Essential Eligibility Criteria

1. For projects working with children, young people and/or vulnerable adults a rigorous safeguarding policy must be in place and all staff/volunteers must have a DBS check ( for more information on obtaining a DBS check contact MPAC on 0151 708 0468).

2. Appropriate insurances for activities and projects must be in place before the project starts.

3. Applicants must agree to the terms and conditions of grants laid down by the My Clubmoor Board and these include:

· A commitment to providing monitoring information as requested

· Agreement to assist in the publicity and promotion of My Clubmoor

· A commitment to providing financial information as requested by the My Clubmoor Board

Please make sure you have read the guidance notes below before submitting your form.

Guidance Notes:

1. Tell us about your idea or project

Give your idea or a project a name and tell us exactly what is you are proposing. You don’t need to tell us why at this stage, we’ll ask that later on in the form. For example:

‘We want to run an 8 week programme of activities for older people who are isolated in the community ‘

Tell us who your project is aimed at e.g. children and young people, older people, unemployed people, disabled people etc. We also need to know how often the project will run (e.g. every day, weekly, weekends only etc.) and how long each session is for (e.g. all day, half day, 3 hrs etc.)

2. What is the start date for your project?

Tell us when you would like to start the project and, if it has end date, tell us that too. This is important so that we can make sure that a decision is made before the start date.

3. How will Clubmoor residents benefit from your idea/project?

We’d like you to tell us about the positive benefit for local people from your idea or project. For example, you may want to run a community clean up and this could: bring together people of all ages; improve the look of the area; get people more involved in caring for their community etc.

4. How many people do you think will benefit?

We appreciate you may not know the exact numbers of people who may be involved but we would expect that you will have an estimate of how many people will benefit.

5. How do you know there is a need?

We want to know why you think your idea/project is needed in the area. It may be that you’ve spoken to family and friends or that you are involved in the local community in some way and have spotted a gap in services. It is helpful if you have taken some time to talk to those who might benefit to see if they are interested in getting involved.

6. How much will the idea/project costs? Please provide a breakdown of the costs involved

While we appreciate you may not have exact figures, please give us some idea of how much you need and what it is you need to spend any grant on. For grants over £500 we will ask for additional information.

7. Which of the My Clubmoor aims does your project idea best fit into (please tick below)

These are the wider aims of the My Clubmoor Board and it’s important that projects fit into these wider aims. If you think your projects fits more than one, go ahead and tick a maximum of three boxes.

8. How will you measure the success of your idea/project and what will happen when the grant is spent?

One you have completed your project the My Clubmoor Board will want to know how things have gone, especially if you intend to apply for another grant to grow the idea/project. Please tell us what methods you will use during the project to measure its success. For example, you may want to use questionnaires from those taking part to find out if they feel they have benefited from being involved and you may want to ask other members of the community if they have seen a difference as a result of your idea/project. It’s important that you think about this in advance of starting your project.

We also need to know if you plan to continue the project and, if so, how it will be financed.

9. Please tell us where the project will take place

We want to know where the project will run from. It may be in a local community centre or from home or some other local venue. Either way, we’ll need you to tell us if you are using the venue for free of it is a cost to the project.

10. Is there anything else you’d like to tell us?

Forms are rubbish aren’t they? There’s never enough room and there’s always something else you want to say. So we thought we’d add a box so you can tell us anything else that you think is important about your idea/project or ask us a question about the process or how we can help you with your idea.

And now..?

If you are happy that you have answered all the questions as fully as you can, please return this form, preferably as a Word document, to [email protected]. We will be posting the dates when Panel will meet to make decisions on the My Clubmoor website, so please check that your application goes to a Panel in plenty of time before the start of your proposed project.
