IN THIS ISSUE Hello Pacific Medicare Family, Another year of pharmacy school has passed! As many of last year’s Medicare Part D students prepare themselves for rotations, the new class is gearing up for the fall slate of outreach events that await us. Aside from the 80 or so drugs that Dr. Patel has us studying over break (ugh!) in preparation for a “quiz” on the first day of class next semester, some of us are taking advantage of our short summer break by traveling the world or merely catching up on sleep that was lost studying for that five-hour pharmacokinetics final. As we look forward to helping Medicare beneficiaries, we also find ourselves looking back to admire what the students in the Medicare classes before us have accomplished. We know that Dr. Patel and the Medicare Part D faculty have provided us with the knowledge to help beneficiaries potentially save money when choosing their Medicare Part D prescription drug plan, but we cannot predict the patients that we will encounter at the upcoming outreach events or truly fathom the impact that we will have on the lives of those that we will be serving. Near the end of this issue (Pages #9-14), you will find a beautiful presentation of memories and photos compiled by Zohal Fazel from all 40 students in the 2011 Medicare class. For those former Part Ders, we hope this compilation brings a smile to your face as you sit back and remember your days at Pacific as a proud Medicare Part D student. You built this class and we hope to continue the tradition and create our own memories as well. As you browse through this issue you will also find the finalized dates and times for each of the upcoming Medicare outreach events this fall, learn more about the evolution of the Part D class from its inception to its present day status, read about Medicare research conducted by previous students, and maybe even laugh a little bit at the corny Medicare jokes that will be sporadically interspersed. When looking at a cross section of a tree trunk, one will find concentric rings (growth rings or annual rings) that describe its age. The inner rings describe the early phases of growth, and the outermost rings signify the most recent growth patterns, of the tree. Similarly, we have created a pictorial (seen on the right), the “Medicare Class Tree Trunk”, to describe the evolution of the class since its inception. We also have provided brief descriptions of the learning and services provided by Part D students since the class was first created in 2007.

2007 Medicare Part D Plan Review and Low-Income Subsidy (LIS) Assistance These two services have been provided each year since we began the Part D program in 2007. Through the Medicare Part D plan review students ensure that beneficiaries are enrolled in the optimum (lowest cost) Part D plan based on their specific prescription drug mix and personal preferences (e.g., preferred pharmacy). The LIS assistance is where students will determine if beneficiaries with limited income/assets may qualify for additional governmental assistance with their Part D drug costs.

2009 Pharmaceutical Assistance Program (PAP) Help In 2009, students first began assisting beneficiaries with determining if their non-Part D covered brand name medications could be obtained at a lower cost through a Pharmaceutical Assistance Program.

2010 Medication Therapy Management (MTM) and Vaccine Administration (Flu) Beginning in 2010, students (under the supervision of licensed pharmacists) began providing full blown MTM services to each assisted beneficiary. During the MTM intervention, students utilize their knowledge and also use drug information databases to identify severe medication related issues including: drug-drug interactions, therapeutic duplication, therapeutic interchange opportunities, Beers list medications, and many others. Findings from the MTM intervention, along with specific recommendations, are communicated to the beneficiary’s prescriber and/or pharmacy. Additionally, students first provided the flu vaccine to beneficiaries at 2010 Medicare outreach events.

2011 Vaccines (Pneumococcal and Shingles) In 2011, students were trained and provided three different vaccines to beneficiaries during the Medicare outreach events. In addition to the flu vaccine (provided starting 2010), students also began providing the pneumococcal and shingles vaccines.

2012 Medicare Top 200 Drugs To help increase students’ knowledge and confidence during the provision of MTM services, the Medicare Top 200 was incorporated in this year’s class. The list of drugs was adopted from the Centers for Medicare & Medicaid Services (CMS) and is based on the most commonly filled medications by Medicare beneficiaries. Each week students are required to learn (and are tested) on approximately 10 new drugs from this list. Information that students are required to know about each drug include: brand/generic name, therapeutic class, common indication, dosing range, common adverse events, key drug-drug interactions, contraindications, key counseling points, and whether the medication is on the Beers list. Quizzes are cumulative to ensure that students have repeated exposure to these drugs and are better prepared for outreach.

