JOB DESCRIPTIONJob purpose / Context:

Work flexibly within the service to provide a range clinical and prescribing services to drug and alcohol users across Sandwell.

Responsible to:Team Leader (Clinical)

1. Service Delivery

Provide a clinical service to alcohol and drug users consistent to their needs including evidence based non-medical prescribing service.

Provide tailored treatment interventions including general health assessments and screening, community detoxification, medication titration, stabilization programmes and relapse prevention plans.

Develop clinically safe and effective Detox plans with individuals that are; comprehensive, person-centred and individualised based on a clear assessment of that individual’s physical and mental needs, substance and circumstances.

Ensure accurate notes of all consultations and contacts relating to service users are recorded clearly and fully in clinical notes.

Completing accurate, person-centred and individualised risk assessments and risk management plans.

Offer wound care assessment and basic wound dressing to clients. Organise signposting to other services as required, and ensure clear pathways into other treatment arenas as appropriate.

Undertake phlebotomy and dry blood spot testing, as required, to undertake routine screening and testing, ensuring adherence to identified pathway for blood sample labelling, collection, obtaining results and signposting follow-up and any next steps planning.

2. Human Resources

Ensure that procedures are constantly reviewed to promote best practice and evidenced based practice in accordance with UK Guidelines on Clinical Management, NICE guidance, NMC guidance and Cranstoun: Sandwell clinical protocols and procedures.

Take responsibility of training and registration to ensure practice is up to date with guidelines.

Delivering on your own role performance commitments and seek to maximise own learning and potential.

Train staff in new guidelines and procedures when required.

3. Health, Safety & Facilities

To ensure the services provides a good quality working environment. Ensure the Service has a safe, quality & conducive environment for

employees to work & clients to use. Ensure that any dangerous situations, occurrences and incidents are

appropriately reported and investigated. Work within Cranstoun environmental plan in order to achieve environmental

best practise within this sector.

4. Financial Management

Support the Clinical lead to manage and control medical supply budgets in line with Cranstoun’s financial regulations and policies.

Contribute to effective financial management in Cranstoun by ensuring your day to day activities and workplace decisions reflect an understanding of costs.

Maximise the use and effectiveness of available resources.

5. Outcomes & Performance

Deliver high levels of performance in a way that realises your potential, by;-

Ensuring that the agreed quality and regulatory objectives are met by prioritising, planning and organising own workload.

Contributing to Cranstoun growth and business development plans by advocating for Cranstoun to clients, their families, stakeholders and other external contacts and partners through delivering on commitments and presenting Cranstoun in a positive image.

Pro-actively maintaining an up to date knowledge of medicine management and both local and national guidelines and agreements.

Actively engage in clinical supervision and CPD. Providing effective mentoring, and support to the nursing team and wider

service team. Ensuring all data and information relating to clients is accurate and shared in

the appropriate way. Inputting outcomes data and other information into corporate systems in

accordance with stated policies and procedures.

Observe Cranstoun information management strategy by;-

Carry out day to day tasks in accordance with stated policies, procedures and regulations to assist the service achieve its compliance obligations, including CQC.

Ensure the effective flow of information within the service, with managers and external parties by passing on and seeking information required, raising unresolved concerns and taking an active interest in Cranstoun internal communications.

Help Cranstoun to increase its performance and long term sustainability by optimising the management of IT, facilities and other physical resources that support the achievement of the Service plan.

Project the desired image of Cranstoun by;-

Understanding and promoting Cranstoun values. Being a role model of the values through own example while ensuring they

are demonstrated by the staff in your team. Ensure adherence to confidentiality policy in all aspects of work Undertake any other duties within your capabilities that are relevant to the

Qualifications

RMN/RGN Qualification Demonstrable experience of post registration Demonstrable experience and knowledge working with substance misuse Has a supplementary/independent non-medical prescribing qualification

Experience & Knowledge

Knowledge of the ‘Drug misuse and dependence: UK guidelines on clinical management (2007)’.

Knowledge and understanding of the general health issues affecting drug users in crisis

Knowledge and experience of care planning and case management Experience of working within a multi-disciplinary team Knowledge of primary healthcare & treatment relating to drug users

Abilities and attributes
