor’s authorization. Blanket authorizations, such as dispensing Tylenol at my discretion, are not allowed. Provider will send all medication home when authorization time is up.

Prior to applying sunscreen, teething gel or insect repellant, provider will obtain a written authorization from families. Authorization shall include the brand and the ingredient strength.

Smoking is not permitted on the premises of the center.

SIDS – Children under one year of age will be place to sleep on his or her back in a crib, unless the child physician authorizes another position in writing. Children will not sleep in a crib or playpen that contains materials such as sheepskins, pillow, fluffy blankets, bumper pads or stuffed animals. Children under two years of age cribs and playpens shall contain a tight-fitting mattress and any mattress covering shall fit snugly over the mattress. Sheets or blankets will be tucked tightly under the mattress and shall be kept away from the child’s nose and mouth. If a child falls asleep in a swing or car seat, the child will be removed from the swing or care seat immediately and placed to sleep on his or her back in a crib.

If child becomes ill while at the center, families will be contacted immediately. Ill children will be isolated within sight and sound and made as comfortable as possible. Children should be picked up within one hour of provider call. If child is not picked up within one hour, provider will contact next person on child’s emergency contact list.

Personal cleanliness

Proper hand washing helps keep your child care program safe and healthy for children, their families, and you! Provider and children hands should be washed with liquid soap and water upon arrival, before and after meals or preparing food, after toileting / diapering (for the provider: after each diaper check, completing the diapering procedures, changing soiled clothes, helping children with toileting, after outdoor play, before shared water play and after messy, sand, or water play, after dealing with bodily fluids, after touching contaminated objects or pets and whenever they become dirty.

All meals, snacks, and drinks will be provided. All meals are prepared onsite.

No sugar cereals and sweet drinks will be served. All beverages will be 100 percent fruit juice.

Meals and snack will be nutritional and the daily menus will follow the guidelines recommended by the UDSA Child and Adult Care Food Program.

A weekly menu will be posted. All Children should be dropped off ½ hour before scheduled meal serving times.

Birthday and holiday treats will be provided by the day care. Families are not permitted to bring gum, candy, or other treats.

If child (ren) have any special dietary needs (medical or personal) or has food allergies, families must notify provider in writing.

Weekly record of meals and snacks are available for review upon request. School-age children will be offered an afternoon snack upon arrival from school.

Children younger than twelve (12) months will be served formula or breast milk unless written authorization is on file from child’s physician stating otherwise. Provider will provide Good Start infant formula. All infant bottles and commercial baby food must be labeled with child’s name.

Daily Activities

Provider do not include religious practices in daily activities. Provider do not offer a religious based education program or curriculum. Provider will celebrate the following holidays: Christmas, Easter, Halloween, Independence Day, Dr. Martin Luther King Day, Mother’s Day, Father’s Day, Valentine Day and St. Patrick Day.

Provider will plan activities that will provide children with a variety of developmentally and age appropriate experiences such as:

Language development, books, music, story time, finger plays, flannel board stories. Large muscle skills, balls, hula hoops, bean bags, outdoor play. Small muscle skills, arts / crafts, stringing beads, blocks. Creative expression, dramatic play, music / flannel board. Self-help skills, assist with mealtime preparation, dress self for outdoors.

Literacy skills, books, storyboard, alphabet and writing.

Play is a major component of child care program. Enough time, materials and space is provided for children to actively explore the world around them. Children will have an opportunity to use a variety of art materials and manipulative and housekeeping equipment.

Infants and toddlers will receive individualized care and will be allowed to follow their own schedules.

Caregivers will provide language development and other learning experiences for babies during normal routines (eating and diapering)

Infants will receive physical contact and attention including being held, rocked, talked and sung to.

Daily communication with parents will unsure the child (ren) schedule is as consistent as possible.

Infant will be given individual attention including lots of time for talking.

Body positions of non-mobile infants and their location in the center will be changed frequently.