Save the Dates!

by Benicia Ortiz As many of you know, the Medicare Part D open enrollment period begins on October 15th and

runs through December 7th. During this time window, all Medicare beneficiaries should evaluate their Part D plan options for the upcoming year (2013). Why is this important? We have found that up to 90% of beneficiaries in a Part D plan can potentially save money (~$600) by switching plans during the open enrollment period. As such, it is crucial that we reach out to vulnerable Medicare beneficiaries and provide them with the necessary assistance.

Trained pharmacy students from the University of the Pacific will be holding a series of outreach events during which beneficiaries will be provided Part D plan assistance and Medication Therapy Management services. In addition, at each of these events, outreach attendees will be offered a myriad of health care screenings and services. Below you will find a detailed list of our scheduled events this fall including details about event date/timing and location. As always, to be able to provide these services, we need your help. We are looking for extraordinary pharmacists that would be willing to volunteer their time to precept students at these events. The Northern California College of Clinical Pharmacy (NCCCP) committee hosted the inaugural PharmD Research Day on July 9th for current Pacific pharmacy students to showcase their research efforts and findings, refine their oral presentation skills and receive structured feedback from faculty judges. Under the guidance of faculty members, 25 students (23 took last year’s Medicare class) presented their Medicare-related research projects on this day. The ultimate goal for each of these students is to present their research at a national meeting and to submit a completed manuscript to a peer-reviewed journal. One such example of a research project is the “Healthcare Passport” which focuses on turning the concept of Pacific’s Medicare Part D Outreach program into a reproducible model for other pharmacy schools across the nation to emulate. This poster received the “Outstanding Pharmacy Student Poster” at the NCCCP PharmD Research Day. In addition, the poster “Beneficiaries' Knowledge, Attitudes, and Perceptions of the Medicare Part D Benefit” won the day’s highest prize (“Pharmacy Research Excellence Award”) which included a $100 Grant to help defray student travel costs when presenting their work at a future conference. All 10 Medicare research projects have been accepted for presentation at national conferences this year. A comprehensive list of the Medicare research projects, conferences this work will be presented at, and the students and faculty involved can be found on the following page. We are proud of our fellow Part Ders for their research efforts and wish them continued success!

2013 MEDICARE PART D UPDATES The 10 things you should know about the Part D benefit in 2013:

Inclusion (for the first time) of Benzodiazepines and Barbiturates as Part D covered drugs

o Part D drugs will include, provided they meet all other requirements to be a Part D drug,:

Barbiturates used for the medical indications of epilepsy, cancer, or a chronic

mental health disorder; and

Benzodiazepines

CMS established a Special Enrollment Period that allows eligible Medicare beneficiaries the

opportunity to enroll in a 5-star Part D plan at any point during the year

The maximum allowable deductible for a Part D plan in 2013 is $325

Total (beneficiary + plan) drug costs between $2970 and $6733.75 describe the coverage gap

(‘donut hole’)

Beneficiaries will receive a 52.5% point-of-sale discount on formulary covered brand-name

medications while in the coverage gap

Beneficiaries will receive a 21% point-of-sale discount on formulary covered generic

medications while in the coverage gap

Beneficiaries pay whichever is higher, 5% of the covered drug cost over $6733.75

(‘catastrophic coverage’), or

o $2.65 for formulary generic drugs or

o $6.60 for formulary brand drugs

Until reaching catastrophic coverage, full-benefit dual-eligible (Medicare and Medi-Cal)

beneficiaries will pay:

o $1.15 for formulary generic drugs or

o $3.50 for formulary brand drugs

Full-benefit dual-eligible (Medicare and Medi-Cal) beneficiaries pay nothing ($0) for formulary

medications once reaching catastrophic coverage