Provider will provide safe, open spaces for children who are creeping and crawling.

Infants and toddlers will be encouraged to play with a variety of safe age and developmentally appropriate materials.

Children, including infants and toddlers, will go outside daily when weather permits. The children will be kept indoors during inclement weather such as:

Heavy rain

Temperature above 90 degrees F.

Wind chills of 0 degrees F. or below for children age two (2) and above.

Wind chills of 20 degrees F. or below for children under age two (2).Nap/Rest Time

Most children under five years old benefit from having a short nap/rest period in the early afternoon when they can relax. Often, children don’t realize how tired they are until they slow sown for a few minutes. Having a scheduled rest time lets them find out. We stress that sleep is optional and that we need the children to al least rest their body so they may

have more energy to participate in the afternoon’s activities. After approximately one half hour to forty five minutes of rest, we allow the children who have not fallen asleep to either have books on their mat or we allow the children who have not fallen asleep to either have books on their mat or we allow them to engage in a quiet activity such as puzzles, art, books etc. This system has been very effective especially for the children who do not sleep on a regular basis. Again, it gives the children the opportunity to rest even though they may not think they are tired.

I will provide a nap mat for each child and I will sanitize the mats on a regular basis. Each child needs to have a sheet and blanket of their own which will be laundered weekly. If your child is in the potty training stage, or is used to sleeping with a pull-up, please communicate your child’s needs to me. I will accommodate individual needs as much as possible. I will handle all accidents very discreetly with the utmost attention to each child’s self-esteem.

Connie’s Kids CDC Daily Schedule

Free-Play Time: Children are able to choose to play in the identified areas. (At least (5) main types of play should be offered) Teachers prepare areas so the materials offered reflect the current learning objectives.

Circle Time:Includes attendance and reviewing class activities for the day. Provider may include more activities to help in transitioning from one type of activity to another.

Large Muscle Activities:Primarily outside active provider led play time. May be indoors during inclement weather. Activities will be planned daily that develop coordination and physical development.

Free-Play Time:Children choose to play in the identified areas (All areas of the room could be in use) Activities that require more direct provider attention or assistance are added. (These activities are usually noted on the lesson plan)

Meal Times:Meals/snacks are planned so that they are part of daily activities, not an interruption. Transition activities will be used so waiting time is kept to a minimum

Commonly referred to as “nap time”, this is the time of day when children, who are tired can have a place to rest. Naps will not be forced. Awake children will be able to get off their cots after 30 min, or as they wake up.

Large Muscle Activities:Primarily outside active provider led play time. May be indoors during inclement weather. Activities will be planned daily that develop coordination and physical development.

Meal Times:Meals/snacks are planned so that they are part of daily activities, not an interruption. Transition activities will be used so waiting time is kept to a minimum.

Story Time:(Optional) transition time used when multiple activities are occurring. Focus of the time is to share stories. Story time will not be more than 10-15 minutes.

Provider do not have a swimming pool on the premises. Provider will not use an off premises poll, wading poll, water attraction or beach for enrolled children. If provider do decide to utilize any of these off-premises, water-oriented facilities, provider will follow all safety and supervision requirements as specified by licensing rules.

Provider has an outdoor play space on the premises. If provider choose to provide a wading pool, provider will be within sight and sound of children at all times. Provider will occasionally take walks around the neighborhood. Provider will always have emergency cards and permissions slips for each child whenever the child (ren) leave the premises.

Play and creativity is the basis for all activities at Connie’s Kids CDC. Play integrates physical, mental, social and emotional growth. Creativity is fostered by a free, flexible, accepting, and open environment and by openly discovering, inventing, and creating. In addition to having the opportunity for self-expression, children are practicing small motor skills, eye-hand coordination, left to right progression, and whole-part relationships.

Toilet training

Child care regulations will not permit provider to toilet train children under the age of twenty months. When parents and child (ren) are ready to begin toilet training a cooperative effort between home and center is most effective. If parents undertake the task of potty training when they want it to happen, the task is arduous and painful for all concerned.

When we agree that the time is right for your child and until he or she is totally

successful in his or her toileting, “No” wearing of overalls or clothing with suspenders, snaps,

buckles, belts, or zippers. Elastic waist pants “ONLY”. Anything else sets your child up for failure. In addition, your child’s cubby “MUST” have two complete changes of clothes including socks.

If your child does not have immediate success, he or she will be put back into diapers, the process will be attempted again. This will not harm the child in any way, in fact, keeping a child in “grown-up” pants when he or she is not successful is harmful.

Field Trips

Getting out of the house occasionally and exploring the world is good for provider and children. There is virtually no end to places that young children will find fascinating. Some common field trip destinations are:

The zoo An indoor children’s gym A children’s museum / science center A park or nature preserve with trails for hiking An aquarium A fire station The public library

Provider will always inform families ahead of time of an upcoming field trip and get permission to take each child.

Provider will also try to bring in interesting people occasionally who can talk about their jobs with the children. Some possibilities are:

A nurse An author An artist A police officer

Child Guidance

The goal of Connie’s Kids CDC is to guide children in becoming cooperative and happy through positive, non-threatening techniques. These techniques will include problem solving, active listening, I-messages and negotiating skills. Children behavior is guided by setting clear limits. Provider will communicate with children about expected behaviors and model these behaviors consistently for them.

Provider will state positively what children can or cannot do using specific terms; “you need to walk” rather than “don’t run”. Undesirable behavior will be redirected to another activity. Children are given a variety of age and developmentally appropriate activities to choose from and will be given the attention they need before they demand it. Behavior

management will be for the purpose of helping children develop self-control, self-esteem and respect for the rights of others, not punishment

Provider no not use “time outs”. Provider do recognize that no single technique will work with all children every time. If a child exhibits extreme excessive unacceptable behavior, provider will request a conference with families to consider how to deal with the behavior. If the behavior continues, provider may include referrals to appropriate community resources and or discharge the child from care. There may be times when provider may need family’s advice or assistance, and will not hesitate to call families if provider feel that it is necessary to do so.

In accordance with “Wisconsin rules for licensing family day care” actions that are aversive, cruel, humiliating, and actions that may be psychologically emotionally or physically painful, discomforting, dangerous or potentially injurious are prohibited. Prohibited actions include spanking, hitting, pinching, shaking, slapping, twisting or inflicting any other form of corporal punishment on the child; verbal abuse threats or derogatory remarks about the child or the child’s family; physical restraints, binding or tying the child to restrict the child’s movement or enclosing the child in a confined space such as a closet, locked room, box or similar cubicle; withholding or forcing meals, snacks or naps, or punishing a child for lapses in toilet training. These forms of punishment will not be used even at parent’s request.

Transportation

Provider do not provide transportation. Provider do not contract for transportation services. Public transportation will be used for field trips requiring transportation.

Children will never be left unattended in vehicles. Whenever children are transported provider will have an attendance checklist. All children will be accounted for by name and sight at each transition, inclusive of each time a vehicle is exited. Provider will obtain the driving record of any person who will be transporting children.

Pets

Connie’s Kids CDC does not have pets on the premises. Prior to adding pets to the center, provider will notify all families in writing. If child has any pet allergies, please inform provider verbally and also in writing.

Orientation Plan

Procedure for ensuring each employee, volunteer or substitute receives an orientation before beginning work. Orientation plan will contain:

Names and ages of all children in care. Current arrival and departure information for each child including the names of

people authorized to pick up the child.

Review of children’s records including emergency contact information. Specific information relating to child’s special health care needs including medications,

disabilities or special health conditions. Procedures to reduce the risk of sudden infant death syndrome. Overview of the daily schedule, including meals/snacks, naps and any information

related to eating and sleep schedules of infants and toddlers. Review of center’s procedures for dealing with emergencies. Procedure for reporting suspected abuse and neglect of a child. Plan for evacuating sleeping children. Procedure to contact a parent if a child is absent from the center without prior

notification from the parent. Review of center policies required under 250.04(2) €. Review of DHS 12.7 (1) regarding caregiver reporting requirements.

Emergency back-up provider

Procedure for ensuring that each time a back-up provider is used, back-up will receive an orientation immediately, before being left alone with children. The orientation will contain:

Names and ages of all the children in care. Arrival / departure information for each child and authorized pickup people. Location of children’s files including emergency contact information, consent for

emergency medical treatment, special health care needs. Procedures to reduce the risk of sudden infant death syndrome.

Emergency Procedures

Families should watch local news station for inclement weather closing. In the event that Milwaukee Public Schools are closed due to severe weather, Connie’s Kids CDC will also be closed. Families will be contacted to pick up children within the hour. Provider will contact families / emergency contacts if children need to be pick-up. If families / emergency contacts cannot be reached provider will keep the child in a safe designated area until someone can be reached. All fees will be waived under these conditions.

Fire drills are practiced once a month with children. In addition, tornado drill are practiced during tornado season which is April through October.

In case of emergencies that require an evacuation or fire drills children will be evacuated through the nearest safe exit. The attendance form and list of phone numbers for parents and emergency contacts will be taken along to ensure that all children are accounted for and all families are notified. Children will assembled middle of the block at the end of the alley.

In the event of a tornado warning, provider and children will meet downstairs in the basement at the bottom of the landing. Blankets and a portable radio and flashlights, with extra batteries for both, are kept in the tornado shelter area at all times. The attendance form and emergency contact information brought along.

In the event of a lost or missing child, provider will check all areas of the center. If the child can’t be found, the child’s family or emergency contact and local authorizes will be notified immediately. Provider will notify the DCF within 24 hours after the occurrence. If the center should lose the use of heat, water or electricity before the center opens, provider will notify families as far in advance as possible. Regular fees and tuition will not be charged

If the center should lose the use of heat, water or electricity while children are in attendance, families or emergency contacts will be notified immediately and asked to pick children up ASAP. Regular fees and tuition will not be charged.

If provider is unable to re-enter the building after a necessary evacuation, provider will take children to neighbor directly across the street from center and contact families to pick children up. Regular fees and tuition will not be charged.

In the event that the center receives a threat to the building or its occupants (bomb threat, bodily injury threat, etc.) law enforcement and the families will be notified immediately to advise them of the threat. Depending on the nature of the threat evacuation and / or closure may be required. Regular fees and tuition will not be charged.

What supplies to bring to center

Extra clothes including socks. Large supply of diapers and wipes. Please label bag of diapers with child’s

first and last name. Clean, empty bottles, provider will supply Good Start formula. If you are

breastfeeding and wish to send breast milk, provider will be happy to

accommodate your needs. Provider will not use a microwave to warm infant formula or breast milk.

If child has special allergies or dietary restrictions please bring written statements.

Appropriate shoes and clothing for outside play. Sunscreen Please label everything with permanent marker. Label with child’s first

and last name.

All fees are paid in advance of services provided. Fees are due on the Friday prior to 1st day of attendance. A 20.00 late fee will be assessed for fees paid late, if a portion of the fees are paid by a third party, such as an employer or the county, that payment will be accepted on a separately arranged schedule, described on the contract. All past due fees must be paid, including any late fees, before a child can return to child care.

Fees for non-sufficient funds (NSF) or overdrafts. Families will be charged an additional fee of 25.00 if your check does not clear the bank. If this occurs you will lost all check writing privileges.

The financial terms will be finalized upon signing of the parent-provider contract. If the parent or legal guardian is under age 18, a cosigner must sign the contract to act as guarantor to the contract, and agree to be bound by all financial terms. Families will receive a minimum of 2 weeks’ notice when a rate increase is planned.
